Associations of obesity with socioeconomic and lifestyle factors in middle-aged and elderly men: European Male Aging Study (EMAS).
BACKGROUND: Social and lifestyle influences on age-related changes in body morphology are complex because lifestyle and physiological response to social stress can affect body fat differently. OBJECTIVE: In this study, we examined the associations of socioeconomic status (SES) and lifestyle factors with BMI and waist circumference (WC) in middle-aged and elderly European men. DESIGN AND SETTING: A cross-sectional study of 3319 men aged 40-79 years recruited from eight European centres. OUTCOMES: We estimated relative risk ratios (RRRs) of overweight/obesity associated with unfavourable SES and lifestyles. RESULTS: The prevalence of BMI ≥ 30 kg/m(2) or WC ≥ 102 cm rose linearly with age, except in the eighth decade when high BMI, but not high WC, declined. Among men aged 40-59 years, compared with non-smokers or most active men, centre and BMI-adjusted RRRs for having a WC between 94 and 101.9 cm increased by 1.6-fold in current smokers, 2.7-fold in least active men and maximal at 2.8-fold in least active men who smoked. Similar patterns but greater RRRs were observed for men with WC ≥ 102 cm, notably 8.4-fold greater in least active men who smoked. Compared with men in employment, those who were not in employment had increased risk of having a high WC by 1.4-fold in the 40-65 years group and by 1.3-fold in the 40-75 years group. These relationships were weaker among elderly men. CONCLUSION: Unfavourable SES and lifestyles associate with increased risk of obesity, especially in middle-aged men. The combination of inactivity and smoking was the strongest predictor of high WC, providing a focus for health promotion and prevention at an early age

Fire in the UK Rural-Urban Interface: estimating avoided costs to residential property assets
Wildfire is already a significant problem in parts of the UK’s rural-urban interface (RUI). With escalating demand for housing, urban areas will continue to expand into the countryside, bringing sources of ignition closer to the fuel. This research aims to develop a method for mapping and costing residential property assets at risk for wildfire threat analysis in the RUI and estimating avoided economic losses under different fire spread scenarios. Types of RUI around Crowthorne Wood (Swinley Forest, Berkshire) will be mapped by adapting a GIS-based Wildland-Urban Interface (WUI) model. House prices prior to the 2011 fire will be used to assign monetary values. Overlaying IRS wildfire locations will highlight ignition-prone RUI types. Fire footprints simulated for different weather scenarios using Prometheus fire spread modelling and the actual surveyed footprint (courtesy of Dr Thomas Smith, Kings College London, and the Forestry Commission, respectively) will be overlaid on the costed RUI map. Avoided costs are estimated by comparing costs of the property affected in actual and simulated fire footprints. Many assumptions are required, but a monetary estimate of what might have happened (costs avoided) would raise awareness of wildfire risk, help target prevention and help quantify the economic benefits of good wildfire risk management
Suitability of UK Biobank Retinal Images for Automatic Analysis of Morphometric Properties of the Vasculature.
PURPOSE: To assess the suitability of retinal images held in the UK Biobank--the largest retinal data repository in a prospective population-based cohort--for computer assisted vascular morphometry, generating measures that are commonly investigated as candidate biomarkers of systemic disease. METHODS: Non-mydriatic fundus images from both eyes of 2,690 participants--people with a self-reported history of myocardial infarction (n=1,345) and a matched control group (n=1,345)--were analysed using VAMPIRE software. These images were drawn from those of 68,554 UK Biobank participants who underwent retinal imaging at recruitment. Four operators were trained in the use of the software to measure retinal vascular tortuosity and bifurcation geometry. RESULTS: Total operator time was approximately 360 hours (4 minutes per image). 2,252 (84%) of participants had at least one image of sufficient quality for the software to process, i.e. there was sufficient detection of retinal vessels in the image by the software to attempt the measurement of the target parameters. 1,604 (60%) of participants had an image of at least one eye that was adequately analysed by the software, i.e. the measurement protocol was successfully completed. Support surfaces for pressure ulcer prevention.
