Case Report: Hantavirus Cardiopulmonary Syndrome Diagnostic in the Face of the COVID-19 Pandemic
Hantavirus cardiopulmonary syndrome (HCPS) is an emerging rodent-borne disease in the Americas. The most common initial symptoms of HCPS are similar to those of COVID-19 and other respiratory infections that evolve rapidly to respiratory failure, resulting from pulmonary edema and shock in about 40%of cases. We describe a fatal case of HCPS in a 24-year-old man who was hospitalized with fever, hemoconcentration, thrombocytopenia, leukocytosis, dry cough and a bilateral diffuse alveolar pulmonary infiltrate during the emergence of the COVID-19 pandemic in Brazil. HCPS needs to be ruled out in patients with clinical manifestations compatible with respiratory infections such as influenza and COVID-19
Psychosocial and environmental determinants of physical activity in a Brazilian public university employees - ELDAF: A prospective cohort study protocol
Background: Increased physical activity levels and their determinations are essential issues worldwide. The Longitudinal Study of Physical Activity Determinants (ELDAF) aims to understand the roles of psychosocial and environmental factors in workers' physical activity levels. Methods: A prospective cohort study of non-faculty civil servants from a public university (approximately 1,200 individuals) will start in 2022 (baseline). The primary measurements will be accelerometer- and questionnaire-based physical activity, social support, social network, socioeconomic status, bereavement, job stress, body image, common mental disorders, depression, and neighborhood satisfaction. Additional measurements will include necessary sociodemographic, physical morbidity, lifestyle and anthropometric information. Participants' places of residence will be geocoded using complete addresses. All participants will furnish written, informed consent before the beginning of the study. Pilot studies were performed to identify and correct potential problems in the data collection instruments and procedures. ELDAF will be the first cohort study conducted in Latin America to investigate physical activity and its determinants

Simvastatin and Muscle: Zebrafish and Chicken Show that the Benefits are not Worth the Damage
Simvastatin is one of the most common medicines prescribed to treat human
hypercholesterolemia. Simvastatin acts through the inhibition of cholesterol synthesis.
Unfortunately, simvastatin causes unwanted side effects on muscles, such as soreness,
tiredness, or weakness. Therefore, to understand the mechanism of action of simvastatin, it
is important to study its physiological and structural impacts on muscle in varied animal models.
Here we report on the effects of simvastatin on two biological models: zebrafish embryos and
chicken muscle culture. In the last years, our group and others showed that simvastatin
treatment in zebrafish embryos reduces fish movements and induces major structural
alterations in skeletal muscles. We also showed that simvastatin and membrane cholesterol
depletion induce major changes in proliferation and differentiation of muscle cells in chick
muscle cultures. Here, we review and discuss these observations considering reported data on
the use of simvastatin as a potential therapy for Duchenne muscular dystrophy
Antileishmanial Efficacy of the Calpain Inhibitor MDL28170 in Combination with Amphotericin B
The necessity of drug combinations to treat leishmaniasis came to the surface mainly
because of the toxicity of current treatments and the emergence of resistant strains. The calpain
inhibitor MDL28170 has previously shown anti-Leishmania activity, therefore its use in association
with standard drugs could provide a new alternative for the treatment strategy against leishmaniasis.
In this study, we analyzed the potential of the combination of MDL28170 and the antileishmanial drug
amphotericin B against Leishmania amazonensis and Leishmania chagasi. The compounds were tested
in the combination of the 1/2 IC50 value of MDL28170 plus the 1/4 IC50 value of amphotericin
B, which led to an increment in the anti-promastigote activity when compared to the single drug
treatments. This drug association revealed several and severe morphophysiological changes on
parasite cells, such as loss of plasma membrane integrity, reduced size of flagellum, and depolarization
of mitochondrial membrane potential besides increased reactive oxygen species production. In
addition, the combination of both drugs had a deleterious effect on the Leishmania–macrophage
interaction, reflecting in a significant anti-amastigote action, which achieved a reduction of 50% in the
association index. These results indicate that the combination treatment proposed here may represent
a new alternative for leishmaniasis chemotherapy
Experimental Combination Therapy with Amiodarone and Low- Dose Benznidazole in a Mouse Model of Trypanosoma cruzi Acute Infection
Chagas disease (CD), caused by Trypanosoma cruzi, affects approximately 6
to 7 million people in Latin America, with cardiomyopathy being the clinical manifestation
most commonly associated with patient death during the acute phase. The etiological
treatment of CD is restricted to benznidazole (Bz) and nifurtimox (Nif), which involve long
periods of administration, frequent side effects, and low efficacy in the chronic phase.
