Xanthogranulomatous Prostatitis, a Rare Prostatic Entity
AbstractThere are several benign prostatic pathologies that can clinically mimic a prostate adenocarcinoma. Xanthogranulomatous prostatitis is a benign inflammatory condition of the prostate and a rare entity. A 47-year old male, with 3 years of lower urinary tract symptoms, with a palpable hypogastric tumor, digital rectal examination: solid prostate, of approximately 60 g. Initial PSA was 0.90 ng/mL. He underwent surgical excision of the lower abdominal nodule and prostatectomy. Histopathology showed xanthogranulomatous prostatitis, without malignancy. Xanthogranulomatous prostatitis is an extremely rare entity that can simulate prostate adenocarcinoma, therefore having a correct histopathological diagnosis is essential
Sacha inchi (Plukenetia volubilis L.) shell biomass for synthesis of silver nanocatalyst
AbstractThis article describes a new approach for synthesis of silver nanostructured particles by using a Sacha inchi shell biomass (SISB). Sacha inchi shell biomass should be considered as an important source of phytochemicals for the synthesis and stabilization of silver nanoparticles (AgNPs). The characterization of AgNPs was performed by UV–vis spectroscopy, Transmission Electron Microscopy and Selected Area Electron Diffraction, indicating that AgNps were synthesized successfully with an average size of 7.2nm. Infrared spectrum measurements were carried out to hypothesize the possible biomolecules responsible for stabilization and capping of the silver nanoparticles. Then, AgNps were used as a catalyst in the presence of sunlight for the removal of methyl orange dye (MO, anionic dye) from aqueous solution. The results implied AgNps to be effective photocatalyst for the decomposition of MO (∼60%, pH 2.0) from aqueous solution

AbstractA dedicated neutronics computational scheme is a crucial tool in the preparation and the post-analysis of an irradiation experiment, as it defines the key parameters of the experiment. The complex and very dynamic environment of a Materials Testing Reactor (MTR) core, on the other hand, represents a challenging object for neutronics calculations. Within the framework of the HELIOS experiment conducted at the High Flux Reactor (HFR) in Petten (The Netherlands) in 2009 under the European Project EUROTRANS, a new neutronics scheme has been developed with the objective of finding the optimum balance between accuracy and calculation performance. This scheme is based on the Monte-Carlo technique and employs a two-level approach by separating the problem into overlapping spatial and temporal domains. The underlying physical phenomena are presented, together with elements of validation which gives a further insight into the effects governing the neutron fluxes in the core
Higher-order auditory areas in congenital deafness: Top-down interactions and corticocortical decoupling
AbstractThe theory of predictive coding assumes that higher-order representations influence lower-order representations by generating predictions about sensory input. In congenital deafness, one identified dysfunction is a reduced activation of deep layers in the auditory cortex. Since these layers play a central role for processing top-down influences, congenital deafness might interfere with the integration of top-down and bottom-up information flow. Studies in humans suggest more deficits in higher-order than in primary cortical areas in congenital deafness. That opens up the question how well neurons in higher-order areas can be activated by the input through the deprived auditory pathway after restoration of hearing with cochlear implants. Further it is unclear whether their interconnections to lower order areas are impaired by absence of hearing. Corticocortical anatomical fiber tracts and general auditory responsiveness in both primary and higher-order areas are generally preserved in absence of auditory experience. However, the existing data suggest a dichotomy between preservation of anatomical cortical connectivity in congenital deafness and functional deficits in corticocortical coupling. Further, cross-modal reorganization observed in congenital deafness in specific cortical areas appears to be established by functional synaptic changes and rests on anatomically preserved, genetically-predetermined and molecularly patterned circuitry connecting the sensory systems. Current data indicate a reduced corticocortical functional coupling between cortical auditory areas in congenital deafness, both in bottom-up and top-down information stream. Consequently, congenital deafness is likely to result in a deficit in predictive coding that affects learning ability after late cochlear implantation
The interplay between trauma, substance abuse and appetitive aggression and its relation to criminal activity among high-risk males in South Africa
