Analyse de l’instauration de la sédation profonde et continue chez les patients en fin de vie aux Urgences du CHU Amiens-Picardie FDVU : à propos d’une étude rétrospective Monocentrique sur le CHU Amiens Picardie sur la période 2023-2024
Background: DCSD, regulated by the Claeys-Leonetti law and the recommendations of the French National Health Authority, is an essential medical practice in end-of-life care. However, data regarding its implementation in emergency departments remain limited. The aim of this study is to assess the compliance of practices at Amiens University Hospital with national recommendations on DCSD. Methods: A retrospective single-center observational study was conducted at Amiens University Hospital from January 1, 2023, to December 31, 2024. Eligible patients were adults admitted to the ED with a formal decision of withholding/withdrawing life-sustaining treatments (LATA) and initiation of sedation. Compliance was assessed using the SPHEREU score, developed for this study. Results: Among the 154 patients included, 140 (90.9%) received sedation in accordance with national guidelines (95% CI [84.9–94.7]). The Pearson’s χ² test on the primary outcome was significant (p < 0.01) Conclusion: DCSD at the Amiens ED is largely compliant with national guidelines. Improvement opportunities include documentation, sedation assessment, and palliative care training. A multicenter study and SPHEREU score validation could enhance practice harmonization.Introduction : la SPCJD encadrée par la loi Claeys-Leonetti et les recommandations de la Haute Autorité de Santé est une pratique essentielle en fin de vie. Peu de données existent sur sa mise en œuvre aux urgences. Cette étude vise à évaluer la conformité des pratiques du CHU d’Amiens aux recommandations nationales. Méthodes : étude rétrospective monocentrique observationnelle menée au CHU d’Amiens, surla période du 1er janvier 2023 au 31 décembre 2024. Les patients inclus sont majeurs, pris en charge aux urgences avec décision de LATA et instauration d’une sédation. La conformité est évaluée selon le score SPHEREU, créé pour cette étude. Résultats : parmi 154 patients inclus, 140 (90,9 %) ont reçu une sédation conforme aux recommandations (IC95 % [84,9–94,7]). Le test du χ² de Pearson sur le critère principal était significatif (p < 0,01). Conclusion : la SPCJD aux urgences du CHU d’Amiens est majoritairement conforme aux recommandations nationales. Des axes d’amélioration persistent, notamment la traçabilité, l’évaluation de la sédation et la formation des équipes aux soins palliatifs. Une étude multicentrique et la validation du score SPHEREU pourraient optimiser l’harmonisation des pratiques

Befreiung aus dem Museum: Postmemory als Subversion der Gedenkkultur in Ari Folmans "Where is Anne Frank"
International audienceDer Beitrag untersucht Ari Folmans Animationsfilm Where Is Anne Frank (2021) als Beispiel für die „postmemoriale“ (Hirsch 1997; 2012) Vermittlung des Shoah-Gedächtnisses im Kontext gegenwärtiger erinnerungspolitischer Aushandlungen. In Folmans fiktionalisierter Fortschreibung steht nicht Anne Frank im Mittelpunkt, sondern ihre imaginäre Freundin Kitty, die als erzählerische Instanz das Tagebuch in die Gegenwart überführt. Dadurch wird kanonisierte Holocaust-Literatur für nachrückende Generationen remediatisiert und neu kontextualisiert.Analysiert werden die ethischen und ästhetischen Implikationen dieser Bearbeitung: Folman löst Anne Frank aus dem museal erstarrten Gedenkraum, setzt ihr schriftliches Selbstgespräch dialogisch fort und verbindet es mit einer aktuellen Flucht- und Verfolgungsgeschichte. So aktualisiert der Film die vielfach beschworene ‚Botschaft‘ Anne Franks, macht das Tagebuch wieder anschlussfähig und positioniert sich gegen ritualisierte Formen des Erinnerns sowie gegen identitätspolitische Vereinnahmungen des Holocaust-Gedenkens.Zugleich problematisiert der Beitrag die politische Brisanz historischer Brückenschläge zwischen der Shoah und anderen Gewalterfahrungen. Folmans erklärtes Ziel, zwei Epochen „miteinander zu verknüpfen, ohne sie zu vergleichen“, wird im Kontext der Debatten um die Singularität der Shoah verortet und in Beziehung zu Ansätzen eines pluralisierten Erinnerungsdiskurses (Zimmerer, Rothberg) gesetzt. Abschließend wird gezeigt, mit welchen diskursiven, narrativen und dramaturgischen Mitteln der Film die Postmemory des Genozids als „multidirektionales Erinnern“ (Rothberg 2009) gestaltet, ohne die Shoah zu instrumentalisieren oder gegenwärtige Flüchtlingspolitik mit der nationalsozialistischen Vernichtungspolitik gleichzusetzen.This article examines Ari Folman’s animated film Where Is Anne Frank (2021) as an example of the “postmemorial” (Hirsch 1997; 2012) transmission of Holocaust memory within the context of contemporary negotiations of memory politics. In Folman’s fictionalized continuation, the focus is not