Sensitivity of lumped and semi-distributed hydrological models to 20 gridded precipitation products in a transboundary basin
International audienceThis study proposes a dual approach to assess 20 Gridded Precipitation Products (GPPs) within a transboundary region with complex topography. GPPs were first compared with observed precipitation to assess their spatio-temporal accuracy. They were then integrated into a lumped (GR4J) and a semi-distributed (MGB-IPH) hydrological model to evaluate their impact on streamflow simulations for three basins. Even if most GPPs effectively captured the dominant north-south precipitation gradient shaped by the Andean topography, the results show significant variations in GPP effectiveness across the considered basins. The most reliable GPPs for streamflow simulation across Katari, Ilave, and Ramis basins are MSWEP, CHIRPS, and MSWEP when considering the lumped GR4J model and SM2Rain_CCI, IMERG_FR, and SM2Rain_CCI when considering the semi-distributed MGB model. This discrepancy among the models shows that GPPs' reliability assessment is sensitive to the model structure and that different conclusions could be made according to the selected model. Our findings show that the GR4j lumped model is barely influenced by precipitation bias due to its buffering capacity. In contrast, the semi-distributed MGB-IPH model is sensitive to precipitation bias in space and time and therefore is more suitable to reveal GPP inconsistencies. Overall, this study not only provides GPP reliability feedback but also new insights on the respective limits and advantages of different assessment methods (i.e., gauges, lumped and semi-distributed models). These findings support the development of a practical framework for GPP selection according to the forecast use.</div
Effects of variety and culture site on nutritional composition of cowpea in Burkina Faso
This volume follows on from the Rencontres Francophone Légumineuses (RFL) held in March 2024, organized by CIRAD, INRAE, Terres Inovia and Terres Univia, and co-organized by the Institut Sénégalais de Recherches Agricoles (ISRA). This unique event brings together all those involved in development, the supply chains and research into grain and forage legumes. It's an opportunity to exchange views, share knowledge and accelerate innovation around legumes. The RFLs focus on seed, forage and woody legumes, and aim to cover the diversity of production systems, as well as the diversity of uses: food, feed and non-food. Pulses are a major lever for meeting the global challenges of climate change and food and nutritional security. All over the world, they play a strategic role, providing nutritional benefits and ensuring protein and food sovereignty. The many species of pulses also represent a formidable breeding ground for food innovation.International audienceCowpea is a legume, rich in protein, B vitamins and minerals (magnesium, zinc), but also rich in antinutritional factors such as α-galactosides and phytic acid. The nutritional composition of cowpea seeds is therefore a criterion that should be taken into account to improve varietal selection. The nutritional composition of 13 varieties of cowpea cultivated in Burkina Faso was characterised in order to identify varietal selection criteria based on nutritional potential. Nine varieties were cultivated in Tenkodogo (T), 10 in Kamboinsé (K), and 6 in both localities. Protein content were determined by the Kjeldahl method, mineral content by ICP-OES, thiamine content by UHPLC-FLR, phytic acid content by HPAEC-CD and α-galactosides and low-molecular-weight sugars content were quantified by HPAEC-PAD. Variance analysis allowed to evaluate the effect of variety using results obtained in each site (nT=9, and nK= 10), as well as effect of locality using results from varieties cultivated in both sites. The protein contents of the samples studied vary between 19,9 and 26,2 g/100 g DM depending on the variety. The phytate content was higher on the Kamboinsé plot (with results ranging from 0,70 to 1,17 g IP6/100 g DM) than on the Tenkodogo plot (0,42 to 0,80 g IP6/100 g DM). Thiamine equivalent contents, ranging from 0,34 to 0,73 mg/100 g DM, showed a significant variety and plot effect. The main forms of thiamine (di-phosphate and hydrochloride thiamins) were present in variable proportions depending on the variety and the plot. Iron and zinc content were mainly affected by variety and varied between 3,70 to 5,92 mg/100 g DM and from 3,30 to 5,14 mg/100 g DM respectively. The total α-galactoside content was between 3,27 and 6,09 g/100 g DM, and the sucrose content ranged from 1,29 to 5,80 g/100 g DM, both influenced by variety. All components were also influenced by the interaction between variety and plot. Results of nutritional composition should be related to agriculture and processing practices to identify processes decreasing antinutritional factors (α-galactosides, phytic acid) while taking account consumers preferences. Phyate, iron, zinc, thiamin, α-galactoside contents could constitute additional criteria in varietal selection of cowpea, in Burkina Faso.</p
Context: Sexual and gender-based violence (SGBV) against private general practitioners (GPs) perpetrated by their patients, are common, affecting practitioners’ mental health and their relationships with patients. To date, there are only a very few qualitative studies exploring the experiences of GPs facing this phenomenon. Objective: Explore GPs’ experiences in private practice facing SGBV from their patients, focusing on perceived psychological impact and coping strategies of physicians. Method : This is a phenomenological qualitative study. Participants were recruited by purposive sampling. Eight in-depth interviews were conducted between august 2023 and April 2024 with GPs who experienced SGBV from their patients and analyzed according to a method inspired by interpretative phenomenological analysis. Results: SGBV can take many forms, from gender-based remarks and inappropriate physical behaviors to harassment. The psychological impact differs between male and female GPs, with the latter reporting feelings of fear, insecurity and self-doubt. To protect themselves, physicians adopt postural prevention strategies, modifying their attitude and appearance, and organizational strategies, including working in group practices or selectively choosing their patient base. The sensitivity of these doctors, reinforced by their personal experiences, is reflected in their commitment to the fight against SGBV. In response to this recurring problem, all participants stressed the importance of raising awareness and training future GPs to meet these challenges. Conclusion: Our study sheds light on the reality of sexism and sexual violence suffered by GPs from their patients, their psychological repercussions and the coping measures developed to face them. It also underlines the active involvement of doctors in the fight against SGBV and the need to raise awareness and train future GPs in these challenges, to ensure a safe and respectful working environment.