AbstractBackgroundIn persistently unsafe environments, the cumulative exposure to violence predicts not only the development of posttraumatic stress disorder (PTSD), but also of increased aggression and violent outbursts. Substance use disorders interact with these developments, as drug consumption may blunt symptoms and also reduce the threshold for violent acts. Investigating the interplay between these variables and the possible cumulative effect of drug abuse on the attraction to cruelty is a crucial step in understanding the cycle of violence and developing intervention programs that address this cycle in violence-troubled communities such as low-income urban areas in South Africa.MethodsYoung males at risk (N=290) were recruited through a reintegration center for offenders in Cape Town. We assessed types of traumatic events experienced, PTSD symptom severity, appetitive aggression, committed offenses and patterns of drug abuse prior to the perpetration of violence.ResultsPath-analyses confirmed a positive relationship between exposure to traumatic events and PTSD symptom severity, appetitive aggression, the number of committed offenses and drug abuse prior to violence. PTSD symptoms were positively associated with the propensity toward aggression. Furthermore, more severe drug abuse was related to higher attraction to violence and more committed offenses.ConclusionsWe conclude that like exposure to violence, drug abuse may play a key role in the attraction to aggression and criminal acts. Measures of violence prevention and psychotherapeutic interventions for trauma-related suffering may not be effective without enduring drug abuse rehabilitation
Antiaging effects of the mixture of Panax ginseng and Crataegus pinnatifida in human dermal fibroblasts and healthy human skin
AbstractBackgroundHuman skin undergoes distinct changes throughout the aging process, based on both intrinsic and extrinsic factors. In a process called photoaging, UVB irradiation leads to upregulation of matrix metalloproteinase-1, which then causes collagen degradation and premature aging. Mixtures of medicinal plants have traditionally been used as drugs in oriental medicine. Based on the previously reported antioxidant properties of Panax ginseng Meyer and Crataegus pinnatifida, we hypothesized that the mixture of P. ginseng Meyer and C. pinnatifida (GC) would have protective effects against skin aging.MethodsAnti-aging activity was examined both in human dermal fibroblasts under UVB irradiation by using Western blot analysis and in healthy human skin by examining noninvasive measurements.ResultsIn vitro studies showed that GC improved procollagen type I expression and diminished matrix metalloproteinase-1 secretion. Based on noninvasive measurements, skin roughness values, including total roughness (R1), maximum roughness (R2), smoothness depth and average roughness (R3), and global photodamage scores were improved by GC application. Moreover, GC ameliorated the high values of smoothness depth (R4), which means that GC reduced loss of skin moisture.ConclusionThese results suggest that GC can prevent aging by inhibiting wrinkle formation and increasing moisture in the human skin
Potentially inappropriate medications prescribed for elderly patients through family physicians
AbstractThe elderly population is increasing throughout the globe, resulting in higher healthcare costs. Potential inappropriate medication (PIM) prescriptions are a major health problem affecting the elderly persons. Due to limited studies in PIM use in primary care and home healthcare in Saudi Arabia, we aim to examine the extent of PIM prescription for and use by elderly patients. This study was carried out with 798 elderly patients, arbitrarily selected from Prince Sultan Medical Military City through the patient register. The mean age of the patients were in the range of 75.2±5.5; 37.8% were males and 62.2% were females. The elderly patients are affected majorly with diabetes (73.9%), hypertension (83.2%) and lipid abnormalities (73.8%). The maximum patients involved in this study were affected with lower hemoglobin levels i.e. 99.2%. Renal impairment was found in 64% and iron supplements were the most commonly used in 23.1%, followed by analgesics and opioids (17%). The 52.5% of participants were using one or more PIMs. Kidney was the only functions and had influence on prescribed decisions. This study indicates PIM is a concern in elderly patients attending clinics and home residents and commonly prescribed ones are atypical antipsychotics, iron overdose, benzodiazepines and opioids. Prescription of drug–drug interactions, cascades and inappropriate drug doses results in preventable adverse effects
Management of Acute Myocardial Infarction in Patients With Dementia: Data From SveDem, the Swedish Dementia Registry
