Detecção do RNA do vírus Chikungunya em fluido oral e urina: uma abordagem alternativa para o diagnóstico?
Objetivo(s): To evaluate whether oral fluid (OF) and urine can be used as alternative samples to serum to diagnose chikungunya virus (CHIKV) infection by qRT-PCR. Material e Métodos: Serum, OF (collected with a rayon-tipped swab in the sublingual region), and urine samples used in the evaluation were obtained from patients enrolled in a prospective surveillance study to detect arbovirus infection among cases of acute febrile illness (≤7 days of symptom onset). The studied samples belonged to three groups of patients with laboratory evidence of chikungunya: patients with qRT-PCR positive for CHIKV in acute-phase sera (n=19); with CHIKV IgM seroconversion between
paired sera (n=12); and with CHIKV IgM detection in acute-phase sera (n=20). The last two groups were negative for CHIKV by qRT-PCR in acute-phase serum. As controls, we used sera, OF, and urine from patients with other acute febrile illnesses (n=28) and from patients with RT-PCR-confirmed dengue (n=16). All samples underwent RNA extraction (Maxwell®, Promega, Madison, USA), followed by CHIKV qRT-PCR (CDC-TRIOPLEX). CHIKV IgM antibodies were investigated using IgM ELISA kits (InBios, Seattle, USA). Resultados e Conclusão: Of the 19 patients with qRT-PCR positive for CHIKV in sera, 9 (47.4%) and one (5.2%) were also positive by qRT-PCR in OF and urine, respectively. Of the 12 patients with
evidence of CHIKV infection solely based on CHIKV IgM seroconversion, 2 (16.7%) were positive for CHIKV by qRT-PCR in OF, and none in urine. None of the 20 patients belonging to the group with CHIKV IgM antibodies in acute-phase sera were positive for CHIKV by qRT-PCR in OF and urine. None of the samples from the control groups were positive (specificity: 100%).Although the CHIKV qRT-PCR in OF was positive in some patients with CHIKV IgM seroconversion who were negative by qRT-PCR in sera, the qRT-PCR sensitivity in OF was about 50% among patients who had a positive CHIKV qRT-PCR in serum. Thus, while OF may be useful as an alternative non-invasive specimen to diagnose acute
chikungunya, a negative result cannot rule out an infection. The frequency at which acute-phase urine showed detectable levels of CHIKV RNA in patients with detectable CHIKV RNA in sera was too low to suggest the use of urine as an alternative acute-phase sample. Further research is needed to evaluate whether OF and urine collected later in the disease course may be helpful in diagnosing chikungunya
Interleukin 21 receptor affects adipogenesis of human adipose-derived stem/stromal cells
Dysfunctions in adipose tissue cells are responsible for several obesity-related metabolic diseases. Understanding the process of adipocyte formation is thus fundamental for understanding these diseases. The adipocyte differentiation of adipose-derived stem/stromal cells (ADSCs) showed a reduction in the mRNA level of the interleukin 21 receptor (IL21R) during this process. Although the receptor has been associated with metabolic diseases, few studies have examined its function in stem cells. In this study, we used confocal immunofluorescence assays to determine that IL21R colocalizes with mitochondrial protein ATP5B, ALDH4A1, and the nucleus of human ADSCs. We demonstrated that silencing and overexpression of IL21R did not affect the cell proliferation and mitochondrial activity of ADSCs. However, IL21R silencing did reduce ADSC adipogenic capacity. Further studies are needed to understand the mechanism involved between IL21R and the adipogenic differentiation process

Educação em Saúde Bucal com plataformas digitais em tempos de COVID-19: relato de experiencia
Introdução:A pandemia da COVID-19 impôs o isolamento social e causou um reordenamento da educação em todos os níveis e modalidades. Objetivo:Relatar umaexperiência bem-sucedida de readaptação de ações em Educação e Promoção de Saúde Bucal realizadas remotamente em um município do Estado de Mato Grosso, por meio de plataformas digitais, como forma de reduzir os danos relacionados com a falta de orientação profissional presencial.Resultados:A experiência mostrou que as crianças se adaptaram ao modelo virtual e mantiveram-se motivadas a realizarem a higiene bucal. Não foi possível realizar a escovação supervisionada nas crianças pois os pais não aderiram às palestras realizadas de maneira virtual. Conclusão:Essa experiência permitiu verificar que atividades educacionais curtas e pontuais, realizadas de maneira virtual e síncrona, podem ser estratégias úteis para levar a Educação em Saúde Bucal para crianças, sobretudo em contextos de isolamento social.Introduction:The pandemic of COVID-19 imposed social isolation and caused a reordering of education at all levels and modalities. Objective:To report a successful experience of readaptation of actions in Education and Oral Health Promotion performed remotely in a municipality of Mato Grosso State, through digital platforms, as a way to reduce thedamage related to the lack of face-to-face professional guidance.Results:The experience showed that the children adapted to the virtual model and remained motivated to perform oral hygiene. It was not possible to perform supervised brushing on the children because their parents did not adhere to the virtual lectures. Conclusion:This experience allowed to verify that short and punctual educational activities, carried out in a virtual and synchronous way, may be useful strategies to take Oral Health Education to children, especially in contexts of social isolation

A new focus of spotted fever caused by Rickettsia parkeri in Brazil
Spotted fever (SF) is a tick-borne zoonosis caused by bacteria of the genus Rickettsia.
The disease varies in severity from mild clinical forms to severe cases. In Brazil, Rickettsia
rickettsii SF is the most serious rickettsiosis and can result in death if not diagnosed and
treated at the onset of symptoms. The SF mild form is caused by Rickettsia parkeri strain
Atlantic Rainforest, and this etiological agent has been reported in the South, Southeast
and Northeast regions of the country, in areas of preserved or little antropized Atlantic
Rainforest. Amblyomma ovale is the proven vector and dogs are the hosts associated with the
bioagent cycle. During a SF case investigation in Paraty municipality, Rio de Janeiro State,
an Atlantic Rainforest biome area in Southeastern Brazil, the human pathogen R. parkeri
strain Atlantic Rainforest was detected by PCR in a sample of human skin inoculation eschar
and in a female A. ovale tick collected from a dog. These results expand the known area of
occurrence of this mild form rickettsiosis in Brazil. In addition, the results of the present
study indicate the importance of implementing programs to control canine ectoparasites and
to raise awareness of the risks of infection, signs and symptoms of SF caused by R. parkeri
strain Atlantic Rainforest
