Furosemide and Serum Protein-Bound Uremic Toxin Concentrations in Patients With CKD
International audienceIntroduction: Furosemide is commonly prescribed to patients with chronic kidney disease (CKD) but may impair the kidney's excretion of protein-bound uremic toxins (PBUTs) via the organic anion transporters 1 and 3 (OAT1 and OAT3). We evaluated the association between furosemide prescription (status and dose level) and the serum concentrations of free OAT1/3-inhibiting uremic toxins (UTs) in patients with CKD.Methods: We included 2342 patients with CKD (stages 2-5) from the CKD-Renal Epidemiology and Information Network (CKD-REIN) cohort and with centralized serum UT assay data at baseline. The UTs were assayed using liquid chromatography - tandem mass spectrometry. The OAT1/3-inhibiting UTs identified in a literature review included indoxyl sulphate (IS), kynurenine (Kyn), p-cresyl sulphate (PCS), and indole-3-acetic acid (IAA). Multiple linear regression was used to assess each PBUT or their sum ( ΣUTsfree) as the dependent variable.Results: Patients prescribed furosemide (n = 799, 34%) were older and had a lower estimated glomerular filtration rate (eGFR), a higher C-reactive protein (CRP) concentration, more comorbidities, and more concomitant medications than patients not prescribed furosemide. After adjustment for potential confounders, patients prescribed > 120 mg furosemide had significantly higher serum concentrations of ΣUTsfree (+19.1%), IS (+31.9%), Kyn (+9.3%), PCS (+29.3%), and IAA (+162.9%) than patients not prescribed furosemide. Using a smooth function to model the association between the furosemide dose level and PBUTs, we observed (for ΣUTsfree and each free UT) a steep increase between 80 and 100 mg and then a high plateau.Conclusion: In patients with CKD, furosemide (particularly at a dose level > 120 mg) is independently associated with higher serum free PBUT concentrations. Our findings suggest that drug-UT competition contributes to PBUT accumulation
Development of in-house ELISA for Scedosporium/Lomentospora serodiagnosis
International audienceScedosporium/Lomentospora species are ubiquitous molds that can cause deep-seated infections and allergic bronchopulmonary mycoses (ABPM). Serodiagnosis is currently performed by immunoprecipitation techniques (IP), which are time-consuming and lack reproducibility. In addition, as antigenic extracts for these fungi are not commercially available, many centers stopped performing this analysis. Therefore, there is a need for automated quantitative alternatives, such as ELISA. The aim of this study was to develop an ELISA for serodiagnosis of Scedosporium/Lomentospora infections. All sera received for Scedosporium/Lomentospora serodiagnosis expertise from April 2022 to February 2024 were tested in parallel using IP and an in-house ELISA with antigenic extracts from both S. apiospermum and L. prolificans. Clinical and biological data such as positive culture, total IgE level and final diagnosis retained were also collected prospectively. The concordance between techniques was calculated, with chi² tests performed to investigate the correlation between ELISA and culture results or final diagnosis. We tested 58 serum samples from 41 different patients. The concordance between IP and ELISA was 64% for S. apiospermum and 62% for L. prolificans. ELISA results obtained with S. apiospermum antigen extract were significantly correlated with culture results (p < 0.001, chi² test). ELISA was also more effective than IP to diagnose ABPM. The Scedosporium/Lomentospora ELISA gave satisfactory results, particularly for S. apiospermum. Further validation on a larger cohort is required implementing this ELISA for routine practice instead of IP. In addition, studies should be conducted on purified native proteins or combination with recombinant antigens to improve standardization

