    HIV-treatment adherence: Exploring experiences of peer counsellors and young-adult clients in HIV Care centres in Eswatini

    Achieving viral load suppression is a persistent challenge amongst young people in HIV care in Southern Africa, Eswatini in particular. Research has shown that treatment-adherence counselling has been central in the global response to HIV and is tipped as central in adherence support to increase treatment efficacy as well as help prevent drug resistance. The Ministry of Health introduced a National Expert Client programme in 2007 to shift tasks including Pre-ART counselling and preparation for ART for newly identified clients. Since then, the scope of expert clients has expanded to include ongoing counselling for clients in care to enhance adherence to care and treatment. This study aimed at exploring experiences of peer-counsellors and young-adult clients on HIV-treatment adherence counselling in one HIV care centre in Eswatini. Data were collected over a period of 3 months and involved 6 peer counsellors who worked in the facility for at least 6 months, and 12 young-adult clients (15-24 years of age) who had at least 6 months in HIV care and treatment. This study used a combination of in-depth interviews and observation methodological approaches. Collected data was transcribed, translated and analysed using systematic text condensation. The main findings of the study include how participants viewed living with HIV as burden; How dedicated social support played a role in treatment support and adherence as well as stigma both experienced and anticipated. From these, we discussed two dimensions of burden that generally were expressed from the findings which included living with HIV and the need to take ART daily to achieve optimum health. The findings also demonstrated that stigma remains a huge threat to access and actual retention in adherence, care and treatment for clients and counsellors alike in the facility and community. Young-adults accessing care did not feel as part of the decision-making process about their care but instead were dictated to and had to conform to healthcare workers' orders.     Pediatric Diabetes Care in Norway : Glycemic Results and Health-Related Quality of Life

    Bakgrunn I Norge har det blitt gjort en rekke tiltak for å forbedre pediatriske diabetesomsorgen og for å følge opp barn med diabetes på best mulig måte. I tillegg til å ta tidlig i bruk ny diabetesteknologi, har norsk barne-diabetesomsorg også fokusert på standardisert pasientopplæring fra starten av diabetessykdommen, nøyaktig insulindoseberegning, systematisk kvalitetsforbedring og klart definerte behandlingsmål. Alle barn og ungdom med type 1 diabetes i Norge mottar rutinemessig diabetesomsorg fra diagnosetidspunktet til 18 års alder ved barneavdelingene i det norske offentlige helsevesenet. Dermed kan barneavdelingene levere omfattende data, inkludert relevante behandlingsresultater, til de barna og ungdommene med diabetes som følges opp til Norsk Barnediabetesregister (NCDR). Datakomplettheten for disse er på ≥98%. Mål De overordnede målene med denne avhandlingen var å studere glykemisk status, det vil si glykosylert hemoglobin A1c (HbA1c), og forekomst av diabetisk ketoacidose (DKA) og alvorlig hypoglykemi (SH), samt diabetes-spesifikk helserelatert livskvalitet (HRQOL) hos barn og ungdom med type 1 diabetes i Norge. Vi ønsket også å identifisere faktorer som har vært assosiert med bedre glykemisk kontroll og diabetes-spesifikk HRQOL de siste ti år. Materiale og metoder Denne avhandlingen er basert på data samlet inn for NCDR fra 2013 til 2022, inkludert 27 214 årlige registreringer fra 6 775 barn og ungdom (0-17 år) med type 1. I tillegg brukte vi resultatene fra en undersøkelse om diabetes-spesifikk HRQOL gjennomført som en del av den årlige NCDR datainnsamlingen i 2017. Artikkel I var basert på 2017-kohorten (N=2 623) i en tverrsnittsstudie. Vi brukte lineære blandet-effekt regresjonsanalyser for å vurdere sammenhengen mellom bruk av diabetesteknologi og HbA1c. Logistiske regresjonsanalyser ble brukt for å vurdere sammenhengen mellom bruk av diabetesteknologi og forekomsten av DKA og SH. Artikkel II var en tverrsnittsstudie basert på DISABKIDS -spørreskjemaets diabetes-spesifikke modul (DDM-10). Barn som var 10 år og eldre, og foreldre til alle barn uavhengig av alder, ble invitert til å fylle ut spørreskjemaet som en del av NCDRs årlige datainnsamling i 2017. Av 2 059 barn som var 10 år eller eldre, fullførte 1 059 barn (50%) spørreskjemaet. Av de 2 725 foreldrene deltok 1 441 (53%). Vi analyserte mulige sammenhenger mellom diabetes-spesifikk HRQOL og HbA1c, alder, kjønn, diabetesvarighet og bruk av diabetesteknologi. I artikkel III analyserte vi longitudinelle registerdata fra 2013 til 2022. Ved hjelp av lineære blandet-effekt regresjonsanalyser vurderte vi longitudinelle sammenhenger med tanke på glykemiske utfall (HbA1c, DKA og SH), og bruk av diabetesteknologi og karbohydratberegning for måltidsbolusberegning. Vi gjorde dette for