Background: Childhood obesity requires, according to the High Authority of Health (HAS), a coordinated multiprofessional approach with the implementation of Patient Therapeutic Education (PTE). In 2022, the Pediatric Resource Centre (PRC) in La Réunion developed the PTE program “Sois bien”. In 2023, for the first time, this program was made available (MA) to a Multiprofessional Health Center (MHC) for their patients. The objective of this study was to explore the perceptions of professionals who participated in this project.Method: A qualitative study inspired by grounded theory. We conducted 8 semi-structured interviews with professionals from the MHC, the PRC and the Regional Health Agency (RHA) of La Réunion from May 2023 to May 2024, and observed a collective workshop of the PTE program “Sois bien”.Results: Patient absenteeism, perceived lack of motivation, family complexities, underestimated workload and insufficient remuneration were major obstacles to the smooth running of the project. At the same time, the overinvestment and frustration of professionals led to a risk of exhaustion and deterioration of the caregiver-patient relationship. However, patient progress, the feeling of better knowing the families and improve the care thanks to collaborative approach with the teamwork were sources of satisfaction and allowed the project to succeed. Conclusion: The development of PTE closer to patients' living environment by a multiprofessional team that knows them seems a relevant approach to develop quality of care. This approach requires the practice of Motivational Interview in parallel of the program, and a quality approach to teams to prevent exhaustion and ensure the sustainability of the program.
Nutritional and microbiological dynamics in the preparation of prahoc fish paste
International audiencePrahoc is a traditional fermented fish widely consumed in Cambodia. Nevertheless, the processing and nutritional values of this daily-consumed product were poorly described. This study offers a detailed analysis of the biochemistry, nutritional composition, and microbiota during the six-month Prahoc incubation. Macronutrients (e.g. lipids, proteins) are rather well preserved during the preparation of the fish paste but the fatty acid and amino acid profiles are slightly modified at the end of the unit operation. Free amino acids increased, which facilitates the in vitro digestibility of the final paste, while beneficial fatty acids, such as eicosapentaenoic and docosahexaenoic acids, decreased. At the end of the process, the peroxide value was nearly five times greater than the limit set by the Codex Alimentarius (10 meq O 2 /kg). Biogenic amines, particularly cadaverine, were present but remained within acceptable limits. Metabarcoding analysis revealed that salt-tolerant bacteria dominated the fermentation process, while fungal activity was minimal. Lactic acid bacteria, such as Vagococcus and Streptococcus , were predominant before salt addition, while the fish pathogen Aeromonas established itself immediately after. Clostridium remained steady throughout, and Lentibacillus became dominant after six months. Food safety concerns related to biogenic amines, peroxides, and Clostridium highlight the need for establishing standard operational practices among national processors to mitigate food risks
Background: Recent data suggest the benefit of early use of high-efficacy therapies (HET) in relapsing-remitting multiple sclerosis (RR-MS). The aim is to analyze real-world evolution of anti-CD20 prescriptions at Bordeaux University Hospital and to compare the effectiveness of early introduction of HET with anti-CD20 monoclonal antibodies versus therapeutic escalation strategy.Method: This is a retrospective monocentric study based on the local EDMUS database. RR-MS patients receiving anti-CD20 treatment and followed for at least two years were classified into the therapeutic escalation (ESC) group if they had received at least one platform treatment before anti-CD20 or into the Early Intensive Treatment (EIT) group. Propensity score-matched linear regression analyses were conducted to compare the EDSS at two years of treatment and at last follow-up. The profile of patients treated since 2021 was compared to that of patients treated before 2021.Results: 217 patients were included in the ESC group and 102 in the EIT group. The median EDSS at 2 years and at last follow- up (median duration 12 years) were lower in EIT group (1.5 in EIT group, 2.5 in ESC group, p 0.001). Patients treated with anti-CD20 since 2021 received fewer treatment lines, had a lower annualized relapse rate and less gadolinium- enhancement at treatment initiation. Conclusion: This study shows that early initiation of high-efficacy treatment with anti-CD20 in RR-MS has an impact on long-term neurological disability. In real world, anti-CD20 are used earlier in the therapeutic sequence.
