Use of Passive Heat and Moisture Exchangers in Invasively Ventilated Patients
Get PDFThis Master's thesis deals with the use of passive heat and moisture exchangers (HME filters) in invasively ventilated patients. The theoretical part deals with the physiology of humidification and describes the consequences of insufficient and excessive humidification of the airways. It also summarizes the history of the development of HME filters and discusses other available methods of airway humidification. One of the chapters discusses in more detail the use of HME filters in clinical practice, focusing on the differences between different types of filters and contraindications to their use. The final part of the theoretical section describes the findings regarding the frequency of filter replacement. The aim of the practical part was to map the current practice of using passive heat and moisture exchangers in invasively ventilated patients in anaesthesiology and resuscitation departments in the Czech Republic and to verify whether their use is in accordance with the latest recommendations of professional sources. Specifically, this was a cross-sectional study conducted through an anonymous online questionnaire distributed to head and station nurses of ARO units. Data collection was conducted over a three-month period and resulted in data from 71 units, which showed that passive humidification...Tato diplomová práce se zabývá použitím pasivních výměníků tepla a vlhkosti (HME filtrů) u invazivně ventilovaných nemocných. Teoretická část se věnuje fyziologii zvlhčení vzduchu a popisuje důsledky nedostatečného a nadměrného zvlhčení dýchacích cest. Dále shrnuje historii vzniku HME filtrů a věnuje se i jiným metodám zvlhčení dýchacích cest, které jsou k dispozici. Jedna z kapitol se podrobněji věnuje použití HME filtrů v klinické praxi, zaměřuje se na rozdíly mezi jednotlivými druhy filtrů a na kontraindikace jejich použití. Závěrečná část teoretické části popisuje poznatky týkající se frekvence výměny filtrů. Cílem praktické části bylo zmapovat aktuální praxi používání pasivních výměníků tepla a vlhkosti u invazivně ventilovaných nemocných na anesteziologicko-resuscitačních pracovištích v České republice a zároveň ověřit, zda je jejich použití v souladu s nejnovějšími doporučeními odborných zdrojů. Jednalo se konkrétně o průřezovou studii realizovanou prostřednictvím anonymního online dotazníku, distribuovaného vrchním a staničním sestrám ARO pracovišť. Prevention of ventilator-associated pneumonia - a clinical guideline
Get PDFThis diploma thesis focuses on the prevention of ventilator-associated pneumonia (VAP) and the development of a clinical guideline. Within this guideline, five priority areas have been defined: patient positioning, cuff pressure management, suctionig of the oral cavity, lower respiratory tract and subglottic space, oral hygiene and ventilation circuit hygiene. The thesis develops a standardized clinical guideline for the prevention of VAP, as no unified approach currently exists in the Czech Republic. Healthcare professionals follow various preventive care bundles. Based on a comprehensive literature review, a clinical guideline for the prevention of VAP has been created. Its aim is to reduce the incidence of VAP and to facilitate nursing care. Additionally, the guideline seeks to unify the key priority areas in VAP prevention. The theoretical part provides an overview of ventilator-associated pneumonia, its prevention strategies, existing clinical practice guidelines, and opportunities for education in VAP prevention. The empirical part, based on the literature review, consists of the developed clinical guideline for VAP prevention, presented in both textual and visual form. V rámci klinického doporučeného postupu bylo stanoveno pět prioritních oblastí, které se skládají z polohování pacienta, tlaku v obturační manžetě, odsávání z dutiny ústní, dolních cest dýchacích a subglotického prostoru, hygieny DÚ a ventilačního okruhu. Diplomová práce zpracovává ustálený klinický doporučený postup pro prevenci VAP, a to z důvodu, že v České republice neexistuje sjednocený postup a zdravotnický personál se řídí několika preventivními balíčky. Výstupem práce za pomoci literárního přehledu (rešerší) vznikl klinický doporučený postup prevence ventilátorové pneumonie. Měl by sloužit ke snížení výskytu VAP a usnadnění ošetřovatelské péče. V neposlední řadě by měl sjednotit prioritní oblasti pro prevenci VAP. V teoretická části se obecně popisuje ventilátorová pneumonie, oblasti prevence VAP, dostupné pokyny pro klinickou praxi a možnosti vzdělávání v oblasti prevence VAP. Comparative analysis of disinformation narratives in the 2017 and 2021 elections to the Chamber of Deputies of the Parliament of the Czech Republic on Facebook
