Intra-abdominal Migration of a Uterine Device
The intrauterine device (IUD) is one of the most commonly used contraceptive methods in the world. However, this method can be associated with complications, including uterine perforation with secondary migration into the abdominal cavity. Intra-abdominal migration of the IUD is a rare but serious complication. We report a clinical case of intra-abdominal migration of an IUD in a 39-year-old woman with no significant medical history, which required laparoscopic diagnosis and treatment
The Periodontal Health of Abutment Teeth Retaining Overdentures: A Systematic Review
Objective: This systematic review aims to evaluate the effect of the overdenture on the periodontal state of the dental roots supporting the prosthesis.Sources: This report followed the PRISMA Statement. A systematic search was done using a search equation in different databases: PubMed/MEDLINE, Web of Science, EBSCOhost, and Science Direct. The search was limited to publications in English and French.Study selection: All observational clinical studies that were interested in evaluating the effect of the overdenture on the periodontal health of abutment teeth were included in our systematic review. Of the 694 articles initially identified, eight studies met our inclusion criteria and were included in the systematic review.Results: There was no significant change in the alveolar bone surrounding the pillar root. The difference in pocket depth and dental mobility was insignificant. However, the loss of attachment increased over time and the height of the attached gingival decreased, especially in the mandible. This decrease was statistically significant
Isolated Epididymal Tuberculosis: A Challenging Diagnosis
There is an increasing number of cases of isolated genital tuberculosis. However, it remains challenging to diagnose this condition. The clinical manifestation is often nonspecific. There are no specific and sensitive noninvasive investigations to help confirm its diagnosis. We report the case of a young patient who presented at the Urology Clinic with chronic left scrotal swelling. We initially considered the diagnosis of either epididymal-testicular tuberculosis or left testicular tumor. An epididymal biopsy was taken. The histology report of the biopsy confirmed the diagnosis of epididymal tuberculosis
Surgical Management of Incidental Intercostal Nerve Schwannoma
Schwannomas, also known as neurilemmomas or neurinomas, are benign nerve sheath tumors, representing approximately 5% of benign soft-tissue neoplasms. Thus, intercostal nerve Schwannoma is a rare entity, and biopsy-excision is usually used to establish the diagnosis. Herein, we report a clinical case of a 18 years-old male with a large chest wall mass fortuitously found on a chest X-ray that was performed as part of an employment evaluation. Evaluation of Quality of Endodontic Retreatment: A Cross-sectional Study among Private Moroccan Dentists
Objectives: To study the attitude of dentists towards root canal retreatment performed in the private sector in Casablanca. Material and methods: To conduct a descriptive cross-sectional study, an anonymous questionnaire was distributed to a representative sample of 314 Moroccan dentists. Sample size calculation and sampling were performed using Epi info software. Data were analyzed using the SPSS (Statistical Package for the Social Sciences) software. Qualitative variables were expressed by their effective and percentage, and quantitative ones by their mean and standard deviation. Results: 91.5% of dentists used endodontic retreatment in their daily practice, and 84.4% of these dentists routinely indicated it when faced with a missing root canal filling or a painful apical lesion. When disobturating, 51.1% of dentists used Desocclusol as a root canal solvent, and 57.4% opted for a combination of manual and rotary mechanized instrumentation. The most common accidents encountered were instrument fractures (68.4%), followed by stoppers (45.7%). Conclusion: The results of this study show that dentists in Casablanca were not complying with the preliminary stages of endodontic retreatment. However, we noted an evolution towards the use of new mechanized preparation techniques and digital radiography
Malignant Peripheral Nerve Sheath Tumors of the Brachial Plexus - A Case Report
Introduction: Malignant peripheral nerve sheath tumors represent all the neoplasias developed at the expense of cells located in the peripheral nerve sheaths. Complete surgical excision with microscopically healthy edges remains the only curative treatment. Through a clinical case, and a review of the literature, we will report our experience in the management of a tumor of the peripheral nerve sheaths. Observation: Madam K.L, 34 years old, admitted for management of a spontaneously painful left axillary mass, and in whom the clinical examination found a patient, in good general condition, with the presence of a 8cm left axillary mass hard to fixed palpation in relation to the 2 planes, painless on palpation. Thoracic computed tomography revealed an encapsulated left axillary tissue mass compressing the axillary pedicle measuring 84mm*67mm with areas of necrosis. MRI had objectified the presence of a fusiform mass extended to the inter-scalene space most likely evoking a tumor of the malignant peripheral nerve sheaths. The patient had undergone complete tumor excision with section of the left subclavian artery and end-to-end anastomosis, with simple postoperative consequences. Conclusion: MPNSTs are rare tumors. Surgery occupies an essential place in the treatment of this disease, it must be planned in order to avoid vascular and nervous risks. Radiotherapy may be indicated as an adjuvant or in the event of an unhealthy margin
