    Accidental Trismus Leading to Failed Endotracheal Intubation and Abandoned Surgery Following Rapid Sequential Induction of General Anesthesia: A Case Report

    A 64-year-old female patient, weighing 46 kg, 156 cm tall, and BMI of 18.9 kg/m², was scheduled to have the left tibial intramedullary needle under general anesthesia. General anesthesia induction was performed sequentially using sufentanyl 15 μg, propofol 120 mg, and cis-atracurium 10 mg intravenously. 2.5 minutes after good mask administration of oxygen by artificial ventilation, the anesthesiologist planned to intubate the trachea and found that her mandible and jaw joint were fixed, the masseter muscle was stiff, and the mouth could not open. Propofol 50 mg was added to deepen the anesthesia, which remained so. The surgery was cancelled. After waking up from anesthesia, the patient recovered her spontaneous mouth opening activity with a maximum mouth opening of 3.3 cm. During this period, the patient\u27s nasopharyngeal body temperature monitoring was unchanged. In recent years, there have been a few cases of masseter muscle spasm (MMS) induced by general anesthesia, and the mechanism of which is still unclear. This sudden MMS after general anesthesia induction led to the anesthesiologist’s mental tension and failure of endotracheal intubation. In such an emergency, if the anesthesiologist\u27s skills and associated equipment are inadequate, the patient should be decisively awakened from anesthesia and the abandoned operation

    The role of RNA 2′-O-methylation in prostate cancer

    2′-O-methylation (Nm) represents a pervasive RNA modification occurring in multiple RNA species, including messenger RNA (mRNA), ribosomal RNA (rRNA), transfer RNA (tRNA), microRNA (miRNA), and PIWI-interacting RNA (piRNA). This chemical mark exerts critical regulatory functions in RNA stability, transcriptional control, and translational efficiency. Recent advances in high-throughput sequencing technologies have enabled transcriptome-wide mapping of Nm sites, uncovering increasing evidence that aberrant Nm modifications contribute to oncogenesis and tumor progression. Prostate cancer, the second most commonly diagnosed malignancy in men in the United States, has emerged as a disease context in which dysregulated Nm-related pathways are of particular significance. As sequencing-based investigations continue to expand, delineating the activities of Nm-modifying enzymes across distinct RNA classes in prostate cancer is anticipated to provide mechanistic insights into disease biology, facilitate the discovery of novel therapeutic targets, and ultimately guide the development of Nm-centered anticancer strategies

    Faculty Perspectives: Teaching in a Competency-Based Education Program

    Competency-based education shifts the focus from teacher-led instruction to student-centered learning, where faculty take on the role of facilitator of learning. While a fair amount of literature exists around the administrative decision-making process and student experiences in competency-based education, there is little in the way of research surrounding the faculty experience. This study sought to better understand how faculty in higher education perceive that they are best supported during transition to a competency-based education environment. Developing a richer understanding of faculty perspectives via cross-case synthesis provides insights for those in higher education to understand how best to support faculty as they transition to competency-based education. Understanding and operationally adjusting to this new perspective of teaching and learning requires a shift not only in the day-to-day functions of a faculty member, but also to basic assumptions of what it is to be a faculty member in higher education. Faculty perceive that they are best supported in a transition to a competency-based education environment by legitimizing the value of competency-based education, supporting scholarly activities, providing ongoing professional learning specific to instructional strategies and assessment strategies that support student-centered learning, collaboration that extends beyond competency-based education programs, and ongoing administrative support

    State Policy and Competency-Based Education: A Practitioner-Based Case Study

    Despite increased attention to competency-based education (CBE), research from a state policy perspective remains limited. This is problematic considering state policy can serve as either an incentive or barrier to implementing high-quality CBE. Using a practitioner-based case study design, this study analyzes the South Dakota Board of Technical Education\u27s (SDBOTE) development of three system-wide CBE policies: credit hour equivalency, academic calendar and term structure, and tuition and fee model. For each policy, the authors outline the options considered and describe the decision made. Two of the three authors served as the primary project managers for the SDBOTE\u27s CBE policy development, which provides first-hand knowledge into the considerations and decision-making processes. Across the three policies examined, a consistent tension between supporting educational innovation and working within existing structures is identified. Three themes that emerged from SDBOTE’s approach to managing this tension and their implications for policy development in state higher education systems are explored: evaluating capacity, identifying constraints, making intentional compromises. This study provides one example of how state higher education systems can develop system-wide CBE policies and has implications for state policymakers, state higher education system administrators, and policy researchers that are considering and evaluating CBE implementation

    Standardized protocols for collecting data on bee-flower interactions and the associated floral community

    Pollen and nectar from flowers constitute the primary food resources of bees, inextricably linking bee and flowering plant communities in space and time. Thus, our understanding of bee biology and distribution can be greatly enhanced by documenting interactions between bees and their host plant species. Plant-pollinator interaction data are routinely collected in studies with diverse research goals, but the lack of standardization in data collection has limited our ability to integrate datasets and address outstanding questions in bee ecology, conservation, and pollination biology. Here, we provide standardized protocols for (A) documenting plant-pollinator interactions and (B) quantifying associated floral resources available to foraging bees. These protocols can be combined for a more detailed understanding of plant-pollinator interactions and can be applied in inventories, surveys, and monitoring programs of bees. We also provide case studies demonstrating their application. We discuss tradeoffs that are inevitable in any methodological approach, including the use of lethal versus non-lethal sampling approaches, and highlight the need to prioritize rigorous testing of the scalability and generalizability of current methodologies. These protocols are part of a series developed in association with the U.S. National Native Bee Monitoring Network to standardize bee monitoring practices
