    재가장기요양서비스 이용 노인의 삶의 의미 영향요인: 서술적 단면 조사 연구

    Purpose: The purpose of this study was to investigate the effects of activities of daily living, depres-sion, and social support on the meaning in life in older adults using services under the long-term care insurance system. Methods: This was a cross-sectional study. A total of 121 older adults were recruit-ed from seven senior daycare centers in two cities in Korea. Data collection was performed between September and October 2023. The data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson correlation coefficients, and hierarchical multiple regression analysis. Results: The results of this study showed that social support (β=.45, p<.001) and depression (β=-.16, p=.048) were signifi-cantly affecting factors to the meaning in life in older adults using services under the long-term care insurance system. Conclusion: Efforts to enhance social support and reduce depressive symptoms are critical to improve the meaning in life of older adults utilizing services under the long-term care insurance system. Specifically, it is necessary to closely identify older adults’ depressive symptoms and further strengthen social support, including policy support and individualized support from healthcare professionals in the facilities to improve older adults’ meaning in life

    Robust Symptomatic Subtypes of Typical Alzheimer’s Disease Characterized by Differential Neuropathological Patterns and Functional Decline

    BACKGROUND: Alzheimer's disease (AD) is classically viewed as a predominantly amnestic syndrome, with other cognitive and neuropsychiatric symptoms (NPS) being non-integral associations. Emerging Evidence suggests that within typical AD, these symptoms are core features from the onset. METHODS: We employed K-modes clustering on 2483 cognitively impaired (CI) individuals (CDR ≥ 0.5), excluding participants diagnosed with atypical AD, non-amnestic MCI, or CI due to non-AD dementias from five cohorts: TRIAD, ADNI, BICWALZS, OASIS-III, and Pittsburgh. Cluster-specific clinical and pathological profiles were established through comparison with 2670 cognitively unimpaired (CU) participants across plasma biomarkers (Ptau-181, Ptau-217, GFAP, Nfl, AB42/40 ratio) and neuroimaging (amyloid and tau PET, white matter hyperintensity, MRI-derived degeneration maps). The rate of functional decline, modeled by increase in CDR-SB, was assessed using a Cox-proportional hazards model and a linear mixed-effect model. RESULTS: We identified five distinct clinical phenotypes within typical AD: 'Pure Amnestic' (30.2%), 'Linguistic-Hyperactive' (14.7%), 'Visuospatial-Affective' (14.7%), 'Frontal' (27.6%), and 'Global' (19.3%) (Figure 1, Figure 3). These phenotypes demonstrated consistency regardless of amyloid status or cohort. Each phenotype exhibited a unique neuropathological signature and a distinct pattern of neurodegeneration (Figure 2). A critical aspect of our findings is the differential rate of functional decline across these phenotypes. The 'Pure Amnestic' group showed the slowest decline, followed by 'Linguistic-Hyperactive' (HR = 1.39; B = 0.2), 'Visuospatial-Affective' (HR = 2.13; B = 1), 'Frontal' (HR = 2.23; B = 1.53), and the 'Global' phenotype showing the fastest decline (HR = 3.52; B = 2.66) (Figure 2, Figure 3). CONCLUSION: Our results challenge the concept of "typical" AD by uncovering five robust clinical phenotypes with divergent neuropathological profiles and clinical characteristics. These results mark an important advancement toward personalized medicine in the landscape of emerging AD treatments

    Contribution of monocyte and macrophage extracellular traps to neutrophilic airway inflammation in severe asthma

    Background: Increased blood/sputum neutrophil counts are related to poor clinical outcomes of severe asthma (SA), where we hypothesized that classical monocytes (CMs)/CM-derived macrophages (Mφ) are involved. We aimed to elucidate the mechanisms of how CMs/Mφ induce the activation of neutrophils/innate lymphoid cells (ILCs) in SA. Methods: Serum levels of monocyte chemoattractant protein-1 (MCP-1) and soluble suppression of tumorigenicity 2 (sST2) were measured from 39 patients with SA and 98 those with nonsevere asthma (NSA). CMs/Mφ were isolated from patients with SA (n = 19) and those with NSA (n = 18) and treated with LPS/interferon-gamma. Monocyte/M1Mφ extracellular traps (MoETs/M1ETs) were evaluated by western blotting, immunofluorescence, and PicoGreen assay. The effects of MoETs/M1ETs on neutrophils, airway epithelial cells (AECs), ILC1, and ILC3 were assessed in vitro and in vivo. Results: The SA group had significantly higher CM counts with increased migration as well as higher levels of serum MCP-1/sST2 than the NSA group. Moreover, the SA group had significantly greater production of MoETs/M1ETs (from CMs/M1Mφ) than the NSA group. The levels of MoETs/M1ETs were positively correlated with blood neutrophils and serum levels of MCP-1/sST2, but negatively correlated with FEV1%. In vitro/in vivo studies demonstrated that MoETs/M1ETs could activate AECs, neutrophils, ILC1, and ILC3 by increased migration as well as proinflammatory cytokine production. Conclusions: CM/Mφ-derived MoETs/M1ETs could contribute to asthma severity by enhancing neutrophilic airway inflammation in SA, where modulating CMs/Mφ may be a potential therapeutic option
