Efficacy and safety of branded vs generic lacosamide in epilepsy: a retrospective real-world study
Purpose Lacosamide (LCS) is a third-generation antiseizure medication (ASM) approved for focal-onset seizures and generalized
epilepsy. Although the branded formulation, Vimpat®, has shown efficacy and safety, the introduction of generic
versions, such as Stutan®, raises concerns about clinical equivalence, especially considering the potential for therapeutic
fluctuations that could result in breakthrough seizures or adverse events. This study aimed to compare the real-world efficacy,
safety and tolerability of branded lacosamide (Vimpat®) versus its generic counterpart (Stutan®) in patients with focal or
generalized epilepsy.
Methods A multicenter, retrospective, observational study was conducted at two epilepsy centers in Southern Italy. Sixty
adult patients were included and divided into two groups: Group A (n = 30) received branded LCS and Group B (n = 30)
received the generic formulation. Data were collected at treatment initiation (T0) and the first follow-up (T1), including
seizure frequency, adverse events and dose adjustments. The primary outcome was the responder rate (≥ 50% reduction in
seizure frequency), with secondary outcomes including seizure freedom, adverse events and dose changes.
Results Baseline characteristics were similar between groups. The average daily LCS dose was significantly higher in the
Vimpat® group (275 ± 121 mg) compared to the Stutan® group (168 ± 89 mg, p < 0.001). Despite this, efficacy outcomes
were comparable, with 60.0% of patients in Group A and 43.3% in Group B achieving a ≥ 50% seizure reduction (p = 0.08).
Adverse events were mild or moderate.
Conclusions In this real-world setting, generic LCS (Stutan®) demonstrated comparable efficacy, safety and tolerability to
Vimpat®, supporting its clinical use as a valid alternative in epilepsy management
Julius Evola: dal Dada ai miti della ‘T’radizione
Nell’ambito delle relazioni politico-culturali tra Italia fascista e Germania nazionalsocialista, il contributo propone una ricostruzione del discorso mitopoietico di Julius Evola non per “fasi” – una prima di tipo artistico-letterario e una seconda più politica – ma come coerente sviluppo di un medesimo discorso, che porta l’impronta delle esperienze inziali come epigono del Dada, in una elaborazione maturata a contatto con Tristan Tzara. Si tratta di un’adesione all’avanguardia dai tratti esteriori ed estetizzanti, frutto di un coinvolgimento postumo e selettivo, seguita da una fase di speculazione filosofica, in cui l’idealismo hegeliano si combina con elementi teoretici eterogenei, finalizzati all’esaltazione filosofica dell’Io. L’immersione nella fucina ideologica del Terzo Reich avviene sotto la medesima spinta aporetica degli esordi: dal “fascismo non fascismo”, al costrutto di “razzismo spirituale”, una diade concettuale ossimorica che prende spunto dalla «dottrina di Ludwig Ferdinand Clauss, nonché dalle relazioni con altri ideologi del pensiero conservatore tedesco, di reciproca influenza. Comprehensive assessment of caregiver burden and strain in early-stage Parkinson’s disease: An exploratory study
Parkinson’s Disease (PD) is a progressive neurodegenerative disorder affecting motor and cognitive functions, reducing the quality of life (QoL) for both patients and caregivers. This study explored correlations
between clinical and psychological factors in early-stage PD patients and their caregivers, focusing on disease
burden and caregiver stress. Nineteen PD patients and their caregivers were assessed. For patients, we evaluated disease severity (Hoehn and Yahr), cognitive functions (Montreal Cognitive Assessment, MoCA), anxiety levels (Hamilton Rating Scale for Anxiety, HRS-A), and quality of life (Parkinson’s Disease Questionnaire,
PDQ-39). For caregiver, we assessed caregiver burden (Caregiver Burden Inventory, CBI), functional social
support (Family Strain Questionnaire, FSQ), and anxiety level (HRS-A). Spearman’s correlation analyzed relationships between patient factors and caregiver burden. Cognitive impairment (MoCA) negatively correlated
with patient QoL (PDQ-Cogn; r = - 0.48, p = 0.04). Caregiver anxiety (HRS-A) was positively correlated
with physical (r = 0.65, p = 0.003) and emotional stress (r = 0.57, p = 0.01), and higher family stress (FSQ).
