"Jedyna taka szansa w życiu". Przegląd doświadczeń studentów kierunku Pielęgniarstwo związanych z rozpoczęciem przez nich pracy zawodowej w czasie pandemii SARS-CoV-2
"JEDYNA TAKA SZANSA W ŻYCIU". PRZEGLĄD DOŚWIADCZEŃ STUDENTÓW KIERUNKU PIELĘGNIARSTWO ZWIĄZANYCH Z ROZPOCZĘCIEM PRZEZ NICH PRACY ZAWODOWEJ W CZASIE PANDEMII SARS-COV-2
Wprowadzenie. Wybuch pandemii wirusa SARS-CoV2 sparaliżował funkcjonowanie systemów ochrony zdrowia. Najistotniejszym problemem było zapewnienie odpowiedniej liczby osób personelu medycznego. Sposobem uzupełnienia braków kadr medycznych było umożliwienie studentom ostatniego roku studiów medycznych rozpoczęcie pracy zawodowej.
Cel pracy. Analiza dostępnego piśmiennictwa podejmującego zagadnienie rozpoczęcia pracy zawodowej przed studentów kierunków medycznych w czasie pandemii wirusa SARS-CoV-2.
Metoda. Przeszukano zasoby pięciu baz bibliograficznych: PubMed, Scopus, ProQuest, Web of Science, CINAHL Complete via EBSCOhost. Kryteria włączenia: 1) publikacje w języku angielskim, 2) prace opublikowane w recenzowanych czasopismach naukowych, 3) rodzaj publikacji, 4) prace opublikowane pomiędzy 01.01.2020 a 21.12.2020. Zidentyfikowano 2,341 publikacji. Do przeglądu włączono 17 publikacji.
Podsumowanie. Analiza wskazała, że jedną z najważniejszych grup kompetencji, umożliwiających sprawne wdrożenie do pracy są kompetencje miękkie (n=13). Studenci zwracali uwagę na problemy etyczne, z którymi zetknęli się rozpoczynając pracę zawodową w pandemii (n=8). Przegląd wskazał dwa czynniki, mogące mieć znaczenie dla kształtowania się w przyszłości tożsamości zawodowej studentów rozpoczynających pracę zawodową w pandemii: zaburzonego procesu adaptacji zawodowej oraz konieczność natychmiastowego wdrażania zmian do procesów kształcenia studentów kierunków medycznych"TRULY ONCE IN A LIFETIME OPPORTUNITY". AN OVERVIEW OF NURSING STUDENTS EXPERIENCES OF STUDY-TO-WORK TRANSITION IN THE SARS-COV-2 PANDEMIC
Introduction. The outbreak of the SARS-CoV2 pandemic resulted in a paralysis of healthcare systems. The most important problem was the provision of an appropriate number of healthcare personnel. One of the ways was to enable fi nal-year students of medical sciences to start professional work.
Aim. To analyse the available literature dealing with student study-to-work transition during the SARS-CoV-2 pandemic outbreak.
Method. The resources of fi ve databases: PubMed, Scopus, ProQuest, Web of Science, CINAHL Complete via EBSCOhost were researched. The inclusion criteria: 1) English language, 2) papers published in peer-reviewed journals, 3) type of publication, 4) papers published between 01.01.2020 and 31.12.2020. We have found 2,341 articles. The 17 papers were included in the fi nal analysis.
Results. The analysis pointed to soft competences as one of the most important group of competences enabling effi cient studyto- work transition (n=13). Students pointed to the ethical problems encountered at the start of work in the pandemic (n=8). Two important factors should be highlighted as crucial for the future formation of the professional identity of a group of students starting their professional career in the pandemic: the impact of the disturbed process of professional adaptation and the need to implement changes to the processes of education of students of medical science
A qualitative analysis of reasons for satisfying professional career after completing higher education studies of public health
Introduction. Public health as an academic discipline is taught at different academical levels all around the world. In Poland, the first School of Public Health was launched in 1991 in the city of Krakow, becoming then a part of the Jagiellonian University Medical College. Through more than three decades, this educational entity (Institute of Public Health Jagiellonian University Medical College; IPH JU MC) has educated hundreds of graduates who work for various healthcare stakeholders.
Aim. The goal of this study was to find the benefits resulting from studying public health, based on the experiences of IPH JU MC graduates, as well as to characterize the experiences gained and the skills developed during university studies of public health.