on Anne Frank herself but on her imaginary friend Kitty, who functions as a narrative agent transferring the diary into the present. In this way, canonized Holocaust literature is remediated and recontextualized for subsequent generations.The analysis addresses the ethical and aesthetic implications of this adaptation. Folman removes Anne Frank from the museum-like, ossified space of official commemoration, dialogically extends her written self-conversation, and links it to a contemporary story of flight and persecution. In doing so, the film updates the oft-invoked “message” of Anne Frank, restores the diary’s relevance and connectivity, and positions itself against ritualized forms of remembrance as well as against identity-political appropriations of Holocaust memory.At the same time, the article problematizes the political sensitivity of historical bridge-building between the Shoah and other experiences of violence. Folman’s stated aim of “connecting two epochs without comparing them” is situated within debates on the singularity of the Shoah and related to approaches advocating a pluralized discourse of remembrance (Zimmerer, Rothberg). Finally, the article demonstrates the discursive, narrative, and dramaturgical strategies through which the film shapes the postmemory of the genocide as “multidirectional memory” (Rothberg 2009), without instrumentalizing the Shoah or equating contemporary refugee politics with National Socialist extermination policy
Entre changement climatique et écologisation de la société, la filière forêt-bois en transition ? État des lieux de la recherche, pour une mise en perspective du numéro thématique
International audienceThe article introduces the eponymous issue by providing perspective based on an overview of research on forests conducted mainly in France. The proliferation of theses and journal issues shows the enduring relevance of this subject within the human and social sciences, proving that a new field of research is opening up. The authors focus on the questions that drive this research and guided the call for papers that led to this issue. They conduct a cross-analysis of the included articles, which are considered representative of current studies on the forestry-wood sector. These articles are then presented individually, following a path from upstream to downstream, via the territories. The authors conclude on the significance of these writings as a key to reading and understanding transitions beyond forests.L’article introduit le dossier éponyme, par une mise en perspective fondée sur un état des lieux des recherches menées surtout en France sur les forêts. La multiplication des thèses et des numéros de revue montre l’ancrage durable de cet objet d’étude au sein des shs, prouvant l’ouverture d’un front de recherche. Les auteurs s’attardent sur les questions qui animent ces travaux et ont guidé l’appel dont émane ce dossier. Ils procèdent à une analyse croisée des articles inclus, jugés représentatifs des études en cours sur la filière forêt-bois. Ces articles sont ensuite présentés individuellement, en suivant un cheminement de l’amont à l’aval, via les territoires. Les auteurs concluent sur la portée de ces écrits, comme clé de lecture et de compréhension des transitions, au-delà des forêts
Driver Fatigue Detection using EEG Microstate Features and Support Vector Machines
International audienceBackground: Driver fatigue detection is crucial for traffic safety. Electroencephalography (EEG) signals, which directly reflect the human mental state, provide a reliable approach for identifying fatigue.Objective: This study aimed to investigate the effectiveness of EEG microstate analysis in detecting driver fatigue by analyzing variations in microstate features between normal and fatigued states.Material and methods: This analytical study aimed to develop a supervised machine learning approach for driver fatigue detection using EEG microstate features. EEG data were collected from 10 individuals in both normal and fatigued states. Microstate analysis was performed to extract key features, including duration, occurrence, coverage, and Microstate Mean Power (MMP), from four types of microstates labeled A, B, C, and D. These features were then used as inputs to train and test a Support Vector Machine (SVM) for classifying each EEG segment into either normal state or fatigue state.Results: The classification achieved high accuracy, particularly when combining MMP and occurrence features. The highest accuracy recorded was 98.77%.Conclusion: EEG microstate analysis, in combination with SVM, proves to be an effective method for detecting driver fatigue. This approach can be utilized for real-time driver monitoring and fatigue alert systems, enhancing road safety
Le Mini Cognitive Evaluation (MCE) est-il supérieur au MMSE et MoCA pour repérer les troubles neurocognitifs à un stade léger ?
