Struma Ovarii Associated with Pseudo- Meigs’Syndrome
Struma ovarii is a specialized ovarian teratoma composed predominantly of mature thyroid tissue. It is associated with pleural effusion and ascites (Pseudo-Meigs’ syndrome) in 5% of cases. Majorities of the strumas are benign, however occasionally malignant transformation may be seen. We report a case of a 45 years old postmenopausal woman who presented with gradually increasing dyspnoea and distention of abdomen of five months duration. USG abdomen revealed a bulky right ovary with a solid and cystic components and ascites. Her chest X-ray showed bilateral pleural effusion. Hence, clinical diagnosis of malignant ovarian tumor was kept; however, both the ascetic & pleural fluids were cytologically negative for malignant cells. The patient was operated forhysterectomy with bilateral salpingo-ophorectomy. The histopathological examination of the ovarian mass confirmed the diagnosis of struma ovarii. Postsurgical follow up of patient showed spontaneous regression of pleural effusion and ascites. The coexistence of an ovarian tumor, ascites and bilateral pleural effusion that resolves spontaneously on resection of the ovarian mass is known as pseudo-Meigs’ syndrome. Patient with pseudo-Meigs’ syndrome may present a diagnostic problem as they masquerade as carcinoma with malignant effusions. In addition, the coexistence of struma ovarii and pseudo-Meigs’ syndrome is a very rare event
Distributed Estimation Based on Prior Information
In this paper, we propose an approach for distributed estimation algorithm using Bayesian-based forward backward (FB) Kalman filter (KF) used on a stochastic singular linear system. The approach incorporates generalized versions of KF presented for cases with complete or incomplete a-priori information with bounds, followed by estimation fusion for these cases. The proposed approach is then validated on a coupled tank system to ensure its effectiveness
Adult Pure Yolk Sac Tumor of The Testis.
Adult pure yolk sac tumor (YST) is extremely rare. In childhood pure yolk sac tumors are most frequently seen, whereas component of mixed germ cell testicular tumors observed in adults. They have different biological behavior than the childhood tumors. We herein report a case of adult pure yolk sac tumor of left testis in 40 years old man. Until now limited number of the cases has been reported in the literature
A Rare Giant Pleural Thymoma in Posterior Mediastinum.
Thymoma is an epithelial neoplasm of the thymus, which commonly lies in the anterior mediastinum. Unusually, thymomas can also be found in other locations. Surgical excision, when feasible, appears to provide good results. We encountered a rare case of a thymoma that developed in the right thoracic cavity, and originating from the pleura in posterior mediastinum. Stillbirth in a Tertiary Care Referral Hospital in North Bengal - A Review of Causes, Risk Factors and Prevention Strategies
Background and Aims: Stillbirth is one of the most common adverse outcomes of pregnancy, accounting for half of all perinatal mortality. Each year approximately 4 million stillbirths are reported, with 97% occurring in developing countries. The objective of the present study was to evaluate the stillbirth rate, exploring the risk factors and causes of stillbirth and suggest policies to reduce it. Settings and Design: A retrospective study of stillbirth among all deliveries over 5 years at North Bengal Medical College, a referral tertiary care teaching hospital in a rural background. The stillbirth rate and its trend were defined and the probable causes and risk factors were identified. Results: Stillbirth rate is 59.76/1000 live births, and Perinatal Mortality 98.65/1000 births. Of the still births, 59.72% were fresh and 40.27% were macerated. Among the causes of stillbirths, poor antenatal attendance and low socioeconomic status were important; other risk factors included prematurity, PIH, birth asphyxia, poor intrapartum care including prolonged and obstructed labour. In 23% cases, the cause remained unexplained. Conclusion: In addition to poor antenatal care, low socioeconomic condition, poor referral service, suboptimal intrapartum care in health facilities including tertiary centre were mainly responsible for majority of still births which could have been prevented. We speculate that upgrading the existing health system performance, particularly high quality intrapartum care by skilled health personnel, will reduce stillbirths substantially in our institut
Increasing Trends of Methicillin Resistant Coagulase Negative Staphylococcus in Neonatal Septicaemia - A Study in a Tertiary Care Hospital, Mysore, South India
