    Amoeboma- An Interesting Case Report With an Unusual Presentation

    Amebiasis is an infectious disease caused by Entamoeba Histolytica is more common in devolping countries, with various clinical manifestations and sequalae. Amoeboma , is an unusual sequelae which manifests as a tumor like mass within the colon. We present an unusual case of 5 year old child with signs of acute abdomen. A mass was found on expoloratory laparotomy attached to the small intestine. Histopathological examination of the mass revealed numerous amoebic trophozoites and the diagnosis of amoeboma was made. Our case is unusual because amoebomas presenting as an extraluminal mass attached to small intestine is extremely uncommon

    Schizophrenia, Substance Use and Aggressions: What Are The Relationships?

    Objectives: The objective of the study is to determine the prevalence of substance abuse for alcohol, cannabis, opiates, stimulants, solvent and other substances among patients with schizophrenia in Hospital Bahagia Ulu Kinta (HBUK), Perak , Central Peninsular of Malaysia. This study also aims to determine the association of substance abuse with aggression, the demographic characteristics and total duration of hospitalization. Methods: This was a retrospective cross-sectional study whereby the first 194 subjects diagnosed to have schizophrenia based on International Classification of Disease, 10th edition (ICD-10) criteria were taken from data registry of patients admitted to HBUK from January until February 2004. The subjects’ medical files were examined for documentation of substances abuse, aggression and accumulative duration of hospitalization. Results: The results showed the prevalence of substances misuse among patients with schizophrenia in general (including alcohol) was 24.7%. Cannabis 16.7%, alcohol 13.4%, opiates(heroin) 6.7%, Amphetamine type stimulants (amphetamine, metamphetamine, ecstacy) 5.7%, and other substances (benzodiazepine, solvents) 1.5%. Conclusion: There is higher prevalence of substance misuse in patients with schizophrenia as compared to general population. Male patients with history of substance misuse are more likely to have aggression than female. This group needs special precaution and probably in need of specialist help

    Varenicline – a new pharmacotherapy for smoking cessation in primary care practice

    Background: Cigarette smoking causes significant morbidity and mortality and is a major public health concern worldwide. Primary care doctors are in a unique position that enables them to promote smoking cessation, as smokers are more aware of their health at the time of their clinic visit. It is thus important to offer effective treatment to help smokers stop smoking. Methods: A search of PubMed was done up to 16 December 2009, using the keywords “varenicline” alone, “varenicline” and “family medicine”, and “varenicline” and “primary care”. The search produced 426 articles on varenicline. The articles that were chosen were case reports, meta-analyses, review articles and clinical trials published in English. Results: A new drug called varenicline has recently been introduced to assist smoking cessation. It is an α4β2 nicotinic acetylcholine receptor partial agonist. Varenicline has a unique action that relieves the cravings and withdrawal symptoms that occur during smoking abstinence, while blocking the receptor and preventing any reward from additional smoking. It has been shown to be efficacious for smoking cessation in normal smokers; however, its safety in smokers with mental health problems needs to be elucidated. Conclusion: The currently available data support the effectiveness of varenicline to treat nicotine dependence, but caution is needed in smokers with mental health problems. Thus, primary care doctors have a new pharmacological option to offer smokers who wish to stop smoking

    Recognizability of Individual Creative Style Within and Across Domains: Preliminary Studies

    It is hypothesized that creativity arises from the self-mending capacity of an internal model of the world, or worldview. The uniquely honed worldview of a creative individual results in a distinctive style that is recognizable within and across domains. It is further hypothesized that creativity is domaingeneral in the sense that there exist multiple avenues by which the distinctiveness of one’s worldview can be expressed. These hypotheses were tested using art students and creative writing students. Art students guessed significantly above chance both which painting was done by which of five famous artists, and which artwork was done by which of their peers. Similarly, creative writing students guessed significantly above chance both which passage was written by which of five famous writers, and which passage was written by which of their peers. These findings support the hypothesis that creative style is recognizable. Moreover, creative writing students guessed significantly above chance which of their peers produced particular works of art, supporting the hypothesis that creative style is recognizable not just within but across domains

    Awareness regarding female breast cancer in Kashmiri males - A study

    Breast cancer is a major killer disease in females globally and in developing regions, where the early cancer detection facilities are unavailable, prognosis is even worse. Awareness about this disease can lead to early detection and thereby decrease the morbidity and mortality. A self designed questionnaire was used to study the level of awareness regarding breast cancer among males. The questionnaire had 15 questions and on the basis on score attained, the subjects were classified as having poor, average or good breast cancer awareness. Out of 624 participants, 555(89%) had poor breast cancer awareness and 47(7.5%) had average awareness. Only 22 (3.5%) had good awareness about breast cancer. The level of awareness regarding female breast cancer in Kashmiri males is very low. Measures need to be taken to spread awareness about this disease in males so that they can play a vital role in early detection of this disease

    Comparison of the Knowledge in Core Policies of Essential Drug List Among Medical Practitioners and Medical Students in Galle, Sri Lanka

    Selection of the best and safest medicine should be one of the national economic policies in a country for good health care services. Introduction of detailed module in rational use of medicine (RUM) to pharmacology syllabus needed prior analysis of the existing knowledge among health care workers. Therefore we assessed the knowledge and attitudes of essential drug list (EDL) on medical practitioners (MPs) and medical students (MSs). Forty two MPs and 120 MSs from hospital and Faculty of Medicine were given a pretested structured questionnaire related to core policies of EDL, contents, criteria for selection and time frame for revision in RUM. Our study showed that only 29% of MPs were confident about their knowledge in EDL and 17% of them had marked it as don’t know. Study was expanded for quantitative analysis of the knowledge on the core policies of EDL on them. Knowledge on contents, criteria for selection and the time frame for revision of EDL were 63%, 83% and 17% in MPs. Of MSs, 87% had sound knowledge in core contents, 32% in criteria for selection and only 50% of MSs were aware about the correct time frame of revision of EDL. Knowledge in contents of EDL was higher in MSs (87%) than MPs(63&). MPs were not aware about EDL preparing criteria such as inclusion of generic names, common ailment, majority ailments of the people (59%, 56% and 56% respectively). In contrast, MSs had > 93% of the knowledge in all three areas. However MSs had poorer knowledge (32%) in criteria for selection of EDL than MPs (83%). Knowledge in time frame for revision of EDL was 17% in MPs and 50% in MSs. We found that MPs in the service were not convinced about their knowledge in EDL. Deficiency was significant in the core contents of the EDL preparation. Therefore we suggest that MPs need repetitive in-service training programme for practicing of RUM in the national health facilities. We need to reiterative programme in the core curriculum regarding the criteria for EDL selection. Though MSs had good knowledge in content of EDL, they are poor in criteria for selection and time frame for revision
