    Successful Pregnancy Outcome In Maternal Crigler Najjar Syndrome Type II.

    Estimated incidence of Crigler-Najjar syndrome(CNS) is 1 case per 1,000,000 births(1 million). The overall prevalence of CN syndrome is unknown, with only several hundred people reported to have this disease. It is interestingly very rare to encounter a pregnant adult women with congenital jaundice. Pregnancy in CN type II patients is a diagnostic and a therapeutic challenge because of the high risk of bilirubin encephalopathy with serious neurological damage as life-threatening complications for the fetus. To date 8 pregnancy outcome have been reported from 5 women and we report the6 woman with a successful 9 th pregnancy outcome.     A Rare Case of Metastatic Intra-abdominal Melanoma Following Exenteration of Right Eye for Primary Choroidal Melanoma

    The most common primary intraocular malignant tumour is choroidal melanoma. Reported incidence of intraocular melanoma is less than 1 per 100,000. Liver is the most frequent site of metastasis, and involvement of the other sites generally occurs in association with liver metastasis. The present case had an unusual presentation of, rapidly refilling massive ascites and right pleural effusion, following exenteration of the right eye 4 years earlier for primary choroid melanoma. To the best of our knowledge this is the first case of intra abdominal melanotic rapidly refilling ascites following primary choroidal melanoma masquerading as an ovarian tumour. We have discussed the atypical presentation, difficulties in arriving at a diagnosis preoperatively, intraoperative dilemmas and the post operative management

    Based on formal-theoretical principles about the \ud sign processes involved, we have built synthetic experiments \ud to investigate the emergence of communication based on \ud symbols and indexes in a distributed system of sign users, \ud following theoretical constraints from C.S.Peirce theory of \ud signs, following a Synthetic Semiotics approach. In this paper, we summarize these computational experiments and results regarding associative learning processes of symbolic sign modality and cognitive conditions in an evolutionary process for the emergence of either symbol-based or index-based communication

    Technology Use in the Virtual R&D Teams

    Problem statement: \ud Although, literature proves the importance of the technology role in the effectiveness of virtual Research and Development (R&D) teams for new product development. However, the factors that make technology construct in a virtual R&D team are still ambiguous. The manager of virtual R&D teams for new product development does not know which type of technology should be used. \ud Approach: \ud To address the gap and answer the question, the study presents a set of factors that make a technology construct. The proposed construct modified by finding of the field survey (N = 240). We empirically examine the relationship between construct and its factors by employing the Structural Equation Modeling (SEM). A measurement model built base on the 19 preliminary factors that extracted from literature review. The result shows 10 factors out of 19 factors maintaining to make technology construct. \ud Results: \ud These 10 technology factors can be grouped into two constructs namely Web base communication and Web base data sharing. The findings can help new product development managers of enterprises to concentrate in the main factors for leading an effective virtual R&D team. In addition, it provides a guideline for software developers as well. Conclusion: The second and third generation technologies are now more suitable for developing new products through virtual R&D teams

    Clinical Decision Support System for Oncologist: Perception, Expectations and Implementation

    Background: Clinical Decision Support System is an integral application in modern healthcare practice to assist the clinicians with patient information and domain knowledge during patient care. Purpose: Assessment of perception and Identification of expectation are the prerequisite for the implementation of Clinical Decision Support System (CDSS). Methodology: A descriptive study was conducted among the 100 Oncologists of cancer hospitals and research centers of Southern India. A validated survey questionnaire related to the perception and expectation towards CDSS was provided to the participants and asked them to mark their response on a 5-Point Likert Scale i.e. from Strongly Agree to Strongly Disagree. The time motion study was also conducted to understand the issues related to the access of patient information and domain knowledge. Result: The Oncologist perception towards CDSS was found to be significant where the Oncologists felt that the implementation of CDSS will decrease the cost of healthcare and increase the productivity of the hospital. The Oncologists expected that the system should have the features to automatically capture the clinical knowledge, practice guidelines and updates related to cancer care. The time motion study revealed that the Oncologists spend their maximum time in documentation & retrieval of patient information during patient care and evaluation because they largely depend on printed text. A web enabled CDSS is developed, implemented, and tested against the requirement of the Oncologists where the system was found to be highly acceptable by the Oncologists. Conclusion: The expectations of the end users should be fulfilled to make the CDSS more acceptable and sustainable as the similar result had been observed and found in the present research

    Adenoid cystic carcinoma of the trachea is a rare primary tracheal malignancy. Obstructive symptoms such as dyspnoea, hoarseness of voice, dysphasia are commonly seen symptoms. Combined modality treatments including surgery and radiation therapy are considered as optimal treatment for these tumours. A case of adenoid cystic carcinoma in a 35 years old male patient who was treated successfully by surgical excision and external beam radiation therapy is presented

    Frequency of Parental Psychopathology in Children with Attention Deficit Hyperactivity Disorder

    Parental mental health concerns place children at a significantly greater risk of lower social, psychological and physical health than children in families not affected by mental illness. Several studies have examined the extent to which psychopathology in children are closely related to parental substance use disorders or to other mental disorders. This study intended to investigate the frequency of occurrence of parental psychopathology in children with Attention Deficit/Hyperactivity Disorder and compare the characteristics of children with and without parental psychopathology. Fifty children between 7-14 years of either sex fulfilling the ICD-10 criteria for ADHD comprised the sample of this study.It would be a chart review of in-patients and out-patients who have consulted at Kasturba Hospital, Department of Psychiatry, Manipal over a two year period. Parents diagnosed with Substance Use Disorders (SUD) and other psychiatric morbidity according to ICD-10 was compared with those parents without any psychiatric illness on various Sociodemographic, clinical characteristics. Most of the parents were in the second or third decade of their life. Of the fifty patients 41(82%) were males and 9 (18%) were females. In 36(87.0 %) of the fathers had a history of substance use disorder and 5(13.0 %) it was absent. Other morbidities were also present in both parents. The findings are discussed in light of the implications

The article aims to introduce the subject to medical fraternity in India and recommends that the theory should be included in the syllabus of post graduate students of community medicine so that future generations of public health experts are aware of the theory and its potential use to find tangible solutions to public health issues

    Human quadrupedalism is not an epiphenomenon caused\ud by neurodevelopmental malformation and ataxia

    Two cases with quadrupedal locomotion (QL) were presented. In both cases, cognitive and psychiatric functions were normal and, no neurological deficits were observed, except for a sequel paralysis of left leg in Case 2. It was suggested that human QL (1) should not be considered as an epiphenomenon caused by neurodevelopmental malformation and\ud ataxia, but (2) may be considered as a re-emergence of the ancestral diagonal QL, and (3) it may spontaneously emerge in humans with entirely normal brains, by taking advantage\ud of neural networks such as central pattern generators that have been preserved for about 400 million years

    Effective Virtual Teams for New Product Development

    At present, the existing literature shows that the factors which influence the effectiveness of virtual teams for new product development are still ambiguous. To address this problem, a research design was developed, which includes detailed literature review, preliminary model and field survey. From literature review, the factors which influence the effectiveness of virtual teams are identified and these factors are modified using a field survey. The relationship between knowledge workers (people), process and technology in virtual teams is explored in this study. The results of the study suggest that technology and process are tightly correlated and need to be considered early in virtual teams. The use of software as a service, web solution, report generator and tracking system should be incorporated for effectiveness virtual teams