Thus, combined therapies emerge as an important tool in the treatment of CD, allowing
the reduction of Bz dose and treatment duration. In this sense, amiodarone (AMD), the
most efficient antiarrhythmic drug currently available and prescribed to CD patients, is a
potential candidate for combined treatment due to its known trypanocidal activity.
However, the efficacy of AMD during the acute phase of CD and its interaction with Bz
or Nif are still unknown. In the present study, using a well-established murine model of
the acute phase of CD, we observed that the Bz/AMD combination was more effective in
reducing the peak parasitemia than both monotherapy treatments. Additionally, the Bz/
AMD combination reduced (i) interleukin-6 (IL-6) levels in cardiac tissue, (ii) P-wave duration,
and (iii) frequency of arrhythmia in infected animals and (iv) restored gap junction integrity
in cardiac tissue. Therefore, our study validates AMD as a promising candidate for
combined therapy with Bz, reinforcing the strategy of combined therapy for CD.
IMPORTANCE Chagas disease affects approximately 6 to 7 million people worldwide,
with cardiomyopathy being the clinical manifestation that most commonly leads to patient
death. The etiological treatment of Chagas disease is limited to drugs (benznidazole and
nifurtimox) with relatively high toxicity and therapeutic failures. In this sense, amiodarone,
the most effective currently available antiarrhythmic drug prescribed to patients with
Chagas disease, is a potential candidate for combined treatment due to its known trypanocidal
effect. In the present study, we show that combined treatment with benznidazole
and amiodarone improves the trypanocidal effect and reduces cardiac damage in
acutely T. cruzi-infected mice
Evaluation of the performance of commercial tests for Covid-19 immunodiagnosis
Introdução: Ensaios moleculares são considerados o teste de referência para o diagnóstico precoce de indivíduos sintomáticos e/ou com suspeita clínica da COVID-19. Contudo, tais ensaios mostram-se ineficazes para a identificação e rastreio de casos assintomáticos, sendo necessária a utilização de ensaios sorológicos indiretos. Após o início da pandemia diversos testes sorológicos foram disponibilizados no mercado e, devido à urgência em sua utilização, nem todos foram submetidos à uma avaliação sistemática de seu desempenho. Objetivo(s): Avaliar o desempenho de três testes comerciais para o diagnóstico sorológico da COVID-19. Material e Métodos: Quinhentas e doze amostras foram selecionadas para a realização deste estudo. Deste total, 353 foram de pacientes COVID-19 (RT-PCR positivo para SARS-CoV-2) internados em hospitais de Salvador - BA, e 159 de doadores de sangue negativos para a COVID-19 obtidas em período anterior à pandemia. As amostras foram categorizadas em 4 grupos: 0-7 dias após o aparecimento dos sintomas, 8-14 dias, 15-21 dias e 22 ou mais dias, e submetidas aos kits comerciais Gold ELISA COVID-19 IgG+IgM (REM Diagnóstica, Brasil), Anti-SARS-CoV-2 NCP ELISA (IgG) (Euroimmun, Brasil) e Anti-SARS-CoV-2 NCP ELISA (IgM) (Euroimmun, Brasil). Resultados e Conclusão: O valor de sensibilidade variou conforme o tempo pós-aparecimento dos sintomas. O Gold ELISA apresentou o maior valor (57,5%), seguido pelo Euro IgM (46,5%) e Euro IgG (41,4%). A especificidade manteve-se elevada para os 3 kits (>96,2%), enquanto a acurácia variou de 70,5% para o Gold ELISA e 61,9% para o Euro IgM à 59,6% para o Euro IgG. Os três ensaios apresentaram AUC superior a 70%, indicando capacidade discriminatória moderada. O índice de Kappa revelou concordância moderada entre o Gold ELISA e o Euroimmun IgM, enquanto o Euroimmun IgG apresentou fraca concordância com Gold ELISA