BACKGROUND: Pressure ulcers (i.e. bedsores, pressure sores, pressure injuries, decubitus ulcers) are areas of localised damage to the skin and underlying tissue. They are common in the elderly and immobile, and costly in financial and human terms. Pressure-relieving support surfaces (i.e. beds, mattresses, seat cushions etc) are used to help prevent ulcer development. OBJECTIVES: This systematic review seeks to establish:(1) the extent to which pressure-relieving support surfaces reduce the incidence of pressure ulcers compared with standard support surfaces, and,(2) their comparative effectiveness in ulcer prevention. SEARCH METHODS: In April 2015, for this fourth update we searched The Cochrane Wounds Group Specialised Register (searched 15 April 2015) which includes the results of regular searches of MEDLINE, EMBASE and CINAHL and The Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2015, Issue 3). SELECTION CRITERIA: Randomised controlled trials (RCTs) and quasi-randomised trials, published or unpublished, that assessed the effects of any support surface for prevention of pressure ulcers, in any patient group or setting which measured pressure ulcer incidence. Trials reporting only proxy outcomes (e.g. interface pressure) were excluded. Two review authors independently selected trials. DATA COLLECTION AND ANALYSIS: Data were extracted by one review author and checked by another. Where appropriate, estimates from similar trials were pooled for meta-analysis. MAIN RESULTS: For this fourth update six new trials were included, bringing the total of included trials to 59.Foam alternatives to standard hospital foam mattresses reduce the incidence of pressure ulcers in people at risk (RR 0.40 95% CI 0.21 to 0.74). The relative merits of alternating- and constant low-pressure devices are unclear. One high-quality trial suggested that alternating-pressure mattresses may be more cost effective than alternating-pressure overlays in a UK context.Pressure-relieving overlays on the operating table reduce postoperative pressure ulcer incidence, although two trials indicated that foam overlays caused adverse skin changes. Meta-analysis of three trials suggest that Australian standard medical sheepskins prevent pressure ulcers (RR 0.56 95% CI 0.32 to 0.97). AUTHORS' CONCLUSIONS: People at high risk of developing pressure ulcers should use higher-specification foam mattresses rather than standard hospital foam mattresses. The relative merits of higher-specification constant low-pressure and alternating-pressure support surfaces for preventing pressure ulcers are unclear, but alternating-pressure mattresses may be more cost effective than alternating-pressure overlays in a UK context. Medical grade sheepskins are associated with a decrease in pressure ulcer development. Organisations might consider the use of some forms of pressure relief for high risk patients in the operating theatre

Temporal expression extraction with extensive feature type selection and a posteriori label adjustment
The automatic extraction of temporal information from written texts is pivotal for many Natural Language Processing applications such as question answering, text summarisation and information retrieval. It allows to filter information and infer temporal flows of events. This paper presents ManTIME, a general domain temporal expression identification and normalisation system, and systematically explores the impact of different features and training corpora on the performance. The identification phase combines the use of conditional random fields along with a post-processing pipeline, whereas the normalisation phase is carried out using NorMA, an open-source rule-based temporal normaliser. We investigate the performance variation with respect to different feature types. Specifically, we show that the use of WordNet-based features in the identification task negatively affects the overall performance, and that there is no statistically significant difference in the results based on gazetteers, shallow parsing and propositional noun phrases labels on top of the morpho-lexical features. We also show that the use of silver data (alone or in addition to the human-annotated ones) does not improve the performance. We evaluate six combinations of training data and post-processing pipeline with respect to the TempEval-3 benchmark test set. The best run achieved 0.95 (precision), 0.85 (recall) and 0.90 (Fβ=1) in the identification phase. Normalisation accuracies are 0.86 (for type attribute) and 0.77 (for value attribute).
