Alle Experten gehen davon aus, dass die Fallzahlen wegen der zunehmenden Alterung unserer Gesellschaft in den kommenden Jahren weiter deutlich steigen werden – und dass neurodegenerative Erkrankungen wie Parkinson und Alzheimer bis zum Jahr 2040 nach Herz-Kreislauf-Erkrankungen die zweithäufigste Todesursache sein werden. "Diese Entwicklung stellt unser Gesundheitssystem, vor allem den Pflegebereich, vor neue und große Herausforderungen", prophezeit Dr. Inga Claus. Die Neurologin fordert im "Umdenken"-Podcast der Universität Münster eine umfassendere Aufklärung, mehr Personal sowie zusätzliche spezialisierte Zentren, um die Patientinnen und Patienten bestmöglich zu versorgen

    Rethinking Privacy:How to take care of what we know about others

    Das in dieser Dissertation begründete Konzept der Privatheit als Nichteinmischung (PANIC) liefert eine neuartige Perspektive auf das Private und verlagert die Betrachtung vom Akt der Weitergabe persönlicher Informationen an sich auf die Konsequenzen, die sich daraus ergeben, dass durch das Teilen persönliche Informationen für andere zugänglich werden. PANIC liefert Erklärungen für unsere alltäglichen sozialen Praktiken und stimmt zudem mit unserer intuitiven Auffassung von Privatheit überein. Die zentrale These lautet, dass die Privatheit einer Person nur dann vermindert wird, wenn sich andere in bestimmte Lebensbereiche derart einmischen, dass dies die betroffene Person darin einschränkt, ihr Leben nach ihren eigenen Vorstellungen zu gestalten. Dies stellt eine grundlegende Abkehr von traditionellen Ansätzen dar, die Privatheit unter Bezugnahme auf Geheimhaltung und Kontrolle definieren und beim Schutz der individuellen Privatheit das Individuum selbst in der Verantwortung sehen. PANIC hingegen sieht die Hauptverantwortung für den Schutz der individuellen Privatheit in der Gemeinschaft.The privacy-as-non-interference conception (PANIC) proposed in this dissertation provides an adequate conceptualization of privacy by offering a fresh perspective on privacy that shifts the focus from the act of sharing personal information to the consequences that arise once the information becomes accessible to others. This perspective aligns with our common social practices and our intuitive understanding of privacy. Essentially, PANIC asserts that an individual’s privacy is diminished only when others interfere with specific aspects of their life in a manner that restricts their ability to organize and live their life according to their own choices. This represents a fundamental shift from traditional privacy conceptions that emphasize secrecy or control and place the burden of protecting one’s privacy primarily on the individual. Instead, PANIC suggests that the primary responsibility for safeguarding an individual’s privacy lies with others or society as a whole

    Retrospektive Analyse der megaendoprothetischen Rekonstruktionen von Knie und Hüfte nach periprothetischem Infekt

    Diese retrospektive Studie untersucht 150 Patienten, die nach periprothetischer Infektion des Knie- und/oder Hüftgelenks mit MUTARS®-Megaendoprothesen versorgt wurden. Analysiert wurden Komplikationen, Revisionsraten, das infektfreie sowie das amputationsfreie Implantatüberleben und das Risiko von Reinfektionen. Die häufigsten Ursachen für Implantatversagen waren Reinfektionen und Weichteilkomplikationen. Besonders beim totalen Femurersatz zeigte sich eine erhöhte Reinfektions- und Amputationsrate. Die Kaplan-Meier-Analyse verdeutlichte, dass das Langzeitergebnis durch ein komplexes Zusammenspiel mehrerer Faktoren beeinflusst wird. Insgesamt unterstreicht die Arbeit die hohe Komplexität der septischen Revisionsendoprothetik und die Notwendigkeit optimierter chirurgischer Strategien zur Verbesserung der klinischen Ergebnisse

    Scratching increases epidermal neuronal branching and alters psychophysical testing responses in atopic dermatitis and brachioradial pruritus

    Background: Chronic scratching imposes a major stress on the skin and can lead to itch intensity worsening, and consequently, patients may enter an itch–scratch cycle. This repetitive mechanical stress can result in lichenification, worsening of epidermal barrier function, and enhanced cutaneous inflammation. Furthermore, a reduction of intraepidermal nerve fibers was previously described in lichenification. Aim: The aim of this study was to investigate the influence of chronic scratching on the epidermal neuroanatomy and on sensory changes, in particular the prevalence of hyperknesis and alloknesis in patients after mechanical, chemical, and electrical stimuli. Methods: Analyses were performed on pruritic lichenified (chronically scratched), pruritic non-lichenified (not chronically scratched), and non-pruritic non-lesional (unaffected) skin areas of patients with inflammatory pruritus, i.e., atopic dermatitis (n = 35), and neuropathic pruritus, i.e., brachioradial pruritus (n = 34) vs. healthy matched controls (n = 64). Our fine-grained spatial skin characterization enabled specifically studying the differential effects of chronic scratching in inflammatory and neuropathic itch. Results: Analysis of intraepidermal nerve fiber density showed rarefaction of fibers in all three skin areas of patients compared with healthy controls in both diagnoses. Even more, the two pruritic areas had significantly less nerve fibers than the unaffected skin, whereas electrically induced itch was massively increased. Epidermal branching of the remaining nerve fibers in lichenified/chronically scratched skin was increased, particularly in patients with brachioradial pruritus, which may contribute to the pronounced local neuronal sensitivity. Hyperknesis and alloknesis were found to increase independently of lichenification. Conclusion: Our results indicate that chronic scratching may not affect intraepidermal nerve fiber density but leads to a stronger branching pattern of intraepidermal nerve fibers, which may contribute to local hypersensitivity. The increased sensitivity in the pruritic areas suggests mechanisms of peripheral sensitization, whereas the increased sensation of electrically and chemically induced itch in unaffected skin indicates central sensitization for itch

