    Endoscopic Management of Esophageal Motility Disorders : Diagnostic and Therapeutic Outcomes

    Bakgrunn: Motilitetsforstyrrelser i spiserøret (EMD) er kroniske tilstander som kan ha stor påvirkning på pasientenes livskvalitet, der svelgevanske er et fellessymptom. For å kunne stille riktig diagnose og gi riktig behandling, er det avgjørende med presis diagnostikk. De siste årene har forskning gitt oss ny kunnskap og nye metoder for å håndtere disse tilstandene. Kirurgisk myotomi, der man deler muskulaturen i nedre del av spiserøret, som ble først utført av Ernst Heller i 1913, har siden vært gullstandarden for behandling av akalasi og primær EMD. Peroral Endoskopisk Myotomi (POEM) representerer en ny minimalt invasiv teknikk, som er et mer skånsom enn kirurgi. Selv om POEM har mange fordeler, mangler det data for langtids-oppfølging. I tillegg har POEM begrensninger når det gjelder å utføre myotomi i øvre del av spiserøret. Retrograd Endoskopisk Myotomi (REM) ble derfor utviklet for å løse dette problemet, og tillater myotomi av proksimale den av spiserøret og øvre lukkemuskel i spiserøret, gjennom en gastrostomi. Eosinofil øsofagitt (EoE), som er den vanligste sekundære EMD, behandles vanligvis med endoskopisk dilatasjon og lokale steroider. Diagnosen stilles med tilfeldige vevsprøver fra spiserørsslimhinnen der man mikroskopisk kan påvise betennelse med over 15 eosinofile granulocytter per høyoppløselig mikroskopifelt, som i vår studie er definert til 0.25 mm². Fordi betennelsen kan ha ujevn utbredelse i spiserøret, kan man gjennom tilfeldig biopsitaking få ulike svar, og diagnosen kan overses. Formål med avhandlingen: Målet med denne avhandlingen var å vurdere effekten og sikkerheten av POEM og REM ved akalasi, og evaluere en ny diagnostisk teknikk for EoE, som representerer de to vanligste EMD i spiserøret. Studie I: Å undersøke langtids-sikkerhet og effekt av POEM i en norsk kohort under en oppstartsfase og innføring av en ny prosedyre. Studie II: Å undersøke gjennomførbarhet og sikkerhet av REM, kombinert med en prototyp navigasjonssystem for fleksibel endoskopi, for å behandle terapiresistent akalasi i øvre del av spiserøret. Studie III: Å evaluere en ny metodikk for å øke diagnostisk treffsikkerhet for EoE. Metoder: Studiene inkluderte pasienter som ble fulgt fra 2014 til 2021. I den første studien analyserte vi de første 84 pasientene med primær akalasi som ble behandlet med POEM, og ble fulgt opp opptil seks år. De ble evaluert med høyoppløselig manometri, barium svelgrøntgen, 24-timers pH-målinger og Eckardt symptomskår. I den andre studien, fokuserte vi på en pasient med terapiresistent akalasi type III i øvre del av spiserøret som gjennomgikk REM. Den tredje studien inkluderte 49 pasienter med svelgevansker som fikk gastroskopi med jod-kalium-jodid (IPIS). Blant disse hadde 30 pasienter histologisk bekreftet EoE, mens 19 ble brukt som kontrollgruppe. Hovedresultater: I den første studien, ble 84 pasienter behandlet med POEM, derav 50 menn og 34 kvinner. Av disse hadde 43 (51%) aldri blitt behandlet før, mens 24 (28,6%) hadde fått tidligere behandling som botulinumtoksin, pneumatisk ballongutvidelse eller begge deler, og 17 (20,2%) hadde fått Hellers myotomi tidligere. Etter POEM var median symptomskår 1 (0–9) ved 12 måneder, sammenlignet med 7 (4–12) før behandlingen (p<0,01). Klinisk suksess, definert som Eckardt score lik eller under 3, ble oppnådd hos 74 pasienter (88,1%) etter 12 måneder, med høyest suksessrate på 95% (41/43) blant de uten tidligere behandling og 70,6% (12/17) blant de som hadde hatt Hellers myotomi. I en oppfølging etter 5–6 år av 42 pasienter, var suksessraten på 94%, med milde bivirkninger hos 6% og reflux hos 46% etter POEM. Den andre studien viste at REM utført via en midlertidig gastrostomi er en trygg og gjennomførbar behandling for proksimal akalasi. Under REM ble et navigasjonssystem brukt til å identifisere anatomiske strukturer i mediastinum og nakken, slik at det ble mulig å planlegge lengden og posisjonen til muskelspaltingen presist. Den tredje studien undersøkte om en jod-kalium-jod (IPIS) kunne avdekke aktiv betennelse ved EoE. Tre ulike IPIS-mønstre ble observert blant EoE-pasientene: 17 pasienter (57%) hadde