Introduction: Neonatal septicaemia is one among the leading causes of neonatal mortality in India. For the last 20 years CoNS has been identified as a major cause of neonatal septicaemia in NICU. Different studies show 70-75% of CoNS as resistant to methicillin. Material and methods: Blood samples from neonatal septicaemia cases during Dec 2007 to Dec 2010 were processed & isolates were identified. CoNS were biotyped & methicillin resistance was detected using cefoxitin 30 µg disk. Results: Of 2256 blood samples from neonatal septicaemia cases, 587(36.98%) were CoNS. Staphylococcus epidermidis 234(39.86%) was the commonest isolate. Methicillin resistance was noted in 286 (48.72%). MRCoNS prevalence during 2008, 2009 & 2010 were 41.57%, 47% and 57.36% respectively. Conclusion: Our study showed increased trends of MR-CoNS in neonatal septicaemia leading to increased usage of vancomycin & other glycopeptides, resulting in emergence of multidrug resistant strains, thus narrowing the treatment options in neonates

DNA computing is a discipline that aims at harnessing individual molecules at the nano-scopic level for computational purposes. Computation with DNA molecules possesses an inherent interest for researchers in computers and biology. Given its vast parallelism and high-density storage, DNA computing approaches are employed to solve many problems. DNA has also been explored as an excellent material and a fundamental building block for building large-scale nanostructures, constructing individual nano-mechanical devices, and performing computations. Molecular-scale autonomous programmable computers are demonstrated allowing both input and output information to be in molecular form. Knowledge based Replica Management in Data Grid Computation
As the number of scientific disciplines has increased, the large data collections are emerging as important community resources. In domains as high energy physics, and computational genomic, the volume of interesting data is already measured in terabytes and will soon total peta bytes. The Research communities need to access and analyze this data using complex computational and access techniques. No data management infrastructure provides solution against the complex computational analysis of huge and geographically distributed data sets. Most of queries usually search analyzed data from terabytes of distributed data repository, over wide area networks. Replicas, and other advanced techniques collectively maximize the use of scarce storage, networking, and computing resources. Existing data grid replication technique no doubt provide us with availability of required data sets, but in order to create replica it has to bear an overhead of huge computation for required data sets. Large network traffic cause the performance unsatisfactory. Our goal in this effort is to provide users with replication infrastructure in Grid that uses Knowledgebase having learning capability so as to reduce the computation for creating dataset on each user request
About A Rare Cause Of Primary Hyperparathyroidism
Introduction: Primary hyperparathyroïdism is observed in 35 to 44 subjects/ 100000 persons. The increased production of parathyroid hormones is secondary to primary glandular modifications consisting mainly in adenomas. The authors report a clear-cell hyperplasia causing primary hyperparathyroidism. Observation: We report the case of a 25-year-old man who was admitted to explore pathologic fractures of the left arm and a malignant hypercalcaemia. Complementary laboratory tests revealed primary hyperparathyroidism. A multiple endocrine neoplasia was excluded by radiologic examinations. Cervical ultra-sound examination revealed 2 parathyroid adenomas and per-operative exploration showed 3 « adenomas ». Microscopic examination of the 4 parathyroid glands specimen concluded to a clear cell hyperplasia. Conclusion: Clear cell hyperplasia is a benign cause of primary hyperparathyroidism. The diagnosis is based upon histologic findings and examination of the 4 glands
Culture and sensitivity pattern of micro-organism isolated from diabetic foot infections in a tertiary care hospital
Aim & Method: The current study was designed to elucidate the protective effect of Syzygium cumini seed extract (SCE) on skin carcinogenesis induced by a single topical application of 7,12-dimethylbenz(a)anthracene (100 μg/100 μl of acetone) and 2 weeks later promoted by repeated application of croton oil (1% in acetone/three times a week) till the end of the experiment (16 weeks). Result: Oral administration of SCE at a dose of 125 mg/kg b.wt./day for 15 days at the peri-initiational stage (i.e., 7 days before & 7 days after DMBA application) and for 14 weeks at the promotional stage (i.e., from the time of croton oil application), revealed a significant reduction in lipid peroxidation (p<0.05-0.001) along with an elevation in the activities of enzymatic antioxidants (superoxide dismutase, p<0.05-0.001 & catalase, p<0.05-0.001), non-enzymatic antioxidant (reduced glutathione, p<0.05-0.01 & vitamin-C, p<0.01-0.001) and total proteins levels (p<0.01-0.001) when compared to the carcinogen treated control animals. Histopathological study revealed that dyskeratosis of the epidermis, deposition of keratinous pearl and epidermal hyperplasia in skin tumors of DMBA treated control and the same were found to be of lesser degree in both the SCE treated experimental animals. Conclusions: These results demonstrate that SCE ameliorate the DMBA/croton oil induced adverse biochemical and histopathological alterations during skin carcinogenesis in mice
