Evaluation de l’accès aux psychothérapies pour troubles légers/modérés (2) : enquêtes transverses
International audienceContexte/Objectif : Un dispositif d’accès aux psychothérapies destiné aux adultes de 18 à 60 ans souffrant de troubles anxieux ou dépressifs légers à modérés a été expérimenté entre 2018 et 2022 par l’Assurance Maladie, dans quatre départements. Ce dispositif a fait l’objet d’une évaluation multidimensionnelle dont un deuxième volet est présenté ici.Méthodes : L’évaluation vise à apporter un éclairage détaillé sur le fonctionnement, l’acceptabilité et l’impact clinique, organisationnel et économique du dispositif. Ce deuxième volet s’appuie sur une enquête transversale menée en octobre 2020 auprès des médecins généralistes et psychologues participants.Résultats/Enseignements : 603 médecins généralistes (18 % des participants) ont répondu à l’enquête, et 436 psychologues (59 % des participants), ces derniers formés principalement aux approches psychodynamique (56 %) et cognitivo-comportementale (21 %). L’activité des répondants dans le dispositif était significative (plus de 5 patients orientés pour 73 % des médecins, plus de 10 suivis pour 72 % des psychologues).L’acceptabilité du dispositif a été jugée bonne : peu d’annulation des entretiens initiaux (aucune pour 65 % des psychologues), patients « tout à fait satisfaits » à 55 et 48 %, et « plutôt satisfaits » à 43 et 47 % respectivement pour les médecins et psychologues. La perception de l’évolution clinique était positive, une amélioration étant perçue pour « tous ou la majorité » des patients sur le plan de la santé mentale (64 % des médecins et 79 % des psychologues) et du fonctionnement social (55 % et 66 %), et pour « une partie » des patients pour la quasi- totalité des répondants. 75 % des psychologues ont poursuivi les séances avec certains patients après la prise en charge.L’utilité du dispositif était évaluée à 9/10 et 8,9/10, son fonctionnement à 6,3/10 et 4,6/10, sa généralisation souhaitée par 94 % et 84 % des généralistes et psychologues. Les principaux freins évoqués concernaient la rémunération des séances (citée par 96 % des psychologues souhaitant la généralisation), les critères d’inclusion, les tâches administratives, les modalités de collaboration et l’avis psychiatrique.Discussion/Conclusion : De nombreux médecins et psychologues ont adhéré au dispositif, perçu positivement son impact, et souhaité sa généralisation sous forme adaptée. Les différents résultats de l’évaluation ont ouvert la voie à la mise en place du dispositif MonParcoursPsy
Hébergement des personnes exilées : l’action des « espaces associatifs oppositionnels »
International audienceÀ partir de matériaux qualitatifs récoltés dans trois villes bretonnes, nous proposons de mieux comprendre ce que sont des espaces associatifs oppositionnels dans le champ de l’hébergement des personnes exilées. Des associations qui ne sont ni totalement informelles ni très structurées avec de nombreuses personnes salariées, prennent en charge les personnes exilées non hébergées dans le cadre des dispositifs publics. Nous analysons leurs actions à travers trois axes. Nous rappelons d’abord le contexte de crise de l’hébergement des personnes exilées pour expliquer les publics que les associations sont amenées à accueillir. Nous nous focalisons ensuite sur les contraintes des espaces associatifs hébergeant les personnes non prises en charge. Nous expliquons enfin les logiques autonomes que les associations peuvent tout de même construire. L’étude de ces associations permet de mieux comprendre les espaces associatifs oppositionnels et leur rôle global dans la prise en charge des personnes exilées présentes localement
12-3 - La défaveur sociale et les comorbidités expliquent-elles le moindre accès des femmes à la greffe rénale en France ?
