    Le renouvellement de la norme dans les villes médiévales : l’exemple du scripteur urbain

    International audienceThis article looks at how the concept of renewal can be applied to the norm, both in terms of its content in relation to its objective, and in terms of the producer of the norm, between permanence and (re)legitimisation in relation to the latter. The study shows that renewal applied to the norm is never sufficient in itself in medieval towns. Depending on the case, renewal takes the form of restarting, replacing, repeating, extending or reiterating.Cet article traite de la manière dont le concept de renouvellement peut être appliqué à la norme, à la fois sur son contenu par rapport à l’objectif de celle-ci et sur le producteur de la norme, entre permanence et (re)légitimation concernant ce dernier. L’étude démontre que le renouvellement appliqué à la norme n’est jamais suffisant en soi dans les villes médiévales. Il est alors nécessaire de le préciser ; le renouvellement se faisant selon les cas par recommencement, remplacement, répétition, prorogation ou réitération

    Cosmetic potential of phenolic-rich extracts from Cannabis sativa L. seeds: Inhibition of enzymes involved in skin aging, inflammation and hyperpigmentation

    International audienceThe demand for natural and multifunctional ingredients in cosmetic formulations has surged in recent years, driven by growing consumer awareness of skin health. In this context, the present study investigates the cosmetic potential of Cannabis sativa L. seeds. Two fractions, obtained by solvent partitioning with petroleum ether and ethyl acetate, were assessed for their radical scavenging ability (DPPH, ABTS, HRSA) and reducing power (FRAP, CUPRAC, TAC). The ethyl acetate (EA) fraction exhibited significantly higher antioxidant activity and was therefore selected for further evaluation of its inhibitory effects on key enzymes associated with skin aging, inflammation, and hyperpigmentation. The phenolic compounds in the EA fraction were analyzed by HPLC-DAD coupled with mass spectrometry in negative ion mode using an untargeted approach. The EA fraction demonstrated strong enzyme-inhibitory activities (IC 50 = 31.05 µg mL -1 , 75.53 µg mL -1 , 5.71 µg mL -1 , and 141.72 µg mL -1 , respectively for elastase, collagenase, lipoxygenase, and tyrosinase,), likely due to its high contents of hydroxycinnamic acid amides (240.90 mg g -1 extract, predominantly represented by caffeoyltyramine) and lignanamides (449.34 mg CTE g -1 extract, dominated by cannabisins A and B). These findings indicate that phenolic-rich fractions from cannabis seeds represent promising natural ingredients for cosmetic formulations targeting oxidative stress, skin aging, hyperpigmentation, and cutaneous inflammation. The results further contribute to the valorization of cannabis seed by-products and support the development of safer, plant-based alternatives in the cosmetic industry.</div

    International audienceLa transition vers l’âge adulte des patients présentant une encéphalopathie épileptique reste un défi majeur : les crises persistent souvent et impactent fortement l’autonomie, la socialisation, et la scolarité.Une transition réussie doit débuter précocement et intégrer une évaluation multidisciplinaire portant sur les dimensions médicale, cognitive, psychiatrique, sociale et éducative. À l’âge adulte, la prise en charge doit rester globale : gestion nutritionnelle (déglutition, régime céto, carences), suivi respiratoire (SAOS, hypoventilation, VNI), santé sexuelle et reproductive (contraception, interactions thérapeutique), accompagnement psychologique et prise en charge fonctionnelle motrice.La coordination étroite entre neurologues, MPR, pneumologues, diététiciens, gastro-entérologues, psychologues, travailleurs sociaux et familles est indispensable pour éviter les ruptures de soins.     International audienceIntroductionLe diagnostic précoce des troubles cognitifs post-AVC est utile à la prise en charge. Il n’existe pas de test de dépistage suffisamment sensible et spécifique pour les troubles cognitifs légers.ObjectifsL’objectif de cette étude était d’examiner les capacités de discrimination d’un nouveau test de repérage, le MCE, chez des patients avec au maximum un trouble cognitif à un stade léger défini par un MMSE ≥ 20.MéthodesL’objectif principal était de déterminer si le MCE était supérieur au MMSE et MoCA pour discriminer patients AVC des témoins. Les objectifs secondaires étaient d’examiner le test le plus discriminant pour identifier les patients déficitaires à une batterie de tests(BNP) ; les associations avec les localisations d’atrophie cérébrale(analyse VBM). Nous avons inclus 122 patients en unité neuro-vasculaire (UNV) du CHU d’Amiens ; 436 témoins ; 89 patients avec BNP(44 déficitaires).RésultatsLe MCE était supérieur (p = 0,0001) aux MMSE et MoCA pour (1) discriminer les patients (ROC AUC = 0,791 ; IC95 % : 0,731–0,851) des témoins ainsi que pour (2) discriminer les patients déficitaires à la batterie (ROC AUC = 0,896 ; IC95 % : 0,838–0,954). Le score au MCE était corrélé au volume cérébral global (p ≤ 0,01 corrigé). En analyse VBM, le score au MCE était associé (p ≤ 0,001 non corrigé) à l’atrophie de nombreuses structures cérébrales avec une prédominance gauche (noyau caudé gauche, thalamus antérieur gauche et droit, régions temporales supérieures et moyennes gauche, insula droit, régions postérieures du cervelet gauche et carrefour pariéto temporal gauche).DiscussionCette étude extrêmement exigeante, puisque ne portant que chez des patients avec au maximum un trouble cognitif à un stade léger, a montré que le MCE était un test très prometteur pour identifier les patients AVC avec un déficit cognitif même à un stade léger.ConclusionLes propriétés diagnostiques du MCE restent à définir pour identifier son rôle dans la filière de soins