e o Euroimmun IgM. A análise de Bland-Altman mostrou baixa variabilidade dos resultados, apesar da regressão de Deming apontar presença de erro aleatório e sistêmico entre os três testes analisados. Apesar da baixa sensibilidade, os testes apresentaram bom desempenho diagnóstico. Foi observado que a sensibilidade é condicionada ao tempo entre a coleta do material biológico e aparecimento dos sintomas. Assim, é necessário o desenvolvimento de testes de elevada sensibilidade analítica capazes de detectar anticorpos anti-SARS-CoV-2 logo após o aparecimento dos sintomas

Este trabalho está inserido, simultaneamente, em duas pesquisas. A primeira é a de título “Alimentação saudável em tempos de covid-19: circularidade e sentidos em um contexto de pandemia”, desenvolvida por Raíssa Ramos no âmbito do curso de mestrado e que contou com auxílio da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (Capes); a segunda, de título “O SUS como política pública positivada: subjetividade, midiatização e poder”, vem sendo desenvolvida por Wilson Borges no Programa de Pós-Graduação em Informação e Comunicação em Saúde, da Fundação Oswaldo Cruz (Fiocruz)
Epidemiology and Treatment of Canine Tegumentary Leishmaniasis caused by Leishmania braziliensis
Introdução: Objetivo(s): Determinar a prevalência e incidência de leishmaniose canina (LTC) em uma área de transmissão de Leishmania braziliensis. Avaliar a eficácia do tratamento intralesional com antimoniato de meglumina em cães com LTC em uma área de transmissão de L.braziliensis. Material e Métodos: A incidência da infecção canina por L. braziliensis e da LTC foram determinadas pela identificação de cães infectados e de cães doentes no período de um ano na vila de Corte de Pedra. O diagnóstico para detecção da infecção canina foi realizado por Reação em Cadeia da Polimerase (PCR) e pelo teste imunoezimático (ELISA). O segundo estudo foi um ensaio clínico randomizado controlado com o objetivo de avaliar a eficácia e efetividade do Glucantime intralesional comparado com o uso de solução intralesional de cloreto de sódio (placebo) em cães com LTC causado por L. braziliensis. Foram selecionados 32 animais dos 214 cães estudados no primeiro estudo que foram alocados em dois grupos. O grupo 1 (16 animais) foi tratado com Glucantime intralesional e o grupo 2 (16 animais) recebeu placebo nos dias zero, 15 e 30. A cura foi definida como a cicatrização completa da lesão e ausência de bordas elevadas e a falha terapêutica foi considerada quando a úlcera permaneceu ativa após 90 dias. Resultados e Conclusão: De um total de 214 animais estudados,142 animais foram positivos para infecção por L. braziliensis, com uma prevalência de 66%. Desses 142 cães positivos, 66 (47%) apresentaram lesões clássicas de LTC semelhantes às lesões humanas. Após um ano, 138 cães incluídos no estudo foram reavaliados, onde a incidência da doença foi de 14%. As lesões de LTC foram predominantemente observadas na orelha, bolsa escrotal e focinho. Dos 32 cães participantes do 2º estudo de acordo com o tratamento recebido não foram observadas diferenças quanto à idade, duração da doença e tamanho da lesão. O local predominante da úlcera foi a bolsa escrotal, em 9 animais (56%) no grupo tratamento e em 10 (62%) no grupo placebo. A cura no dia 90 foi observada em 14 animais (87,5%) tratados com Glucantime comparado com o grupo placebo onde a cura foi observada em dois animais (12,5%). Em uma área de transmissão de Leishmania braziliensis a prevalência da infecção em
cães é alta assim como a incidência, sugerindo que o cão pode participar da cadeia de transmissão desse parasito. O uso de Glucantime por via intralesional é uma estratégia terapêutica eficaz no tratamento da LTC