International audienceThis article aims to study the relationship between agility and open innovation within high-tech SMEs. With an exploratory aim, it relies on a case study of an SME specializing in consulting and engineering in high technologies. Our results highlight a dynamic interconnection between the two approaches of open innovation and agility, providing an in-depth understanding of their mutual influence in this context. Thus, we demonstrate that the capabilities of agility can stimulate and support inbound and outbound innovation practices, while highlighting that open innovation itself fosters these capabilities. However, it is important to note that agility is not enough to overcome all the barriers that open innovation faces within high-tech SMEs.Cet article se propose d’étudier la relation entre l’agilité et l’innovation ouverte au sein des PME de haute technologie. À visée exploratoire, il s’appuie sur l’étude de cas d’une PME spécialisée dans l’ingénierie, le conseil en informatique et l’innovation technologique. Nos résultats mettent en évidence une interconnexion dynamique entre les deux approches de l’agilité et de l’innovation ouverte, offrant une compréhension approfondie de leur influence mutuelle dans ce contexte. Ainsi, nous démontrons que les capacités de l’agilité peuvent stimuler et soutenir les pratiques d’innovation entrante et sortante, tout en soulignant que l’innovation ouverte contribue à les favoriser. Cependant, il est important de noter que ces capacités ne suffisent pas à surmonter toutes les barrières auxquelles la PME est confrontée dans le domaine de l’innovation ouverte

Maisons-Alfort : Anses, 44 p.L’Anses a été saisie, le 19 juillet 2023, par la Direction générale du travail (DGT) pour la réalisation de l’expertise suivante : « Evaluation des indicateurs biologiques en vue de la recommandation de valeurs limites biologiques et de valeurs d’imprégnation populationnelle pour la surveillance biologique des expositions professionnelles à l’oxyde d’éthylène ».Dans le cadre du protocole d’accord entre l'Anses et le ministère du travail pour la mise en œuvre du programme de travail d'expertise scientifique en matière de valeurs limites atmosphériques et biologiques pour les expositions professionnelles, établi en juillet 2018 et reconduit en 2023, la direction générale du travail (DGT) a saisi l’Anses, dans un premier temps, afin d’identifier, à partir des recommandations existantes, un ou plusieurs indicateurs biologiques d’exposition (IBE) et les valeurs biologiques associées pouvant être utilisés pour la surveillance des expositions professionnelles à l’oxyde d’éthylène.A cet effet, il est demandé à l’Anses :- d’évaluer la pertinence des IBE déjà proposés par les organismes reconnus aux niveaux européen et international pour cet agent chimique (les adduits à l’hémoglobine N-(2-hydroxyéthyl)valine ou HEV dans le sang et l’acide S-(2-hydroxyéthyl)mercapturique ou HEMA dans les urines), et identifier, le cas échéant, d’autres IBE (et/ou indicateurs biologiques d’effets) potentiels ;- d’évaluer les valeurs sanitaires et valeurs issues des données d’imprégnation en population générale proposées par ces organismes, à partir des données scientifiques les plus récentes ;- de préciser les facteurs pouvant influencer l’interprétation des résultats si cela n’est pas indiqué dans les travaux d’expertise de ces organismes (ou si des incohérences avec les données de la littérature sont mises en évidence lors de l’évaluation de l’Anses) ;- d’identifier et évaluer les méthodes de mesure (et préconiser les modalités de prélèvement de chaque IBE).Dans un second temps, il est demandé à l’Anses de mettre en œuvre une expertise collective si les résultats des travaux de la première phase en montre la nécessité
Effects of breathing a hyperoxic gas mixture on perceptual, biochemical and performance recovery following simulated soccer match play
International audienceThe aim of this study was to examine the effect of breathing a hyperoxic gas mixture on recovery kinetics after a simulated soccer match protocol. In a double-blind, randomized design, twenty-eight semi-professional soccer players completed the Loughborough Intermittent Shuttle Test (LIST) followed by the administration of either a hyperoxic (FIO2 = 99.5%: HYP) or a normoxic gas mixture (FIO2 = 21%: NORM). HYP and NORM were administered immediately after LIST and daily for the next 3 days for 15 minutes. Physical performance (squat jump: SJ, countermovement jump: CMJ, maximal voluntary contraction: MVC, and 20 m sprint: SP), blood concentrations of muscle damage markers (creatine kinase: CK, lactate dehydrogenase: LDH), marker of inflammation (C-reactive protein: CRP) and Hooper index (HI) were assessed