International audienceIntroductionPlusieurs études menées au niveau international ou dans certaines régions françaises, font état d'un faible accès à la greffe rénale des femmes. Cette étude vise à analyser l'accès à la greffe rénale des femmes en France en prenant en compte la défaveur sociale et les comorbidités.MéthodeLes données de tous les patients, âgés de 18-85 ans, ayant démarré la dialyse entre 2017-2019, ont été extraites du registre REIN. Les modèles de COX et de Fine and Gray ont été utilisés pour étudier le lien entre les caractéristiques des patients, la défaveur sociale (EDI) et l'accès à la liste d'attente puis l'accès à la greffe rénale au niveau national et régional.RésultatsOnt été inclus 29 395 patients (35 % de femmes). Les hommes avaient plus de comorbidités au démarrage de la dialyse. Après ajustement sur l'EDI et les comorbidités, les femmes étaient moins susceptibles d’être inscrites sur la liste d'attente que les hommes un an (aHR: 0,91(0,87-0,96)) et trois ans (aHR: 0,87(0,84-0,91)) après le démarrage de la dialyse. Cependant cette disparité n'a été observée que chez les femmes de plus de 60 ans (aHR: 0,76(0,71-0,82) et 0,75(0,71-0,81)). En revanche, aucune différence entre les hommes et les femmes n'a été constatée en ce qui concerne l'accès à la greffe, deux ans après être inscrit sur la liste (aHR: 1,01(0,94- 1,10)). Enfin, les femmes avaient moins de chance d’être greffées 4 ans après le démarrage de la dialyse (aHR: 0,93(0,88-0,99)). Des résultats similaires ont été retrouvés dans plusieurs régions.ConclusionLes femmes âgées de plus de 60 ans ont un accès moindre à la liste d'attente et à la greffe rénale et cette disparité n'est expliquée ni par les comorbidités ni par la défaveur sociale du lieu de résidence. Une étude qualitative est en cours pour mieux comprendre les raisons de ces disparités
Early Childcare and Developmental Delay Risk at 3.5 Years: Insights from the French ELFE Cohort
International audienceWe tested the association between early childcare attendance in the first three years of life and child development at age 3.5 years in the French context, where early childcare is subsidized. In the ELFE (Étude Longitudinale Français depuis l'Enfance) birth cohort study set in metropolitan France, children’s development was reported by parents at age 3.5 years (n = 11,033) via the Child Development Inventory (CDI) questionnaire. CDI scores were transformed into a development quotient (DQ), with a DQ < 90 corresponding to possible and a DQ < 85 corresponding to a probable developmental delay. Inverse probability weighted multivariable regression models were used to analyse whether early childcare in the first three years of life (centre-based, childminder, informal or parental care) was associated to development delay. Compared to children in exclusive parental care, those in centre-based childcare (CBC) or with a childminder prior to school entry were significantly less likely to experience possible (OR = 0.56, [95% CI = 0.51–0.61] for CBC and OR = 0.77, [95% CI = 0.72–0.83] for childminder attendance) and probable developmental delay (OR = 0.62, [0.58–0.67] for CBC and OR = 0.80 [0.76–0.83] for childminder). Informal childcare attendance was not significantly associated with children’s possible nor probable developmental delay ((OR = 0.97, [0.84–1.12]) and (OR = 0.97, [0.82–1.15]), respectively). Conclusions: Overall, our findings add to the existing scientific literature, showing that in the French context, where childcare can start as early as 3 months of age, early childcare attendance can contribute to child’s development
La politique de l’étiquette. L’individualisation du gouvernement des risques face aux pollutions de l’air intérieur
International audienceFor almost forty years, indoor air pollution in enclosed spaces (homes, offices, schools) has been identified as a major health risk due to the presence of numerous toxic pollutants. In France, the government has taken administrative and technical steps to deal with this new risk. This structuring of public action followed warnings issued by international academic and technical circles as early as the 1970s, but did not come to fruition until three decades later when regulations were adopted, particularly the labelling of certain construction materials based on their potential to emit pollutants and how dangerous they are. This means of addressing the problem is particularly discreet, in the sense that it takes the form of specialised technical regulation in which a small number of experts and organisations have played a central role, in particular by working to define the issues, their relative importance, the tools for understanding that are to be used and the levers for action that are to be prioritised. This article sets out to reconstruct the genesis of the techno-administrative framing of the problem of indoor air pollution in order to understand, from the perspective of a sociology of public action and its producers, how a policy based on labelling ultimately prevailed, placing the responsibility for change on individual consumers.Depuis près de quarante ans, la pollution de « l’air intérieur » des espaces clos (domiciles, bureaux, écoles) a été identifiée comme un risque sanitaire important, du fait de la présence de nombreux polluants toxiques. En France, l’État s’est organisé administrativement et techniquement pour prendre en charge ce nouveau risque. Cette structuration de l’action publique fait suite aux alertes émises par des milieux universitaires et techniques internationaux dès les années 1970, mais ne débouche que trois décennies plus tard avec l’adoption de régulations, dont principalement l’étiquetage de certains matériaux de construction selon leur degré d’émissivité et de dangerosité. Ce traitement du problème est particulièrement discret, au sens où il prend la forme d’une régulation technique spécialisée dans laquelle un petit nombre d’experts et d’organismes a joué un rôle central, en particulier en agissant sur la définition des enjeux, de leur importance relative, des outils de connaissance à mobiliser et des leviers d’action à privilégier. Cet article propose de reconstituer la genèse de l’encadrement techno-administratif du problème des pollutions de l’air intérieur pour comprendre, sous l’angle d’une sociologie de l’action publique et de ses producteurs, comment une « politique de l’étiquette » a finalement prédominé, faisant porter la responsabilité du changement sur les consommateurs individuels