gutter og jenter hver for seg. Videre sammenlignet vi de glykemiske resultatene fra de 9 barneavdelingene i NCDR som deltok i et nasjonalt kvalitetsforbedringsprosjekt (IQ Norway) fra 2017 til 2019 med de glykemiske resultatene fra de 17 ikke-deltakende avdelingene. Resultater I 2017 brukte 75% av barna i NCDR en insulinpumpe, og 53% brukte kontinuerlig glukoseovervåking (CGM). Tverrsnittsanalyser av 2017 NCDR-kohorten viste at bruk av insulinpumpe var assosiert med høyere HbA1c sammenlignet med bruk av insulinpenn, mens bruk av CGM var assosiert med lavere HbA1c sammenlignet med ikke å bruke CGM. DKA og SH var ikke assosiert med bruk av insulinpumpe eller CGM. Imidlertid var bruk av patch-pumper assosiert med en betydelig lavere odds ratio for DKA sammenlignet med andre pumpetyper. Diabetes-spesifikk HRQOL hos barn med type 1 diabetes generelt var høy. Foreldre vurderte diabetes-spesifikk HRQOL som lavere enn barna selv gjorde. Det var ingen tverrsnittssammenheng mellom diabetes-spesifikk HRQOL og bruk av diabetesteknologi. Høyere diabetes-spesifikk HRQOL var assosiert med lavere HbA1c og det å være gutt hos barn som var 10 år og eldre, men ikke hos yngre barn. Fra 2013 til 2022 sank gjennomsnittlig HbA1c hos barn og ungdom med type 1 diabetes i Norge jevnt fra 66,6 mmol/mol til 55,4 mmol/mol. Andelen barn som nådde en HbA1c under 53,0 mmol/mol økte fra 13% til 43%. Forekomsten av akutte komplikasjoner falt fra 4,8% til 1,0% (DKA) og 4,9% til 2,1% (SH). Bruken av diabetesteknologi økte kontinuerlig over observasjonsperioden, slik at 91% av barna brukte insulinpumpe og 97% brukte CGM i 2022. Bruk av insulinpumpe, CGM og karbohydratberegning var assosiert med bedre glykemiske resultater i denne longitudinelle studien. De 9 barneavdelingene som deltok i kvalitetsforbedringsprosjektet IQ Norway forbedret sitt gjennomsnittlige institusjonelle HbA1c mer enn de 17 ikke-deltakende avdelingene. Denne forskjellen i HbA1c forble stabil over de neste tre årene etter at dette kvalitetsforbedringsprosjektet va avsluttet. Ved alle årlige registreringer var det å være jente assosiert med høyere HbA1c i 10-årsperioden. Vi fant uventede forskjeller mellom gutter og jenter når det gjelder HbA1c, bruk av diabetesteknologi og karbohydratberegning. Konklusjoner og konsekvenser Denne avhandlingen identifiserte viktige faktorer som sannsynligvis bidro til den betydelige forbedringen av glykemiske resultater i den norske barnepopulasjonen fra 2013 til 2022. Vi viste kjønnsforskjeller når det gjelder HbA1c og diabetes-spesifikk HRQOL som taler for en mer personlig og kjønnstilpasset tilnærming for gutter og jenter i diabetesomsorgen. Bruk av diabetesteknologi var ikke assosiert med bedre diabetes-spesifikk HRQOL i 2017, men på grunn av den enorme utviklingen i diabetesteknologi siden 2017, kan man muligens forvente en mer positiv sammenheng mellom slik teknologi og HRQOL i framtiden. Som vist i denne avhandlingen var NCDR svært nyttig i arbeidet med å identifisere faktorer som bidrar til forbedret diabetesomsorg og behandlingsresultater over tid på nasjonalt nivå. Resultatene av denne avhandlingen kan være relevante og virke inspirerende for fremtidig utvikling av barne-diabetesomsorg i Norge, og de kan virke veiledende i andre populasjoner, hvor man har mål om å bedre glykemiske resultater og HRQOL. For dette formålet kan det å etablere et diabetesregister for å overvåke behandlingsresultatmålene og livskvaliteten, sammenligne barneavdelingene og identifisere faktorer som bidrar til forbedring av diabetesomsorgen på nasjonalt eller lokalt nivå, i kombinasjon med universell tilgang til diabetesteknologi og spesialisert diabetesomsorg, være viktige hjørnesteiner.Background In Norway, a variety of efforts have been made to improve pediatric diabetes care and to support children with diabetes in the best possible way. Aside from the early adoption of new diabetes technology, Norwegian pediatric diabetes care has also focused on standardized patient education starting from the onset of diabetes, precise insulin dose calculation, systematic quality improvement and clearly defined treatment targets. Children and adolescents with type 1 diabetes in Norway routinely receive diabetes care from diagnosis and until the age of 18 years solely at the pediatric departments within the Norwegian public health care system. Thus, the pediatric departments annually deliver comprehensive data, including relevant treatment outcomes, of the children and adolescents with diabetes under follow-up to the Norwegian Childhood Diabetes Registry (NCDR), with a data completeness of ≥98%. Aims The overall aims of this thesis were to study the glycemic status, characterized by glycated hemoglobin A1c (HbA1c), and the incidence of diabetic ketoacidosis (DKA) and severe hypoglycemia (SH), and the diabetes-specific health-related quality of life (HRQOL) of children and adolescents with type 1 diabetes in Norway. We also aimed to identify factors associated with better glycemic control and diabetes-specific HRQOL in the last