Training for the Practice of a Biotechnological and Human Medicine
During the last decades, medicine has known a beneficial leap due to research and innovation in new biotechnologies permitting more efficient, accurate, and targeted care for many diseases. At the same time, better prevention in safety conditions and patient-centered-care organization involved tight cooperation between multiple care-health professionals according to the patient's needs. Medical students have to be trained to acquire technology advances and prepare to operate in the team before caring for patients? Such practice training in simulation centers aims to develop authentic real-life scenarios of patient care situations, including individual or team interactions skills and clinicians' ability for shared decision and human relationship as listening, announcement and coaching based on ethical questioning and careful of person
Improving Management of Chronic Diseases by Generalist Practitioner
Chronic diseases lead to most deaths and health expenses in developed countries. Early diagnosis prevents these diseases from complications and decreases their cost. General practitioners seem to be the most available physicians to assume the health path of such patients. However several factors may affect their efficacy: the dramatic decrease of French practitioners since several years, the lack of adequate formation and medical time whereas two factors may affect the quality of therapeutic control during the follow up: poor compliance in drug use by the patient and clinical inertia by the general practitioner, both leading to reduced efficiency of therapy, increased complications and health expenses. Therapeutic education of the patients is the best response to improve the management of chronic diseases. However, most practitioners are unable to assume all the steps of health’s path including early diagnosis, prevention of chronic diseases, control, and therapeutic education. The reasons are the lack of formation, lack of time, and financial difficulties. To improve this situation we suggest: 1- to include psychological, social, and therapeutic education in the course of medical studies with a practical formation in urban and rural areas with particular attention for deprived populations. 2-to develop therapeutic education and the use of e-health for general practitioners. 3- to improve the links between private and hospital practices. 4- to increase the number of medical centers including medical and paramedical practitioners participating in a global health network coordinated by the general practitioner
Health Cooperation, France, a Strong Commitment
Health has been identified by the government as a field where France can strengthen its international presence. The global health landscape is complex and many actors need coordination at the national level and in countries where the partnerships are the most important. Our national pharmaceutical and medical device companies as well as our research institutes are at the best global level. The French health system is one of the more powerful in the world. These assets strengthen the relevance to improve the international role of France. Specific actions in progress are able to increase more and more the collective effectiveness. The shared conception of the practice of medicine between France and Morocco opens wide scope and prospects to strengthen cooperation between medical sites of our two countries. In the teaching and research fields shared projects already exist and request to be developed on a clear financial basis. The participation of industrialists to strengthen the scope of these partnerships must be imagined
Evolution of the Quality of Life Concept in Morocco Case of Rheumatoid Arthritis and Ankylosing Spondylitis
Rheumatoid arthritis (RA) and ankylosing spondylitis (AS) are at the head of chronic inflammatory rheumatisms. The suffering caused by chronic rheumatic pain, the repercussions on the patients daily life and their entourage, undoubtedly alter their quality of life. We summarize the studies that have addressed this aspect in Moroccan patients suffering from RA and AS. These studies unanimously show the disturbances caused by the disease and their negative impact on the Moroccan patients’ quality of life. Indeed the consequences are significant on the physical, psychological, and social domains.Longitudinal studies carried out in particular in RA, attest to the importance of adequate management of those chronic rheumatic diseases to improve our patients’ quality of life
Emergencies and Disaster Medicine
Human concentration, the development of technologies, threats of destruction, and contamination are at the origin of an awareness of the need for civilian assistance. Urgent medical aid, which is developing and moving towards network medicine, in partnership with all the structures contributing to the emergency, is contributing to the rationalization of social policy. The actors of emergency medical aid thus contribute to safeguarding, while improving, a health and social sector, which is now one of the foundations of democracy and one of its foundations in the face of the challenges of the future
Relationship Between Disease Activity and Hearing Loss in Rheumatoid Arthritis Patients A Case Control Study
Objectives: Rheumatoid arthritis (RA) is well known to affect many different organ systems. Previous work suggests that this includes the auditory system. The objectives of this work are to evaluate the pattern of hearing impairment in patients with rheumatoid arthritis and also to examine the possible associations between hearing impairment and related RA features especially disease activity. Materials and methods: Thirty RA patients (mean age of 44.5 ± 9.9 years; female sex (90%)) and 17 healthy controls (mean age of 41.5±9.1 years; female sex (76.4%)) were included in our study. The 2 groups were matched for age and sex (p>0.05). The otoscopic examination was normal in all participants. No subject of the 2 groups has had any abnormalities at the otoscopic examination. Hearing impairment was evaluated by pure tone audiometry and tympanometry including static compliance, middle ear pressure, stapedial reflex threshold test. In all patients, the clinical features, laboratory data, X-rays, disease activity index-DAS 28 were performed. Results: Hearing loss was more prevalent in RA patients compared to healthy controls (56.7 vs 11.8%; p=0.005). RA patients have conductive, sensorineural, and mixed hearing loss in respectively 43.4, 3.3, and 10 % of cases. Association analysis between hearing characteristics and remission in RA patients shows that RA patients in remission have significantly lower mean hearing thresholds (12.8±5.2dB vs 18.8±6.9 dB; p=0.04). Conclusion: This study suggests that hearing loss risk is higher in RA patients and seems to be associated with disease duration. Hearing loss in RA was directly proportional to the disease activity index-DAS 28. An audiological evaluation must be performed periodically to identify possible audiological damage
A Chronic Psychogenic Vomiting Case of Dramatic Response to Escitalopram
This case report describes the treatment of an interesting patient with vomiting for years. The patient admitted to the family health center with chronic vomiting and weight loss. Her physical examination was unremarkable. The complaint of the patient in who organic pathogen was excluded by biochemical and radiological examinations was evaluated psychologically; her complaints were ended following the initiation of escitalopram therapy, unlike previous treatment. In this report, we represent a specific patient for the escitalopram treatment, and thanks to this, it contributes a unique sample to the literature about escitalopram usage in the treatment of chronic vomiting
Erosive Adenomatosis of the Nipple
Erosive adenomatosis of the nipple is a rare benign neoplasm caused by a complex proliferation of the lactiferous ducts that affects primarily middle-aged women. Clinically, the differential diagnosis is breast carcinoma, Paget disease, and galactophorous dilatation. A correct diagnosis is fundamental to avoid unnecessary major surgery, which can increase treatment morbidity and harm the patient
Educational Use of Digital Video in the Context of Practical Work in Dentistry Prospective Evaluation Survey of the Optimal Duration of Educational Films by Second-Year Students of Dentistry in Rabat
The transmission of knowledge is a critical step that requires dedication and logistic assets to achieve the desired objective.Among these advantages is the online launch of educational films on the university's platform which aims at the improvement of understanding by students, and the information provided during practical work. For this, we have made three videos on the implementation of shaped clasps, while fixing the duration of the films successively at 8min50, 5min05, and 3min30. The videos were evaluated by the entire class through a questionnaire distributed to students who were present in the practical work training session. The biostatistical analysis revealed the preference to be for the 8min50 video with an initial viewing percentage of 59.3%. At a second and another viewing, preference was successively in favor of the 5min05 video (54.5%) and that of 3min 30 (43.4%). Furthermore, the students’ favorite videos were those with a time interval of 10 to 20 minutes (46.9%) for upcoming educational materials