redusert opptak gjennom hele spiserøret, fem (17%) hadde segmentert opptak, og åtte (27%) hadde flekkete mønster. Alle EoE-pasientene viste redusert opptak av IPIS i ulik grad, med 100% sensitivitet. Hos pasientene med segmentert eller flekkete mønster var det signifikant flere eosinofiler i områder med redusert opptak sammenlignet med områder med normalt opptak (100,2 versus 6,5 eosinofiler per synsfelt, p=0,005). Konklusjon: De første to studiene konkluderer at POEM er en effektiv og sikker behandling for primær akalasi, som gir langvarig symptomlindring med få milde bivirkninger, men med en økt forekomst av gastroøsofagal refluks. REM er en trygg og gjennomførbar behandling for terapiresistent akalasi i øvre del av spiserøret når den kombineres med et navigasjonssystem. Kombinasjonen av POEM og REM muliggjør komplett myotomi i hele spiserøret, fra cardia til øvre øsofagal sfinter. Den tredje studien identifiserer tre ulike mønstre ved EoE, og viser at målrettede biopsier etter IPISfarging øker diagnostisk sensitivitet, med selektive prøvetaking. Samlet sett støtter resultatene bruk av endoskopi som hovedverktøy for diagnostisering og behandling av EMD.Background: Esophageal motility disorders (EMDs) are chronic conditions that can significantly impact patients' quality of life, with dysphagia being the most common symptom. Proper diagnostic workup and treatment are crucial for accurately diagnosing and managing these disorders. For a century, surgical myotomy of the distal esophagus has been the gold standard treatment for primary EMDs. However, in recent years, per-oral endoscopic myotomy (POEM) has emerged as a minimally invasive alternative. Despite several benefits, POEM has limitations in performing myotomy of the proximal esophagus due to safety margins. Retrograde Endoscopic Myotomy (REM) was developed to address this limitation, allowing for the myotomy of the proximal esophagus and the upper esophageal sphincter by reversing the POEM procedure via a temporary gastrostomy. Eosinophilic esophagitis (EoE) is the most common secondary EMD and is typically treated with endoscopic dilation and local steroids. However, the diagnosis can be overlooked due to sampling errors from random biopsies. Aims of the Dissertation: The overall objective of this dissertation was to evaluate the efficacy and safety of various treatment options and a new diagnostic tool for two of the most common esophageal motility disorders: achalasia and EoE. Study I: To assess the long-term safety and efficacy of POEM in a Norwegian population. Study II: To examine the feasibility and safety of REM combined with a prototype navigation system for treating achalasia in the proximal esophagus. Study III: Evaluate a new technique that may improve the diagnostic yield of EoE. Methods: The studies prospectively included patients and collected data from 2014 to 2021. The first paper analyzed 84 patients with primary achalasia who underwent POEM and were followed for up to six years. They were evaluated using high-resolution manometry, timed barium esophagogram (TBE), 24-hour pH measurements, and a symptom score. The second paper focused on a patient with therapy-resistant type III achalasia in the proximal esophagus who underwent REM. The third paper included 49 patients with dysphagia who underwent gastroscopy using iodine potassium iodide (IPIS). Among these, 30 patients had histologically proven EoE, while 19 with dysphagia and symptoms compatible with EoE served as control subjects. Main results: The first paper studied 84 patients treated with POEM, including 50 males and 34 females. Among these patients, 43 (51%) were treatment-naïve, while 24 (28.6%) had prior treatments such as botulinum toxin, pneumatic balloon dilation, or both, and 17 (20.2%) had undergone Heller myotomy (HM). The median post-POEM Eckhart score at 12 months was 1 (0-9), compared to 7 (4-12) pre-POEM (p<0.01). Seventyfour patients (88.1%) achieved clinical success at 12 months, with the highest success rate of 95% (41/43) among treatment-naïve patients and 70.6% (12/17) among those previously treated with HM. In a 5–6-year follow-up, 42 patients demonstrated a clinical success rate of 94%, with mild adverse events in 6% and 