decade. Material and methods This thesis is based on data collected for the NCDR from 2013 to 2022, including 27,214 annual registrations of 6,775 children and adolescents (0-17 years) with type 1 diabetes and HbA1c levels analyzed at the same DCCT reference laboratory. In addition, we used the results of a survey on diabetes-specific HRQOL conducted as part of the 2017 annual NCDR data collection. In Paper I, we included the 2017 NCDR cohort (N=2,623) in a cross-sectional study and used linear mixed-effects regression analysis to assess the association of diabetes technology use and HbA1c. Logistic regression analysis was used to assess the association between diabetes technology use and the incidence of DKA and SH. Paper II was a cross-sectional study based on the DISABKIDS diabetes-specific module (DDM 10). Children 10 years and older, and parents of all children regardless their age were invited to complete the questionnaire as part of the NCDR’s 2017 annual data collection. Of 2,059 children 10 years or older, 1,059 children (50%) completed the questionnaire. Of the 2,725 parents, 1,441 (53%) participated. We analyzed potential associations between diabetes-specific HRQOL and HbA1c, age, sex, diabetes duration and the use of diabetes technology. For Paper III, we analyzed longitudinal registry data from 2013 to 2022. Using linear mixed-effects regression analysis, we assessed longitudinal associations of glycemic outcomes (HbA1c, DKA and SH), and the use of diabetes technology and carbohydrate counting for meal bolus calculation, separately for boys and girls. Further, we compared the glycemic outcomes of the 9 pediatric departments in the NCDR that participated in a national quality improvement project (IQ Norway) from 2017 to 2019 with the glycemic outcomes of the 17 nonparticipating departments. Results In 2017, 75% of children in the NCDR used an insulin pump, and 53% used continuous glucose monitoring (CGM). Cross-sectional analyses of the 2017 annual NCDR cohort showed that insulin pump use was associated with a higher HbA1c compared to insulin pen use, whereas CGM use was associated with a lower HbA1c compared to not using CGM. DKA and SH were not associated with insulin pump or CGM use. However, the use of patch pumps was associated with a notably lower odds ratio for DKA compared to other pump types. Diabetes-specific HRQOL in children with type 1 diabetes was generally high. Parents assessed diabetes specific HRQOL as lower than the children themselves. There was no cross-sectional association between diabetes-specific HRQOL and the use of diabetes technology. Higher diabetes-specific HRQOL was associated with lower HbA1c and male sex in children 10 years and older, but not in younger children. From 2013 to 2022, mean HbA1c in children and adolescents with type 1 diabetes in Norway decreased steadily from 66.6 mmol/mol to 55.4 mmol/mol. The proportion of children reaching an HbA1c of under 53.0 mmol/mol increased from 13% to 43%. The incidence of acute complications declined from 4.8% to 1.0% (DKA) and 4.9% to 2.1% (SH). The use of diabetes technology increased continuously over the study period, resulting in 91% of children using an insulin pump and 97% using CGM in 2022. Insulin pump and CGM use as well as carbohydrate counting were associated with better glycemic results in this longitudinal study. The 9 pediatric departments that participated in the quality improvement project enhanced their average institutional HbA1c more than the 17 nonparticipating departments. This difference in HbA1c remained stable over the following three years after the completion of ‘IQ Norway’. At all annual registrations, female sex was associated with higher HbA1c during this 10-year period. We found unexpected differences between boys and girls regarding HbA1c, the use of diabetes technology and carbohydrate counting. Conclusions and consequences This thesis identified important factors that likely contributed to the considerably improving glycemic outcomes seen in the Norwegian pediatric population from 2013 to 2022. We showed sex specific differences regarding HbA1c and diabetes-specific HRQOL that advocate for a more personalized approach for boys and girls in diabetes care. Diabetes technology use was not associated with better diabetes-specific HRQOL in 2017, but due to the enormous development in diabetes technology since 2017, the potential association of such technology with HRQOL may be more positive in future assessments. As shown in the present thesis, the NCDR was very useful in the identification of factors contributing to improved diabetes care and monitoring of glycemic outcomes over time at a national level. The results of this thesis can be inspiring and relevant for the future development of pediatric diabetes care in Norway and guide health care efforts in other populations, aiming for better glycemic results and HRQOL. For this purpose, the establishment of a diabetes registry to monitor treatment outcome measures and HRQOL, benchmark the pediatric departments, and identify factors that contribute to the improvement of diabetes care at a national or local level, in combination with universal access to diabetes technology and specialized diabetes care, might be important cornerstones.Doktorgradsavhandlin

