    Modified Trochanteric Flip Osteotomy in Varus Intertrochanteric Osteotomy for Treatment of Legg–Calvé–Perthes Disease

    Background/Objectives: Legg–Calvé–Perthes disease (LCPD) presents challenges in treatment due to its varied course and unclear etiology. This study aimed to evaluate the efficacy of combining proximal femoral varus osteotomy (PFVO) with a modified trochanteric flip osteotomy to address biomechanical consequences and improve hip abductor muscle strength. Methods: We present a modified approach combining PFVO with a trochanteric flip osteotomy. In this technique, the greater trochanter in compound with its muscular insertions is separated from the femur and attached distally using a varization blade plate. Nine patients (ten hips, mean age 8 years) with LCPD were treated using this technique. Clinical examination findings and radiographic evaluations were retrospectively analyzed. The median follow-up was 33 months. Results: At the last follow-up, two patients exhibited Trendelenburg gait, but hip abduction was improved in all patients. Radiographically, consolidation at the osteotomy site was observed in all cases with no delayed union or non-union. The median CE angle improved by 7°, while the median CCD decreased by 18°. The median MPFA decreased by 13°, resulting in a median of 82°. Conclusions: Combining PFVO with a modified trochanteric flip osteotomy addresses biomechanical issues associated with PFVO, potentially improving hip containment and abductor muscle strength. This approach may offer advantages over traditional osteotomy techniques in treating LCPD, and it appears to produce a superior functional outcome in particular in regard to limping when compared to conventional PFVO. Despite satisfactory radiological outcomes in most cases, further research is needed to assess long-term effectiveness and address challenges such as femoral head enlargement and persistent gait abnormalities

    On various notions of distance between subalgebras of operator algebras

    Given any irreducible inclusion B ⊂ A of unital C*-algebras with a finite-index conditional expectation E : A → B, we show that the set of E-compatible intermediate C*-subalgebras is finite, thereby generalizing a finiteness result of Ino and Watatani [11]. A finiteness result for a certain collection of intermediate C*-subalgebras of a non-irreducible inclusion of simple unital C*-algebras is also obtained, which provides a C*-version of a finiteness result of Khoshkam and Mashood [18]. Apart from these finiteness results, comparisons between various notions of distance between subalgebras of operator algebras by Kadison–Kastler, Christensen and Mashood–Taylor are made. Further, these comparisons are used satisfactorily to provide some concrete calculations of distance between operator algebras associated to two distinct subgroups of a given discrete group

    Parental Evaluation of a Responsive Parenting Program for Infants with Hearing Loss

    Background: Parental satisfaction is an important factor in the evaluation of early intervention programs but is rarely investigated. The Muenster Parental Program (MPP) is a short, evidence-based early intervention program that focuses on parental responsiveness. It is a family-centered intervention for parents of infants aged 3–18 months who have recently been diagnosed with hearing loss and fitted with hearing devices, including prior to or following cochlear implant surgery. Objective: We aim to receive feedback from parents regarding the process and outcomes of their participation in the MPP. Method: Following their participation, all participants of the MPP were asked to complete an evaluation questionnaire. This article reports feedback from the first 52 participants (44 mothers, 7 fathers, and 1 godmother). Their infants (N = 45) had moderate to complete hearing loss, they were aged 2–20 months, and 40% of them had an additional disease, disorder, and/or developmental delay. Results: Parents reported high levels of satisfaction with the content, didactics, setting, and individual benefits of the intervention, and a high recommendation rate (92%). The aspects most appreciated were meeting other affected parents and the concrete individual support of parent–child communication, including video feedback. Almost all parents (96%) reported a change in their communication style with their child. This confirms the results of a previous controlled intervention study on the enhancement of parental responsiveness via the MPP. Conclusions: This evaluation of the MPP from a parental point of view has revealed equally high satisfaction with the content, setting, and didactics amongst all parents regardless of any potentially influential parent or child variables. The MPP is well suited to a wide range of close caregivers’ needs despite the known diversity of children with hearing loss and their parents or families

    Retrospektive Analyse zum Einfluss eines Mismatch der Minorhistokompatibilitätsantigene HA-1 und HA-2 auf die Therapieergebnisse nach allogener Stammzelltransplantation bei akuter myeloischer Leukämie

