    Impact of Vitamin D in the improvement of respiratory function in sickle cell disease adult patients

    The study aimed to discover the role of vitamin D in improving the respiratory function in sickle cell disease patients. In this prospective study, 188 adults were enrolled, drawn from the out-patients unit of Thalassemia Cente The collected data were categorized into three groups: group 1 included the data of 100 healthy individuals of age range of 18-50 years as control; group 2 included the data of 88 (40 males and 48 females) sickle cell disease patients in steady state before supplementation of vitamin D; group 3 represented the data of the same 88 sickle cell disease patients as measured and recorded after 6 weeks of vitamin D supplementation. Laboratory measurements including pulmonary function tests and hematological parameters, while vitamin D levels were estimated for all groups to compare the data before and after supplementation of vitamin We found significant differences in the pulmonary function tests when comparing groups. Mean value of FEV1 revealed significant differences between group 1 and 2 (p >0.05), while vitamin D supplement created a significant difference between group 2 and 3 (p >0.05). The improvement in FEV1 did not reach to that of healthy (group 1), as referred by the significant variation between group 1 and group 3 (p>0.05). The same findings were indicative within most pulmonary function tests, and the percentages of combined cases (restrictive and obstructive) were also deceased in group 3. Conclusion: supplement of vitamin D in SCD patients could result in relative improvement in lung function

    AN ASSESSMENT OF THE QUALITY OF LIFE IN CHRONIC HEART FAILURE PATIENTS Aim. The aim of this cross-sectional study was to evaluate quality of life (QoL) and selected aspects of patients with chronic heart failure. Material and methods. The Minnesota Living with Heart Failure Questionnaire was used for an assessment of QoL. Data were obtained on depression, illness perception, social support, self-sufficiency, and severity of heart failure according to NYHA classifi cation. Data were analysed using descriptive statistics, the Kruskal-Wallis and Mann-Whitney test, and the Spearman correlation coeffi cient. Linear regression analysis was also performed. Results. Correlation analysis (p < 0.05) indicated that patients with reduced self-sufficiency (r = -0.3529) and patients with more severe heart failure (r = 0.2642) reported a poorer QoL. Worse the illness perception (r = 0.4113), more frequent depression (r = 0.5470) and a worse subjective assessment of the state of health (r = 0.4394) indicated a worse QoL. The predictors of the total QoL score were depression (p = 0.000), illness perception (p = 0.001), self-suffi ciency (p = 0.008), and subjective assessment of the state of health (p = 0.005). Conclusions. A comprehensive approach with an emphasis on improving QoL is necessary in the care of patients with chronic heart failure.OCENA JAKOŚCI ŻYCIA PACJENTÓW Z PRZEWLEKŁĄ WADĄ SERCA Cel pracy. Celem badań przekrojowych była ocena jakości życia (QoL) oraz wybranych aspektów pacjentów z przewlekłą niewydolnością serca. Materiał i metody. Do oceny QoL wykorzystano kwestionariusz Minnesota Living with Heart Failure. Uzyskano dane dotyczące depresji, postrzegania choroby, wsparcia społecznego, samowystarczalności oraz ciężkości niewydolności serca według klasyfikacji NYHA. Dane analizowano za pomocą statystyki opisowej, testu Kruskala-Wallisa i Manna-Whitneya oraz współczynnika korelacji Spearmana. Przeprowadzono również analizę regresji liniowej. Wyniki. Analiza korelacji (p < 0,05) wykazała, że pacjenci z obniżoną samodzielnością (r = -0,3529) oraz pacjenci z cięższą niewydolnością serca (r = 0,2642) zgłaszali gorszą QoL. Gorsze postrzeganie choroby (r = 0,4113), częstsze występowanie depresji (r = 0,5470) oraz gorsza subiektywna ocena stanu zdrowia (r = 0,4394) wskazywały na gorszą jakość życia. Predyktorami całkowitej punktacji QoL były depresja (p = 0,000), postrzeganie choroby (p = 0,001), samowystarczalność (p = 0,008) oraz subiektywna ocena stanu zdrowia (p = 0,005). Wnioski. W opiece nad chorymi z przewlekłą niewydolnością serca konieczne jest kompleksowe podejście z naciskiem na poprawę jakości życia