at baseline, 15 minutes and 24 h, 48 h and 72 h following the LIST. SJ, CMJ, MVC, and SP were lower at 15 min, 24 h, and 48 h in both conditions compared to the prior LIST (p < 0.05). However, the decrease in MVC was significantly attenuated at 15 min, 24 h and 48 h in HYP compared to the NORM condition (p < 0.01). Likewise, HI, muscle soreness and fatigue scores were significantly lower in HYP compared to the NORM condition up to 48 h following the LIST (p < 0.01). The present study suggests that the application of HYP immediately after a simulated soccer match and for the next 3 days promoted a lower fatigue-induced decrement in MVC and restored perceptual parameters of fatigue in semi-professional players

The spirits conjured by Annis help her through the terrible hardships she has to undergo not only by giving her advice but also by granting her access to her own individual history and giving her the agency and empowerment that will lead to her asserting her own identity. These spirits also tell the story of those before her, of those, like her grandmother, on ships during the Middle Passage or of those who came after, giving her a new perspective on history and bringing about collective memory. This neo-slave narrative is in itself a way to mourn and memorialize the past so it is not forgotten. It can even be considered as what Pierre Nora calls a lieu de mémoire and should also be seen as part of the process of ethnogenesis.Let Us Descend met en scène la façon dont les esprits ne sont pas seulement des guides vers l'identité personnelle, mais aussi vers la construction de l'identité ethnique, ou ethnogenèse. Les esprits invoqués par Annis l'aident à surmonter les terribles épreuves qu'elle doit subir, non seulement en lui donnant des conseils, mais aussi en lui permettant d'accéder à sa propre histoire individuelle et en lui donnant l'agentivité et le pouvoir lui permettront d'affirmer sa propre identité. Ces esprits racontent aussi l'histoire de ceux qui l'ont précédée, de ceux qui, comme sa grand-mère, se trouvaient sur les bateaux lors du Passage du milieu, ou de ceux qui sont venus après, lui donnant une nouvelle perspective sur l'histoire et faisant naître une mémoire collective. Impact de la prise en charge réanimatoire initiale sur la morbi-mortalité et la qualité de vie à 1 an chez les polytraumatisés âgés graves : "traumage", une étude de cohorte rétrospective amiénoise
Background: The elderly population is continuously growing due to increased life expectancy and improved management of comorbidities, which allows for the maintenance of a level of autonomy comparable to that of younger individuals. As a result, they are exposed to similar traumatic situations. The objective of our study was to highlight a more pronounced loss of autonomy one year after trauma in polytraumatized patients over the age of 65 and to identify the contributing factors. Materials and Methods: We conducted a retrospective, single-center study involving 650 polytraumatized patients admitted to the intensive care unit at Amiens University Hospital between 2014 and 2023. We collected data on medical history, initial clinical and trauma assessments, and overall management. Loss of autonomy was defined as a worsening of at least two points on the modified Rankin Scale (mRS) at one-year post-trauma compared to the pre-event mRS score. Patients were divided into two groups based on whether they experienced a ΔRankin ≥ 2 at one-year post-trauma, for multivariate analysis. Results: We found that patients over the age of 65 had a significantly higher risk of experiencing a ΔRankin ≥ 2 one year after trauma (OR = 2.720, 95% CI [1.54–4.8], p < 0.001). Factors associated with this increase in Rankin score included higher Charlson Comorbidity Index, higher Simplified Acute Physiology Score II (SAPS II), higher Injury Severity Score (ISS) upon ICU admission, lower limb injuries, presence of shock, and greater need for blood transfusions and mechanical ventilation. Conclusion: This study highlights the impact of initial trauma management combined with comorbidities on the loss of autonomy one-year post-trauma. In addition to being a mortality risk factor, age was shown to be a predictor of long-term functional decline. It is essential to assess the risk of autonomy deterioration as