Endogenous endophthalmitis complicating infective endocarditis: a multicentre case-matched control cohort
International audienceAims Endogenous endophthalmitis (EE) is a rarely reported complication of infective endocarditis (IE). In an international series, we sought to determine the clinical and microbiological profile, treatment, and outcome of patients presenting with IE-related EE. Methods and results Cases recorded from 2014 to 2023 in nine centres in Europe and the United States were collected. Results were compared to a matched control group. Conclusion Sixty-six patients with EE were reported, mean age of 65.2 ± 14.9 years, 71% (n = 47) male. Blood cultures were positive in 97% (64 cases) of patients, with a predominance of streptococci (46%, n = 30). As compared with the control group (n = 264), the EE group presented with more frequent diabetes (35% vs. 21%, P = 0.02), history of cirrhosis (9% vs. 3%, P = 0.04), glomerulonephritis (15% vs. 0.4%, P < 0.001), embolism before admission (92% vs. 55%, P < 0.001), and Janeway lesions (9% vs. 1%, P = 0.002). Streptococcal infection (46% vs. 26%, P = 0.001) was more frequent and Enterococcal infection (0% vs. 18%, P < 0.001) less frequent in the EE group. The main ocular symptoms were a decrease in visual acuity (96%), red eye (55%), and ocular pain (55%). Treatment of EE consisted of intravitreal antibiotic injection in 55 (83%) patients and vitrectomy in 17 (26%). Improvement of visual acuity was observed in 36 (55%) patients. Conclusion EE is a serious complication of IE with severe residual vision impairment. Patients with IE should be evaluated for ocular complications, since early detection of EE is crucial to prevent delays in management and to preserve visual function
Exponential stabilization of quasi-one-sided Lipschitz systems with time delay
International audienceThis paper aims to design Observer-Based (OB) controllers that ensure the exponential stability of a class of nonlinear time-delay systems. The nonlinear part of the system satisfies a weak Quasi-One-Sided Lipschitz (QOSL) condition characterized by the matrices , as well as a QOSL condition characterized by the matrices . First, we derive a sufficient condition formulated as a Linear Matrix Inequality (LMI) via a Lyapunov–Krasovskii (LK) functional. The main advantage of this design is that the controller and observer gains are computed in a single step. However, its main drawback is that the matrices , , and are fixed rather than treated as decision variables. To overcome this limitation, we propose an improved design in which the matrices , and are treated as decision-variable matrices with a fixed structure. By using an appropriate decoupling technique, this approach provides greater flexibility in the selection of matrices and reduces conservatism. The efficacy of the developed OB controllers is demonstrated via a suitable numerical example
Anti-interleukines 17 : taux de maintien, efficacité, et tolérance dans le psoriasis de l’enfant. Cohorte internationale rétrospective en vie courante
International audienceIntroductionLe psoriasis sévère, justifiant un traitement systémique, touche 10 à 20 % des enfants souffrant d’un psoriasis. Les biothérapies, utilisées le plus souvent après échec d’un traitement systémique conventionnel, ont transformé leur prise en charge. L’efficacité de ces biothérapies a été évaluée dans des essais thérapeutiques sur de faibles effectifs, souvent moins de 50 enfants. Les données en vie courante sur de plus grands effectifs sont nécessaires pour mieux appréhender leur place dans l’arsenal thérapeutique. Le taux de maintien comparatif, l‘efficacité, la tolérance ont été évalués pour l’étanercept, l’adalumimab et l’ustékinumab dans les cohortes BiPe, réalisées avant l’arrivée des anti-interleukines 17 (Il17), sécukinumab (SECU) et ixékizumab (IXE).La cohorte Secu-Ped est une cohorte rétrospective visant à évaluer les anti-Il17 en vie courante dans le psoriasis de l’enfant : taux de maintien, efficacité, et tolérance.Matériel et méthodesCohorte rétrospective internationale (France, Italie, Portugal, Espagne, Pays-Bas, Argentine, et Maroc) dans laquelle étaient inclus tous les enfants (<18 ans) recevant un anti-Il17 pour un psoriasis