    COUVADE SYNDROME AMONG PARTNERS OF WOMEN WITH PHYSIOLOGICAL AND COMPLICATED PREGNANCY Aim. To assess the prevalence of couvade syndrome and analyze the factors determining the occurrence of this syndrome in the partners of pregnant women. Material and method. The research material was collected by a diagnostic survey, using an author’s survey questionnaire. Results. Couvade Syndrome occurs in 94.3% of partners of women with complicated pregnancies. Among the most common somatic symptoms there are: back pain (56.2%), headaches (53.3%), appetite changes (48.6%), indigestion (34.3%) and abdominal pain of unknown origin (31.4%), while mental ones include: sleep disturbances (37.1%), irritability (34.3%), tension (32.4%), regardless of the course of partner’s pregnancy. Men, who were married, were more likely to have Couvade symptoms than those in an informal relationship, regardless of the course of partner’s pregnancy (p=0.02). Respondents without Couvade Syndrome were more likely to have low levels of involvement in their partner’s current pregnancy than those with symptoms, regardless of the course of this pregnancy (p=0.03). Men with symptoms of the syndrome were more likely to be less involved in their responsibilities at work than those without symptoms, regardless of the course of partner’s pregnancy (p=0.0006). Conclusions. The majority (94.3%) of partners of women with complicated pregnancies have a Couvade Syndrome. The most common somatic symptoms include back pain, headaches, appetite changes, indigestion and abdominal pain of unknown origin, while the mental ones: sleep disturbances, irritability, tension, anxiety and nervousness. Married men are more likely to have Couvade Syndrome than those in an informal relationship. Low levels of involvement in the current pregnancy are shown by men who do not have the syndrome. Men, who have the Couvade Syndrome, are less involved in their work responsibilities. SYNDROM KUWADY WŚRÓD PARTNERÓW KOBIET W CIĄŻY FIZJOLOGICZNEJ I POWIKŁANEJ Cel pracy. Ocena częstości występowania syndromu kuwady oraz analiza czynników determinujących występowanie tego syndromu wśród partnerów kobiet w ciąży fizjologicznej i powikłanej. Materiał i metoda. Materiał zebrano metodą sondażu diagnostycznego, za pomocą autorskiego kwestionariusza ankiety. Wyniki. Syndrom kuwady występuje u 94,3% partnerów kobiet w ciąży, w przebiegu której wystąpiły powikłania. Do najczęstszych objawów somatycznych u mężczyzn, bez względu na przebieg ciąży ich partnerki i towarzyszące powikłania, należą: bóle pleców (56,2%), bóle głowy (53,3%), zmiany apetytu (48,6%), niestrawność (34,3%), bóle brzucha niewiadomego pochodzenia (31,4%), zaś do psychicznych: zaburzenia rytmu snu (37,1%), rozdrażnienie (34,3%), napięcie (32,4%), niepokój (26,7%), nerwowość (22,9%). Mężczyźni w związku małżeńskim istotnie częściej mieli objawy kuwady niż ci w związku nieformalnym, niezależnie od przebiegu ciąży partnerki (p=0,02). Badani bez objawów syndromu kuwady częściej prezentowali niski poziom zaangażowania w obecną ciążę partnerki niż ci z objawami, bez względu na to, czy obecna ciąża partnerki przebiegała w sposób fizjologiczny, czy powikłany (p=0,03). Mężczyźni z objawami syndromu częściej byli mniej zaangażowani w swoje obowiązki w pracy niż ci bez objawów, niezależnie od przebiegu ciąży partnerki (p=0,0006). Wnioski. Syndrom kuwady występuje u większości (94,3%) partnerów kobiet w ciąży powikłanej. Wśród objawów somatycznych u mężczyzn, bez względu na przebieg ciąży ich partnerki i towarzyszące powikłania, występują najczęściej: bóle pleców, bóle głowy, zmiany apetytu, niestrawność oraz bóle brzucha niewiadomego pochodzenia, zaś wśród psychicznych: zaburzenia rytmu snu, rozdrażnienie, napięcie, niepokój oraz nerwowość. Zespół kuwady częściej występuje wśród mężczyzn będących w związku małżeńskim niż w związku nieformalnym, niezależnie od tego, czy aktualna ciąża partnerki przebiegała w sposób fizjologiczny, czy powikłany. Mężczyźni, u których występuje syndrom kuwady, bez względu na przebieg obecnej ciąży partnerki, nie tylko wykazują niski poziom zaangażowania w aktualną ciążę, ale także mniej angażują się we własne obowiązki zawodowe