Background : Acute Mountain Sickness (AMS) occurs at altitudes above 2500m. Without rapid treatment, the progression of this pathology can quickly progress to death. Prevention and knowledge of this disease are essential, even more so in isolated environments where medical evacuation is inherently difficult. The objective of this study is to assess the prevalence and severity of AMS in newcomers to the Franco-Italian scientific base Concordia located in Antarctica at 3233m altitude. We also examined the influence of potential risk factors and the Lake Louise Questionnaire Score after it’s update in 2018.Methods: in this observational cross-sectional study, AMS was assessed by medical staff on the day of arrival using the Lake Louise Score (LLQS, 2018 version) and daily until a maximum follow-up of 5 days. Severity was graded into mild, moderate and severe. Various parameters such as barometric pressure, gender, age and medications were also collected. Results: the overall prevalence of acute mountain sickness (AMS) according to the 2018 Lake Louise Questionnaire Score (LLQS) classification was estimated at 16.5% at 3233m. The AMS severity increased until the 1st day of follow-up and decreases until it disappears after 2 days of follow-up. A total of 53 participant (54.6% [44.7%; 64.2%]) reported experiencing headaches. Gastrointestinal symptoms were reported by 14 participant (14.4% [8.8%; 22.8%]). Fatigue was reported by 46 participant (47.4% [37.8%; 57.3%]). Dizziness was reported by 14 participant (14.4% [8.8%; 22.8%]). There were only mild severity AMS according to the LLQS classification, but one participant had to be evacuated from the site for a High-altitude pulmonary oedema. Conclusion: AMS is common at 3233m on Concordia Station. It can be severe and lead to medical evacuation without having classic warning signs detectable by LLQS2018.. Better knowledge and corrective action must be taken for security and well-being at work.Introduction : Le Mal Aigu des Montagnes (MAM) survient à des altitudes supérieures à 2 500 m.
Introduction: High-impact exercises, such as trail running, increase intra-abdominal pressure, placing stress on pelvic floor structures. This study hypothesized that trail running could promote the development of pelvic floor disorders (PFD) and that specific associated factors could be identified. Methods: A cross-sectional observational study was conducted via an online questionnaire. Collected data included demographic and anthropometric information, trail running practices, PFD symptoms, and medical and obstetric history. Prevalence rates were estimated with 95% confidence intervals, and associations between variables were analyzed using logistic regression. Results: A total of 3,283 female trail runners completed the questionnaire. Nearly one in two women (48.8%) reported currently experiencing at least one PFD, and the overall prevalence including past symptoms reached 67.3%. Stress urinary incontinence (SUI) was the most frequently reported disorder (37.9%), followed by pelvic pain (20%), urgency urinary incontinence (UUI, 15.5%), anal incontinence (10%), and pelvic organ prolapse (1.4%). Main associated factors included parity, vaginal delivery (linked to SUI), a BMI ≥ 25 (linked to SUI and UUI), chronic constipation (linked to UUI and anal incontinence), and family history of PFD.Conclusion: Trail running, through its specific biomechanical stresses, may contribute to the onset or worsening of PFD, particularly SUI. These results highlight the need to raise awareness among female trail runners and to develop targeted prevention strategies.
Improvement of Nanopore sequencing provides access to high quality genomic data for multi-component CRESS-DNA plant viruses
International audienceBackground Faced with the recrudescence of viral CRESS-DNA plant diseases, the availability of efficient and costeffective tools for routine diagnosis and genomic characterisation is vital. As these viruses possess circular singlestrand DNA genomes, they have been routinely characterised using rolling circle amplification (RCA) coupled with Sanger sequencing. However, while providing the basis of our knowledge of the diverse CRESS-DNA viruses, this approach is laboratory-intensive, time-consuming and ultimately ineffective faced with co-infection or viruses with multiple genomic components, two common characteristics of these viruses. Whereas alternatives have proved effective in some applications, there is a strong need for next-generation sequencing methods suitable for small-scale projects that can routinely produce high quality sequences comparable to the gold standard Sanger sequencing. ResultsHere, we present an RCA sequencing diagnostic technique using the latest Oxford Nanopore Technology flongle flow cells. Originally, using the tandem-repeat nature of RCA products, we were able to improve the quality of each viral read and assemble high-quality genomic components. The effectiveness of the method was demonstrated on two plant samples, one infected with the bipartite begomovirus African cassava mosaic virus (ACMV) and the other infected with the nanovirus faba bean necrotic stunt virus (FBNSV), a virus with eight genomic segments. This method allow us to recover all genomic components of both viruses. The assembled genomes of ACMV and FBNSV shared 100% nucleotide identity with those obtained with Sanger sequencing. Additionally, our experiments demonstrated that for similar-sized components, the number of reads was proportional to the segment frequencies measured using qPCR. ConclusionIn this study, we demonstrated an accessible and effective Nanopore-based method for high-quality genomic characterisation of CRESS-DNA viruses, comparable to Sanger sequencing. Face with of increasing challenges posed by viral CRESS-DNA plant diseases, integrating this approach into routine workflows could pave the way for more proactive responses to viral epidemics.</div