Material and Methods. Based on 46 interviews conducted with public health graduates, a qualitative analysis was made using NVivo 12 software. Interviews’ content was analyzed with the use of eight codes related to issues mentioned by graduates: EPH (EuroPubHealth), EBM, data analysis, English, project, HTA, practical placement, and students’ scientific clubs.
Results and Conclusions. In the interviews the graduates indicated the role of skills gained during education at IPH JU MC, e.g., drawing conclusions based on scientific research, taking part in many public health projects, better language skills thanks to English specialization path, working in the international environment. The results have shown that experiences gained during university studies of public health allow for career development in the healthcare sector
A mother’s diet as a predictor of her child’s current and future health
The body’s ‘nutritional programming’ scheme assumes that the influence of environmental factors during the so-called “critical periods’” of human development associated with, among other things, excessive or deficient nutrients can lead to permanent metabolic changes. The negative impact of external factors, poor nutrition, different gene expression during the fetal period with accompanying fetal growth retardation can permanently “reprogram” the metabolism and the course of many physiological processes, causing metabolic complications in adulthood. More and more researchers and theoreticians are considering and verifying the relationship between the early influence of environmental factors and the occurrence of civilisation diseases in the context of nutritional programming. Recently, many researchers have pointed out that the infant years are a critical time for metabolic programming.The impact of a pregnant woman’s nutrition on the development of obesity, cardiovascular disease, insulin resistance or metabolic syndrome in her child’s later life has been empirically proven. Similar results have been reported for the importance of breastfeeding and also of nutritional deficiencies or excesses in the first 1000 days of the child. This period is particularly susceptible to all the factors that affect the body and builds the foundations of the child’s broader health at present and in the future. The aim of this study is to show, on the basis of an analysis of research reports, that both woman’s nutrition during the pre-conceptional period and during pregnancy, as well as her child’s nutrition after birth, are of crucial importance for the child’s development and somatic health

DOKUMENTACJA PIELĘGNIARSKA NOWORODKÓW Z MASĄ URODZENIOWĄ PONIŻEJ 1500 G PRZEBYWAJĄCYCH NA WŁOSKIM ODDZIALE INTENSYWNEJ TERAPII NOWORODKÓW: MAPOWANIE KRZYŻOWE W CELU OPRACOWANIA PODZBIORU ICNP®
Cel pracy. Niniejsze badanie ma na celu weryfikację możliwości opracowania podzbioru Międzynarodowej Klasyfikacji Praktyki Pielęgniarskiej (ICNP) dla noworodków przedwcześnie urodzonych hospitalizowanych na Oddziale Intensywnej Terapii Noworodka (OITN).
Materiał i metody. To retrospektywne badanie opisowe jest leksykalnym mapowaniem krzyżowym z notatek pielęgniarek do ICNP. W badaniu przeanalizowano wygodną próbę 115 notatek pielęgniarskich noworodków z Oddziału Intensywnej Terapii Noworodków. Próba obejmowała wcześniaki z masą urodzeniową poniżej 1500 g hospitalizowane przez co najmniej trzy dni. Obliczono rozkład częstotliwości i procentowy rozkład częstotliwości terminów ICNP wraz z terminami ICNP dla każdej potrzeby włoskiego modelu koncepcyjnego pielęgniarstwa.
Wyniki. Aż 99,8% zjawisk pielęgniarskich w notatkach pielęgniarek odpowiadało terminom ICNP. Łączna liczba 203 terminów ICNP opisywała zjawiska pielęgniarskie, z czego 161 należało do wstępnie skoordynowanych osi. Częstotliwość występowania terminów ICNP została skategoryzowana zgodnie z włoskim modelem koncepcyjnym pielęgniarstwa. Potrzeba odżywiania i nawadniania, potrzeba oddychania oraz potrzeba eliminacji moczu i jelit były potrzebami pacjentów z największą liczbą terminów ICNP.
Wnioski. Wyniki badania sugerują możliwość opracowania podzbioru ICNP dla OITN. Konieczne są dalsze badania w celu walidacji podzbioru i nowych terminów ICNP.NURSING DOCUMENTATION OF NEWBORNS WITH BIRTH WEIGHT UNDER 1500 G IN AN ITALIAN NEONATAL INTENSIVE CARE UNIT: A CROSS-MAPPING STUDY TO DEVELOP AN ICNP® SUBSET
Aim. This study aims to verify the possibility of developing an International Classification for Nursing Practice (ICNP) Subset for preterm newborns hospitalized in Neonatal Intensive Care Unit (NICU).