cutané en dehors d’essais thérapeutiques. Inclusions de janvier à mai 2025. L’efficacité était évaluée sur le taux de Physician Global Assessment (PGA) 0–1 et la réduction du Psoriasis Area and Severity Index (PASI) de 75 et 90 % (PASI75 et 90) au troisième mois (M3). Seuls les évènements indésirables (EI) imputés par l’investigateur au traitement étaient retenus.RésultatsAu total, 152 enfants (âge moyen : 12,9 ± 3,4 ans ; filles : 54 % ; psoriasis en plaques : 58 % ; rhumatisme psoriasique : 9,9 %) ont été inclus dans 36 centres. Ils ont reçu 160 traitements : SECU : n = 139 ; IXE : n = 21. Le taux de maintien à 2 ans était supérieur pour le SECU (75 %) vs IXE (50 %) (p = 0,036) ; une tendance se dessinait pour un meilleur maintien chez les enfants bionaïfs avant introduction d’une 1ère cure de SECU (p = 0,10), mais il n’y avait pas de différences en fonction des groupes de poids.Le taux de PGA0-1 à M3 était de 58 % et 92 % pour le SECU et l’IXE respectivement, de PASI75 : 45 vs 70 %, et de PASI90 : 32 % et 60%.Les principaux EI étaient pour le SECU : eczéma (n = 5), candidose, exacerbation de psoriasis, bronchite, et rhinite (n = 3), et pour l’IXE : douleur au point d’injection (n = 4), rhinite, asthénie, nausées, et eczéma (n = 2)DiscussionCette cohorte internationale en vie courante d’enfants psoriasiques montre un taux de maintien supérieur pour le SECU que pour l’IXE et un niveau d’efficacité (PGA, PASI75/90) inférieur à celui rapporté dans les essais publiés. La fréquence des EI était très faible avec notamment des eczémas, rhinites sous anti-Il17, des douleurs aux points d’injection pour l’IXE, et des candidoses et exacerbation du psoriasis pour le SECU.ConclusionCes cohortes en vie courante sont indispensables afin de mieux apprécier la place respective des différents traitements dans le psoriasis de l’enfant
Efficacité et tolérance de l’infliximab dans l’hidradénite suppurée : étude rétrospective multicentrique
International audienceIntroductionL’arsenal thérapeutique de l’hidradénite suppurée (HS) comporte actuellement 3 biothérapies. Certains patients peuvent être en échec ou présenter des contre-indications à certains de ces traitements. Les données concernant l’infliximab (IFX) pour l’HS, en particulier françaises, sont limitées.Matériel et méthodesÉtude rétrospective multicentrique visant à décrire le profil des patients traités par IFX pour HS et évaluer son efficacité et sa tolérance. Critères d’inclusion : patients HS ayant reçu ≥6 mois d’IFX. Éléments recueillis : caractéristiques des patients, historique médicamenteux, sévérité de l’HS(Hurley, HS PGA), efficacité (définie par réduction ≥2 points de l’HS PGA/baseline) à 6 mois et à la dernière date d’évaluation, survie et tolérance de IFX.RésultatsAu total 86 patients étaient inclus dans 8 centres, dont les caractéristiques sont détaillées. L’IFX était la 1ère ligne de biothérapie chez 42 patients, la 2e ligne chez 22, la 3e ligne chez 9, la 4e ligne chez 10 et la 5e ligne chez 3. Le score HS PGA médian à baseline était de 4. Vint-et-un patients avaient une atteinte du visage. À 6 mois, l’HS PGA médian était de 2, 50 % des patients obtenaient une amélioration ≥2 points de l’HS PGA/baseline, 76,2 % avaient une amélioration de l’atteinte du visage. La durée médiane de traitement par IFX était de 17 mois (8, 23–30) ; 44 patients interrompaient l’IFX : échec primaire n = 10, échec secondaire n = 11, effet secondaire n = 5, perdu de vue n = 5, passage à IFX sous cutané (SC) n = 5, succès thérapeutique n = 3, demande du patient n = 2, grossesse n = 1. Les effets secondaires comprenaient : réaction anaphylactoïde à IFX n = 4, prise de poids n = 4, psoriasis paradoxal n = 3, hépatite médicamenteuse n = 2, asthénie n = 2, mononévrite multiple n = 1, lupus induit n = 1, eczéma n = 1, condylomes profus n = 1, infections ORL à répétition n = 1, céphalées n = 1. À leur dernière évaluation (après une durée médiane de 22 mois d’IFX), le HS PGA médian des 42 patients qui poursuivaient l’IFX au-delà de 6 mois, était de 2 et 81,8 % avaient une amélioration du HS PGA ≥2 points/baseline. Trente-trois patients bénéficiaient d’une modification de la dose et/ou de l’intervalle entre les perfusions d’IFX (dont 3 allègements de traitement). Quinze