    Construction Laborer Crushed by Asphalt Truck while Paving Interstate Highway

    On October 24, a 34 year old male construction laborer was fatally injured when he was crushed beneath an asphalt-carrying tractor trailer while paving an interstate highway. The victim, facing away from the tractor-trailer, was shoveling old asphalt from around a catch basin adjacent to the high-speed lane. Working approximately 12 feet behind the trailer, the victim could not be seen by the truck driver. The vehicle had no backup alarm. Co-workers, seeing the truck begin to move yelled to the driver and the victim, but it was too late. The victim was run over by the left rear dual axle and was killed instantly. State police on the scene for highway traffic control called the local medical examiner who removed the body to a county hospital. The MA FACE Director concluded that to prevent similar occurrences in the future: 1. Construction company employers should ensure that one person be designated as a signalman to direct trucks backing up within highway construction sites. 2. Trucking company employers should design, develop and implement a comprehensive safety program that includes, but is not limited to, training for truck drivers in hazard recognition on construction sites and providing back-up alarms on trucks. 3. Highway paving contractors should design, develop and implement a comprehensive safety program that includes, but is not limited to, training for highway workers in controlling traffic hazards on highway construction sites. In addition, tractor-trailer manufacturers should: 4. Consider providing back-up alarms and back-up lights as standard equipment on new vehicles.Cooperative Agreemen

    Sewer maintenance worker killed by nozzle tip on jet/vac hose

    On August 6, 1999, a 29 year old male public works employee died when he was struck by the nozzle of a high pressure water hose. Two employees were doing routine cleaning of the sanitary sewer line using a combination jet/vac truck equipped with a water-powered hose and nozzle. The nozzle somehow came back up through the manhole and struck the victim in the head. His co-worker was at the next manhole some distance away, and not in visible sight due to a ravine and landscape between the two positions. His co-worker stated that as the sewer cleaning was in process, he heard the nozzle approaching his end of the sewer line, indicating there were no obstructions in the line, and radioed the victim. The victim started reeling the hose back to the truck. The co-worker heard the truck idle down and knew that the hose was back at the start of the sewer line. The victim radioed the co-worker to confirm that he was shutting the unit down. The co-worker started fastening down the manhole cover following normal procedures, when he heard the truck idle back up to operating speed. He then heard boys screaming and knew something had happened. It took him some time to get through the ravine and back to the truck. Upon arriving at the truck, he found the victim lying by the manhole with massive head injuries. The nozzle had somehow come back up through the manhole and struck the victim in the head. Bystanders reported that they heard something explode from the back of the truck. They saw the hose spraying water into the air and the victim lying on the street, and immediately called 911. The city police and fire department responded, and the victim was unconscious when they arrived. Life Flight helicopter was notified and transported the victim to a local hospital. The victim remained unconscious and died two days later. Recommendations based on our investigation are as follows: 1. Employees must follow manufacturer's instructions when using equipment. 2. Safety issues should be re-emphasized periodically during staff meetings.Cooperative Agreemen

    Farmer Dies When Skid Steer Loader Rolls Over Him After it Plunged Over Edge of Bluff

    On October 22, 2003, a 43 year-old farmer died after his skid steer loader plunged over the edge of a bluff and rolled over him. He was filling in ruts on a dirt road close to the edge of a bluff by his field and planned to cut up a tree that had fallen across the road. He was using a new Case Model 40XT skid steer loader that he had owned about two months. The victim's father came to the farm and realized the victim had not returned home. He took the tractor up to the field to check on the victim and found the skid steer loader lying on its side over the embankment. The victim was lying beside it. The father returned to the house and notified the Emergency Medical System. The sheriff was dispatched within 20 minutes and the emergency personnel were present when he arrived. They detected no signs of life and contacted the County Coroner. The Coroner received the call and pronounced the victim dead at the scene. The seat belt on the skid steer loader was not fastened and the safety restraint bar was up. The chainsaw the victim was going to use to cut the tree was located about 15 yards from the top of the hill lying in the vicinity of the path taken by the skid steer loader. The FACE investigator concluded that to help prevent similar occurrences, farmers should: 1. Always fasten the seat belt and keep the operator's restraint bar down at all times when operating a skid steer loader. 2. Walk around the work area before starting to work and look for hazards. Use extreme caution when operating machinery on or near sloped terrain or embankments and consider doing work by hand where machine use may be too hazardous. 3.     Background:Despite the high risks associated with occupational fatigue in healthcare, few organizations require nurses to screen and report fatigue symptoms. As a result, little is known about if and how nurses would report fatigue while on the job.Objective:To determine if hospital-based pediatric nurses reported fatigue as part of an active injury reporting method.Methods:This secondary analysis of qualitative data used a descriptive design with content analysis. Data from the parent study were collected at a U.S. pediatric hospital where nurses verbally reported on-shift injuries or near misses and pre- and post-shift health status via a digital voice recorder. Researchers used content analysis to independently code data for nurses\u2019 references to fatigue. Codes were then analyzed for patterns and themes.Results:Approximately 30% (n=104) of participants reported fatigue-related content. Emergent themes were Work Stressors, Individual Risk Factors, Fatigue Descriptors, Adverse Outcomes from Fatigue, Fatigue Buffers, Descriptors for Buffered Fatigue, and Favorable Outcomes from Buffered Fatigue. Fatigue descriptions align with prior literature, demonstrating the accuracy of the voice recorder data collection method. In addition, nurses expressed uncertainty about the appropriateness of reporting fatigue symptoms.Conclusion:Findings from this study support pediatric nurses will report fatigue, when provided an opportunity.     ObjectiveThis study is to compare socio-demographic, HIV testing, and prevention factors experienced by insured low-income heterosexual Black women and men.MethodsWe examined cross-sectional data from Black women and men (n = 5837) recruited in 23 U.S. cities for National HIV Behavioral Surveillance June\u2013December 2019. We compared socio-demographic and behavioral factors between groups using log-linked Poisson regression models, producing adjusted prevalence ratios and 95% confidence intervals.ResultsBlack women were less likely than Black men to have private insurance (aPR 0.61, 95% CI 0.50\u20130.74, p < 0.0001). Black women were more likely than Black men to have incomes at or below the poverty line (aPR 1.04, 95% CI 1.01\u20131.07, p = 0.02), be aware of PrEP (aPR 1.20, 95% CI 1.12\u20131.28, p < 0.0001), and have been recently tested for HIV (aPR 1.12, 95% CI 1.04, 1.20, p < 0.01).ConclusionsDespite insured status, many Black women and men experienced suboptimal access to and utilization of HIV testing and prevention services.