Our study highlights the complex relationships between clinical and psychological factors in PD patients and
their caregivers. This supports the need for integrated approaches that address the clinical management of
PD and the caregivers’ psychological well-being

Estimation of Equivalent Capacitance in Three-Level NPC Inverter SiC-Based
This paper presents an algorithm to estimate the equivalent capacitance of the DC bus in a three-phase, three-level neutral point-clamped inverter. The proposed method involves the injection of a low-frequency current through the midpoint of the converter, inducing a low-frequency oscillation in the DC bus. This technique enables the estimation of the equivalent capacitance of the capacitors without the need for external equipment. The algorithm demonstrates effectiveness under different operating conditions. The analysis incorporates considerations of the limitations of the injected current. The efficacy of the proposed method is supported by experimental and simulation tests conducted using a three-phase synchronous reluctance motor driven by a three-phase, three-level inverter
Endoplasmic reticulum stress-related genes contribute to lung cancer risk: a multiomics data integration study
Background: The role of endoplasmic reticulum stress (ERS) in lung cancer remains inadequately explored, with existing studies reporting conflicting results. This study aimed to investigate the causal relationships between ERS-related genes and lung cancer risk. Methods: This study used two large-scale genome-wide association studies (GWAS) datasets on lung cancer and integrated multiomics data, including methylation, expression and protein quantitative trait loci, to determine the causal relationships between methylation, gene expression, protein abundance, and lung cancer risk via summary data-based Mendelian randomization (SMR) and colocalization analyses. The findings revealed regulatory effects of methylation on gene and protein expression. Moreover, using cancer databases, key genes and proteins with prognostic significance were validated, and their correlations with immune cell infiltration within the tumor microenvironment were examined. Results: We identified 168 methylation sites associated with lung cancer risk linked to 97 genes. Among these, 20 genes showed altered messenger RNA levels, and 9 had modified protein abundance. Reduced methylation at cg23090046 correlated with increased KLC1 transcription, protein abundance, and lung cancer risk. Similarly, reduced methylation at cg12873919 and cg13989999 was associated with elevated BCL2L1 transcription and heightened lung cancer risk. Patients with high BCL2L1 expression exhibited greater M2 macrophage infiltration, while those with high KLC1 expression exhibited greater B-cell immune suppression, with the high expression of both genes being associated with poor prognosis [KLC1: hazard ratio (HR) =1.29, 95% confident interval (CI): 1.14–1.45; BCL2L1: HR =1.17, 95% CI: 1.01–1.36]. Conclusions: This study systematically investigated the causal relationship between ERS-related genes and lung cancer risk, revealing that elevated expression of KLC1 and BCL2L1 may contribute to increased lung cancer risk and be associated with poor prognosis in affected patients

Restored Intensities from Customized Crops of NMR Experiments (RICC-NMR) to Gain Better Insight on Chemometrics of Sicilian and Sardinian Extra Virgin Olive Oils
The paper describes the application of mono-dimensional high-resolution nuclear
magnetic resonance (NMR) spectroscopy to extra virgin olive oil (EVOO) samples to extract
the chemical composition reasonably featured by specific genotype (cultivar) and detailed
environmental conditions (terroir). To achieve this goal, we designed a suitable spectroscopic protocol made up of four NMR experiments: (I) standard 1H{13C}, (II) multiple
pre-saturated 1H{13C}, (III) 1H selective excitation at 9.25 ppm, and (IV) 13C{1H} acquisition.
The three 1H-NMR experiments (I–III) were merged into a single informative 1H-NMR
trace. This “Restored Intensities from Customized Crops” (RICC-NMR) allowed us to
extract, in just one 1H-NMR dataset, combined information about (a) main components,
(b) less-represented components, and (c) minor but key-ruling secoiridoid species, respectively. Selected integrations of the RICC-NMR trace, together with selected integrations
gathered from the 13C-NMR profile, led, for each sample, to the quantification of variables
able to sort out distinct EVOOs. In this paper, this original methodology was applied to
rationalize eighteen EVOOs from Sicily and nineteen from Sardinia, which were chemometrically compared and discussed