    Carboxyl ester lipase in pancreatic disease : In vitro and in vivo studies of human pathogenic CEL mutations

    Karboksylester-lipase (CEL) er et fordøyelsesenzym som hovedsakelig er uttrykt i bukspytt-kjertelen. Det siste eksonet i CEL-genet inneholder et variabelt antall tandem repetisjoner (VNTR), altså en region av repeterte DNA-sekvenser. To sykdomsfremkallende CEL-varianter, kjent som CEL-MODY og CEL-HYB1, har begge en endret VNTR-region sammenlignet med det normale genet. CEL-MODY inneholder en enkelt-base delesjon som gir et skift i leseramme, mens i CEL-HYB1 stammer VNTR-regionen fra pseudogenet CELP. CEL-MODY-varianten forårsaker den arvelige sykdommen MODY8, som gir både diabetes og eksokrin dysfunksjon i bukspyttkjertelen. CEL-HYB1-varianten er en risikofaktor for kronisk pankreatitt, og er fem ganger overrepresentert i europeiske kohorter med pankreatitt-pasienter. CEL-MODY og CEL-HYB1 virker mest sannsynlig gjennom den såkalte misfolding-avhengige mekanismen for pankreatitt. Det overordnete målet med denne avhandlingen var å få en bedre forståelse av hvordan patogene CEL-varianter forårsaker sykdom i bukspyttkjertelen. I Artikkel Ⅰ brukte vi HEK293 celler for teste om noen enkelt-base delesjoner i VNTR-regionen er mer patogene enn andre. Celler som uttrykte varianter som ligger fremst i regionen viste nedsatt sekresjon, økt protein-aggregering og endoplasmatisk retikulum stress, trolig på grunn av et økt antall cysteiner og samtidig redusert O-glykosylering i VNTR´en. I Artikkel Ⅱ laget vi en ny modell for kronisk pankreatitt ved å sette VNTR-regionen fra CEL-MODY inn i mus. Musene utviklet spontan kronisk pankreatitt, med kjennetegn som atrofi, inflammasjon, fibrose og fettvev i bukspyttkjertelen. Musene ble ikke diabetiske, selv når de ble gjort homozygote for CEL-MODY eller ble satt på høy-fett diett. I Artikkel Ⅲ laget vi transgene mus for risiko-allelet CEL-HYB1 etter samme strategi som for CEL-MODY. Også Cel-HYB1-musene utviklet spontan kronisk pankreatitt, men med en mildere fenotype enn Cel-MODY. I tillegg observerte vi nedsatt glukosetoleranse for 12-måneder gamle Cel-HYB1 mus. I Artikkel Ⅳ screenet vi en norsk kohort med kronisk pankreatitt-pasienter for å finne bærere av CEL-HYB1. To av 154 pasienter (1.3%) var positive, og vi beskrev disse pasientene klinisk og kartla familien deres. Vi fant at CEL-HYB1 mest sannsynlig forårsaker pankreatitt i kombinasjon med andre risikofaktorer som røyking og pankreas divisium. Oppsummert har vi gjennom studier på celler, mus og pasienter fått ny og bedre forståelse for sykdomsmekanismene bak CEL-MODY og CEL-HYB1. Vi har lykkes i å lage helt nye musemodeller for kronisk pankreatitt. Disse gjenspeiler den eksokrine fenotypen knyttet til de to sykdomsvariantene, men vi avdekket ikke den karakteristiske endokrine dysfunksjonen for MODY8.Carboxyl ester lipase (CEL) is a digestive enzyme mainly expressed in the pancreas. The last exon of the CEL gene includes a variable number of tandem repeats (VNTR), i.e., a region of repeated DNA sequences. Two pathogenic CEL variants, denoted CEL-MODY and CEL-HYB1, both have altered VNTR regions compared with the normal gene. CEL-MODY contains a single-base deletion that changes the reading frame of the VNTR sequence, while the VNTR of CEL-HYB1 stems from the pseudogene CELP. The CEL-MODY variant causes the inherited disease MODY8, which is characterised by both diabetes and exocrine pancreatic dysfunction. The CEL-HYB1 variant is a risk factor for chronic pancreatitis, with a 5-fold overrepresentation in European materials of pancreatitis patients. CEL-MODY and CEL-HYB1 are considered to work through the so-called misfolding-dependent pathway of pancreatitis. The overall aim of this thesis was to gain better knowledge about how pathogenic CEL variants cause pancreatic disease. In Paper Ⅰ, we used HEK293 cells to test if some single-base deletions of the VNTR are more pathogenic than others. Cells expressing the proximal variants of this region displayed reduced secretion, increased protein aggregation and endoplasmic reticulum stress, attributed to both an increased number of cysteine residues and decreased O-glycosylation within the VNTR domain. In Paper Ⅱ, we created a novel model for chronic pancreatitis by inserting the VNTR from CEL-MODY in mouse. The mice developed spontaneous chronic pancreatitis, with hallmarks like atrophy, inflammation, fibrosis and fatty replacement of exocrine tissue. The mice did not become diabetic, even when made homozygous for CEL-MODY or exposed to a high-fat diet. In Paper Ⅲ, we generated transgenic mice carrying the CEL-HYB1 risk allele, using the same strategy as for CEL-MODY. The Cel-HYB1 mice, too, developed spontaneous chronic pancreatitis, although with a somewhat