46% experiencing post-POEM reflux. The second paper demonstrates that REM performed via a temporary gastrostomy is a safe and feasible treatment for proximal achalasia. During REM, a prototype navigation system aided in identifying the anatomical structures of the mediastinum and neck, allowing for precise assessment of the position and length of the myotomy and planning of the procedure. The third paper investigated whether IPIS could detect active inflammation in eosinophilic esophagitis (EoE). Three IPIS staining patterns were observed in EoE patients. Seventeen patients (57%) exhibited diminished uptake throughout the esophagus, five (17%) showed segmented uptake, and eight (27%) displayed a patchy pattern. All EoE patients demonstrated reduced uptake of IPIS to varying extents, yielding 100% sensitivity. In EoE patients with segmental or patchy patterns, areas with diminished uptake exhibited significantly more eosinophils than areas with normal uptake (100.2 versus 6.5 eosinophils/HPF, p=0.005). Conclusions: Papers one and two address achalasia and conclude that POEM is an effective and safe treatment for primary achalasia, providing long-term symptom relief with minimal mild adverse events. However, it is associated with an increase in esophageal reflux. When combined with a navigation system, REM is a feasible and safe treatment for therapy-resistant achalasia in the proximal esophagus. Paper three identifies three distinct inflammation patterns in EoE and demonstrates that targeted biopsies using the IPIS stain improve diagnostic sensitivity compared to standard random biopsy protocol.     AI and the quest for legitimacy in the modern society How does the use of artificial intelligence in public governance impact institutional legitimacy? The case of Norway.

    This study examines the impact of using Artificial Intelligence (AI) in public governance on institutional legitimacy by using a multilevel mixed methods research design. Institutional legitimacy is an essential ingredient for organizational success and survival. However, as our societies become more complex, more advanced solutions are sought to address the challenges that emerge. Consequently, there is a growing desire to incorporate AI into public governance to mitigate these challenges. Therefore, this study aims to research the potential impact of using AI in public governance through the research question: How does the use of artificial intelligence in public governance impact institutional legitimacy? Institutional legitimacy in this thesis is understood and studied as a multilevel and multifaceted concept. Hence, central is the reflection on the following question: Legitimate to whom? Approaching legitimacy as such demands the integration of several levels of analysis to produce an adequate answer. In this thesis, I explore the political, organizational, and individual levels. Moreover, when discussing the potential challenges the incorporation of AI in public governance may generate, one of the most controversial cases remains unexplored: high-discretion tasks. Thus, to answer the research question, I combine a survey experiment from the Norwegian Citizen Panel at the individual level with discourse analyses of public documents at the organizational and systemic levels. The findings indicate that there is a schism in the understanding and legitimacy of using AI in public governance. Political leaders frame AI as a necessary innovation, a “magic concept” necessary for the country’s future, and the regulations needed as a “necessary evil”. Meanwhile, the citizens consider the regulations an essential part of the process. In the “eyes” of the public, humans remain the most legitimate decision-makers, but using AI in high-discretion tasks may be legitimized through the use of accountability and transparency measures.     Performance of Distributed and Shared Memory Parallel Sparse-Matrix Vector Multiplication

