    Cross-sectional study on Brucella spp., Leptospira spp. and Salmonella spp. in bats from Montes Claros, Minas Gerais, Brazil

    The understanding on the role of bats in the ecology of zoonotic diseases, especially its relevance as a carrier of pathogens, is important for the determination of preventive measures considering the One Health context. The present study aimed to investigate the presence of Brucella spp., Leptospira spp. and Salmonella spp. in blood (n = 163), liver (n = 35) and spleen (n = 62) samples from bats captured in Montes Claros, Minas Gerais, Brazil. Only Salmonella spp. was found in a blood sample of an insectivorous female bat of the species Lasiurus blossevilli, evidencing the capacity of this animal species to host this pathogen. In conclusion, our results in bats from Montes Claros indicate that they do not act as hosts for Brucella spp. and Leptospira spp., although being potential carriers of Salmonella spp. in a low prevalence

    Simple Summary: Culicoides genus insects are spread throughout the world, and some species are vectors of important human and animal diseases. Genetic identification, as well as the local occurrence of these insects, are fundamental to the development of risk profiles and entomological surveillance of transmitted diseases. We investigated the Culicoides occurrence in Alagoas State, northeastern Brazil. Midges were captured with light traps, being morphologic and genetic identified. After PCR analysis and GenBank database comparison, it was confirmed that the captured midges belong to Culicoides insignis. This was the first formal report of Culicoides insignis occurrence in Alagoas State, northeastern Brazil. Abstract: The species of the Culicoides genus are hematophagous, and some of them are vectors of important human and animal diseases. This group of insects is distributed worldwide, varying according to local species. Knowledge of the geographic distribution of specific species is crucial for the development and implementation of control strategies. The aim of this work was to investigate the occurrence of Culicoides in the state of Alagoas in northeast Brazil. Midges were captured with CDC light traps, and their identification and morphological analyses were performed by the Ceratopogonidae Collection of the Oswaldo Cruz Foundation (FIOCRUZ/CCER) in Rio de Janeiro, Brazil. Morphological analyses were performed using the key to Culicoides from the guttatus group and comparison with other deposited specimens. DNA sequencing, genetic analysis and comparison with sequences in the Genbank database, confirmed the identification of the flies as Culicoides insignis. This was the first formal report of C. insignis being found in Alagoas

As populações migrantes têm diferentes necessidades, sendo o acesso à saúde algo primordial para garantir a qualidade de vida. Assim, este estudo objetivou analisar a produção científica sobre atenção à saúde de imigrantes internacionais haitianos em diferentes contextos e países, buscando compreender quais são os desafios e as perspectivas para a atenção à saúde dessa população nos contextos investigados. Trata-se de uma Revisão Integrativa de Literatura, realizada a partir de pesquisas nas bases Sistema Online de Busca e Análise de Literatura Médica (Medline/PubMed) e Scientific Electronic Library Online (SciELO), com os descritores padronizados ‘Health Care’ (‘Public Health’), ‘Haiti’ e ‘Emigrants and Immigrants’, combinados por meio do operador booleano AND. Selecionaram-se artigos publicados no período de 2015 a 2019 que fossem em português, inglês ou espanhol e que tratassem desse tema. A partir da análise dos artigos, identificaram-se três categorias de análise temática: Impactos das regulamentações imigratórias na integração dos imigrantes haitianos; Competência intercultural para a atenção à saúde de imigrantes haitianos; e Condições de saúde e acesso a serviços por imigrantes haitianos. Destaca-se a importância da inclusão de aspectos políticos interculturais nas políticas de saúde voltadas à atenção à saúde do imigrante.With globalization, migration flows have intensified and financial reasons are one of the main drivers of these events. Migrant populations have different needs, and access to health is essential to ensure their quality of life. Accordingly, this study aimed to analyze the scientific production on health care of international Haitian immigrants in different contexts and countries, seeking to understand what are the challenges and perspectives for the health care of this population in the investigated contexts. This is an Integrative Literature Review, performed from searches in the Medical Literature Analysis and Retrieval System Online (Medline/PubMed) and Scientific Electronic Library Online (SciELO), with the standardized descriptors ‘Health Care’ (‘Public Health’), ‘Haiti’ and ‘Emigrants and Immigrants’, combined through the Boolean operator AND. We selected articles published in the period 2015-2019, in Portuguese, English or Spanish, that dealt with this theme. From the analysis of the articles, we identified three categories of thematic analysis: Impacts of immigration regulations on the integration of Haitian immigrants; Intercultural competency for the health care for Haitian immigrants; and Health conditions and access to services by Haitian immigrants. We should highlight the importance of including intercultural political aspects in health policies focused on the health care for immigrants.Con la globalización, los flujos migratorios se han intensificado y las razones financieras son uno de los principales impulsores de tales eventos. Las poblaciones migrantes tienen diferentes necesidades, siendo el acceso a la salud fundamental para garantizar la calidad de vida.     Evidence of Zika virus circulation in asymptomatic pregnant women in Northeast, Brazil