milder phenotype. In addition, we noted impaired glucose tolerance in Cel-HYB1 mice aged 12 months. In Paper Ⅳ, we screened a Norwegian cohort of chronic pancreatitis patients to identify carriers of CEL-HYB1. Two of 154 screened cases (1.3%) were positive. We described these patients clinically and mapped their families. We found that CEL-HYB1 most likely causes pancreatitis when acting in combination with other risk factors, such as smoking and pancreas divisium. In summary, through cellular, animal, and clinical studies, we have gained important new insight into the disease mechanisms involving CEL-MODY and CEL-HYB1. We have successfully created two fundamentally new mouse models for chronic pancreatitis. They recapitulate the exocrine phenotype associated with the two variants, although we did not uncover the endocrine dysfunction which is characteristic of MODY8.Doktorgradsavhandlin

    The Role of Ocean Heat Content on the Madden–Julian Oscillation

    Det overordnede målet med denne avhandlingen er å forstå hvilken rolle varmeinnholdet i den øvre delen av havet og havdynamikken rundt ekvator spiller for Madden-Julian svingningen (MJO). Mens havets respons til atmosfæren på tidsskalaer kortere enn en sesong har vært gjenstand for omfattende studier, har påvirkningen av havets varmeinnhold på MJO ikke vært systematisk undersøkt. Nylig har det blitt forsket på samspillet mellom havdynamikk, varmeinnhold i havet og MJO i Indiahavet. I Indiahavet er synkronisering mellom bølger i havet langs ekvator og MJO mulig, delvis på grunn av den romlige skalaen, forplantningshastigheten til ekvatoriale bølger og tidsskalaen til MJO-variabilitet. Det er i konteksten av å forstå variabilitet og samspill mellom MJO, dynamikken og varmeinnholdet i havet at resultatene i denne avhandlingen er basert. Dette undersøkes i en serie på tre artikler. For å forstå variasjonen i varmeinnhold i forbindelse med MJO brukes et dynamisk og kinematisk konsistent estimat av havets tilstand, Estimating the Circulation and Climate of the Ocean (ECCO), i artikkel I. Et lukket varmebudsjett viser at horisontal adveksjon er den dominerende drivkraften bak variasjonen i varmeinnhold i den øvre delen av havet på tidsskalaer kortere enn en sesong. Fasesammensetninger av MJO viser at den horisontale adveksjonen som driver variasjoner i varmeinnhold i forbindelse med MJO er knyttet til kelvin- og rossbybølger i havet langs ekvator. Anomalier av varmeinnhold nord og sør for ekvator, delvis modulert av rossbybølger, forsterkes når MJO starter over Indiahavet i fase 2-3. Økningen i varmeinnhold som er modifisert av rossbybølger i havet ved starten av MJO hendelser indikerer synkronisering mellom rossbybølger og MJO i Indiahavet. Denne studien viser at havdynamikken knyttet til kelvin- og rossbybølger i havet dominerer i moduleringen av varmeinnholdet i øvre delen av havet på MJO-tidsskalaen. En lukket kvantifisering av det øvre havets varmebudsjett på tidsskalaer kortere enn en sesong for Indiahavet har ikke tidligere blitt utført ved hjelp av tilstandsestimater som er begrenset av observasjoner, og er et viktig bidrag fra denne studien. Artikkel II undersøker hvordan varmeinnholdet i de øvre lagene av Indiahavet påvirker hvordan MJO forplanter seg forbi det maritime kontinentet. I denne studien brukes MJO-hendelser identifisert av Kerns and Chen (2020) ved hjelp av TRMM-nedbør til å undersøke effekten av varmeinnhold i Indiahavet på MJO. Positivt varmeinnhold forplanter seg vestover i Indiahavet før begynnelsen av MJO-hendelser. Dette varme vannet blir delvis modulert av rossbybølger som reflekteres ved den østlige grensen av Indiahavet. Noen dager før starten av propagerende MJO-hendelser forsterkes de varme anomaliene lokalt når de synkroniseres med MJO. Før MJO-hendelser som ikke propagerer, er derimot de varme anomaliene som forplanter seg vestover, relativt sett, mindre signifikante. Synkroniseringen mellom rossbybølger og MJO, ved modifikasjoner av varmeinnhold i øvre deler av Indiahavet, bidrar til propageringen av MJO forbi det maritime kontinentet. Til slutt undersøkes havdynamikkens tilbakekobling til MJO i den koblede norske jordsystemmodellen versjon 2 (NorESM2) i artikkel III. Horisontale strømmer basert på MJO-fasekompositter avledet fra ECCOs havtilstandsestimat introduseres i en gjentagende syklus i NorESM2. Denne gjentatte syklusen med sterke havstrømmer assosiert med MJO, som i stor grad er relatert til havdynamikk, brukes til å forstå tilbakekoblingen av MJO-styrt havdynamikk på modellens MJO-lignende variabilitet. Tilbakekoblingen fra MJO-assosierte havstrømmer forbedrer representasjonen av den sonale vinden (U850) i lavere deler av troposfæren assosiert med modellens MJO. Men den iboende MJO variabiliteten i modellen forblir lik i eksperimenter med og uten disse havstrømmene. Samlet sett bidrar disse resultatene til vår forståelse av havets respons og tilbakekobling til atmosfæren og gir ytterligere støtte til den potensielt viktige rollen som den øvre delen av havet ved ekvator spiller