    Sparse-Matrix Vector Multiplication (SpMV) is a computational kernel encountered in many scientific applications. It has an inherently low computational intensity, and its performance is memory-bound. This thesis evaluates the performance metrics of five increasingly selective communication strategies for SpMV in a shared and distributed memory hybrid environment. Experiments were conducted on single-socketed and dual-socketed multicore systems across various configurations. Findings show that reducing the communication volume yield better performance on a single node. In multi node setups, the findings show that performance is impacted by the communication volume, but that other factors such as network and hardware characteristics also play a large role in performance.Masteroppgave i informatikkINF399MAMN-PROGMAMN-IN

    Denne oppgåva undersøkjer ulike utfordringar som informantane møter i sjukepleiaryrket. Eg har gjennomført åtte intervju med sjukepleiarar i alderen mellom tjuetre og femtiåtte år, der fem av dei arbeider innanfor den kommunale helse- og omsorgstenesta og tre av dei arbeider innan spesialisthelsetenesta. Omgrepet omsorgsrasjonalitet bidreg til å underbyggje funna i analysen. Dessutan vil pull- og pushfaktorar bli brukt som eit analyseverktøy for å framheve problemstillinga. Oppgåva er delt inn i fire analysekapittel som tek føre seg uføreseieleg arbeidskvardag, omsorgsrasjonalitet, push- og pullfaktorar i yrket, og til slutt utfordringar knytte til kjønn i omsorgsyrket. Det fyrste analysekapittelet tek føre seg omgrepet uføreseieleg arbeidskvardag, som er knyta til utfordringar innan turnusarbeid og endringar i arbeidskvardagen som fylgje av koronapandemien. På bakgrunn av at datamaterialet viser at pandemien har ført til at turnusen har blitt endra og at informantane har fått fleire utfordrande arbeidsoppgåver, har omgrepet uføreseieleg arbeidskvardag blitt etablert. Det andre analysekapittelet ser på utfordringar knytt til omsorgsutøving. Her vil omgrepa stress og tidspress bli diskuterte som gjennomgåande utfordringar som fører til konsekvensar for sjukepleiarane sin måte å utøve omsorg på, fordi arbeidskvardagen er styrt av ytre krav og rammer. På den måten har sjukepleiaren gjerne ikkje god nok tid til å setje seg ned med pasienten og høyre korleis han eller ho faktisk har det. Dette er gjerne i strid med prinsippa for utøving av omsorgsrasjonalitet som framhevar at sjukepleiaren må kunne setje seg inn i pasienten sin situasjon for å kunne utøve god og nødvendig omsorg. Det tredje analysekapittelet ser på sjukepleiarane sine positive og negative tankar om yrket. Kapittelet tek i bruk push- og pullfaktorar som eit analyseverktøy for å diskutere kvifor informantane ynskjer å gå på jobb sjølv om dei fortel om dagar utan tilstrekkeleg opplæring, dårleg arbeidsmiljø eller lite ressursar på jobb. Ved å fokusere på informantane sine positive og negative sider ved yrket, vert utfordringane som informantane fortel om, diskutert i ljos av push- og pullfaktorar. Det fjerde og siste analysekapittelet diskuterer utfordringar knytt til kjønn i sjukepleiaryrket. Her blir kjønnsbalansen på arbeidsplassen til informantane diskutert, og informantane fortel om forskjellar i arbeidsmåten til menn og kvinner. Vidare vil ein diskutere om arbeidsmiljøet hadde vore betre dersom det hadde vore likevekt mellom kjønna. Avslutningsvis i kapittelet vil utfordringar som både menn og kvinner kan oppleve i møte med pasientane, bli presentert og diskutert. Empirien i oppgåva viser at tidspress og stadige krav om effektivisering medfører at sjukepleiaren utset arbeidsoppgåver og at kvaliteten i helsehjelpa vert redusert. Dette står gjerne i strid med dei grunnleggjande sjukepleiarverdiane som inneber ideal om å utøve heilskapleg pasientsentrert pleie av høg kvalitet. Det krevjar at sjukepleiaren har god kompetanse, tydelege verdiar, viser respekt og fokuserer på pasienten sine fysiske og psykiske behov. Kari Wærness sitt omgrep om omsorgsrasjonalitet inneber at ein ikkje må ekskludere kjensler, men heller omfatte desse for at handlingar skal kunne reknast som rasjonelle (Christensen &amp; Syltevik, 1999, s. 13). Slik meiner Wærness at omsorgsarbeidet ikkje er rasjonelt dersom ein ikkje bruker hjartet i arbeidet. På bakgrunn av dette og i tråd med prinsippa for omsorgsrasjonalitet må sjukepleiaren ha tid til å setje seg inn i pasienten sin situasjon for å kunne utøve god omsorg.     Use of psychotropic medications among glioma patients in Denmark, Norway, Sweden, and Wales