Introduction : le diagnostic précoce au stade de trouble cognitif léger des pathologies ou complications cognitives des pathologies encéphaliques est indispensable pour établir des stratégies préventives et thérapeutiques. Cependant, il n’existe pas actuellement de test de dépistage suffisamment sensible et spécifique. L’objectif principal était de déterminer si le test de dépistage MCE était supérieur au MMSE et MoCA pour discriminer les patients des témoins. Les objectifs secondaires étaient d’examiner le test le plus discriminant chez les patients AVC, CMRR et déficitaires au BNP, et d’examiner les associations entre MCE et localisation de l’atrophie. Population et méthodes : nous avons inclus 215 patients (AVC : n= 122 ; CMRR : n=93) et 436 témoins. La discrimination du MCE pour le dépistage des TCL a été comparée à celle des MMSE et MoCA en analysant leurs aptitudes à discriminer patients des témoins. Ces analyses ont été menées avec des courbes ROC dans la population totale puis dans les sous-groupes AVC, CMRR et chez les patients déficitaires au BNP (n=194). L’association entre MCE et localisation d’atrophie cérébral a été faite par analyse VBM. Résultats : le MCE était supérieur (p=0,01) pour discriminer les patients (AUC=0,838 ; IC 95% : 0,805-0,870), résultat également trouvé dans les sous-groupes AVC (AUC = 0,791) et CMRR (AUC = 0,898). Il n’était pas retrouvé de supériorité pour discriminer les patients déficitaires au BNP (AUC = 0,870). Le score au MCE était associé à l’atrophie de nombreux clusters (région calcarine gauche, noyau caudé gauche, cervelet postéro-latéral gauche) et aux volumes cérébraux. Discussion et Conclusion : ces résultats ont montré que le MCE était plus discriminant que le MMSE et MoCA pour distinguer patients des témoins et pour les sous-groupes à l’exception des patients déficitaires au BNP. Il pourrait être envisagé une poursuite de la recherche afin d’optimiser sa sensibilité
Federated Learning for IoMT Security: Enhancing Attack Detection with hybrid CNN-LSTM ModeL
International audienceIntegrating threat detection, while maintaining privacy, into IoMT networks presents the unique challenge of balancing threat detection and patient protection. We designed a federated learning framework that leverages a hybrid CNN-LSTM architecture capable of capturing the spatio-temporal characteristics of network traffic. When we evaluated our system on the Edge-IIoTset and CICIOMT 2024 datasets, the results showed accuracies of 96% and 98.89%, respectively and an accuracy of 99.89% on DDoS attacks

EUNIS habitat maps: enhancing thematic and spatial resolution for Europe through machine learning
International audienceThe EUNIS habitat classification is crucial for categorising European habitats, supporting European policy on nature conservation and implementing the Nature Restoration Law. To meet the growing demand for detailed and accurate habitat information, we provide spatial predictions across Europe (EEA39 territory) for 260 EUNIS habitat types at hierarchical level 3, together with independent validation and uncertainty analyses. Using ensemble machine learning models, together with high-resolution satellite imagery and ecologically meaningful climatic, topographic and edaphic variables, we produced a European habitat map indicating the most probable habitat overall at 100-m resolution across Europe. Additionally, we provide information on prediction uncertainty and the most probable habitats at level 3 within each EUNIS level 1 formation. This product is particularly useful for both conservation and restoration purposes. Predictions were cross-validated at European scale using a spatial block cross-validation and evaluated against independent data from France (forests only), the Netherlands and Austria. The maps achieved strong predictive performance, with F1-scores ranging from 0.61 to 0.94 in spatial cross-validation and from 0.33 to 0.95 in external validation datasets with distinct trade-offs in terms of recall and precision across habitat formations.