Material and methods. This retrospective descriptive study is a lexical cross-mapping from nurses’ notes to the ICNP. The study analyzes a convenience sample of 115 newborns’ nursing notes of a NICU. The sample included preterm newborns with birth weight under 1500 g hospitalized for at least three days. The frequency distribution, and the percentage frequency distribution of ICNP terms, together with ICNP terms for each need of the Italian nursing conceptual model were calculated.
Results. As many as 99.8% of nursing phenomena in nurses’ notes found correspondence with ICNP terms. A total number of 203 ICNP terms described nursing phenomena, of which 161 belonged to pre-coordinated axes. Frequency of ICNP terms was categorized according to an Italian nursing conceptual model. Need of Nutrition and Hydration, need of Breathing and need of Urinary and Bowel Elimination were the patients’ needs with the highest number of ICNP terms.
Conclusions. Results of the study suggest the possibility of developing an ICNP Subset for the NICU. Further studies are needed to validate a Subset and new ICNP terms
Komfort czy dyskomfort u pacjentów w paliatywnej opiece domowej? – badanie pilotażowe
COMFORT OR DISCOMFORT FOR PATIENTS IN PALLIATIVE HOME CARE? – A PILOT STUDY
Introduction. Ensuring comfort in patients in palliative home care is a difficult task. Care, in which patient’s comfort is fundamental, was first introduced by Katharine Kolcaba.
Aim. Assessing comfort levels of patients in palliative home care, including selected determinants of this condition.
Material and methods. The study included 55 palliative care patients in home settings, receiving services provided by the home palliative care team and a group of 63 informal caregivers providing home care. A pilot study, by diagnostic survey, using a survey questionnaire with the PAPI technique was conducted among patients in palliative care treated in home-based settings in the first half of 2022.
Results. There was a statistically significant relationship between satisfaction with care, and the level of comfort provided (p<0.05). Those who strongly indicated satisfaction were characterized by significantly higher levels of comfort provided (M=35.43; SD=5.32 vs. M=33.38; SD=4.63), compared to other patients.
Conclusions. There is a correlation between the level of comfort perceived by patients, and methods of providing comfort to patients in home settings used by informal caregivers. Individuals, who are characterized by a higher feeling of comfort provision, at the same time achieve higher degree of comfort provided by the caregiver.KOMFORT CZY DYSKOMFORT U PACJENTÓW W PALIATYWNEJ OPIECE DOMOWEJ? – BADANIE PILOTAŻOWE
Wstęp. Zapewnienie komfortu u pacjentów w paliatywnej opiece domowej jest trudnym zadaniem. Opiekę, w której komfort pacjenta stanowi fundamentalne znaczenie, wprowadziła Katharine Kolcaba.
Cel pracy. Ocena poziomu komfortu u pacjentów w paliatywnej opiece domowej z uwzględnieniem wybranych uwarunkowań tego stanu.
Materiał i metody. Badaniem objęto 55 pacjentów leczonych paliatywnie w środowisku domowym, korzystających ze świadczeń realizowanych przez zespół domowej opieki paliatywnej oraz grupę 63 opiekunów nieformalnych sprawujących opiekę domową. Badania pilotażowe, metodą sondażu diagnostycznego, z wykorzystaniem kwestionariusza ankiety techniką PAPI przeprowadzono wśród pacjentów w opiece paliatywnej leczonych w środowisku domowym w pierwszym półroczu 2022 roku.
Wyniki. Wykazano istotną statystycznie zależność miedzy zadowoleniem z opieki, a poziomem zapewnianego komfortu (p<0.05). Osoby, które zdecydowanie wskazywały na zadowolenie cechowały się istotnie wyższym poziomem zapewnianego komfortu (M=35,43; SD=5,32 vs. M=33,38; SD=4.63) w porównaniu z pozostałymi pacjentami.