AbstractObjectivesWe aimed to (1) study factors that determine the use of invasive procedures in the management of acute myocardial infarction (AMI) in patients with dementia and (2) determine whether the use of invasive procedures was associated with their better survival.DesignCohort study based on patients registered in the Swedish Dementia Registry (SveDem), 2007–2012. Median follow-up time was 228 days.SettingPatients diagnosed with dementia in specialist memory clinics and primary care units in Sweden.ParticipantsA total of 525 patients with dementia who suffered AMI (mean age 89 years, 54% women).MeasurementsInformation on AMI and use of invasive procedures (coronary angiography and percutaneous coronary intervention) was obtained from Swedish national health registers. Binary logistic regression was applied to study associations of patients’ characteristics with the use of invasive procedures; odds ratios (ORs) with 95% confidence intervals (CIs) were calculated. Survival was analyzed with Kaplan-Meier curves; log-rank test was used to compare survival of patients who received an invasive procedure versus those who did not receive it. Cox regression was applied to study association of the invasive procedures with all-cause mortality; hazard ratios (HRs) with 95% CIs were calculated.ResultsOne hundred ten patients (21%) with dementia received an invasive procedure in the management of AMI. After multivariate adjustment, lower age and higher global cognitive status were associated with the use of invasive procedures. The invasively managed patients survived longer (P = .001). The use of invasive procedures was associated with a lower risk of all-cause mortality, adjusting for type of AMI and dementia disorder, age, gender, registration unit, history of AMI and comorbidity score (HR 0.35, 95% CI 0.21–0.59), or total number of drugs (HR 0.34, 95% CI 0.20–0.58).ConclusionAge and cognitive status determine the use of invasive procedures in patients with dementia. This study suggests that the invasive management of AMI has a benefit for survival of patients with dementia
Mimicking the Impact of Infant Tongue Peristalsis on Behavior of Solid Oral Dosage Forms Administered During Breastfeeding
AbstractAn in vitro simulation system was developed to study the effect of an infant's peristaltic tongue motion during breastfeeding on oral rapidly disintegrating tablets in the mouth, for use in rapid product candidate screening. These tablets are being designed for use inside a modified nipple shield worn by a mother during breastfeeding, a proposed novel platform technology to administer drugs and nutrients to breastfeeding infants. In this study, the release of a model compound, sulforhodamine B, from tablet formulations was studied under physiologically relevant forces induced by compression and rotation of a tongue mimic. The release profiles of the sulforhodamine B in flowing deionized water were found to be statistically different using 2-way ANOVA with matching, when tongue mimic rotation was introduced for 2 compression levels representing 2 tongue strengths (p = 0.0013 and p < 0.0001 for the lower and higher compression settings, respectively). Compression level was found to be a significant factor for increasing model compound release at rotational rates representing nonnutritive breastfeeding (p = 0.0162). This novel apparatus is the first to simulate the motion and pressures applied by the tongue and could be used in future infant oral product development
Component-wise conditionally unbiased widely linear MMSE estimation
AbstractBiased estimators can outperform unbiased ones in terms of the mean square error (MSE). The best linear unbiased estimator (BLUE) fulfills the so called global conditional unbiased constraint when treated in the Bayesian framework. Recently, the component-wise conditionally unbiased linear minimum mean square error (CWCU LMMSE) estimator has been introduced. This estimator preserves a quite strong (namely the CWCU) unbiased condition which in effect sufficiently represents the intuitive view of unbiasedness. Generally, it is global conditionally biased and outperforms the BLUE in a Bayesian MSE sense. In this work we briefly recapitulate CWCU LMMSE estimation under linear model assumptions, and additionally derive the CWCU LMMSE estimator under the (only) assumption of jointly Gaussian parameters and measurements. The main intent of this work, however, is the extension of the theory of CWCU estimation to CWCU widely linear estimators. We derive the CWCU WLMMSE estimator for different model assumptions and address the analytical relationships between CWCU WLMMSE and WLMMSE estimators. The properties of the CWCU WLMMSE estimator are deduced analytically, and compared by simulation to global conditionally unbiased as well as WLMMSE counterparts with the help of a parameter estimation example and a data estimation/channel equalization application