    Allogene hämatopoetische Stammzelltransplantationen (aHSZT) stellen einen entscheidenden Therapieansatz bei akuter myeloischer Leukämie (AML) dar. Durch den Spender-gegen-Leukämie-Effekt (GVL) gelingt eine Senkung der Rezidivrate. Dennoch zeigt sich das Gesamtüberleben unverändert durch das Auftreten akuter und chronischer Spender-gegen-Empfänger-Erkrankungen (GVHD) und begleitender Immunsuppression. Ziel der Forschung an den hämatopoetischen Minorhistokompatibilitätsantigenen HA-1 und HA-2 ist eine Trennung von GVL und GVHD, um die Langzeitergebnisse nach aHSZT zu verbessern. In dieser retrospektiven Studie an 284 Spender-Empfänger-Paaren, die zwischen 2013 und 2017 eine aHSZT bei AML erhielten, konnte eine deutlich niedrigere Rezidivrate bei immunrelevantem HA-1-/HA-2-Mismatch (p=0.051) als Hinweis für einen gesteigerten GVL-Effekt bei gleichbleibender GVHD gezeigt werden. Ein signifikanter Einfluss auf das Gesamt- und das Rezidiv-freie Überleben konnte nicht nachgewiesen werden

    Tolerance Induction in Vascularized Composite Allotransplantation—A Brief Review of Preclinical Models

    Pre-clinical studies are an obligatory tool to develop and translate novel therapeutic strategies into clinical practice. Acute and chronic rejection mediated by the recipient’s immune system remains an important limiting factor for the (long-term) survival of vascularized composite allografts (VCA). Furthermore, high intensity immunosuppressive (IS) protocols are needed to mitigate the immediate and long-term effects of rejection. These IS regiments can have significant side-effects such as predisposing transplant recipients to infections, organ dysfunction and malignancies. To overcome these problems, tolerance induction has been proposed as one strategy to reduce the intensity of IS protocols and to thereby mitigate long-term effects of allograft rejection. In this review article, we provide an overview about animal models and strategies that have been used to induce tolerance. The induction of donor-specific tolerance was achieved in preclinical animal models and clinical translation may help improve short and long-term outcomes in VCAs in the future

    Knotless Suture Anchors Display Favorable Elongation but an Inferior Ultimate Failure Load Versus Titanium Suture Anchors and All-Suture Anchors: A Biomechanical Comparison in a Porcine Model

    Background: Several types of suture anchors, which differ in their working principles, are available for fixation of ligamentous structures in knee surgery. How the choice of a suture anchor type influences the biomechanical stability of ligament fixation is largely unknown. Purpose: To compare the biomechanical properties of different suture anchor designs regarding primary stability for tendon fixation and repair in medial collateral ligament (MCL) surgery. Study Design: Controlled laboratory study. Methods: The primary stability of MCL fixation was assessed in a porcine model utilizing 1 of 3 suture anchor types: a 5.5-mm titanium suture anchor (TSA), a 2.8-mm all-suture anchor (ASA), or a 5.5-mm polyether ether ketone knotless suture anchor (KLSA). Primary stability was assessed using a uniaxial material testing machine. Cyclic loading at 50 N and 100 N was applied for 500 cycles each, followed by a load-to-failure test. Results: After 500 cycles at 50 N, the KLSA (2.4 ± 0.3 mm) showed significantly (P < .05) reduced elongation in comparison to the TSA (4.0 ± 0.9 mm) and ASA (3.6 ± 0.7 mm), and after 500 cycles at 100 N, the KLSA (6.5 ± 1.4 mm) again showed significantly (P < .05) reduced elongation in comparison to the TSA (11.0 ± 2.2 mm) and ASA (12.0 ± 3.6 mm). However, the KLSA (213 ± 27 N) showed a significantly (P < .05) inferior ultimate failure load in comparison to the TSA (300 ± 20 N) and ASA (348 ± 23 N). In comparison to the TSA (113.0 ± 11.0 N/mm) and ASA (113.6 ± 14.4 N/mm), the KLSA (150.7 ± 11.6 N/mm) displayed the highest stiffness (P < .05). No significant differences were observed regarding yield load. Conclusion: KLSAs displayed significantly reduced elongation, at the cost of a reduced ultimate failure load, in comparison to TSAs and ASAs. Clinical Relevance: Surgeons should be aware that differences exist between different suture anchor types regarding their biomechanical stability. KLSAs may be favorable for fixation of peripheral ligaments in knee surgery