Wnioski. Istnieje związek pomiędzy poziomem komfortu postrzeganym przez pacjentów, a metodami zapewnienia komfortu pacjentom w warunkach domowych stosowanymi przez opiekunów nieformalnych. Osoby, które cechują się wyższym odczuciem zapewniania komfortu jednocześnie uzyskują wyższy stopień zapewnienia komfortu przez opiekuna
Zmiany zapalne błon śluzowych jamy ustnej jako powikłanie leczenia systemowego
ZMIANY ZAPALNE BŁON ŚLUZOWYCH JAMY USTNEJ JAKO POWIKŁANIE LECZENIA SYSTEMOWEGO
Wprowadzenie. Zapalenie błony śluzowej jamy ustnej (oral mucositis – OM) spowodowane leczeniem systemowym stanowi duży problem terapeutyczny. Powikłania z tym związane mogą powodować komplikacje w przebiegu zaplanowanego leczenia, co ma wpływ na rokowanie i wyniki leczenia onkologicznego. Dokładne monitorowanie przebiegu całego procesu terapeutycznego i nacisk na edukację pacjenta i rodziny może znacznie ograniczyć występowanie objawów niepożądanych.
Cel pracy. Celem pracy jest przedstawienie problemu jakiego doświadczają pacjenci onkologiczni poddani leczeniu systemowemu. W pracy opisano patogenezę zapalenia błony śluzowej jamy ustnej, obecne standardy w leczeniu oraz podkreślono rolę pielęgniarki onkologicznej w opiece nad pacjentem z objawami tego schorzenia.
Metoda. W pracy dokonano przeglądu wybranego piśmiennictwa omawiającego zapalenie jamy ustnej u pacjentów onkologicznych Prowadzone są liczne badania kliniczne dotyczące poszukiwania leków, które mogłyby mieć zastosowanie w zapobieganiu oraz leczeniu (oral mucositis – OM).
Podsumowanie. Rozwój terapii onkologicznych, tym leczenie celowane, immunoterapia znacząco wydłużyły czas życia chorych oraz czas wolny od progresji choroby. Odpowiednia profilaktyka, edukacja chorych, systematyczna obserwacja pozwala na wczesne wykrycie schorzenia co pozwala na szybkie wdrożenie leczenia objawowego. Podstawą skutecznej terapii onkologicznej jest ścisła współpraca pacjenta z lekarzem i pielęgniarką oraz czynny i świadomy udział w całym procesie leczenia i pielęgnacjiINFLAMMATORY CHANGES IN THE ORAL MUCOSA AS A SIDE SYMPTOM OF SYSTEMIC TREATMENT
Introduction. Introduction OM caused by systemic treatment is a major therapeutic problem. The related complications may causa complications in the course of planned treatment, which in turn may aff ect the prognosis and results of oncological treatment.
Aim. The aim of the study is to present the problem experienced by oncological patients undergoing systemic treatment.The paper describes the pathogenesis of oral mucositis, the current treat-ment standards and emphasizes the role of an oncology nurse in ca ring for a patient with symptoms of this disease.
Method. The paper reviews selected literature on stomatitis in cancer patients. Numerous clinical trials are conducted to search for drugs that could be used in the prevention and treatment – OM.
Summary. The development of oncological therapies, including targeted treatment and immunotherapy, signifi cantly extended the life span of patients and the time free from disease progression. Appropriate prophylaxis, patient education, systematic observation allow for early detection of the disease, which allows for quick implementation of symptomatic treatment. Evaluation of parenteral injection skills of medical undergraduates on mannequins in a tertiary care teaching hospital in South India – A cross-sectional study
Appropriate knowledge and expertise of parenteral injection is vital in the medical field. Hence we impart training on mannequins before the students practice on patients. This study was conducted to evaluate the skill exhibited by the students in parenteral injection skills, namely intravenous, intramuscular, intradermal and subcutaneous injections. The students were assessed for step based on the pre-validated check list created for Objective Structured Practical Examination. In this study, 16 % of the students failed to check the prescription order before starting the procedure. The name and the expiry date of the drug was not checked by 22 % of the students. Hundred percent of the students sterilized their hands. Loading of drug was done properly by 80 % of the students and 88 % of the students checked for air bubbles in the syringe and removed them properly. The site for injection was not sterilized by 26 percent of the students. Tourniquet was not applied by 19 percent of the students and 28 % of the students did not hold the bevel up during insertion for intravenous route. The steps where frequent faltering happened needs to be emphasized for them to perform better in a more efficient manner during the summative assessment as well as for future practice
Perineural invasion in nearby tissue adjacent to colorectal carcinoma with CD166 stem cell marker expression
Pathological evaluation of colorectal cancer resection samples play a vital role in management and prognosis of postoperative cancer patients. Difficulties exist in the assessment and outcomes of these specimens for therapy protocol. PNI is a perineural invasion associated with increased mortality in many malignancies including colon cancer. In colorectal cancer (CRC) and nearby tissue, PNI evaluation as a potential prognostic indicator with the use of CD 166 stem cell marker remains to be clearly defined for providing a convenient information for future management and prognosis. The incidence and significance of histological neural invasion in nearby tissue was conducted in 52 patients with colorectal carcinoma operated on for the period from June 2017 to June 2020 retrospectively. Tumors were subjected to histopathological and immunohistochemical study (IHC) with CD166 stem cell marker for PNI in tissue adjacent to CRC. Data collected and analyzed, histopathological pictures was obtained and studied.Outcomes showed that neural invasion was expressed by Cd166 stem cell marker as strong and severe in patients with stage B and C in tissues nearby tumor which reveals bad prognostic features.