Analyzing arguments on tobacco tax increases. Focus on French parliamentary questions and responses, 2000–2020
International audienceIntroduction: Tax increases are the most effective but still the least-used tobacco control measure. The tobacco industry (TI) employs lobbying strategies to oppose the implementation of tax policies on its products. Over the past two decades, French tobacco tax policies have been characterized by a relative inconsistency. This research aims to understand why, by analyzing the arguments of French policymakers (MPs and government) between 2000 and 2020 in favor or against tax increases.Methods: To capture parliamentary debates, we performed an advanced term search on the French National Assembly website, using the keyword 'tobacco'. The search returned 5126 available documents out of which 1106 (12.6%, 645 questions, 461 responses) covered price and taxation and were included. They were analyzed using descriptive statistics and thematic content analysis (NVivo) and were compared, when relevant, to arguments raised in the international literature on TI lobbying against taxation increases.Results: We found 3176 arguments on tobacco taxation: 77.2% were against tobacco tax increases and 22.7% were in favor of tax policies. Arguments varied depending on the source: 92.4% of MPs' arguments were against tax increases, while 52.1% of arguments from government responses were in favor. The anti-tax arguments were similar to those identified in the international literature that singled out negative economic and social consequences (illicit trade, penalizing tobacconists). Other arguments that were more specific to the French context, highlighted the key economic and social role played by tobacconists in France. Pro-tax arguments highlighted the health, economic and social benefits of tax policies.Conclusions: This is the first French tobacco research on parliamentary documents, although Parliament is a place of direct TI lobbying. It will enable public health actors to better understand the arguments used by the TI in order to counter them in front of MPs, and to better monitor debates in Parliament
Meconium concentrations of pesticides and risk of hypospadias: a case–control study in Brittany, France
International audienceBackground - Hypospadias is a male genital tract defect for which an increase in prevalence has been documented over the last few decades. A role for environmental risk factors is suspected, including prenatal exposure to pesticides. Objectives - To study the risk of hypospadias in association with multiple pesticide measurements in meconium samples. Methods - The Brittany Registry of Congenital Anomalies (France) conducted a case-control study between 2012 and 2018. Cases were hypospadias, ascertained by a pediatrician and a pediatric surgeon, excluding genetic conditions, following European Surveillance of Congenital Anomalies guidelines (N = 69). Controls (N = 135) were two male infants without congenital anomaly born after each case in the same maternity unit. Mothers in the maternity units completed a self-administered questionnaire, we collected medical data from hospital records, and medical staff collected meconium samples. We performed chemical analysis of 38 pesticides (parent compound and/or metabolite) by UHPLC/MS/MS following strict quality assurance/quality control criteria and blind to case-control status. We carried out logistic regression accounting for frequency-matching variables and major risk factors. Results - Among the 38 pesticides measured, 16 (42%) were never detected in the meconium samples, 18 (47%) were in <5% of samples, and 4 (11%) in ≥5% of the samples. We observed an association between the detection of fenitrothion in meconium and the risk of hypospadias (OR = 2.6 [1.0-6.3] with n cases = 13, n controls = 21), but not the other pesticides. Conclusions - Our small study provides a robust assessment of fetal exposure. Fenitrothion's established antiandrogenic activities provide biologic plausibility for our observations. Further studies are needed to confirm this hypothesis
#1111 The impact of the Covid-19 pandemic on mortality: a study based on the National Health Data System (SNDS)
International audienceBackground and Aims: The Covid-19 pandemic had a major impact on healthcare systems, challenging the continuity of chronic disease management. Patients with Chronic Kidney Disease (CKD) were identified as particularly at risk. The aim of this study was to measure the impact of the Covid-19 pandemic on their mortality using the French National Health Data System (SNDS). Method: The SNDS contains the outpatient and inpatient care of all the French population. We applied an algorithm to identify individuals with CKD based on the healthcare consumption available in the SNDS. We identified two cohorts: individuals identified with CKD in 2016 and in 2019. These two cohorts were followed for one year after the year of identification. Patients’ characteristics were extracted from the SNDS and compared between the two cohorts. The event of interest was death documented in the SNDS with death certificates. The follow-up year was divided into periods according to the chronology of the epidemic: pre-epidemic, first lockdown, post-lockdown and second lockdown. Results: The 2016 and 2019 cohorts included 570,701 and 674,008 individuals respectively. The two cohorts didn't differ in terms of age and sex or comorbidities (median age = 76 years old, 51% female). At 31/12 of the year following identification, 10.90% (n = 62,234) of the 2016 cohort had died, compared with 10.94% (n = 76,756) of the 2019 cohort. Among the 60-year-old and less, mortality in the 2019 cohort was lower than the mortality in the 2016 cohort in all periods of the follow-up year. This remained true among those aged ≥60 years old during the pre-epidemic period (Week 1-Week 10). During the first week of the first lockdown, the instantaneous risk of death for the 2019 cohort reached 5.42 individuals per 1000 in the ≥75-year-old group, compared with 3.12 in the equivalent period for the 2016 cohort (+68%). Conclusion: All results suggest overall lower mortality in the 2019 cohort of CKD. However, an increase in mortality was observed during strong epidemic periods in the age group of those over 75. These results will be completed with the analyses of the healthcare consumption in the two cohorts describe above
Déployer l’Ésat hors des murs suffit-il à déségréguer les travailleur·euses handicapé·es ?