for MJO.The overall goal of this dissertation is to understand the role of upper ocean heat content (OHC) and equatorial ocean dynamics on the Madden-Julian Oscillation (MJO). While the response of the ocean to atmospheric forcing on intraseasonal timescales has been studied extensively, the feedback of OHC on the MJO has not been systematically investigated. Recently, a new line of research has emerged that highlights the interaction between ocean dynamics, OHC, and the MJO in the Indian Ocean (IO) basin. In the IO, synchronization between oceanic equatorial waves and the MJO is possible because of the basin-scale, the propagation speed of oceanic equatorial waves, and the timescale of MJO variability. In a series of three papers, this thesis aims to contribute to understanding the variability and interactions between the MJO, equatorial ocean dynamics, and OHC in the IO basin. To understand the variability of upper OHC associated with the MJO, a dynamically and kinematically consistent ocean state estimate, Estimating the Circulation and Climate of the Ocean (ECCO), is used in Paper I. A closed heat budget reveals horizontal advection as the dominant driver of intraseasonal upper OHC variability. Phase composites of the MJO show that the horizontal advection driving OHC variations associated with the MJO are connected to oceanic equatorial Kelvin and Rossby waves. Off-equatorial OHC anomalies, partly modulated by Rossby waves, are enhanced as the MJO is initiated over the central IO during phases 2 – 3. The occurrence of enhanced off-equatorial upper OHC modulated largely by oceanic Rossby waves during the convective onset of MJO events indicates potential synchronization between oceanic Rossby waves and the MJO in the IO basin. This study highlights the dominance of ocean dynamics connected to oceanic equatorial Kelvin and Rossby waves in modulating the upper OHC on MJO timescales in the IO basin. A closed quantification of upper ocean heat budget terms on intraseasonal timescales for the IO basin had not previously been carried out using state estimates constrained by observations and is an important contribution of this study. Paper II examines the influence of IO upper OHC on MJO propagation past the Maritime Continent (MC). In this study, MJO events identified by Kerns and Chen (2020) using TRMM precipitation are used to investigate the effect of IO basin upper OHC on the MJO. Warm OHC anomalies are observed to propagate westward in the IO basin prior to the onset of propagating MJO events. These westward-propagating warm OHC anomalies are partly modulated by Rossby waves reflected at the eastern IO boundary. A few days before the onset of propagating MJO events, the warm OHC anomalies are locally enhanced as they synchronize with the MJO. On the contrary, before the onset of non-propagating MJO events, the Rossby wave modulated westward propagating warm OHC anomalies are comparatively less significant. Synchronization between oceanic Rossby waves with the MJO, through modulation of upper OHC in the IO, contributes to MJO propagation across the MC. Finally, the feedback of ocean dynamics to the MJO is investigated in the fully coupled Norwegian Earth System Model version 2 (NorESM2) in Paper III. Anomalous intraseasonal horizontal currents based on MJO phase composites derived from the ECCO ocean state estimate are introduced in a repeating cycle in NorESM2 by nudging. This repeating cycle of strong MJO-associated ocean currents, largely related to equatorial ocean dynamics, is used to understand the feedback of MJO-forced ocean dynamics on the model MJO-like variability. The feedback of MJO-associated ocean currents improves the representation of the lower-level zonal wind (U850) associated with the model MJO. However, the intrinsic MJO variability remains similar in experiments with and without intraseasonal ocean currents. This result suggests that the feedback of ocean dynamics in the IO may not be important for the MJO in NorESM2. Due to the limitations of our model experiments, we recommend improved experiments for future research to assess the impact of IO basin ocean dynamics on the MJO. This dissertation examines the interaction between ocean dynamics, OHC, and the MJO. I find that upper OHC is strongly modulated by equatorial ocean dynamics, and its enhancement can contribute to the propagation of the MJO across the Maritime Continent, with implications for improving MJO-related predictability. IO basin ocean dynamics is not found to be important for the intrinsic MJO variability in NorESM2. However, with some limitations in the experiment nudging parameters, I offer suggestions for an improved experiment for future research. Taken together, these results contribute to our understanding of the ocean's response and feedback to the atmosphere and further motivate the potentially important role of the equatorial upper ocean for the MJO.Doktorgradsavhandlin

    Tracing the Image : The Absence/Presence of the Holy Shroud in Sixteenth-Century Bologna.