No full textThe work focuses on the analysis of disinformation narratives in Facebook posts of a total of five political actors (ANO 2011, KSČM, ODS, Piráti, SPD) in the period before the elections to the Chamber of Deputies of the Parliament of the Czech Republic in 2017 and 2021. The primary research method is a qualitative thematic analysis of disinformation narratives, which, after detailed processing, was supplemented by a comparative analysis of the development of narrative strategies in both periods under study. The posts published on the official Facebook pages of selected political movements or political parties in the time interval of fourteen days before the set start of the elections to the Chamber of Deputies in the respective years are examined. The analysis shows a shift in the thematic areas used to construct disinformation narratives, further follows a shift in framing in posts containing disinformation narratives and also monitors how the perception of posts containing disinformation narratives has changed through their reach. The work contributes to a better understanding of the use of disinformation in political communication in electoral processes and points to changes in the content and style of rhetoric in the pre-election period. The topic that the work deals with is very attractive...Práce se zaměřuje na analýzu dezinformačních narativů ve facebookových příspěvcích celkem pěti politických aktérů (ANO, KSČM, ODS, Piráti, SPD) v období před volbami do Poslanecké sněmovny Parlamentu České republiky v letech 2017 a 2021. Primární výzkumnou metodu představuje kvalitativní tematická analýza dezinformačních narativů, která byla po podrobném zpracování doplněna komparativní analýzou vývoje narativních strategií v obou zkoumaných období. Zkoumány jsou příspěvky, zveřejněné na oficiálních facebookových stránkách vybraných politických hnutí či politických stran v časovém intervalu čtrnácti dnů před stanoveným začátkem voleb do Poslanecké sněmovny v příslušných letech. Analýza ukazuje posun v tematických okruzích využívaných pro konstrukci dezinformačních narativů, dále sleduje posun v rámování v příspěvcích obsahující dezinformační narativy a také sleduje, jak se proměnilo vnímání příspěvků obsahující dezinformační narativy prostřednictvím jejich dosahů. Práce přispívá ke zdárnějšímu pochopení využívání dezinformací v politické komunikaci ve volebních procesech a poukazuje na změnu obsahu a stylu rétoriky v předvolebním období. No full textThis thesis examines volatility spillovers among East Asian semiconductor com- panies, regional exchange rates, and relevant energy and metal commodities from 2011 to 2025. Using the Diebold-Yilmaz connectedness framework on weekly range-based volatilities, we estimate static and dynamic spillovers be- tween these assets and visualize the underlying network at several points in time. Static results show modest spillovers, but the dynamic index ranges from 43% to 73%, peaking at the onset of COVID-19. Currencies, especially the Korean Won, act as persistent volatility transmitters, while West Texas Inter- mediate crude is usually a receiver. Silicon shows limited connections to the other assets, but significant spillovers to SMIC, Samsung, and TSMC, during the silicon price shock in 2021. To a lesser extent, we found similar results for copper during its price rally in 2021. Our findings highlight the tangible impact of commodity price swings and exchange rate volatility on the semiconductor companies in the region. Phenylketonuria in the lives of selected groups of children, students and adults
Get PDFThe bachelor thesis deals with phenylketonuria in the lives of selected groups of children, students and adults. The aim of the thesis is to find out what problems phenylketonurics face. The thesis is divided into theoretical and practical parts. The theoretical part of the thesis deals with the history of phenylketonuria and newborn screening. Also heredity, diagnosis of phenylketonuria and its treatment. It also deals with selected problems in phenylketonuria. These selected issues include the financial demands of diet, food availability, low protein diet, menu planning and design and phenylalanine calculation, the impact of phenylketonuria on the life and education and pregnancy of the phenylketonuria patient. The second part of the thesis is a research investigation to find out the problems faced by phenylketonuric patients and their families. This research investigation is conducted by telephone interviews directly with adult phenylketonuria patients or with parents of minor phenylketonuria patients. The results of the research show which problems most bother phenylketonurics and their families. In the discussion of the paper, possible solutions are suggested that could help eliminate or at least partially relieve some of the problems. Cílem práce je zjistit, s jakými problémy se fenylketonurici potýkají. Práce je rozdělena na teoretickou a praktickou část. Teoretická část práce se zabývá historií fenylketonurie a novorozeneckým screeningem. Také dědičností, diagnostikou fenylketonurie a její léčbou. Dále se zabývá vybranými problémy při fenylketonurii. Mezi tyto vybrané problémy patří finanční náročnost diety, dostupnost potravin, nízkobílkovinná dieta, plánování a sestavování jídelníčku a výpočet fenylalaninu, dopady fenylketonurie do života a vzdělávání a těhotenství pacientky s fenylketonurií. Druhou částí práce je výzkumné šetření, které zjišťuje, s jakými problémy se potýkají fenylketonurici a jejich rodiny. Toto výzkumné šetření probíhá pomocí telefonních rozhovorů přímo s dospělými pacienty s fenylketonurií nebo s rodiči nezletilých pacientů s fenylketonurií. Výsledky výzkumu ukazují, které problémy nejvíce trápí fenylketonuriky a jejich rodiny. V diskusi práce jsou navržena možná řešení, která by mohla některé problémy pomoci odstranit nebo alespoň částečně od nich ulevit. Abstract: This bachelor thesis deals with the possibilities of using the principles and methods of garden therapy in occupational therapy. It explores gardening and planting from the perspective of the occupational therapist, and relates these activities to meaningful occupation.
