Prise en charge précoce de l’obésité pédiatrique : définition et impact sur les résultats de prise en charge. Étude de 1510 enfants et adolescents dans le RéPPOP Aquitaine de 2006 à 2015
Background: Childhood obesity has been recognized as a major public health issue. The importance of early detection of overweight and obesity in children has been highlighted in many recommendations. The main objective of this study is to assess the impact of an "early management" of pediatric obesity, on the BMI Zscore for children monitored within RéPPOP Aquitaine. Population and methods: Materials and methods: 1510 children, aged 2 to 18, who received care in RéPPOP Aquitaine between January 2006 and January 2015 were included. We created the variable "management delay" (duration in years between the beginning of the onset of overweight and the initiation of care in RéPPOP); care is defined as "early" if the child has a management delay strictly lower than the median of the management delay for his age group and as "late" if the child has a management delay greater than or equal to the median of the management delay for his age group.Results: 1510 children aged on mean 10.5±3.1 years were included, 58.5% were girls and 59.5% were obese at inclusion. The mean BMI Zscore at inclusion was 3.48±1.05 and 3.10±1.13 at the end of care. 77.2% of patients had decreased their BMI Zscore. Early management delay was significantly associated with a significant decrease in relative ∆BMI Zscore in both univariate (-0.12±0.21VS-0.10±0.17) and multivariate (-0.061[-0.111;-0.012] analysis. Conclusion: Early management of pediatric obesity, within RéPPOP Aquitaine has a positive impact on reducing the corpulence of children.Introduction : L’obésité infantile a été reconnue comme un problème majeur de santé publique. L’importance de repérer précocement le surpoids et l’obésité de l’enfant a été mise en avant dans de nombreuses recommandations. L’objectif principal de ce travail est d’évaluer l’impact d’une prise en charge précoce de l’obésité pédiatrique sur le Zscore d’IMC d’enfants suivis par le RéPPOP Aquitaine.Population et méthodes : Nous avons inclus 1510 enfants, âgés de 2 à 18 ans, ayant bénéficié d’une prise en charge dans le RéPPOP Aquitaine entre janvier 2006 et janvier 2015. Nous avons créé la variable « délai de prise en charge » (durée en années entre le début de l’apparition du surpoids et l’initiation de la prise en charge dans le RéPPOP) ; la prise en charge est définie comme « précoce » si l’enfant a un délai de prise en charge strictement inférieur à la médiane du délai de prise en charge de sa tranche d’âges et comme « tardive » si l’enfant a un délai de prise en charge supérieur ou égal à la médiane du délai de prise en charge de sa tranche d’âges. Résultats : 1510 enfants âgés en moyenne de 10,5±3,1 ans ont été inclus, 58,5% étaient des filles et 59,5% étaient obèses à l’inclusion. Le Zscore moyen d’IMC à l’inclusion était de 3,48±1,05 et de 3,10±1,13 en fin de prise en charge. 77,2% des patients avaient diminué leur Zscore d’IMC. Le délai de prise en charge précoce était associé de façon significative à une diminution plus importante du ∆Zscore d’IMC relatif aussi bien en analyse univariable (-0,12±0,21VS-0,10±0,17) que multivariable (-0,061[-0,111;-0,012]). Conclusion : Une prise en charge précoce de l’obésité pédiatrique, au sein du RéPPOP Aquitaine a un impact positif sur la diminution de la corpulence des enfants
Surveillance in the field: Over-identification of Ebola suspect cases and its contributing factors in West African at-risk contexts
International audienceDuring an Ebola outbreak, the WHO recommends that health professionals consider people as suspect cases (SCs) when they show key signs such as the sudden onset of high fever or specific symptoms after having had contact with a suspect or confirmed Ebola case. SCs should then get care, be isolated and be reported to health authorities until the Ebola virus disease is confirmed through a lab test. This exploratory study aims to understand this identification process in the field based on a qualitative analysis of the diagnosis and therapeutic itineraries of 19 SCs in Cote d’Ivoire and Senegal (2014–2015). Results indicate that the main criteria for SC identification at the field level were fever (understood broadly) and provenance from a highly affected country (applied indiscriminately). WHO criteria were not followed in at least 9 of the 19 cases. Several medical, social and cultural factors favour over-identification of people as SCs, including relativism in defining ‘high fever’, placism, humanitarian or securitarian bias, issues in categorising SC's contact cases, and the context of fear. To avoid undue categorisation and its possible harmful social effects, the WHO definition should be implemented more carefully in various contexts and with greater consideration for ethical issues, while prioritising diagnosis strategies with higher specificity

A giant squid (<i>Architeuthis dux</i>) off Reunion Island, western Indian Ocean: the greatest giant ever?
International audienceA freshly dead individual of the giant squid Architeuthis dux, presumably mutilated by a predator of bigger or comparable size, is reported from the south-western Indian Ocean in proximity to Reunion Island. The species was identified from body and beak morphology and validated genetically. The dorsal mantle length (DML) estimated from beak measurements (lower rostral length, LRL) varied between 2153 and 3060 mm depending on the allometric equation used. The Architeuthis dux individual described here is the biggest giant squid ever reported for the region
The videographic approach in marketing research Which methodological protocol?
International audienceThis article proposes a protocol for the videographic method in marketing research. Its aim is to develop the toolbox available to researchers by outlining how academic videography can be used and by identifying the challenges and opportunities inherent in such an approach. Providing researchers with a concrete guide in this way is also a response to the calls for more widespread use of this method in the academic community. The article presents both the application and an evaluation of the videographic method through four successive phases pre-production, production, post-production and dissemination. This protocol is based on an analysis of the origins of videographic research – primarily North American. This is complemented by an analysis of the corpus of French-language productions presented at national and international conferences, with the aim of providing a broad and non-ethnocentric vision so as to specify the methodological process and the various choices involved, thereby contributing to the development of creative and innovative studies