    Purpose: Glioma patients often suffer from psychiatric and neurological conditions. However, little is known about the patterns of use of psychotropic drugs pre- and post-glioma diagnosis. Therefore, we assessed temporal patterns of psychotropic prescriptions among glioma patients, compared to an age and sex matched comparison cohort in four European countries. Methods: Incident gliomas were identified in Wales from the Secured Anonymized Information Linkage Databank (2005–2016) and population-based registries in Denmark (2001–2016), Norway (2006–2019), and Sweden (2008–2018). From each data source, a cancer-free comparison cohort was matched to the glioma cases by age and sex. We calculated rates of new psychotropic prescriptions and any psychotropic prescriptions during the 2 years prior to and post glioma diagnosis. Analyses were stratified by histological subtypes and subclasses of psychotropic medications. Results: We identified 16,007 glioma patients. The rate of new psychotropic drug use increased from 7 months before diagnosis, peaking around the month of glioma diagnosis (with peak rates ranging from 227 to 753 new psychotropic drugs per 1000 person-months). New use remained substantially higher among glioma patients than comparators throughout the 2-year follow-up period after glioma diagnosis, though rates of new use continued to decline throughout. New use was largely driven by antiepileptics, anxiolytics, hypnotics, and sedatives. Patterns were similar when analyses were stratified by histological subtype. Conclusion: Psychotropic drug use among glioma patients was high, and elevations observed around the time of cancer diagnosis, largely driven by antiepileptics, anxiolytics, hypnotics, and sedatives, are likely associated with the consequences of the disease.publishedVersio

    Into the Blue: An ERC Synergy Grant Resolving Past Arctic Greenhouse Climate States

    The Arctic Ocean is turning blue. Abrupt Arctic warming and amplification is driving rapid sea ice decline and irreversible deglaciation of Greenland. The already emerging, substantial consequences for the planet and society are intensifying and yet, model-based projections lack validatory consensus. To date, we cannot anticipate how a blue Arctic will respond to and amplify an increasingly warmer future climate, nor how it will impact the wider planet and society. Climate projections are inconclusive as we critically lack key Arctic geological archives that preserved the answers. This “Arctic Challenge” of global significance can only be addressed by investigating the processes, consequences, and impacts of past “greenhouse” (warmer-than-present) climate states. To address this challenge, the ERC Synergy Grant project Into the Blue (i2B) is undertaking a program of research focused on retrieving new Arctic geological archives of past warmth and key breakthroughs in climate model performance to deliver a ground-breaking, synergistic framework to answer the central question: “Why and what were the global ramifications of a “blue” (ice-free) Arctic during past warmer-than-present climates?” Here, we present the proposed research plan that will be conducted as part of this program.     Relative sea-level trends in southern Norway during the last millennium

    Geological reconstructions of relative sea level (RSL) from southern Norway show falling RSL during the last 7000 cal. a BP, but tide gauge measurements document a slow RSL rise since at least 1960 CE. With an age gap of c. 1400 years between the youngest geologically reconstructed sea-level index point (SLIP) and the installation of the Tregde tide gauge in southernmost Norway, the exact nature and timing of the onset of RSL rise in southern Norway remain unknown. To fill this gap, we collected peat cores from a salt marsh to reconstruct RSL trends over the past 1000 years using a multiproxy approach, including 210Pb and 14C dating, grain-size analysis, loss-on-ignition (LOI), geochemistry (stable carbon isotopes, carbon to nitrogen ratios and XRF) and diatoms. Our data suggest decreasing tidal current strength and salinity over most of the last millennium, suggesting falling RSL. Sediment geochemistry also appears to vary with wetter and drier climatic periods. An increase in marine-brackish diatoms in combination with an acceleration in sedimentation rates after 1930 CE (1899–1954 CE) suggest that the onset of RSL rise began around this time in southernmost Norway. While most of the proxy data appear to have delayed sensitivity to RSL changes and may be linked to other causal processes, they, nonetheless, provide valuable insight into the environmental response of high-latitude temperate salt marshes to slow rates of RSL change.publishedVersio