    Background: Zika virus (ZIKV) is a flavivirus associated with microcephaly and other fetal anormalities. However, evidence of asymptomatic ZIKV infection in pregnant women is still scarce. This study investigated the prevalence of Zika infection in asymptomatic pregnant women attending two public maternities in Maranhão state, Northeast Brazil. Methods: A total of 196 women were recruited at the time of delivery by convenience sampling from two maternity clinics in São Luís, Maranhão, Brazil, between April 2017 and June 2018. Venous blood, umbilical cord blood and placental fragments from maternal and fetal sides were collected from each subject. ZIKV infection was determined by reverse transcription polymerase chain reaction (RT-qPCR) for ZIKV and by serology (IgM and IgG). Nonspecific laboratory profiles (TORCH screen) were obtained from medical records. Results: The participants were mostly from São Luís and were of 19-35 years of age. They had 10-15 years of schooling and they were of mixed race, married, and Catholic. ZIKV was identified in three umbilical cord samples and in nine placental fragments. Mothers with positive ZIKV RT-qPCR were in the age group older than 19 years. Of the 196 women tested by ZIKV rapid test, 6 and 117 women were positive for anti-ZIKV IgM and anti-ZIKV IgG antibodies, respectively. Placental Immunohistochemistry study detected ZIKV in all samples positive by RT-PCR. The newborns did not show any morphological and/or psychomotor abnormalities at birth. Conclusions: Asymptomatic ZIKV infection is frequent, but it was not associated to morphological and/or psychomotor abnormalities in the newborns up to 6 months post-birth. Although pathological abnormalities were not observed at birth, we cannot rule out the long term impact of apparent asymptomatic congenital ZIKV infection

    Toward the Establishment of a Single Standard Curve for Quantification of Trypanosoma cruzi Natural Populations Using a Synthetic Satellite Unit DNA Sequence

    Accurate diagnostic tools and surrogate markers of parasitologic response to treatment are needed for managing Chagas disease. Quantitative real-time PCR (qPCR) is used for treatment monitoring, but vari ability in copy dosage and sequences of molecular target genes among different Trypanosoma cruzi strains limit the precision of quantitative measures. To improve qPCR quantification accuracy, we designed and evaluated a synthetic DNA molecule containing a satellite DNA (satDNA) repeat unit as standard for quantification of T. cruzi loads in clinical samples, independently of the parasite strain. Probit regression analysis established for Dm28c (TcI) and CL-Brener (TcVI) stocks similar 95% limit of detection values [0.903 (0.745 to 1.497) and 0.667 (CI, 0.113 to 3.927) copy numbers/mL, respectively] when synthetic DNA was the standard for quantification, allowing direct comparison of loads in samples infected with different discrete typing units. This standard curve was evaluated in 205 samples (38 acute oral and 19 chronic Chagas disease patients) from different geographical areas infected with various genotypes, including samples obtained during treatment follow-up; high agreement with parasitic load trends using standard curves based on DNA extracted from spiked blood with counted parasites was obtained. This qPCR-based quantification strategy will be a valuable tool in phase 3 clinical trials, to follow up patients under treatment or at risk of reac tivation, and in experimental models using different parasite strains

