A cognitive behavioural case formulation approach to the treatment of psychosis.
The term 'psychosis' describes a broad range of experiences, including hallucinations, delusions and confused thinking. Schizophrenia, the most serious form of psychosis, affects 1 in 100 and is traditionally characterised as a loss of contact with reality. The symptoms of psychosis are classified as positive and negative. Positive symptoms include hallucinations, mainly auditory but also visual, tactile or olfactory, and disorders of thought, including delusions. Negative symptoms include cognitive dysfunction, loss of volition, anhedonia and poor self-care skills. Schizophrenia is frequently associated with impairment of cognition and emotion. Vocational and social functioning are often disrupted and the problem is associated with considerable social and economic burden to the individual, their carers and society as a whole
Mobile Financial Services at the Base of the Pyramid:A Systemic view for Cross-Sector Governance and Embedded Innovation
Mobile financial services is one of the uprising movements to bank the unbanked by integrating philanthropic and business approaches for financial inclusion. In this chapter, the authors address how a systemic view helps integrate the Philanthropic Initiatives (PI) and the Commercial Initiatives (CI) to get a sustainable impact on the unbanked micro-entrepreneurs. However, each approach has pros and cons as they go along the stages of design, deployment, and sustainability. Using the soft system thinking, the authors theorise the Base of the Pyramid (BoP) as a business system to mix-up the relatively high start-up capabilities of the PI with the relatively sustainable impact of CI. The mobile money case shows that donors, local private enterprises, and multinational corporations follow the BoP strategy to develop an online grid that offers a reconciled balanced scorecard for economic returns, social benefits, and local impact. Such a strategy guarantees flexible, long-term investments and facilitates developing innovative financial services
'Looking after granny': a transnational ethic of care and responsibility
Recent writing in geography has focused on the ways in which we can think and act responsibly in relation to place as well as ‘distant others’. Others have argued for a more ambiguous stance towards responsibility. In most of this literature, however, responsibility is seen as flowing from the centre to the periphery. In this paper I argue that we should also be paying attention to the spaces where responsibility flows in the opposite direction. Using the example of Bolivian elderly care workers in Spain, this paper shows that many elderly care workers are practising a transnational ethic of care that goes beyond the remit of their marketised responsibilities. Many elderly care workers place their ‘grannies’ into the realm of familial social relations and express feelings of responsibility towards them. This allows them to accept or at least bear their demanding and often demeaning work. However, this same process contributes to them staying in often exploitative working conditions while denying responsibilities of care to their own families and to themselves. The paper argues that shifting our attention to how responsibilities and care are practised in transnational social fields, allows for the recognition of the many benefits that richer countries continue to draw from the migration of people from the Global South and contributes to the construction of less Eurocentric perspectives on care and responsibility
Vaccine Induced Herd Immunity for Control of Respiratory Syncytial Virus Disease in a Low-Income Country Setting.
BACKGROUND: Respiratory syncytial virus (RSV) is globally ubiquitous, and infection during the first six months of life is a major risk for severe disease and hospital admission; consequently RSV is the most important viral cause of respiratory morbidity and mortality in young children. Development of vaccines for young infants is complicated by the presence of maternal antibodies and immunological immaturity, but vaccines targeted at older children avoid these problems. Vaccine development for young infants has been unsuccessful, but this is not the case for older children (> 6m). Would vaccinating older children have a significant public health impact? We developed a mathematical model to explore the benefits of a vaccine against RSV. METHODS AND FINDINGS: We have used a deterministic age structured model capturing the key epidemiological characteristics of RSV and performed a statistical maximum-likelihood fit to age-specific hospitalization data from a developing country setting. To explore the effects of vaccination under different mixing assumptions, we included two versions of contact matrices: one from a social contact diary study, and the second a synthesised construction based on demographic data. Vaccination is assumed to elicit an immune response equivalent to primary infection. Our results show that immunisation of young children (5-10m) is likely to be a highly effective method of protection of infants
Endothelial Progenitor Cell Phenotype and Function Are Impaired in Childhood-Onset Systemic Lupus Erythematosus.
Biallelic mutations in the autophagy regulator DRAM2 cause retinal dystrophy with early macular involvement.
Retinal dystrophies are an overlapping group of genetically heterogeneous conditions resulting from mutations in more than 250 genes. Here we describe five families affected by an adult-onset retinal dystrophy with early macular involvement and associated central visual loss in the third or fourth decade of life. Affected individuals were found to harbor disease-causing variants in DRAM2 (DNA-damage regulated autophagy modulator protein 2). Homozygosity mapping and exome sequencing in a large, consanguineous British family of Pakistani origin revealed a homozygous frameshift variant (c.140delG [p.Gly47Valfs(∗)3]) in nine affected family members. Sanger sequencing of DRAM2 in 322 unrelated probands with retinal dystrophy revealed one European subject with compound heterozygous DRAM2 changes (c.494G>A [p.Trp165(∗)] and c.131G>A [p.Ser44Asn]). Inspection of previously generated exome sequencing data in unsolved retinal dystrophy cases identified a homozygous variant in an individual of Indian origin (c.64_66del [p.Ala22del]). Independently, a gene-based case-control association study was conducted via an exome sequencing dataset of 18 phenotypically similar case subjects and 1,917 control subjects. Using a recessive model and a binomial test for rare, presumed biallelic, variants, we found DRAM2 to be the most statistically enriched gene; one subject was a homozygote (c.362A>T [p.His121Leu]) and another a compound heterozygote (c.79T>C [p.Tyr27His] and c.217_225del [p.Val73_Tyr75del]). DRAM2 encodes a transmembrane lysosomal protein thought to play a role in the initiation of autophagy. Immunohistochemical analysis showed DRAM2 localization to photoreceptor inner segments and to the apical surface of retinal pigment epithelial cells where it might be involved in the process of photoreceptor renewal and recycling to preserve visual function
Phase II anti-inflammatory and immune-modulating drugs for acute ischaemic stroke
Stroke is the second leading cause of death worldwide and the leading cause of adult neurological disability. Despite advances in stroke unit care, and increasing use of thrombolysis, there remains an urgent need for safe and effective treatments for acute ischaemic stroke. However, this is against a backdrop of multiple failures in translational drug development. Cerebral ischaemia initiates a complex cascade of immune and inflammatory pathways in the brain microvasculature and periphery, which contribute to the evolution of cerebral injury, resolution and repair. Targeting specific inflammatory or immune pathways, therefore, represents an attractive treatment strategy in acute ischaemic stroke. Although anti-inflammatory drugs have already failed in clinical trial development, several are currently at the Phase II developmental stage. Areas covered: The authors highlight several candidate drugs, which modulate a range of inflammatory and immune pathways, and have been investigated in pre-clinical and Phase II studies to date. Expert opinion: Drugs targeting inflammatory and immune pathways offer theoretical advantages including potentially longer therapeutic time windows and effects complementary to thrombolysis (ameliorating reperfusion injury). Fundamental changes in the approach to pre-clinical and clinical drug development are required to facilitate successful translation of promising candidate drugs into clinical practice