    The impact of COVID-19 pandemic restrictions on general practitioner contacts among patients with common and at-risk diagnoses: a Norwegian registry-based study

    Background: Disease prevention strategies during the COVID-19 pandemic could potentially disrupt the continuity of care. Aims: (1) describe changes in contacts with general practitioner (GP) before and during the pandemic, (2) compare consultation rates for common and at-risk diagnoses, and (3) examine the impact of COVID-19 restrictions on monthly consultation rates across patient subgroups. Methods: Register study with data from the Norwegian Registry for Primary Health Care (NRPHC) for the period 1 April 2018 to 30 March 2021. Changes in consultation rate were the main outcome. Twenty-five diagnoses representing common and at-risk for mortality diagnoses were studied. Interrupted time series analyses were applied. Results: Face-to-face consultations decreased from 2935 to 2304 per 1000 inhabitants per year from the pre-pandemic period to the first pandemic year, with an increase in e-consultations from 60 to 846 per 1000 inhabitants per year. Consultations for common diagnoses decreased by 25.6%, whereas those for high-risk diagnoses decreased by 10.7%. In the group of common diagnoses, the decrease in average monthly consultation rates was less among the eldest compared to the youngest age group. A similar decrease was found in the group with multiple morbid conditions compared to single or no morbidity. Conclusions: The pandemic resulted in fewer face-to-face consultations, but there was a substantial rise in electronic consultations. Consultations for at-risk for mortality diagnoses decreased relatively less than for common diagnoses. Consultation rates for older adults and those with multiple decreased less, suggesting that sicker patients continued to visit their GP during the pandemic.publishedVersio

    Resilience or Resemblance? : Lived experiences of adult children of parents and carers with alcohol-related problems in Botswana

    Den overordnede målsetningen med denne avhandlingen er å øke forståelsen vår for erfaringene til voksne barn av foreldre og omsorgspersoner med alkoholrelaterte problemer i Afrika sør for Sahara. Foreldres og omsorgspersoners alkoholmisbruk har i stor grad påvirket barnas psykososiale trivsel som voksne. Virkningen av alkoholmisbruk sprer seg fra enkeltpersoner til familier og samfunn og blir en del av kulturen, noe som fører til et gjentagende mønster. Som en konsekvens opplever voksne barn av foreldre og omsorgspersoner med alkoholrelaterte problemer risiko, men utvikler også håndteringsmekanismer for å trives under oppveksten i hjem der det blir misbrukt alkohol. Til tross for at Botswana er et land med høyt alkoholforbruk, mangler det forskning på hvordan disse faktorene påvirker barna over tid. Denne avhandlingen har som mål å fylle dette tomrommet. Avhandlingen gir innsikt i hvordan miljøet der det blir misbrukt alkohol påvirker velferden til voksne barn og hvordan tilgjengeligheten av ressurser og støtte påvirker veiene deres til motstandskraft. Motstandskraft-teori og en sosioøkologisk tilnærming til helsefremming inspirerte denne avhandlingen. Funnene i denne avhandlingen diskuteres med utgangspunkt i kvalitative data fra voksne barn av foreldre og omsorgspersoner med alkoholrelaterte problemer samt ulike aktører i rusfeltet. Avhandlingen er basert på tre publiserte artikler; to artikler som allerede er publisert og et manuskript som er innlevert. Denne avhandlingen hadde følgende mål, som ble behandlet i tre fagfellevurderte artikler 1) Å utvikle kunnskap om innflytelsen av alkoholmisbruk innenfor ulike sosioøkologiske nivåer