    Efficacy and Safety of Second‐Line Advanced Therapy After Vedolizumab in Ulcerative Colitis: A Multicenter Cohort Study From the GETAID

    International audienceBackground and Aims Vedolizumab has become the preferred first‐line advanced therapy in ulcerative colitis (UC). However, the optimal second‐line treatment following vedolizumab failure remains unclear. We aimed to evaluate the effectiveness and safety of second‐line therapies after first‐line vedolizumab. Methods We conducted a multicenter retrospective study including UC patients from 31 centers who received infliximab (IFX), subcutaneous (SC) anti‐TNFs, or ustekinumab after vedolizumab failure. The primary endpoint was steroid‐free clinical remission (SFCR) at week 14. Predictors of remission were identified using multivariate logistic regression. Results Among 196 patients, 99 received IFX, 27 anti‐TNF SC, and 70 ustekinumab. At week 14, SFCR was achieved in 78 patients (39.8%): 38 (38.4%) with IFX, 8 (29.6%) with anti‐TNF SC, and 32 (45.7%) with ustekinumab, with no significant difference between groups ( p = 0.32). Median treatment persistence ranged from 8 to 9.2 months. Baseline corticosteroid use was associated with lower odds of SFCR (OR = 0.37, 95% CI [0.18–0.73]). Adverse events occurred in 15.8% of patients, including 12.2% serious events. Overall adverse events were less frequent with ustekinumab than with IFX (10.0% vs. 24.2%, p = 0.02), while serious events were comparable (5.7% vs. 16.1%, p = 0.08). Discontinuation due to adverse events was more frequent with IFX (12.1%) and anti‐TNF SC (14.8%) than with ustekinumab (2.9%, p = 0.045 and 0.049). Conclusion In UC patients failing vedolizumab, second‐line IFX, anti‐TNF SC, and ustekinumab showed similar effectiveness and persistence. Infliximab remains a robust option for rapid control in high inflammatory burden, whereas ustekinumab may be preferred for its superior safety profile in high‐risk patients

    The process of online purchase intention in the era of live streaming commerce: Central role of flow experience

    International audienceThis study proposes an integrated model explaining how consumer trust and the flow experience complementarily align in the process of purchase intention during live streaming commerce (LSC). This model, grounded in social exchange theory and flow theory, introduces two primary routes: the cognitive route establishes the link between streamer characteristics and trust in the product, while the affective route associates LSC characteristics with the flow experience. These two pathways converge toward purchase intention. The mixed route represents the relationship between trust and flow experience and demonstrates the sequential mechanisms leading to online purchase intention. First, a qualitative study consolidated the relevance of the concepts integrated into the research model. Subsequently, a quantitative study conducted in France with 555 respondents was used to test the proposed model. The results reveal the full mediation of trust regarding the effect of streamer characteristics on purchase intention. They also highlight the full mediation of flow experience. Trust thus represents a necessary cognitive basis but insufficient for increasing online purchase intention. It is only through its mediated effect via the flow experience that this intention is intensified. The interplay between trust (cognition) and flow (affect) sheds light on the underlying mechanisms of online purchase intention and opens up new avenues for research

    Contemporary neoadjuvant strategies for rectal cancer: The GRECCAR snapshot study

    International audienceAim Neoadjuvant treatment for rectal cancer has evolved markedly with the growing adoption of total neoadjuvant therapy ( TNT ), organ‐preservation strategies and selective omission of radiotherapy. Recent trials support risk‐based personalization, but its application in real‐world settings remains poorly documented. The aim was to describe current neoadjuvant treatment practices for mid‐low rectal cancer in French expert centres and identify tumour‐ and patient‐related factors influencing decisions. Method This observational study included patients with non‐metastatic rectal adenocarcinoma ≤10 cm from the anal verge, discussed in tumour boards (October 2022 to March 2023) across GRECCAR centres. Tumours were classified as early, intermediate‐risk or locally advanced rectal cancer (LARC). Neoadjuvant treatments were analysed according to tumour extension, location and age. Results Among 463 patients from 27 centres, the most frequent regimen was induction chemotherapy, mainly FOLFIRINOX, followed by long‐course chemoradiotherapy (CRT) (65%). This approach was used in 51%, 66% and 71% of patients in the early, intermediate‐risk and LARC groups, respectively ( p = 0.0060). TNT was more frequently administered for low‐ than mid‐rectal cancers, especially in LARC (86% vs. 71%, p = 0.016). In patients &gt;75 years, CRT + consolidation chemotherapy and radiotherapy alone were proportionally more frequent. Among the early rectal cancers, those treated with induction chemotherapy + CRT had more advanced features than those treated with CRT alone (cT3: 80% vs. 43%, cN+: 62% vs. 10%, tumour size: 3.4 vs. 2.3 cm; all p &lt; 0.001). Conclusion TNT with induction chemotherapy is the predominant neoadjuvant approach in French expert centres. Tumour classification, location and patient age significantly influence treatment choices, reflecting a shift towards personalized context‐specific care

    Eigenvector-based acceleration strategies for gradient-type methods

    Several strategies are described and analyzed to speed-up gradient-type methods when applied to the minimization of strictly convex quadratics and strictly convex functions. The proposed techniques focus on relaxing the traditional optimal step length associated with gradient methods, including the steepest descent (SD) and the minimal residual (MR) methods. Such a relaxation avoids the well-known negative zigzag effect and allows the iterates to move in the entire space which in turn implies that every so often the search direction approaches some eigenvector of the underlying Hessian matrix. The proposed speedups then rely on taking advantage of the properties of the Lanczos method once a search direction that approaches an eigenvector has been identified in order to accelerate the convergence towards the global minimizer. After analyzing the proposed strategies, we illustrate them on the global minimization of strictly convex functions