    Bereits 2002 wurden bei einer BWL-Klausur zwei PISA-Aufgaben gestellt und die Rechtschreibung überprüft, was wiederum mit den Noten bzw. Punkten der Klausur korreliert wurde. Es gibt interessante Ergebnisse und Zusammenhänge, die jedoch nicht völlig eindeutig sind.Already in 2002, a written examination in business administration included two PISA tasks and a check of orthography, which in turn were correlated with the grades or points of the examination. There are interesting results and relationships. However, they are not completely unambiguous

    Unterrichtswahrnehmung von (angehenden) Biologielehrpersonen: Umgang mit Schülervorstellungen im Evolutionsunterricht

    Die Studie untersucht die Unterrichtswahrnehmung von Studierenden, Referendar*innen und Lehrpersonen mit dem Unterrichtsfach Biologie, indem die soziale Praktik des Sprechens über den Umgang mit anthropomorphen und teleologischen Schülervorstellungen im Evolutionsunterricht analysiert wird. Zur Datenerhebung wurde eine Videovignette als Impuls für 31 Gruppendiskussionen und 9 Einzelinterviews mit insgesamt 115 (angehenden) Biologielehrpersonen eingesetzt. Die Datenauswertung erfolgt mit der Dokumentarischen Methode, da diese auf die Analyse sozialer Praktiken ausgerichtet ist und die Unterscheidung zwischen explizitem und implizitem Wissen ermöglicht. Ein Subsample von 15 kontrastreichen Fällen wurde vollständig interpretiert und zur Typenbildung herangezogen. Die kennzeichnende Gemeinsamkeit aller 15 Fälle ist der rekonstruierte Bewertungsmodus. Unterschiede zeigen sich insbesondere in der Bedeutung die den Vorstellungen der Schüler*innen für den Unterricht beigemessen wird, sowie im Verständnis der zugrunde liegenden Lehr- und Lernprozesse

    Molecular dynamics simulations of polymer electrolytes in the bulk and at interfaces: methods and atomistic insights

    Polymerelektrolyte haben das Potenzial, die Sicherheit heutiger Lithium-Ionen-Batterien zu erhöhen und neue Zellchemien mit höheren Energiedichten zu ermöglichen. Diese Arbeit liefert atomistische Einblicke in die Koordination und den Transport von Lithium-Ionen in gebräuchlichen PEO-LiTFSA-Elektrolyten im Volumen und an Elektrodengrenzflächen mittels Molekulardynamik-Simulationen (PEO = Polyethylenoxid, LiTFSA = Lithiumbis(trifluormethylsulfonyl)amid). Der Einfluss unterschiedlicher Kettenlängen, Salzkonzentrationen und Elektrodenoberflächenladungen auf die (Grenzflächen-)Struktur und Dynamik der Elektrolyte wird untersucht. Darüber hinaus deckt diese Arbeit Schwachstellen und Fehlinterpretationen von zwei etablierten und weit verbreiteten Nachbearbeitungs- und Analysemethoden für Molekulardynamik-Simulationen auf, und zwar bei der Trajektorienentfaltung und bei der Bestimmung von Lebenszeiten (z.B.\ Koordinationszeiten oder Schichtverweilzeiten) auf der Basis von Autokorrelationsfunktionen. Verbesserungen und Alternativen werden diskutiert.Polymer electrolytes have the potential to increase the safety of today's lithium-ion batteries and to enable new cell chemistries with higher energy densities. This thesis provides atomistic insights into the lithium-ion coordination and transport in common PEO-LiTFSI electrolytes in the bulk and at electrode interfaces using molecular dynamics simulations (PEO = poly(ethylene oxide), LiTFSI = lithium bis(trifluoromethanesulfonyl)imide). The impact of different chain lengths, salt concentrations and electrode surface charges on the (interfacial) electrolyte structure and dynamics is explored. Moreover, this thesis unveils flaws and misinterpretation of two well-established and widely used post-processing and analysis methods for molecular dynamics simulations, namely trajectory unwrapping and lifetime estimation (e.g. of bond lifetimes or layer residence times) from autocorrelation functions. Improvements and alternative methods are discussed