    Sustainable Use of PiWi Vine Leaves: Life Cycle Assessment and Techno-Economic Analysis of a Novel Vine-Leaf-Based Beverage within the Framework of the Circular Bioeconomy

    This study investigates the ecological and economic performance of a novel, alcohol-free beverage derived from the leaves of fungus-resistant (PiWi) grapevines as part of a research project funded by the Federal Ministry of Research, Technology and Space. Using life cycle assessment (LCA) and techno-economic analysis (TEA), the study evaluates both the environmental footprint and the production costs of the beverage across multiple production scenarios. The results indicate that the PiWi-based beverage shows clear ecological advantages compared to already available drinks such as teabased beverages, fermented lemonades, and alcohol-free wine, mainly due to lower pesticide use and energy demand. Economically, production is feasible within competitive cost ranges, especially when existing winery infrastructure is utilized

    Treatment of Proximal Humeral Fractures in Older Patients During COVID-19 Pandemic in Germany

    Purpose: The COVID-19 pandemic had a severe influence on the entire health sector. Until today, the effect of a SARS-CoV-2 infection on older patients with a proximal humeral fracture (PHF) is unknown. This study examined the following questions: Did the incidence of PHF of older people in Germany vary during the pandemic? Did the treatment change between the lockdown and non-lockdown periods? Was a SARS-CoV-2 infection associated with a worse outcome? Methods: Retrospective claims data of the BARMER health insurance were analysed. All in- and outpatient cases of insurance holders ≥ 65 years from 01/2010 to 09/2022, with coded diagnosis of PHF were analysed. Primary endpoints were the 1-month incidence of PHF per 100,000 insurance holders, number of operative therapies, in-hospital death and in-hospital major adverse events (MAEs). Results: 174,898 inpatient PHF cases were included. During the lockdown periods, the total incidence fell, while the outpatient incidence partially increased during the pandemic-period. Regarding the therapy allocation, there were no relevant persisting changes. In a detailed analysis of 23,979 PHF cases from 01/2020 to 09/2022, 4.1% patients suffered from a SARS-CoV-2 infection and showed a notably higher in-hospital mortality (8.1% vs 2.5%; risk adjusted OR 2.79, 95% CI 2.11– 3.70, p < 0.001) and more MAEs (17.0% vs 7.8%; risk adjusted OR 1.43, 95% CI 1.15– 1.77, p < 0.001). Conclusion: During the COVID-19 pandemic, the overall incidence of PHF in older patients was reduced. The treatment allocation did not change between the pre- and intra-pandemic period. An infection with SARS-CoV-2 was associated with higher mortality and more MAEs

    Trotz der Einführung des StaRUG zum 01.01.2021 werden Restrukturierungen krisenbehafteter Unternehmen, insbesondere von KMU, weiterhin selten mittels eines StaRUG-Verfahrens umgesetzt. Dies verwundert, da gerade das StaRUG-Verfahren nach dem Willen des Richtliniengebers eine Möglichkeit bieten sollte, ein klassisches Insolvenzverfahren zu vermeiden und sich bereits im Vorfeld einer Insolvenz zu restrukturieren. Die vorliegende Untersuchung hat zum Ziel, die Gründe für die geringe praktische Relevanz des StaRUG zu analysieren. Dabei stützt sie sich auf die These, dass insbesondere der Tatbestand der drohenden Zahlungsunfähigkeit gem. § 18 InsO maßgeblich für die seltene Anwendung des StaRUG-Verfahrens verantwortlich ist und keinen geeigneten Eröffnungstatbestand darstellt. Die Arbeit kommt zu dem Ergebnis, dass die drohende Zahlungsunfähigkeit mit erheblichen Rechtsunsicherheiten behaftet ist, insbesondere in Abgrenzung zur Überschuldung gem. § 19 InsO und eine Anknüpfung an einen alternativen Tatbestand geboten wäre

    Telemedizin in der Augenheilkunde: Erfahrungen und Erwartungen deutscher Ophthalmologen im ambulanten und stationären Sektor

