Morbidity and Mortality Weekly Report (MMWR): Recommendations and Reports
This report updates the 2023-24 recommendations of the Advisory Committee on Immunization Practices (ACIP) concerning the use of seasonal influenza vaccines in the United States (MMWR Recomm Rep 2022;72[No. RR-2]:1-24). Routine annual influenza vaccination is recommended for all persons aged 656 months who do not have contraindications. Trivalent inactivated influenza vaccines (IIV3s), trivalent recombinant influenza vaccine (RIV3), and trivalent live attenuated influenza vaccine (LAIV3) are expected to be available. All persons should receive an age-appropriate influenza vaccine (i.e., one approved for their age), with the exception that solid organ transplant recipients aged 18 through 64 years who are receiving immunosuppressive medication regimens may receive either high-dose inactivated influenza vaccine (HD-IIV3) or adjuvanted inactivated influenza vaccine (aIIV3) as acceptable options (without a preference over other age-appropriate IIV3s or RIV3). Except for vaccination for adults aged 6565 years, ACIP makes no preferential recommendation for a specific vaccine when more than one licensed and recommended vaccine is available. ACIP recommends that adults aged 6565 years preferentially receive any one of the following higher dose or adjuvanted influenza vaccines: trivalent high-dose inactivated influenza vaccine (HD-IIV3), trivalent recombinant influenza vaccine (RIV3), or trivalent adjuvanted inactivated influenza vaccine (aIIV3). If none of these three vaccines is available at an opportunity for vaccine administration, then any other age-appropriate influenza vaccine should be used.Primary updates to this report include the following two topics: the composition of 2024-25 U.S. seasonal influenza vaccines and updated recommendations for vaccination of adult solid organ transplant recipients. First, following a period of no confirmed detections of wild-type influenza B/Yamagata lineage viruses in global surveillance since March 2020, 2024-25 U.S. influenza vaccines will not include an influenza B/Yamagata component. All influenza vaccines available in the United States during the 2024-25 season will be trivalent vaccines containing hemagglutinin derived from 1) an influenza A/Victoria/4897/2022 (H1N1)pdm09-like virus (for egg-based vaccines) or an influenza A/Wisconsin/67/2022 (H1N1)pdm09-like virus (for cell culture-based and recombinant vaccines); 2) an influenza A/Thailand/8/2022 (H3N2)-like virus (for egg-based vaccines) or an influenza A/Massachusetts/18/2022 (H3N2)-like virus (for cell culture-based and recombinant vaccines); and 3) an influenza B/Austria/1359417/2021 (Victoria lineage)-like virus. Second, recommendations for vaccination of adult solid organ transplant recipients have been updated to include HD-IIV3 and aIIV3 as acceptable options for solid organ transplant recipients aged 18 through 64 years who are receiving immunosuppressive medication regimens (without a preference over other age-appropriate IIV3s or RIV3).This report focuses on recommendations for the use of vaccines for the prevention and control of seasonal influenza during the 2024-25 influenza season in the United States. Machinist Crushed to Death While Cleaning a Grinding Machine
On May 30, 1996, a 22-year-old machinist was crushed to death while servicing an industrial vertical surface grinding machine. The incident occurred when the victim was cleaning out the cooling fluid sump located near the bottom of the grinder. To reach into the sump, the victim laid across the grinder's magnetic chuck (table) that holds and turns the metal to be ground under the grinding stones. While his body was stretched down into the machine, the victim apparently activated a switch with his leg, starting the chuck's traverse mechanism. The chuck moved sideways toward the grinding stones and crushed the victim between the chuck and the side of the machine. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers should develop, implement, and enforce an effective lockout/tagout program. 2. Employees should be trained to safely operate and maintain the machines. 3. Employers should install emergency stop switches on all machines. 4. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. 5. Employers should be aware of educational and training resources for health and safety information.Cooperative Agreemen
Laborer Dies After Falling 25 Feet Through a Skylight
