    Ironworker Dies in Ohio Following a 20-foot Fall Through a Skylight Opening

    The case of a 38 year old ironworker who died after falling 20 feet through a skylight opening was investigated. The employer was a metal fabrication and erection contractor who employed 55 workers, and who had a safety policy, designated safety officer, and a comprehensive safety program which contained specific safe job procedures. The contractor had been hired to supply and erect an enclosure around a bottling operation located inside an existing building. At the time of the accident the steel skeleton and roof of the enclosure had been constructed. The victim and a foreman agreed to work overtime to finish welding support hangers for the HVAC system. While no one observed his fall, it is assumed that the victim was in the process of covering the ductwork with a fire blanket and either tripped on the angle iron and fell into, or stepped backwards into, an 18 by 24 inch skylight opening, falling approximately 20 feet and landing on the concrete floor, striking the back of his head. He died of an extensive skull fracture 2 hours after the accident. It was recommended that skylight openings be guarded by a fixed standard railing on all exposed sides, or a cover capable of supporting the maximum intended load, and that safety programs be developed to include training and educating employees in the proper methods of covering or guarding skylight openings to prevent falls.Publication date provided by the authoring office.     33 Year-Old Apprentice Substation Electrician Fatally Injured

    A 33 year old apprentice substation electrician in the final 6 months of a 6 year training program died after he came in contact with electrical energy and fell from an aerial bucket. Two journeyman substation electricians and two apprentice substation electricians were working as a crew to clean high voltage disconnect circuits at a large substation. An aerial bucket was used to access the switches. The victim had completed cleaning the fourth circuit. He left the fourth circuit closed to drain off any static charge that may have built up in the system. There would have been none had the outgoing side been grounded. The fourth circuit was reenergized. The crew foreman deenergized the incoming line to the fifth circuit. The line was tested and grounds placed on the incoming line. The victim was cleaning the switches when he contacted both sides of one of the three switches, completing the circuit. The victim did not have himself belted into the aerial bucket as required and was thrown out, falling 40 feet to the ground. Cause of death was listed as cerebral injuries received from the fall. The electrical shock was noted as the cause of the fall. The crew had not followed standard operating procedure while cleaning the switches. Recommendations arising from this accident include: the routine presentation of standard operating procedures and hazard awareness at safety meetings; and verification of the deenergization of both sides of a substation circuit before proceeding with work.Publication date provided by the authoring office.     Ranch hand killed by bulldozer while logging

    On September 30, 2004, a 52-year-old ranch hand, working as a bulldozer operator in a logging operation, was killed when the D4E Caterpillar tractor he was operating slid backward over the top of him. The ranch hand was an experienced logger. With the engine left running, gear in reverse/neutral, parking brake set, and front blade raised about 1 ft off the ground, the ranch hand had exited the cab and was busy setting chokers to logs behind the bulldozer to haul them to a landing. The bulldozer evidently did not slip into gear, but the front blade was elevated, and the bulldozer was on a slight incline and the tracks rested on branches and other woody debris, which may have contributed to instability. The victim was pinned under the bulldozer, and was pronounced dead at the scene.Recommendations 1. Before exiting a bulldozer, the machine operator must apply the brake and lower the front blade to the ground. 2. Never rely solely on the brakes of a bulldozer to prevent movement. 3. Be aware of conditions where bulldozer tracks may be unstable. 4. Develop a site-specific safety plan that includes an assessment of hazards and plans to minimize the risks.Cooperative Agreemen

    Two Feedlot Workers Suffocated in Grain Bin

    A 23-year-old mill operator and a 20-year-old maintenance technician were killed when they were engulfed in corn in a 6,000 bushel cone bottom gravity feed bin at a feedlot. The two individuals had been "knocking down" soy bean meal in a gravity feed bin next to the corn bin prior to the incident. For some reason, after their work in the soy bean meal bin they went to the corn bin and both individuals became engulfed in cracked corn. Both of these bins were active, flowing soy bean meal and cracked corn into a "clam" approximately once every five to ten minutes. The "clam" was then emptied into feed trucks. When the corn flowed from the bin, this pulled the victims under the corn. When others at the feedlot realized the two were trapped in the corn, they cut openings in the bottom of the bin to release the corn and rescue the victims. Both individuals died from suffocation. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences employers should: 1. ensure a permit is issued prior to entering bins. 2. ensure all equipment which presents a danger to employees is locked-out, blocked-off, or otherwise prevented from operating. 3. ensure employees are wearing appropriate fall protection anytime they are exposed to a fall into a bin or silo. 4. provide and have in place equipment for rescue operations which is specifically suited for the task being conducted. 5. develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard recognition and abatement. 6. consider installing locks on all accesses to bins and silos and controlling the keys to the locks. 7. consider installing phones in close proximity to bins and silos which are normally entered, or providing teams entering bins and silos with cell phones or two-way radios to provide communications to the mill operator.Cooperative Agreemen

    Background:Nonmedical use of prescription opioids (NUPO) is associated with heroin use and other adverse outcomes among adolescents. To inform the timing of substance use prevention activities and which substances to target, we examined age at NUPO initiation, associations between substance use initiation and current (past 30-day) NUPO, and order of NUPO initiation relative to other substances.Methods:Data from 2,834 students 15 or older participating in the 2017 Virginia Youth Survey, the first Youth Risk Behavior Surveillance System survey to assess age at NUPO initiation and current NUPO, were analyzed in 2019. Students reported current NUPO and ages at initiation for cigarettes, alcohol, marijuana, and NUPO (categorized as 12 or younger, 13 or 14, 15 or older, or never). Associations between age at substance use initiation and current NUPO were examined using adjusted prevalence ratios (aPRs) and 95% confidence intervals (CI).Results:Overall, 12% of students reported lifetime NUPO, with 2.4%, 4.0% and 5.6% initiating at 12 or younger, 13 or 14, and 15 or older, respectively; 5.3% reported current NUPO. Initiating cigarettes, alcohol, and marijuana at each age category (compared with never) was associated with an increased prevalence of current NUPO after adjusting for demographics and initiation of other substances. Among students initiating NUPO, initiating NUPO at 12 or younger (compared with 15 or older) was associated with an increased prevalence of current NUPO after adjusting for demographics (aPR=1.54, 95% CI: 1.10\u20132.16), but not after further adjustment for initiation of other substances (aPR=1.38, 95% CI=0.97\u20131.97). Among students initiating NUPO, 45.4% initiated NUPO before or during the same age as other substances.Conclusions:More than 6% of students initiated NUPO at 14 or younger.