    På sporet av bildet: Fraværet/nærværet av Det Hellige Likkledet (Likkledet i Torino) i 1500-tallets Bologna. I kunsthistoriefaget blir renessansen ofte ansett som en grunnleggende hjørnestein i etableringen av det moderne kunst-begrep. På sporet av bildet ser nærmere på kompleksitetene rundt endringen av bilde-begrepet i en omskiftende tid, og stiller det kritiske spørsmålet: Hva var et bilde på slutten av 1500-tallet? I utforskingen av dette spørsmålet fokuserer avhandlingen på den innflytelsesrike skikkelsen Gabriele Paleotti (1536-1597), kardinal og biskop av Bologna, som strevde med å få oversikt over bildets skiftende rolle og betydning i kjølvannet av reformasjonen og Trientkonsilet (1545-1563). Gjennom studier av primærkilder knyttet til Paleotti – inkludert gjenstander og upubliserte korrespondanse fra sent 1500-tall – kaster På sporet av bildet lys over forhandlingene og diskursene knyttet til bilder i den post-katolske reformasjonen. En ledestjerne i avhandlingen er Det Hellige Likkledet i Torino, et viktig, andakts og bilde-relikvie som fant veien til Bologna i 1578 I form av trykk. Avhandlingen viser hvordan dette bilde-relikviet ble et viktig diskusjonstema i Bologna og påvirket Paleottis påfølgende diskusjoner om bilder I hans verk Discorso intorno alle imagini sacre et profane (1582). Ved bruk av diskursanalyse, undersøker denne avhandlingen språket og retorikken rundt bilder, og avdekker de grunnleggende motsetningene og kompleksitetene i disse diskusjonene. Avhandlingen ser nærmere på forhandlingen av bilde-begrepet innen sosiale nettverk, og viser hvordan bildet selv spilte en rolle i denne forhandlingsprosessen. Avhandlingen utfordrer noen sentrale dikotomier i dagens syn på tidlig moderne kunsthistorie. Her settes det spørsmålstegn ved maleriets og skulpturens dominans over trykket (særlig avtrykket), originalens overlegenhet over kopien, samt ideen om at et «sant» bilde må stemme nøyaktig overens med den visuelle fremtreden. Avhandlingen undersøker også oppfatningen av det å betrakte bilder som en løsrevet, isolert opplevelse snarere enn en sanselig interaksjon. Avhandlingen kritiserer den forestillingen om at bildeframstilling allerede på denne tiden var en sekularisert, liberal kunst. Avslutningsvis undersøker avhandlingen hvordan endringer i humanismen, historiografien og naturvitenskapen endret bildenes rolle som sannhetsformidlere i en tid preget av usikkerhet. Med denne mangefasetterte undersøkelsen søker avhandlingen å utdype vår forståelse av kompleksiteten rundt bildebegrepet, et begrep som ofte tas for gitt.Tracing the Image: The absence / presence of the Holy Shroud in sixteenth-century Bologna. In the discipline of art history, the Renaissance period is often regarded as a foundational cornerstone, shaping contemporary perceptions of art. Tracing the Image delves into the complexities surrounding the concept of the image during this transformative time, positing the crucial question: what was an image at the close of the sixteenth century? To explore this question, this study focuses on the influential figure of Gabriele Paleotti (1536-1597), Cardinal and Bishop of Bologna, who grappled with the evolving significance of images in the wake of the reformations and the Council of Trent (1545-1563). Through an examination of primary sources related to Paleotti – including visual artifacts and unpublished correspondence from the late 1500s – this study sheds light on the negotiations and discourses surrounding images in post-Catholic reform. Guiding the study is the Holy Shroud of Turin, a prominent devotional object known as an image-relic, which found its way to Bologna in 1578 through a print. The study demonstrates how this image-relic emerged as a central topic of discussion in Bologna and influenced Paleotti’s discussions about images in his subsequent publication the Discorso intorno alle imagini sacre et profane (1582). By employing discourse analysis, Tracing the Image closely examines the language and rhetoric surrounding images, revealing the inherent contradictions and complexities within these discussions. It highlights how concepts of image were negotiated within social networks and shows the active role that the image itself played in this negotiation process. This study challenges some of key dichotomies which have shaped early modern art history, questioning the dominance of painting and sculpture over prints (especially the imprint), the superiority of originals over copies, and the idea that a 'true' image must accurately match visual appearance. It also reexamines the perception of viewing images as a detached experience rather than an affective, tactile interaction. Lastly, it critiques the notion that image-making was already a secularised liberal art at this time. Additionally, the thesis explores how shifts in humanism, historiography and natural sciences reshaped the role of images in establishing truth during a time of uncertainty. Ultimately, this multifaceted investigation seeks to deepen our understanding of the complexities surrounding the concept of image, a concept which is all too often taken for granted.Doktorgradsavhandlin