patients bénéficiaient d’une chirurgie large de l’HS sous IFX, sans complication post opératoire rapportée.DiscussionDans notre étude, 50 % des patients obtenaient à 6 mois une amélioration ≥2 points de l’HS PGA, taux de réponse inférieur à celui d’une méta-analyse portant sur 19 publications et 314 patients où il était de 83 % (mais les critères d’évaluation de l’efficacité étaient inhomogènes). La durée médiane de survie du traitement était de 17 mois, sans différence entre les patients traités en 1ère ou ≥2e ligne biothérapie ; elle est supérieure aux données de la littérature (33 à 58 semaines). Le profil de tolérance était celui habituel de l’IFX ; on ne notait en particulier pas de complication postopératoire pour les patients opérés sous IFX pour leur HS. Les limites de notre étude sont le caractère rétrospectif, l’hétérogénéité de sa population (ligne de biothérapie, posologie et intervalle d’administration de l’IFX, traitements associés…) rendant difficiles les comparaisons entre sous-groupes.ConclusionL’IFX est donc une option thérapeutique en cas d’HS, en particulier en échec aux biothérapies ayant l’AMM dans cette indication. Ses modalités d’utilisation dans cette situation particulière mériteraient d’être précisées
The kinase domain of TRPM7 interacts with PAK1 and regulates pancreatic cancer cell epithelial-to-mesenchymal transition
International audiencePancreatic ductal adenocarcinoma (PDAC) is the main and the deadliest form of pancreatic cancer. This is a major problem of public health since it will become the second leading cause of death by cancer in the next few years, mainly due to the lack of efficient therapies. Transient Receptor Potential Cation Channel Subfamily M Member 7 (TRPM7) protein, a cation channel fused with a serine/threonine kinase domain is overexpressed in PDAC and associated with a low survival. In this work, we aim to study the role of kinase domain on pancreatic cell fates by using a model of kinase domain deletion by CRISPR-Cas9. PANC-1 and MIA PaCa-2 PDAC cell lines were used and kinase domain was deleted by CRISPR-Cas9 strategy. Kinase domain deletion (ΔK) was validated by RT-qPCR and western blots. The effect of kinase domain deletion on channel function was studied by patch-clamp and Mn 2+ -quenching. The cell phenotype was studied by MTT and cell migration/invasion assays. Finally, the role of kinase domain was studied in vivo in xenografted mice. Here we show that TRPM7 kinase domain is required to maintain a mesenchymal phenotype in PDAC cells. We also demonstrated that TRPM7 and PAK1 interact in the same protein complexes. Moreover, TRPM7 kinase domain is required for carcinogenesis and cancer cell dissemination in vivo. Intriguingly, the role of TRPM7 kinase is cell specific and may depend on the KRAS oncogene mutation status. In conclusion, TRPM7 kinase domain is required to maintain a mesenchymal and aggressive phenotype in PDAC cells, and it could be a promising target against PDAC
La connaissance et l’acceptabilité de la contraception masculine thermique chez les hommes âgés de 18 à 55 ans en Picardie
Introduction: Male contraception remains less discussed and less used than female contraception. Scientific advances and societal changes have shown that men can also play a role in contraception within a couple. Objective: To determine the acceptability and use of male thermal contraception among men aged 18 to 55 in Picardy. Study design and Methods: This was a quantitative, descriptive, observational, cross-sectional, and multicenter study. A digital questionnaire was administered to men aged 18 to 55 in Picardy through various networks between January 2 and March 24, 2025. A total of 415 men were included. Results: 153 men had little knowledge of male contraceptive methods and 16 had none. 44.82% of men would be open to considering it while 29.4% would be reluctant. The motivations were to avoid the side effects of female contraceptives and to share contraception. The main obstacles were constraints related to its use, effectiveness, and lack of knowledge.Discussion: This study showed a growing interest in thermal male contraception, although it is limited by a lack of scientific and medical data. It would be important to conduct