    Doula Support for Hispanic Women

    Purpose This quantitative study will seek to provide valuable insight into the needs and preferences of Hispanic women during labor with the potential benefit of doula support in minimizing their pain. Methods A quasi-experimental design study will be conducted, randomizing a convenient sample into two groups consisting of one experimental and one control group, with the objective of evaluating the efficacy of doula presence as a source for pain management in Hispanic women. One hundred Hispanic women will be recruited under the following criteria: 18 years or older, a single fetus pregnancy, gestational age 37 weeks and above, and no complications during pregnancy. At the completion of labor, the women will be asked their pain level using the Childbirth Pain Scale. Results The data has not yet been collected, however, the t-test is expected to result in a low p-value, determining that doula support reduces pain during labor in Hispanic women Conclusion Cultural proficiency plays a crucial role in providing effective care, particularly in the context of nursing, because nurses often have the initial interaction to identify cultural distinctions among patients. Hence, the importance of addressing the gap in research concerning the impact of doula support on labor pain among Hispanic women, as their preferences may be influenced by their cultural customs

    A Culturally Sensitive and Intersectional Approach to Healthcare Among Latinx Communities

    Various factors, including immigration status, language barriers, age, and socioeconomic status, influence healthcare service utilization among the Latinx population. This study seeks to assess whether culturally sensitive care addressing intersectional factors enhances health access and outcomes in low-income Latinx communities (Purpose). The research proposes a longitudinal quasi-experimental study involving 400 Latinx participants aged 18 and above in low-income urban Los Angeles neighborhoods. A control group of 200 participants and an intervention group of 200 will be established, with the latter receiving culturally sensitive interventions when seeking care during the study. A pre-study survey will collect demographic information, health history, healthcare utilization, patient satisfaction, barriers, challenges, and open-ended comments. After the survey, participants will continue their usual activities, possibly receiving care. After 12 months, a post-study survey will be conducted, covering pre-study elements and adding health outcomes, feedback, and overall satisfaction (Method). The study anticipates finding that intersectional factors, such as immigration status, impact healthcare utilization irrespective of health insurance status. The chi-squared test is expected to exceed the critical value, indicating a significant difference between control and intervention groups. Overall, the study anticipates demonstrating that culturally sensitive healthcare focusing on intersectional factors leads to better health outcomes and improved access (Results). This research underscores the imperative for healthcare equity in Latinx communities, emphasizing that culturally sensitive healthcare enhances health outcomes, patient satisfaction, and access—a fundamental human right (Conclusion)

    This piece is set in Kittery Point, ME, where my cousins lived, a place in which I spent many summers growing up. I define these summers as pinpoints in my youth that helped me discover the first touches of independence away from my home in Pennsylvania. All of the time I spent alone was prominent for what I remember of this time. I was incredibly shy and detached from my cousins\u27 friends. Though I loved being a young teenager in Maine, I could never quite grasp the social life similar to Wyatt when he is back home in Kittery from Boston to take care of his sick Uncle who suffers from the symptoms of a traumatic brain injury. Wyatt has fallen out of love with his life and back into the twisted and distant love of his high school sweetheart. With his past suddenly at the forefront, Wyatt is forced to navigate a harsh winter on his journey to find hope and agency in his life

    The Impact of Mutant Protein Disulfide Isomerase A3 and p21-Positive Cell Clearance on Cognitive Function in Mouse Models

