Cystopexy raises the post-operative complication rate during laparoscopic hysterectomy for uterine prolapse
Objectives: This study aims to compare the outcomes of laparoscopic colposuspension sec Shull (LCSS) and laparoscopic colposacropexy (LCSP) with and without the addition of cystopexy for the treatment of pelvic organ prolapse (POP) in terms of postoperative complications, recurrence rates, and overall effectiveness. Materials and methods: A retrospective case-control analysis was conducted on women treated for grade 3–4 POP-Q uterine prolapse at the Academic Departments of Gynaecology and Obstetrics of “G. Martino” of Messina, Italy, and “L. Vanvitelli” of Napoli, Italy, between November 2020 and February 2022. Group A consisted of patients who underwent laparoscopic hysterectomy followed by LCSS or LCSP without cystopexy. At the same time, Group B included patients who had the same procedures with the addition of cystopexy. Data on complications were collected using the Clavien-Dindo classification, and prolapse recurrence was monitored according to the POP-Q system. Statistical analysis was performed using Fisher's exact, Chi-squared, and Wilcoxon rank-sum tests. Results: A total of 148 patients were included, with 125 in Group A and 23 in Group B. Group B showed a significantly higher rate of postoperative complications (16%) compared to Group A (2.4%) (p = 0.016), with an Odds Ratio of 7.62 (95% CI 1.59–36.51, p = 0.0017). No significant difference between the groups was found in the recurrence rate of prolapse at 24 months (p > 0.9). Conclusion: Adding cystopexy to LCSS or LCSP increases the risk of postoperative complications without reducing prolapse recurrence rates. Further research is needed to identify patients who may benefit from cystopexy and to evaluate its impact on stress incontinence and patient satisfaction

A 36-kV 12-Mvar T-STATCOM Based on Mixed Si-WBG Open End Winding Multilevel Converter
In 2021, Terna S.p.A. introduced a 36 kV connection standard for renewable energy plants (10-100 MW), optimizing costs, simplifying connections, and reducing substation space requirements. This allows multiple producers to connect to shared bays near 380-60kV/36kV substations, promoting efficiency and scalability in renewable energy integration. The initiative was prompted by the increasing demand for renewable energy plants to connect to the National Transmission Grid (RTN). To regulate AC voltage at the 36 kV Point of Connection (PoC), this paper proposes a transmission-type STATCOM with an open-end winding multilevel converter. The design uses silicon IGBTs in a five-level neutral point clamped (NPC) converter, switching at line frequency (50 Hz) to minimize switching losses, and SiC MOSFETs in a two-level converter, enhancing efficiency due to their low switching and conduction losses. The STATCOM includes a coupling transformer with an open-end secondary winding, eliminating the need for additional series filter reactors. This solution aims to facilitate the integration of wide-bandgap (WBG) devices in grid-connected renewable energy systems like SiC MOSFETs limited at 3.3kV, 1kA. Preliminary simulations show that the design, when applied to a 12 MVAR, 11 kV STATCOM connected to a 36 kV transmission line, exhibits high efficiency, compactness, and enhanced performance, paving the way for the adoption of WBG technology in renewable energy applications

Protein hydrolysates from Hermetia illucens trigger cellular responses to cope with LPS-induced inflammation and oxidative stress in L-929 cells
Insect protein hydrolysates (PH) are emerging as valuable compounds with biological activity. The aim of the present study was to assess the potential cytoprotective effects of PH from the Black Soldier Fly (BPH, in the range 0.1–0.5 mg/mL) against inflammatory conditions and oxidative stress in LPS-challenged L-929 cells. BPH was effective in inhibiting LPS-induced ROS and nitrite production and in reducing the protein and transcript levels of remarkable inflammatory markers, such as TNF-α, IL-6, IL-1α, and IL-1β, as determined by ELISA and/or qPCR. Moreover, the BPH antioxidant and anti-inflammatory activities rely on the induction of selected genes and proteins involved in the antioxidant response (i.e. Cu/ZnSod, MnSod, Gpx, HO-1) through Nrf2, as well as on the inhibition of the activation of NF-κB, a key player in inflammation. These findings suggest that BPH represents effective bioactive compounds with therapeutic potential for mitigating oxidative stress and inflammation in vitro, thus deserving further investigation into the underlying mechanisms before BPH application as novel drugs in the near future
Body understanding measure for pregnancy scale (BUMPS): Psychometric properties and predictive validity with postpartum anxiety, depression and body appreciation among Italian peripartum women