part of the care pathway for polytraumatized patients over 65. This could help guide future management strategies.Contexte : la population des personnes âgés est en croissance permanente avec l’allongement de l’espérance de vie et l’amélioration de la prise en charge des comorbidités permettant le maintien d’une autonomie comparable aux jeunes et sont exposés aux mêmes situations traumatiques. Le but de notre étude était de mettre en évidence une perte d’autonomie à 1 an plus marquée chez le polytraumatisé de plus de 65 ans et d’identifier quels critères pouvaient y participer. Matériel et méthodes : nous avons réalisé une étude rétrospective monocentrique sur 650 patients polytraumatisés hospitalisés en réanimation au CHU d’Amiens de 2014 à 2023 inclus. Nous avons relevé les antécédents, le bilan clinique et traumatologique initial puis les éléments de prise en charge globale. Nous avons défini la perte d’autonomie comme une aggravation du score de RANKIN d’au moins deux points à un an du traumatisme par rapport au RANKIN avant l’évènement aigu. Puis nous avons constitué deux groupes à comparer pour l’analyse multivariée en fonction de la survenue ou non d’un Delta de RANKIN ³ 2 (D RANKIN ³ 2) à 1 an du traumatisme. Résultats : nous avons pu constater que les patients présentaient, de manière significative, un surrisque d’avoir un D RANKIN ³ 2 à un an du traumatisme lorsqu’ils avaient plus de 65 ans (OR = 2,720 IC 95% [1,54-4,8], p < 0,001). Les facteurs participants à cette augmentation de RANKIN sont un score de Charlson, un IGS 2 et un ISS plus élevé à l’entrée en réanimation, une lésion des membres inférieurs, la présence d’un état de choc, un recours plus important à une transfusion et à une ventilation mécanique. Conclusion : cette étude a permis de mettre en lumière l’impact que peut avoir la prise en charge initiale cumulé aux conséquences des différentes comorbidités sur la perte d’autonomie à 1 an du traumatisme. Nous avons pu montrer qu’en plus d’être un facteur de mortalité, l’âge est un facteur de risque de perte d’autonomie. Il est important d'évaluer le risque d’altération d’autonomie dans le parcours de soins du polytraumatisé de plus de 65 ans. Ceci pourrait possiblement orienter nos prises en charge à l’avenir
Expert Perspectives on Incorporating GLP-1 RA in Diabetes and Chronic Kidney Disease – Challenges and Opportunities
International audienceChronic kidney disease (CKD) is a complex and progressive condition ultimately leading to premature death. Diabetes is the leading cause of end-stage kidney disease worldwide. Up till 2024, international clinical guidelines have established three therapeutic pillars to delay CKD progression in people with type 2 diabetes (T2D): renin-angiotensin system inhibitors, sodium-glucose cotransporter 2 inhibitors, and the non-steroidal mineralocorticoid receptor antagonist finerenone. With the recent results from the Evaluate Renal Function with Semaglutide Once Weekly (FLOW) study, the glucagon-like peptide-l receptor agonist (GLP-1 RA) class is now considered a new therapeutic pillar in reducing CKD complications in this patient population. In this expert opinion, we identify patient populations at risk of developing new onset or worsening pre-existing CKD to explore optimal therapeutic strategies, introducing GLP-1 RAs. We highlight the important challenges that remain in optimizing, sequencing, and combining these four therapeutic pillars. Even though the conventional approach of combining the pillars has been based on the historical emergence of evidence, we discuss the factors that would influence physicians’ decision for preferring one pillar over another, and for selecting a certain combination, whether performed simultaneously or sequentially. These factors include the grade of CKD and the level of albuminuria; diabetic control (glycemia); comorbidities: atherosclerotic cardiovascular disease, heart failure, obesity; concomitant medications; biological variables: potassium serum levels. The efficacy and safety profiles of each pillar, as demonstrated in landmark trials that have clearly shown the nephroprotective effects, along with real-world data, should also be carefully considered when selecting the most appropriate therapeutic option