On August 23, 1995, a 21-year-old laborer was killed after falling 25 feet through a skylight while working on a warehouse roof. The victim and a co-worker had been hired as temporary laborers to paint the roof of a large warehouse. At about 3:00 p.m. on their second day at work, the laborers had completed painting for the day and had started to clean another section of the roof. The two decided to take a break and sit down. The victim went over to the plastic domed skylight and pressed against it with his hand, apparently testing its strength. The plastic broke and the victim went through the skylight, falling 25 feet to the warehouse floor below. NJDOH FACE investigators concluded that, in order to prevent similar incidents in the future, these safety guidelines should be followed: 1. Read and follow the recommendations in the attached publication, NIOSH Alert: Preventing Worker Deaths and Injuries From Falls Through Skylights and Roof Openings. 2. Develop, implement, and enforce a comprehensive employee safety program. 3. Equip fixed vertical ladders with a fall protection device.Cooperative Agreemen
Construction Worker Dies After Being Struck by a Falling Excavator Bucket
A 44-year-old construction worker (victim) was killed when a 36-inch-wide bucket weighing more than 1000 pounds, detached from its quick release coupler and fell from an excavator, landing on him as he was working in a trench. The victim and another worker had been in the trench preparing it for a pre-formed concrete manhole that they were about to install. Their supervisor (the excavator operator) sat and waited in the excavator, with the engine running, and the bucket raised a few feet above ground level so that it would be out of the way. The second man in the trench had just turned away from the victim when he heard a "click" and turned back to see the bucket fall. He attempted to move the bucket, but could not. He and the excavator operator used the excavator and a chain to remove the bucket and then called for help. The victim was pronounced dead at the scene upon arrival of emergency medical workers. In order to prevent similar instances from occurring, FACE investigators recommend that: 1. Workers should not work under heavy machinery. 2. A manual-locking pin, although it would require the operator to exit the cab or another worker on the ground to disengage, would dramatically increase the safety of those working with and around the equipment and should be a part of all quick release couplers. Also, the control panel for quick release couplers for heavy equipment should have only two positions, lock and unlock. When in the unlock position, there should be indicator lights as well as audible warnings to alert the operator and others nearby to the coupler's status. The lights should be mounted in a highly visible area. 3. All equipment should be maintained properly, including routine maintenance as recommended by the manufacturer as well as unscheduled repair and replacement of missing, damaged, or worn parts. 4. Trenches deeper than five feet should have the walls shored for workers protection.Cooperative Agreemen
Worker Killed by 90 Foot Fall from Roof Canopy
On August 7, 1997, a 45-year-old water proofer died from injuries sustained in a 90-foot fall from a concrete roof canopy at a large apartment building. Although he used fall protection while working on a scaffold, he was using none when he stood on the roof canopy to move a cable guard. New Jersey FACE investigators concluded that, in order to prevent similar incidents, the following safety guidelines should be followed: 1. Workers should use fall protection whenever in danger of falling further than six feet. 2. A job hazard evaluation should be completed; policies and worker training should be implemented based upon the findings of the hazard evaluation.Cooperative Agreemen
Drill Operator and Mechanic Electrocuted After Contacting A 7,200 Volt Overhead Power Line With A Drill Rig Boom
On July 30, 1991, a 45-year-old male drill operator and a 58-year-old male shop mechanic were electrocuted when their drill rig boom made contact with a 7,200 volt overhead power line. The incident occurred while the two men were moving a drill rig away from the side of a road where they had been drilling test bores. The workers were apparently moving the raised boom of the rig when it contacted the power line, electrocuting the rig operator. The second worker was simultaneously electrocuted while holding a steel-reinforced air hose attached to the drill rig. NJDOH FACE investigators concluded that, in order to prevent similar incidents in the future, the following safety guidelines should be followed: 1. A minimum clearance of 10 feet must be maintained between equipment and energized lines to prevent inadvertent contact. 2. Employers should ensure that employees are properly trained in the safe operation of equipment and that company safety procedures are followed at all times. 3. Drilling equipment should be posted with signs warning of contact with overhead power lines.Formerly NJ9109Cooperative Agreemen