    A multidisiplinary intervention tailored to knee osteoarthritis patients at risk of chronic pain after total knee arthroplasty

    Bakgrunn: MultiKnee studien er en randomisert kontrollert studie (RCT) som har som mål å teste effekten av en intervensjon som kombinerer undervisning, treningsterapi og kognitiv adferdsterapi, når den gis i stedet for eller i tillegg til kneproteseoperasjon hos artrosepasienter. Mål: Målet med denne studien var å: 1) kartlegge den digitale helsekompetansen hos pasienter operert med totalprotese i hofte eller kne og undersøke sammenhengen mellom digital helsekompetanse og helserelatert livskvalitet, 2) utvikle et internettbasert kognitivt adferdsterapi (iCBT) program tilpasset pasienter med artrose i kne, og 3) teste gjennomførbarheten av MultiKnee programmet og studien. Metode: En tverrsnitts-studie ble brukt for å kartlegge digital helsekompetanse hos pasienter med hofte- eller kneprotese, og undersøke sammenhengen mellom digital helsekompetanse og helserelatert livskvalitet. UK Medical Research Counsil’s rammeverk for utvikling og testing av komplekse intervensjoner ble brukt i utviklingen av iCBT programmet og testingen på pasienter og fysioterapeuter. En feasibility RCT ble utført for å teste gjennomførbarheten av en full- skala RCT. Resultat: Eldre pasienter med lav utdanning, hadde lavest digital helsekompetanse. Pasienter med mer engasjement og kontroll rapporterte høyere helserelatert livskvalitet. Et iCBT program ble utviklet og tilpasset artrose pasienter i tillegg til en fysioterapimanual for å veilede fysioterapeutene i oppfølgingen av pasientene. Normdata for digital helsekompetanse ble beskrevet. Feasibility studien viste at det var nødvendig med noen endringer i inklusjonskriteriene underveis i studien for å kunne rekruttere nok pasienter til en full skala RCT, og at det var nødvendig å tilpasse iCBT programmet ytterligere før oppstart av RCT studien. Konklusjon: Norm data for digital helsekompetanse kan brukes for å tilpasse digitale helsetjenester til hofte- og kneprotesepasienter for å gi et likt helsetilbud til alle. iCBT programmet er tilpasset artrosepasienter og pasienter med totalprotese i kne. Feasibility studien viste at en full skala RCT er mulig å gjennomføre for å teste effekten av den kombinerte intervensjonen når den gis enten i tillegg til eller i stedet for operasjon med totalprotese i kne.Background: The MultiKnee study is a randomized controlled trial (RCT) aiming to test the effectiveness of an intervention combining education, exercise therapy, and internet-based cognitive behavioral therapy (iCBT), either as an alternative to or in addition to knee arthroplasty surgery for knee osteoarthritis patients. Aim: The aim of this study was to: 1) assess the digital health literacy (eHL) among patients who underwent total hip arthroplasty (THA) or total knee arthroplasty (TKA) and examine the relationship between eHL and health-related quality of life (HRQoL), 2) develop an internet-based cognitive behavioral therapy (iCBT) program tailored to patients with knee osteoarthritis (OA), and 3) test the feasibility of the MultiKnee intervention and trial. Methods: The UK Medical Research Council’s framework for developing and evaluating complex interventions was used to develop the iCBT program and test it on patients and physiotherapists. A cross-sectional study was conducted to assess eHL among TKA and THA patients and examine the relationship between eHL and HRQoL. A feasibility RCT was conducted to test the feasibility of a full-scale RCT. Results: Norm data for eHL were described. Older patients with lower education had the lowest eHL. Patients with higher engagement with digital services and who are feeling safe and in control reported better HRQoL. An iCBT program tailored for OA patients was developed, along with a physiotherapy manual to guide physiotherapists in patient follow-up. The feasibility study indicated the need for changes in the inclusion criteria during the study to recruit enough patients for a full-scale RCT and further adaptation of the iCBT program before starting the RCT. Conclusion: eHL norm data can be used to tailor digital health services for THA and TKA patients to ensure equal healthcare access. The iCBT program is tailored for OA and TKA and feasible to be tested in a full-scale RCT. The feasibility study showed that a full-scale RCT is feasible to evaluate the combined intervention's effectiveness, either as an alternative to or in addition to surgery.Doktorgradsavhandlin
