    Women’s Insights on Extended Adjuvant Endocrine Therapy for Breast Cancer: Qualitative Online Forums Study

    International audienceIn France, breast cancer is the most commonly diagnosed cancer and the leading cause of cancer-related death among women. For around one-third of women with hormone receptor–positive breast cancer, extending adjuvant endocrine therapy (AET) beyond the initial 5 years is now recommended to reduce the risk of recurrence and mortality. While weighing benefits against potential side effects is essential, little is known about how women seek information about extended AET or how they experience this extension. Objective This study aimed to explore, through online forums, women’s unmet information needs, the factors influencing their decision-making around extended AET, and the needs and expectations they share throughout the treatment journey. Methods A qualitative content analysis was conducted using data from 5 French online forums over a 10-year period. We used a nonparticipant observational approach to identify relevant posts and gain a deeper understanding of forum dynamics related to extended AET. Data were collected in March 2025. After applying inclusion and exclusion criteria, 188 messages posted by 105 women were selected. A netnographic approach, derived from ethnography and suited to exploratory research, was used in conjunction with Braun and Clarke’s 6-phase framework for thematic analysis, integrating both inductive (narrative-based) and deductive (literature-informed) approaches. Results Our findings showed that women turned to forums to seek information about extended AET from both medical and nonexpert sources. However, they often reported difficulties in interpreting complex or inconsistent information. Understanding and acceptance of treatment extension were influenced by the timing and manner of information disclosure. Three decision-making patterns were identified: (1) acceptance, (2) hesitation, and (3) refusal, which could shift over time, shaped by factors such as fear of recurrence, side effect experience and management, trust in treatment effectiveness, interactions with health care professionals, and life changes. Some women on extended AET shared resilient and encouraging experiences, providing reassurance and practical strategies for coping with side effects. Conclusions Online forums offer valuable insights into women’s experiences, unmet information needs, uncertainties, and coping strategies surrounding extended AET, contributing to addressing a gap in knowledge on how women perceive and manage long-term treatment. Providing clear, accessible information and tailored communication tools, especially on benefit-risk balance and side effects management, at the time of prescription and at key points along the care pathway, may support informed decision-making, improve treatment adherence, maintain quality of life, and offer reassurance. With growing numbers of women in long-term treatment, enhanced coordination between primary care and hospital services, including follow-up by trained general practitioners and gynecologists in collaboration with cancer centers, may help ensure continuity of care and comprehensive support

    International audienceLe projet V-GeoSciEd est un projet Erasmus Plus porté par l'Université Paris Cité (Caroline Leininger-Frézal) en partenariat avec les universités de Vienne, Hambourg, Castilla de la Mancha, l'Université Polytechnique d'Athènes (NTUA), Eurogeo et l'UNED (Madrid). Le projet vise à développer des pratiques d'enseignement qui prennent en compte la diversité des élèves dans les contenus enseignés. Le poster présentera les résultats d'entretiens réalisés avec des formateurs d'enseignants. Ces entretiens questionnent la place de la diversité et de l'inclusion dans la formation des enseignants en géographie et en science, en Europe dans une perspective comparative. Les résultats montrent des profils très différents de formation d'enseignants

    Factors associated with early neurological deterioration in minor distal medium vessel acute ischemic stroke: A multinational multicenter study