    Schistosoma mansoni Infection Is Impacted by Malnutrition

    Schistosomiasis remains one of the most important neglected tropical diseases in the world. It mainly affects developing countries, where it often coexists with malnutrition. Despite this, few studies have investigated the relationship between schistosomiasis and malnutrition. Herein, we evaluate the impact of malnutrition on experimental S. mansoni infection. Mice were divided into 5 groups: Control (Ctrl) diet (14% protein and 10% lipids), low-protein 3% (LP 3%), low-protein 8% (LP 8%), low-fat 2.5% (LF 2.5%), and low-fat 5% (LF 5%). Mice were fed with their respective diets and were infected when a difference of approximately 20% in the body weight between mice from any experimental group and mice from the control group was achieved. Nutritional, parasitological, and immunological parameters were assessed either just before infection and/or approximately 50 days later before mice were perfused. Our results showed that the 3% low-protein diet was the only one capable of establishing malnutrition in mice. Mice fed with this diet showed: (i) significant reduction in body weight and serum albumin levels before infection, (ii) decreased levels of all biochemical parameters evaluated before perfusion, (iii) decreased numbers of schistosome eggs trapped in intestines and impaired parasite fecundity, (iv) a delay in the granuloma development with a smaller granuloma area, and (v) reduced levels of IL-4 and IFN-γ in the liver. Our findings demonstrate that low protein supply leads to malnutrition in mice and impacts the cytokine milieu in the liver and granuloma formation. Additionally, the establishment of our murine malnutrition model will enable future studies aiming to better understand the complex relationships between nutrition, immune responses, and infection outcom

    Narrativas como contribuição para construção da Memória da Chikungunya

    A presente dissertação foi inspirada e formulada em paralelo à implantação da pesquisa da Rede de Pesquisas Clínica e Aplicada em Chikungunya (Replick), coordenada pelo Instituto Nacional de Infectologia Evandro Chagas (INI) que busca, dentre outros objetivos, estabelecer a História Natural da Chikungunya. Enquanto uma doença re-emergente, e com características próximas às doenças negligenciadas, a Chikungunya propicia a identificação única de um cenário de interação e coprodução de conhecimento entre especialistas e leigos, em busca de estratégias e soluções para o manejo da doença. Nesse sentido, o comprometimento da Replick com o engajamento do sujeito na pesquisa é colocado como central, ressaltando tanto as dimensões éticas e da centralidade do sujeito, trazendo contribuições fundamentais para melhores resultados de cuidado integral em saúde. O foco desta pesquisa está na busca por estratégias que deem voz aos sujeitos, possibilitando que a clínica da doença seja explicitada e construída junto com a descrição da vida de cada um dos sujeitos, para que seja possível a construção da história social da doença para além da natural. Nesse sentido, parte-se do conceito de narrativas médicas e do sujeito da pesquisa, seu entrelaçamento com a construção social da doença e, por meio de uma imersão na literatura cientifica, busca-se identificar se e como essa estratégia pode ser utilizada na Replick e em demais pesquisas biomédicas e ensaios clínicos, de forma a propiciar a construção de uma memória da Chikungunya a partir das narrativas dos pacientes participantes da pesquisa clínica.This dissertation was inspired and formulated in parallel to the implantation of research by the Clinical and Applied Research Network in Chikungunya (Replick), coordinated by the National Institute of Infectious Diseases Evandro Chagas (INI) which seeks, among other objectives, to establish the Natural History of Chikungunya. As an emerging disease, and with characteristics close to neglected diseases, Chikungunya provides and unique identification of a scenario of interaction and co-production of knowledge between specialists and lay people, in search of strategies and solutions for the management of the disease. In this sense, Replick's commitment to the subject's engagement in research is placed as central, emphasizing both the ethical dimensions and the centrality of the subject, bringing fundamental contributions to better results of comprehensive health care. The focus of this research is on the search for strategies that give voice to the subjects, enabling the clinic of the disease to be made explicit and built together with the description of the life of each one, so that it is possible to build the social history of the disease beyond the Natural. In this sense, it starts with the concept of medical narratives and the research subject, their intertwining with the social construction of the disease and, through an immersion in the scientific literature, we seek to identify whether and how this strategy can be used in Replick and in other biomedical research and clinical trials, in order to promote the construction of a memory of Chikungunya from the narratives of patients participating in clinical research