Pregnancy is a unique phase in a woman's life marked by profound physical transformations, including changes in body shape and weight. The Body Understanding Measure for Pregnancy Scale (BUMPs) was designed to assess body image during pregnancy. Despite its increasing use, the scale has not yet been adapted into Italian, and evidence regarding its predictive validity with respect to anxiety, depression, and body appreciation is lacking. This study aimed to address these gaps to validate the Italian BUMPs and test its predictive validity. A community sample of 726 Italian pregnant women was recruited (age range 18–48, Mage= 31.3 ± 4.79). Participants completed a translated BUMPs and other self-report questionnaires assessing anxiety, depression, and body appreciation. Confirmatory factor analysis supported a three-factor structure for the BUMPs, with dimensions assessing Satisfaction with Appearing Pregnant, Weight Gain Concerns, and Physical Burdens of Pregnancy. BUMPs subscales demonstrated satisfactory internal consistency (ω = 0.765–0.866). Cross-sectional analysis revealed that BUMPs scores correlated with anxiety (r range from 0.25 to 0.32), depression (r range from 0.31 to 0.34), and gestational body mass index (r range from 0.18 to 0.37). Longitudinal analysis associated BUMPs with anxiety, depression, and body appreciation measured after childbirth, providing evidence of predictive validity. Overall, the present study supports the BUMPs as a valid and reliable tool for assessing body image during pregnancy within the Italian context. Additionally, it provides the first evidence of the BUMPs' predictive validity for postpartum mental health outcomes and body appreciation after childbirth
Assessment of sleep bruxism and its association with sleep quality, stress levels, and dental health in pediatric population: a multifactorial analysis
Background: Bruxism is defined as the recurring muscular activity of the jaw, which is distinguished by the unconscious grinding or clenching of teeth or pushing and forcing of the lower jaw. The present study was conducted to assess sleep bruxism and its association with sleep quality, stress levels, and dental health in the paediatric population. Methodology: 384 children aged 6–12 years were recruited for the study. Ethical approval from the institutional ethical committee and informed consent from parents and guardians were obtained. Children with systemic diseases, mental or developmental disorders, CNS-affecting medications, or those whose parents did not provide consent were not included. Bruxism was identified using the American Academy of Sleep Medicine (AASM) criteria through parental reports. Sleep quality and stress levels were assessed using AASM guidelines and the Perceived Stress Scale for Children (PSS-C). A chi-square test was applied to assess association, with a p value < 0.05 considered to be significant. Results: The number of siblings and the mother’s education level were also significantly associated. It was found that sleep bruxism is significantly associated with all parameters of sleep quality except the usage of pillows. When stress levels among children were assessed, it was found that 60 (15.6%) children had very low stress, 131 (34.1%) children had low stress, 95 (24.7%) children had average stress, 92 (24%) children had high stress, and 6 (1.6%) children had a very high level of stress. A statistically highly significant (p = 0.001) association was found between bruxism and stress levels among children. Conclusion: The results of the present study concluded that sleep bruxism was significantly linked with sleep quality and stress levels among children, which ultimately affect dental health
Management of Systemic Anti-psoriatic Drugs in Psoriasis Patients with Concurrent Paraplegia or Tetraplegia: Insights From a 6-Year Multicenter, Retrospective Observational Study
Introduction: Patients with psoriasis (PsO) and permanent spinal cord injuries (SCI) resulting in paraplegia and tetraplegia may experience a higher rate of infections compared to patients with PsO without SCI. It can result in further challenges for therapeutic management with immunosuppressants (biological and non-biological treatments). Thus, we aimed to evaluate the rate of infections in patients with PsO and SCI treated with systemic immunosuppressants. Methods: This multicenter, retrospective observational study enrolled patients with PsO and traumatic SCI undergoing systemic immunosuppressive treatments for at least 5 years. All patients were evaluated by experienced, board-certified dermatologists and neurologists. Demographic and clinical data were collected. Results: We enrolled 23 patients with SCI (16 with paraplegia and 7 with tetraplegia) treated with methotrexate (MTX) and different biologics (tumor necrosis factor (TNF) inhibitors (i) and interleukin (IL)-17i/IL-23i). Globally, patients with SCI treated with MTX displayed higher rates of infection compared to those treated with biologics. Patients with paraplegia had lower rates of infection compared to patients with tetraplegia during anti-psoriatic therapies (p < 0.05). Those treated with TNFi had greater rates of infection than those treated with IL-17i/IL-23i (p < 0.001). Patients with psoriatic arthritis (PsA) experienced a significant diagnostic delay and clinical monitoring of PsA severity was challenging. Conclusion: In patients with moderate-to-severe PsO and concurrent traumatic SCI, dermatologists should consider using IL-17i/IL-23i as first-line therapy
Synthesis, characterization and catalytic application of 2D framework consist of La3+ ions decorated ceric A-sandwiched polyoxometalate and Cu2+ derivative
New inorganic 2D framework of H2[{La3(H2O)21}P2W18Ce3O70(OH)(H2O)3]∙24H2O (La3P2W18Ce3) and Cu[{La3(H2O)21}P2W18Ce3O70(OH)(H2O)3] (CuLa3P2W18Ce3) were synthesized and characterized by FT-IR, X-ray single-crystal diffraction, PXRD, FESEM-EDX and Uv-Vis spectroscopy. The X-ray single-crystal diffraction revealed that La3P2W18Ce3 structure consist of Ce4+ sandwiched A-type polyoxometalate which connected together via different aqua coordinated La3+ ions to form a 2D inorganic framework. The insoluble Cu2+ derivative, CuLa3P2W18Ce3 was synthesized by addition of Cu2+ acetate salt to the La3P2W18Ce3 sparingly soluble. The as-synthesized inorganic frameworks were used as heterogeneous catalysts to promote the synthesis of α-acyloxy amides via the well-known Passirini reaction and synthesis of bis-amides via the well-known Ugi under mild conditions