reliability studies, strengthen professional training and promote the dissemination of this method. Conclusion: Theoretical acceptability was nearly 60%, although actual use is extremely low. Efficacy and safety studies, as well as better dissemination, seem essential.Introduction : la contraception masculine reste moins discutée et moins utilisée que la contraception féminine. Les avancées scientifiques et l’évolution de la société ont montré que les hommes pouvaient eux aussi prendre part à la contraception du couple. Objectif : déterminer l’acceptabilité et l’utilisation de la contraception masculine thermique chez les hommes picards de 18 à 55 ans. Matériels et méthodes : il s’agissait d’une étude quantitative, descriptive, observationnelle, transversale et multicentrique. Un questionnaire informatisé a été proposé aux hommes de 18 à 55 ans en Picardie par divers réseaux entre le 2 janvier et le 24 mars 2025. 415 hommes ont été inclus. Résultats : 153 hommes avaient des connaissances faibles sur les méthodes de contraception masculine et 16 n’en avaient aucune. 44,82% des hommes seraient ouverts à la réflexion et 29,4% peu enclins. Les motivations étaient d’éviter les effets secondaires des contraceptifs féminins et le partage de la contraception. Les principaux freins concernaient les contraintes liées à son utilisation, l’efficacité et le manque de connaissances. Discussion : cette étude a montré un intérêt croissant pour la contraception masculine thermique bien qu’elle soit limitée par un déficit scientifique et médical. Il serait important d’effectuer des études de fiabilité, de renforcer la formation des professionnels et la diffusion de cette méthode. Conclusion : l’acceptabilité théorique était à près de 60% bien que son usage réel soit extrêmement faible. Des études d’efficacité et d’innocuité ainsi qu’une meilleure diffusion semblent essentiels

This paper focus on polymaterial sculpture and glazed terracotta sculpture in Tuscany, and the interplay between materials and lights – natural or artificial. Sculptors, indeed, used different materials and their surfaces – matte, glossy or shiny – in order to reinforce and convey the political or theological message to the beholder. They also adapted the device for the domestic setting, the interior of the ecclesiastical buildings or the façades of churches and civic buildings. Can Like Attract Like? A Study of Homonymous Gathering in Networks
A team of mobile agents, starting from distinct nodes of a network, have to meet at the same node and declare that they all met. Agents execute the same algorithm, which they start when activated by an adversary or by an agent entering their initial node. When activated, agents traverse edges of the network in synchronous rounds. Their perception and communication are strictly local. This task, known as gathering, is a central problem in distributed mobile systems. Most prior work focuses on minimizing its time complexity, i.e., the worst-case number of rounds between the start of the earliest agent and the task completion. To break possible symmetries, deterministic solutions typically assume that agents have pairwise distinct IDs, called labels, known only to themselves. But must all labels be pairwise distinct to guarantee deterministic gathering? We address this question by considering agents that may share the same label. A team L is said to be gatherable if, for every initial setting of L, there is an algorithm that solves gathering. Our contribution is threefold. (1) We give a full characterization of the gatherable teams. (2) We design an algorithm that gathers all of them in poly time, where (resp. ) is the graph order (resp. the smallest label in L). This algorithm requires the agents to initially share only bits of common knowledge, where is the largest label multiplicity in L. (3) We show this dependency is almost optimal to get a poly-time complexity. As a by-product, we get the first deterministic poly-time algorithm requiring no common knowledge to gather any team when all labels are distinct. Known to be achievable for two-agent teams, extending this to any team size faced a major challenge: termination detection. Our techniques to address it may be of independent interest