    Mit zunehmender Verbreitung digitaler Technologien hat sich das Spektrum von Telemedizin in der Augenheilkunde erweitert. Es umfasst unter anderem die Befundung, Überwachung und postoperative Betreuung von Patienten. Diese Arbeit gibt einen Einblick in die Nutzung von Telemedizin in der Augenheilkunde in Deutschland und betrachtet fachspezifische Nutzen, Hürden, Erfahrungen bezüglich ihrer Verwendung sowie Veränderungen im Rahmen der Covid- 19-Pandemie. Die durchgeführte Umfrage enthält 57 Fragen, unterteilt in sechs Blöcke, abgestimmt auf die Tätigkeitsbereiche der Teilnehmer. Die Befragung wurde als Online-Umfrage unter 528 ambulanten und 166 stationären augenärztlichen Einrichtungen in Deutschland vom 15.12.2023 bis zum 30.01.2024 durchgeführt. Insgesamt wurden 190 gültige Teilnahmen an der Umfrage erhalten. Die Ergebnisse zeigen, dass Telemedizin in der Augenheilkunde nicht flächendeckend verbreitet ist. Primäre Hürden sind Diagnosesicherheit, Datenschutz, gesetzliche Restriktionen, Finanzierungsrisiken und technische Ausstattung. Die Tätigkeit im stationären Sektor, größeren Einrichtungen sowie operative Tätigkeit erhöhen die Nutzungswahrscheinlichkeit von Telemedizin signifikant. Hauptgrund für die Nutzung ist die potentielle Einsparung zeitlicher Ressourcen. Die Bedeutung von Telemedizin hat durch die Covid-19-Pandemie für die Hälfte der Befragten nicht zugenommen. Es lässt sich zudem nicht bestätigen, dass durch die Covid-19- Pandemie die Nachfrage von Telemedizin in der Augenheilkunde signifikant gestiegen ist. Die Hälfte der Befragten empfindet Telemedizin als zusätzliche Belastung im Alltag, eine Mehrheit bevorzugt weiterhin den physischen Arztkontakt. Die physische Untersuchung bleibt daher auch in Zukunft essentiell

    The fundamental groupoid: Topology, Haar system and actions

    We equip the fundamental groupoid π1(X) of a locally path connected and semilocally simply connected space X with the Whisker topology to make it a topological groupoid. Then the equivalence of the Whisker topology and the quotient of the compactopen topology on the path space of X is proved. This topological groupoid is locally trivial and not étale. We prove that the fundamental groupoid of a topological group, in particular a Lie group, is a transformation groupoid. To understand when π1(X) is locally compact or Hausdorff, we study its point-set topology. We discuss the existence and give a description of a Haar system on π1(X). For the last main result, let E be the category of π1(X)-spaces for which the momentum maps are local homeomorphisms. We show that this category is isomorphic to that of the covering spaces of X. Using this, we give different characterizations for the free or proper actions in E

    Unraveling the blue shift in porphyrin fluorescence in glioma: The 620 nm peak and its potential significance in tumor biology

    In glioma surgery, the low-density infiltration zone of tumors is difficult to detect by any means. While, for instance, 5-aminolevulinic acid (5-ALA)-induced fluorescence is a well-established surgical procedure for maximizing resection of malignant gliomas, a cell density in tumor tissue of 20–30% is needed to observe visual fluorescence. Hyperspectral imaging is a powerful technique for the optical characterization of brain tissue, which accommodates the complex spectral properties of gliomas. Thereby, knowledge about the signal source is essential to generate specific separation (unmixing) procedures for the different spectral characteristics of analytes and estimate compound abundances. It was stated that protoporphyrin IX (PpIX) fluorescence consists mainly of emission peaks at 634 nm (PpIX634) and 620 nm (PpIX620). However, other members of the substance group of porphyrins fluoresce similarly to PpIX due to their common tetrapyrrole core structure. While the PpIX634 signal has reliably been assigned to PpIX, it has not yet been analyzed if PpIX620 might result from a different porphyrin rather than being a second photo state of PpIX. We thus reviewed more than 200,000 spectra from various tumors measured in almost 600 biopsies of 130 patients. Insufficient consideration of autofluorescence led to artificial inflation of the PpIX620 peak in the past. Recently, five basis spectra (PpIX634, PpIX620, flavin, lipofuscin, and NADH) were described and incorporated into the analysis algorithm, which allowed more accurate unmixing of spectral abundances. We used the improved algorithm to investigate the PpIX620 signal more precisely and investigated coproporphyrin III (CpIII) fluorescence phantoms for spectral unmixing. Our findings show that the PpIX634 peak was the primary source of the 5-ALA-induced fluorescence. CpIII had a similar spectral characteristic to PpIX620. The supplementation of 5-ALA may trigger the increased production of porphyrins other than PpIX within the heme biosynthesis pathway, including that of CpIII. It is essential to correctly separate autofluorescence from the main PpIX634 peak to analyze the fluorescence signal. This article highlights the need for a comprehensive understanding of the spectral complexity in gliomas and suggests less significance of the 620 nm fluorescence peak for PpIX analysis and visualization