i Botswana; 2) å belyse oppfatningene og erfaringene til voksne barn av foreldre og omsorgspersoner med alkoholrelaterte problemer og veier de bruker for å navigere risikoer som kan føre til motstandskraft eller til at de blir som foreldrene sine; 3) å utforske hvordan beskyttelsesfaktorer i det sosiale miljøet til voksne barn av foreldre og omsorgspersoner med alkoholproblemer påvirker deres motstandskraft. Ved å bruke et sosioøkologisk perspektiv på motstandskraft, baserer denne kvalitative studien seg på en fenomenologisk og narrativ tilnærming. Artikkel 1 tar utgangspunkt i tjuetre semi-strukturerte intervjuer og en fokusgruppe med aktører i rusfeltet for å kartlegge den sosio-kulturelle samfunnskonteksten av alkoholmisbruk i landsbyen. Resultatene viser at mønstre av alkoholmisbruk over tid påvirker forskjellige sosio-økologiske systemer i samfunnet. Disse funnene indikerer at selv om det finnes systemer tilgjengelige for å respondere på alkoholmisbruk i samfunnet, trengs det samarbeid mellom aktører i feltet. Artikkel 2 bygget på sosioøkologisk motstandskraft-teori, og undersøker erfaringer med å vokse opp i et hjem der alkoholmisbruk forekommer blant syv voksne. Resultatene viste at voksne barn av foreldre og omsorgspersoner med alkoholrelaterte problemer opplever risikofaktorer for selv å bruke alkohol og dermed kopiere sine foreldre og omsorgspersoner. Videre viser resultatene at det finnes håndteringsmåter som brukes for å mestre oppveksten i et hjem hvor det forekommer alkoholmisbruk, som resulterer i at voksne barn blir motstandsdyktige mot alkoholmisbruk og ikke gjentar adferden til sine foreldre og omsorgspersoner. Artikkel 3 gir et inngående innblikk i erfaringene til voksne barn av foreldre og omsorgspersoner med alkoholrelaterte problemer gjennom en narrativ undersøkelse. Tre deltakere fra Artikkel 2 ble valgt ut for å få en detaljert forståelse av beskyttelsesfaktorene i deres sosiale miljø som påvirket deres motstandskraft til å mestre det å vokse opp i et alkoholmisbrukende hjem. Resultatene avslørte interne og eksterne støttesystemer som gjorde det mulig for voksne barn å trives. Funnene indikerte at alle de sosioøkologiske nivåene - individet, familien, venner og samfunnet- bidro som tilgjengelige ressurser for voksne barn tross tidligere motgang. Basert på funnene i de tre artiklene, ser vi at kulturen for alkoholbruk i Botswana trenger å bli adressert i samfunnet for å redusere den store innflytelsen alkoholmisbruk har på individer, familier og samfunn. Det er funnet risikofaktorer som påvirker barn som vokser opp i miljøer med alkoholmisbruk til samtidig å være motstandsdyktige og å likne sine alkoholmisbrukende foreldre og omsorgspersoner. Videre legger studien vekt på at noen barn kan få tilgang til beskyttelsesfaktorer i form av tilgjengelige ressurser i sine familier og samfunn, som kan hjelpe dem å trives til tross for å vokse opp i hjem hvor alkoholmisbruk forekommer. Selv om Botswana for tiden har systemer for å adressere alkoholmisbruk, må ulike aktører i feltet samarbeide for å implementere politikk og reguleringer, slik som alkoholpolitikken, barne- og ungdomsvernloven og reguleringer for tradisjonelle bryggerier. Avhandlingen tar også til orde for bevissthet om erfaringene til voksne barn som vokste opp i alkoholmisbrukende hjem, som et område som trenger å bli adressert på forsknings- og politikkutviklingsnivå og oversatt til praksis.The overall aim of this thesis is to increase our understanding of the experiences of adult children of parents and carers with alcohol-related problems in sub-Saharan Africa. The misuse of alcohol by parents and carers has greatly influenced children’s psychosocial well-being as adults. The influence of alcohol misuse trickles down from individuals to families and communities and becomes embedded in the culture where there is a feedback loop. As a result, adult children of parents and carers with alcohol-related problems experience risks but also protective factors to thrive while growing up in alcohol-misusing homes. Despite Botswana’s being a country with high alcohol consumption, there is a dearth of research on how these factors affect these children over time. This thesis aims to fill that research