Aneurysmal Bone Cyst of the Twelfth Rib: A Case Report
Benign tumors of the chest wall are rare. Aneurysmal bone cyst is a benign tumor of the skeleton system which mainly develops at the level of the long bones and the vertebrae, and rarely at the rib level. We present a case of a 17-year-old young female with no medical history who presents a mass at the right basal level of the chest wall. The CT scan shows a multiloculated mass at the level of the twelfth ribs, by an elective incision the tumor was resected. The histology was in favor of an aneurysmal bone cyst
Vaginal Metastatic Recurrence of a Bladder Tumor after Anterior Pelvectomy – A Case Report
Background: Vaginal metastatic recurrence after anterior pelvic pelvectomy for bladder tumor is a rare occurrence that remains unexplained and requires specific recommendations to prevent or effectively treat it in a timely manner.Case report: We report on a 69-year-old female patient with a history of hyperthyroidism who had undergone anterior pelvic pelvectomy for bladder tumor and presented a year later with vaginal bleeding. Gynecological examination with a speculum revealed four small budding tumors on the anterior vaginal wall. An anatomopathological study confirmed urothelial carcinoma. We performed surgical excision of all masses and complemented it with radiotherapy.Conclusion: The purpose of our presentation is to discuss predictive factors for vaginal metastatic recurrence and to emphasize the importance of gynecological examination in follow-up after anterior pelvic pelvectomy, which we believe should be included in the recommendations for such follow-up
Management of Post-Traumatic Sternal Fractures in a West African Country: the Case of Niger
Introduction: Traumatic fractures of the sternum are relatively rare injuries and occur against the backdrop of high-energy chest trauma. The severity of these lesions is mainly related to their association with life-threatening cardiovascular lesions. Their treatment is mostly conservative. The aim of this work is to report the results of the management of post-traumatic fractures of the sternum in Niger. Reference Hospital of Niamey. 8 of these patients were treated with conservative methods and 1 benefited from osteosynthesis by a screwed ankle plate adapted to the sternum. Results: The mean age of patients was 41.78 years (14 to 60 years). The male sex was predominant with 7 cases or 77.77%. Public road traffic is the most common occurrence circumstance found in 88.9% of cases. Symptomatology was dominated by presternal chest pain. The site of the fracture was at the level of the manubrium in 66.6% of cases and the most common type of fracture was the simple fracture in 55,56 of cases. Pulmonary contusion and hemothorax were the most associated lesions with respectively 44.4% each. There was 1 case of death in the context of polytrauma. Conclusion: Post-traumatic fractures of the sternum are rare and potentially serious, requiring multidisciplinary management
A Rare association of Mauriac syndrome and Van-Wyk Grumbach syndrome found in a young Saudi girl with poorly controlled type 1 Diabetes and Hypothyroidism: A Case Report and Brief literature review
Type 1 diabetes mellitus (T1DM) is an autoimmune disease characterized by insufficient insulin production of the pancreatic beta-cells. Patients with T1DM will have a higher risk of other autoimmune disorders like celiac and thyroid diseases. Hypothyroidism is the failure of the thyroid gland to secrete an adequate amount of thyroxine, which is required for physical growth, brain development, and cellular metabolism. Most studies reported that children with T1DM have a higher incidence of hypothyroidism than normal children, with 9.6% having hypothyroidism and 19% having positive anti-TPO antibodies. Hypothyroidism will aggravate the condition in a child with T1DM and vice versa. Uncontrolled diabetes for a long time might increase insulin resistance due to complete depression of the hypothalamus-pituitary thyroid axis. A rare complication of poorly controlled T1DM is Mauriac syndrome, characterized by elevated liver enzymes, hyperlipidemia, cushingoid features, growth retardation, and hepatomegaly due to glycogenic hepatopathy. Van Wyk–Grumbach syndrome is also a rare complication of long-standing, untreated hypothyroidism, manifested by breast development, multicystic ovary, uterine bleeding associated with lack of pubic and axillary hair growth, and delayed bone age. Here, we report a case with two rare complications of Mauriac syndrome and Van Wyk–Grumbach syndrome in a child with hypothyroidism and poorly controlled T1DM