    The protein disulfide isomerases (PDIs) are a family of enzymes within the endoplasmic reticulum (ER) that catalyze disulfide bond formation in proteins. This posttranslational modification— a process termed redox protein folding— regulates protein function and structural stability. The genetic loss-of-function mutation c.170G\u3eA in PDIA3 causes a severe intellectual disability (ID) during childhood. To understand how PDIA3 c.170G\u3eA causes this disease, we engineered a knockin mouse model, Pdia3 c.170G\u3eA, using CRISPR/Cas9 technology to model the endogenous disease pathology more accurately. We hypothesized that the ID pathology and behavioral deficits would be present in the Pdia3 c.170G\u3eA knockin mice. In congruence with our hypothesis, assessment of recognition memory, long-term spatial memory, and contextual memory showed a deficit in the Pdia3 c.170G\u3eA heterozygous mice, constituting abnormal behavior reminiscent of the human condition. The spontaneous working memory, exploration/anxiety behavior, general mobility/activity, and weight of these mice, however, were not statistically significantly altered by the mutation. Additionally, we performed proteomics analysis to understand the molecular mechanisms for the impaired learning of the Pdia3 c.170G\u3eA knockin mice. We found pathways enriched to respiratory electron transport, ATP biosynthesis, Alzheimer’s disease, tricarboxylic acid cycle, Huntington’s, and Parkinson’s disease. Overall, our results demonstrated we could model aspects of severe ID with the Pdia3 c.170G\u3eA knockin mouse model. Consequently, the first part of this thesis provides foundational research for the analysis of redox protein folding and the result of stunting PDIA3 function, which is a pivotal part of the redox protein folding machinery. Cellular senescence is a post-mitotic state in which cells secrete proinflammatory factors, may lose cell-type-specific functions, and become apoptosis-resistant. Although senescence has useful biological functions in development and cancer immune response, an accumulation of senescent cells is associated with most age-related diseases including dementia. The first stage in cellular senescence is cell-cycle arrest, induced by either the p16/ retinoblastoma (Rb) or the p21/p53 pathway. Although a great deal of research has been done to understand the p16/Rb pathway, much less is known about the p21/p53 pathway. A recently generated p21-clearance mouse model suggests the removal of p21-positive cells is beneficial in bone aging models. However, how removal of p21 cells impacts cognition is unknown. We therefore hypothesize that the chemical induction of cellular senescence in this mouse model will result in cognitive deficits that may subsequently be ameliorated by the removal of p21-positive cells in the brain. In this pilot project, we attempted to induce brain cell senescence in mice using doxorubicin to study its cognitive impacts. We then measured recognition and spatial memory, general mobility and activity, and weights after doxorubicin treatment. We found no statistically significant trends of decline in long-term spatial memory as a result of doxorubicin treatment and no noticeable deficits in recognition memory, weights, or general mobility and activity. This pilot study concludes that doxorubicin does not have a statistically significant impact on cognitive function in this particular mouse system. Since the trend for our study was toward decreased cognitive function in doxorubicin-treated mice, we may need to use a larger number of mice to make a final conclusion

    ICU Nurse Mental Health: Telehealth vs In-Person Therapy

    Background Nurses working within high-stress specialties, such as Intensive Care Units (ICUs), often face a lack of resources aimed at supporting their mental health. The expectation for the nurses is to independently manage their psychological well-being based on their knowledge and educational background. However, many times it is difficult to manage persistent stress, anxiety, or depression and this could adversely affect the quality of patient care and eventually lead to burnout. Telehealth therapy has become more readily available and used since the COVID-19 pandemic, leading to two forms of treatment delivery. Currently, there is limited research on the effectiveness of telehealth compared to the traditional form of in-person office visits. Objective The primary aim of this literature review and proposal is to improve the mental well-being of ICU nurses while assessing and comparing the effectiveness of telehealth versus in-person therapy in reducing anxiety. The literature review summarizes findings from six studies, categorizing them according to their effect on mental health and the benefits and barriers associated with in-person and telehealth therapy. Method This research proposal will encompass a mixed-methods study involving 25 participants for individual cognitive behavioral therapy, incorporating both qualitative and quantitative approaches. Participants will be recruited from hospitals with ICU departments through emails/flyers. Data will be collected pre- and post-intervention through questionnaires and surveys along with two test tools called the Generalized Anxiety Disorder Scale (GAD-7) and the Maslach Burnout Inventory (MBI).     Effects of California’s Mandated Staffing Ratios

    California passed Mandated Staffing Ratio legislation (A.B. 394) in 2002 to regulate nurse-to-patient ratios in acute care hospitals to address nursing burnout, adverse patient outcomes, and errors. Ratios were determined by the California Health Department.