    International audienceBackground: Patients with acute ischemic stroke secondary to distal and medium vessel occlusion (AIS-DMVO) and minor strokes present a challenge in determining the most appropriate emergent treatment. Factors leading to early neurological deterioration (END) in this patient population are understudied, but END is known to result in poor functional outcomes. Therefore, we aimed to investigate the factors contributing to END in minor AIS-DMVO cases. Methods: We included patients with AIS-DMVO and minor strokes from 37 sites across North America, Asia, and Europe. Minor stroke was defined as a baseline National Institutes of Health Stroke Scale (NIHSS) score of ⩽5. The primary outcome measure, END, was defined as a shift of ⩾4 points in the NIHSS score at day one after treatment compared to baseline. Univariable and multivariable logistic regression analyses were performed to identify factors associated with END. Results: Among 559 consecutive patients with DMVO and minor strokes, END was reported in 68 patients. In multivariable analysis, mechanical thrombectomy (MT) was independently associated with higher odds of END (adjusted odds ratio [aOR] 2.37, 95% CI 1.12–5.02, p = 0.02), while intravenous thrombolysis (IVT) was associated with lower odds of END (aOR 0.46, 95% CI 0.26–0.81, p = 0.008). However, the association between MT and END was no longer statistically significant in the IPTW-adjusted analysis (OR 1.65, 95% CI 0.69–3.98, p = 0.26). Hypertension and antiplatelet use at baseline were also independently associated with END. Among MT-treated patients, successful and excellent recanalization and first-pass effect were protective against END. Conclusion: MT was associated with END in patients with minor AIS-DMVO, although this association was not significant after IPTW adjustment. IVT was independently associated with reduced risk of END. These findings support careful patient selection and further study in randomized trials

    Une occupation groupée du vie-ve s. BC (Hallstatt D) au mont Fiquet à Pont-l’Évêque (Calvados)

    National audienceThe Pont-L’Évêque site, excavated in 2022, is a remarkable example of a grouped settlement, probably dating to the 6th-5th century BC (Hallstatt D). It comprises elliptical, or rectangular, apse buildings with a peripheral foundation trench. Some have superimposed plans. Smaller posthole buildings are interpreted as annexes. Several pits located nearby could be for water storage. These cisterns and the foundation trenches contained large amounts of pottery used to date the settlement. Radiocarbon dates fall on the Hallstatt plateau but their probability span was reduced using Bayesian analyses, giving a probable period of occupation between 550 and 450 BCE. Comparisons with other sites in northern France found common points with sites installed specifically in the Grand-West region (Normandy/Brittany/Pays de la Loire), as well as with the south of England.Le site de Pont-L’Évêque, fouillé en 2022, représente un exemple remarquable d’habitat groupé, vraisemblablement situé entre le vie et le ve s. BC. Ses caractéristiques sont la présence de bâtiments elliptiques ou rectangulaires à abside, à tranchée de fondation périphérique, présentant parfois plusieurs plans superposés. Des bâtiments sur poteaux, de moindre surface, ont été interprétés comme des édifices annexes associés à ces grands bâtiments sur tranchée. Plusieurs fosses installées à proximité ont été interprétées comme de possibles citernes de stockage de l’eau. Elles ont livré un mobilier céramique significatif, issu également des tranchées de fondation des bâtiments, qui permet de conforter la datation de l’ensemble de cet habitat groupé. Les datations 14C sont inscrites sur le plateau du Hallstatt, mais leur nombre élevé a permis de réduire, par le biais des analyses bayésiennes, la période probable d’occupation du site entre 550 et 450 av. n. è. Les comparaisons recherchées dans la moitié nord de la France ont permis d’établir des points communs avec des sites installés spécifiquement dans la région Grand-Ouest (Normandie, Bretagne, Pays de la Loire) et aussi quelques convergences avec le sud de l’Angleterre

    International audienceA large data set of XAS (X-ray Absorption Spectroscopy) Manganese K-edge spectra has been collected operando and studied upon the electrochemical oxidation of the promising Li-ion battery anode material Li 7 MnN 4 . Using chemometric tools such as PCA (Principal Component Analysis) and MCR-ALS (Multivariate Curve resolution -Alternating Least Squares), three independent environment spectra were insulated. Based on the faradaic yield and well-chosen comparison of absorption spectrum energies within the frame of the coordination charge model, these environments were ascribed to unusual oxidation states allowed by nitride chemistry at a low potential (B1.2 V vs. Li + /Li), i.e. Mn 5+ (3d 2 ), Mn 6+ (3d 1 ) and Mn 7+ (3d 0 ). Also, their relative amounts are discussed with regard to the long-range structural variation which can be simply described by two successive biphasic domains followed by a solid-solution behaviour. Gathering this long-range and local structure information provides a complete picture of the redox mechanisms occurring in Li 7 MnN 4 .</div