International audienceThe United Nations Convention on the Rights of Persons with Disabilities (CRPD) adopted in 2006 and ratified by France in 2010 promotes the right to inclusion in an ordinary environment. In terms of employment and work, Article 27 of this Convention defends the right of persons with disabilities to work on an equal basis with others and the implementation of appropriate measures to meet their special needs. Sheltered workshops, like any other specialized institutions, are associated with a form of segregation of persons with disabilities and are therefore called into question. To meet the objective of inclusion, sheltered workshops then tend to develop the placement of workers in other companies which allows disabled workers to have a professional occupation with an ordinary employer while remaining attached to their sheltered workshops. Based on doctoral research in sociology conducted between 2017 and 2021, this article aims to analyze this system by asking to what extent it makes it possible to meet the objective of inclusion.La convention des Nations unies relatives aux droits des personnes handicapées (CDPH) de 2006 et ratifiée par la France en 2010 promeut le droit à l’inclusion en milieu ordinaire. En matière d’emploi et de travail, l’article 27 de cette convention défend le droit au travail des personnes handicapées sur la base de l’égalité avec les autres et la mise en œuvre de mesures appropriées pour répondre à leurs besoins spécifiques. Les établissements et services d’accompagnement par le travail (Ésat), comme l’ensemble des institutions spécialisées, se trouvent associés à une forme de ségrégation des personnes handicapées et sont donc remis en question. Pour répondre à l’objectif d’inclusion, les Ésat tendent alors à développer la mise à disposition en milieu ordinaire qui permet à des travailleur·se·s handicapé·es d’exercer une activité professionnelle chez un·e employeur·e ordinaire tout en restant rattaché·es à leur Ésat. Basé sur une recherche doctorale en sociologie conduite entre 2017 et 2021, cet article entend analyser ce dispositif en se demandant dans quelle mesure il permet de répondre à l’objectif d’inclusion auquel il prétend
Leisure gardening and health: an overview of reviews in developed countries to inform integrated environmental health impact assessments
International audienceObjective - In the context of growing land pressure, new uses are projected on contaminated soils. In order to inform a comprehensive health assessment of gardening activities on contaminated land, this work aims to synthesise both quantitative and qualitative findings on the links between gardening, health, and well-being, taking gardeners’ motivations and practices into account.Methods -A rapid overview of reviews was conducted following PRISMA guidelines. Both therapeutic and commercial gardening were excluded, as was data from developing countries. Reviews describing at least one link between leisure gardening and health or well-being were included. Pubmed and Web of Science were searched for reviews published up to February 2023. The results were summarised in the form of a conceptual model, to support a comprehensive assessment of the links between health and gardening.Results -Gardening has been shown to impact i) quality of the environment through supporting and providing ecosystem services and raised ecological awareness, ii) mental well-being through stress relief, positive affect and the provision of meaning and purpose, iii) social capital through increased and stronger relationships, iv) physical activity through enjoyable, low-to- moderate-intensity activities, v) diet through improved food literacy and access to fruit and vegetables, and vi) (potentially) economic status through food savings. Gardening is associated with lower Body Mass Index (BMI) and better self-reported health. Evidence on other health indicators is too scarce for conclusions to be drawn. We propose a framework aggregating these results.Discussion - As a health-oriented review of reviews, some data may be missing as a result of topic classification or loss from duplicate removal. Some health determinants and contexts are less well documented; caution is needed when addressing these specific issues. Despite these limitations, the evidence gathered is sufficient to support an assessment of the health benefits of leisure gardening