Intérêt du scanner dynamique dans la caractérisation des endofuites complexes : résultats préliminaires de l'étude prospective LEAKDYN
Introduction : Follow-up examinations after EVAR implantation frequently encounter difficulties in visualizing endoleaks (ELs) with complex and non-obvious mechanisms. The objective of this study is to evaluate the diagnostic performance of dynamic computed tomography angiography (d-CTA) for the characterization of complex ELs and to compare its results with CTA and Contrasted Enhanced Ultrasound (CEUS) results. Materials and Methods : This prospective monocentric study has included 30 patients with suspected ELs, aneurysm sac enlargement without identified ELs, or multiple ELs on CTA. Almost all patients had undergone EVAR for a subrenal aortic aneurysm, while two had FEVAR for a juxtarenal aneurysm. A d-CTA followed by CEUS was performed and analyzed by a radiologist and an angiologist, respectively.The primary endpoint compared the characterization of EL types between each imaging modality. Dynamic acquisition was performed on a REVOLUTION CT APEX scanner (GE Healthcare®, Milwaukee, USA) covering 16 cm of the region of interest over 27 seconds. The obtained images were analyzed in dynamic cine mode and processed using multiplanar reconstruction by the radiologist. The dose of contrast agent and radiation was compared between CTA and d-CTA. Reinterventions were performed in cases of high-risk ELs classified by d-CTA, with intraoperative confirmation. Results : Among the 30 patients, the d-CTA classified twenty EF as high-risk (p=0.06): four type 1, eight type 2 with early enhancement, and eight type 3. The d-CTA reclassified 5 EF as high-risk that were not identified by CEUS. There was no significant difference in terms of radiation and contrast volume between the imaging modalities. In 100% of the reinterventions, the EF classified by d-CTA was confirmed intraoperatively, with satisfactory clinical and radiological success in the postoperative period. Conclusion : d-CTA proves to be an essential tool for the in-depth understanding and planning of treatments for complex ELs. It enhances diagnostic accuracy and provides more reliable guidance for therapeutic interventions.Introduction : les examens de suivi après implantation d'EVAR rencontrent fréquemment des difficultés de visualisation des endofuites (EF) à mécanisme complexe et peu évident. L'objectif de cette étude est d'évaluer la performance diagnostique de l'angioscanner dynamique (d-CTA) pour la caractérisation des EF complexes, et de comparer ses résultats à ceux obtenus par l'angioscanner triphasique (CTA) et l'échographie de contraste (CEUS). Matériels et méthodes : cette étude prospective monocentrique a inclus 30 patients présentant, à partir du CTA, un doute sur une EF, un accroissement du sac anévrismal sans EF identifiée ou des EF multiples. Un d-CTA suivi d'un CEUS a été réalisé puis analysé par un radiologue et un médecin vasculaire, respectivement. Le critère de jugement principal comparait la caractérisation du type d'EF entre chaque modalité d'imagerie. L'acquisition dynamique a été réalisée sur un scanner REVOLUTION CT APEX (GE Healthcare®, Milwaukee, USA) couvrant 16 cm de région d'intérêt pendant 27 secondes. Les images obtenues ont été analysées en mode ciné dynamique et traitées par reconstruction multiplanaire par le radiologue. La dose de produit de contraste et d'irradiation a été comparée entre le CTA et le d-CTA. Des réinterventions ont été réalisées en cas d'EF à haut risque classifiée par le d-CTA, avec une confirmation peropératoire de celle-ci. Résultats : sur les 30 patients, le d-CTA a classifié vingt EF à haut risque (p=0,06) : quatre type 1, huit types 2 à rehaussement précoce et huit type 3. Le d-CTA a reclassifié 5 EF en haut risque non identifiés par le CEUS. Il n'y avait pas de différence significative en termes d'irradiation et de volume de contraste entre les imageries. Dans 100 % des réinterventions, l'EF classifié par le d-CTA était confirmée en peropératoire, avec un succès clinique et radiologique satisfaisant en post-opératoire. Conclusion : le d-CTA s'avère un outil essentiel pour la compréhension approfondie et la planification des traitements des EF complexes. Il permet d'améliorer la précision diagnostique et de guider de manière plus fiable les interventions thérapeutiques