gap. This thesis provides insights into how the environment of alcohol misuse influences the well-being of adult children and how the availability of resources and support influences their pathways to resilience. Resilience theory and the socio-ecological approach as a theoretical framework inspired this thesis. The summarised findings of this synopsis are discussed based on qualitative data from stakeholders and adult children of parents and carers with alcohol-related problems. It is based on three articles; two published articles, and a submitted manuscript. This thesis drew guidance from the following aims, which were addressed in three peer-reviewed articles: 1) to develop knowledge on the influences of alcohol misuse within the different socio-ecologies in Botswana; 2) to shed light on the perceptions and experiences of adult children of parents and carers with alcohol-related problems, and the pathways that they use to navigate risks that may lead to resilience or resemblance; and 3) to explore how protective factors in the social environment of adult children of parents and carers with alcohol problems influence their resilience in Botswana. Using a socio-ecological approach to resilience, this qualitative study conducted a phenomenological and narrative inquiry. Article 1 is based on 23 semi-structured interviews and a focus group with stakeholders to provide insights into mapping the sociocultural community context of alcohol misuse in the village, using a socio-ecological approach. The findings reveal that patterns of alcohol misuse over time influence different ecological systems in the community. These findings indicate that, although there are available systems in place to respond to alcohol misuse in the community, there is a need for collaboration among stakeholders. Article 2 drew upon a socio-ecological theory of resilience to examine the meaning of the lived experiences among seven adults of growing up in an alcoholic home. The findings revealed that adult children of parents and carers with alcohol-related problems experience risk factors which led them to use alcohol and to resemble their parents and carers. Furthermore, the findings highlight that there are protective factors used to navigate growing up in alcohol-misusing homes, which result in other adult children being resilient to alcohol misuse and not being like their parents and carers. Article 3 provides an in-depth insight into the lived experiences of adult children of parents and carers with alcohol-related problems through a narrative inquiry. Three participants from Article 2 were selected to gain an in-depth understanding of the protective factors in their social environment that influenced their resilience to thrive while growing up in alcoholic homes. The findings revealed internal and external support systems as protective factors that enabled adult children to thrive. The findings stated that all ecologies, such as individual, family, friends, and communities, contributed as available resources for adult children to thrive through adversities. Based on the findings in the three articles, the culture of alcohol misuse in Botswana needs to be addressed in communities to reduce the great influence it has on individuals, families, and communities. There are risk factors found to influence children growing up in alcohol-misusing environments to be resilient and to resemble their alcohol-misusing parents and carers at the same time. Furthermore, the study highlights that some children could access protective factors since individual traits, family, neighbourhoods, schools, and religion were resources available in their families and communities. This helped them to thrive despite growing up in alcohol-misusing homes. Although Botswana currently has systems to address alcohol misuse, stakeholders need to collaborate and work together to implement these policies and regulations, such as the alcohol policy, the Children’s Act, and traditional brew regulations. The thesis also advocates for awareness of the experiences of adult children who grew up in alcohol-misusing homes as an area that needs to be addressed at the policy research level and translated into practice.Doktorgradsavhandlin