Conclusions: Neurogenesis appeared to have a critical role in colorectal cancer progression. furthermore, current results indicated that neurogenesis functions as an independent predictor of outcomes for therapy protocol
Assessment of the potential role of Trefoil Factor-3 marker as a predictive marker of complication in splenectomized and non splenectomized patients with beta thalassemia major
The study’s goal is to appraise the immunological inflammatory marker Trefoil Factor 3, which interacts with thalassemia pathogenesis particularly following splenectomy, and may offer new therapy options for the illness and its repercussions.This is a case-control study design that included 60 patients identified as β-thalassemia major as participators in this study, in addition to 30 seemingly healthy subjects with age and sex close to the patients group who served as a control group. The participants were distributed into four groups: control group, splenectomized patients, non-splenectomized patients, and total patients. Suitable statistical techniques were employed to investigate the results.The study’s findings demonstrated that there was a significance increase in the serum levels of TFF3 when comparing between (splenectomized, non-splenectomized and total patients) with healthy group (322.16±51.241, p-value = 0.01, 317.20±42.449, p-value = 0.01, 320±46.6, p-value = 0.01), vs (309.38±21.94), respectively. Moreover, a comparison between splenectomized and non-splenectomized showed a significantly decrease in TFF3 (322.16±51.241) vs (317.20±42.449), (p-value = 0.043).The presented study also revealed significant positive correlation between TFF3 level with ferritin, iron, total iron binding capacity, transferrin saturation, transferrin, fasting serum glucose, insulin and homeostasis model assessment-insulin resistance. Furthermore, unsaturated iron binding capacity and homeostasis model assessment-beta found a significant negative correlation with TFF3 level.High serum levels of TFF3 in beta thalassemia patients, especially in splenectomies patients, are downregulated by inflammatory cytokines, which are primarily regarded as traditional inflammatory cytokines and are related to insulin resistance. Hence, TFF3 level can serve as a potential predictive for the early detection of beta thalassemia in the development and progression of complications
Medicinal plants: A promising source of anti-diabetic agents in sub-Sahara Africa
The rising burden of Diabetes mellitus (DM) globally and particularly in sub-Sahara Africa calls for alternative treatment solutions. This is because the currently available drugs for its management are limited due to undesirable adverse effects and high cost. Thus, this review explores diabetes and summarizes its treatment options, focusing mainly on medicinal plants therapy. Information on twenty-five selected medicinal plants from sub-Sahara Africa having hypoglycemic and anti-diabetic potentials was obtained via electronic search of major databases, such as Pubmed/Medline, Scopus, Google Scholar and web of science. Predominant bioactive compounds found in these plants include tannins, carpaine, terpenoids, hexadecenoic acid, luteolin, saponins, glycosides, rutin, quercetin, vindoline and kaempferol. Robust evidence indicates that these medicinal plants and their bioactive components exert their antidiabetic potentials via different mechanisms, including: regeneration of pancreatic β-cell and insulin secretion; inhibition of α-amylase, inhibition of intestinal glucose absorption and liver glucose production; antioxidative stress; limitation of glycogen degradation and gluconeogenesis; anti-inflammatory, immunoregulatory. DM imposes a tremendous burden in the region, and its prevalence is not abating; thus the rich flora of the region with known hypoglycemic and antidiabetic efficacy could be explored as a complementary therapy in its management