    Introduction. A stroke is considered a medical emergency. It is an occurrence of brain disorders lasting more than 24 hours. Survivors are likely to be permanently disabled in a variety of ways. It is for this reason that numerous measures have been taken to reduce the mortality and degree of disability of these patients. Rehabilitation is an integral and extremely important part of the treatment of people who suffered a stroke – both ischemic and hemorrhagic. It should be started as soon as possible, because how quickly and to what extent the patient recovers depends on it. Aim. The aim of the study was to analyze and evaluate the deficit of self-care and self-nursing of 100 post-stroke patients at the Department of Neurology with the Stroke Sub-department of the University Hospital in Zielona Góra.  Material and methods. A self-study questionnaire was used prior to the study. Results. Among the subjects of the study, ischemic stroke was the most common (84%). Almost half of the subjects had a physical disability that limited their freedom of movement to a minor degree (47%). A significant degree of limitation was reported by 32% of the subjects. Statistical analysis showed a significant increase in the frequency of moderate pain reported by individuals after physical rehabilitation that restored their mobility – p<0.000001. The strength of the observed effect was moderate (Cramer’s V = 0.43, 95% CI (0.27; 0.56))

    The issue of expired and unused medications is a significant matter worthy of analysis. The study focused on the analysis of pharmaceutical products returned to public pharmacies, with particular emphasis on expired drugs and those returned within their expiration date, along with an economic assessment of the returned products.The research methodology relied on the analysis of products returned by patients to public pharmacies in the Mazovian voivodeship. The analysis encompassed the content of containers containing expired medications in terms of their expiration date, pharmaceutical form, active ingredient, ATC classification, drug availability category, and monetary value. The analyzed products belonged to various categories, including: medicines, dietary supplements, medical devices, specialized medical foods, cosmetics, biocidal products, and hygiene items.The study revealed that 75.75% of the returned products were drugs. Among the most frequently returned active ingredients was paracetamol, and prescription medications were the predominant category among the returned products. The most commonly returned therapeutic products belonged to the groups related to the Gastrointestinal and Metabolism, Respiratory System, and Nervous System. The economic evaluation showed that the value of returned products was significantly lower for expired items compared to those within their expiration date. The total value of the returned products amounted to 42 218.07 PLN.In summary, the issue of medication waste presents a challenge that requires a comprehensive approach, such as public education and the proposal of efficient solutions

    Fosfomycin activity in vitro against Escherichia coli strains isolated from urine specimens

    Urinary tract infections are caused mostly by uropathogenic Escherichia coli strains. The aim of this paper was to assess the activity of fosfomycin in vitro against 74 E. coli strains isolated from urine samples of outpatients with acute cystitis. Minimum inhibitory concentration (MIC) was determined by the recommended method of serial drug dilution in Mueller-Hinton agar supplemented with glucose-6-phosphate (25 mg/L). The estimated MIC values were in the range of 1-32 mg/L. The prevalence of fosfomycin-sensitive strains was 45.9%, while that of fosfomycin-resistant strains – 54.1%. The time-kill assay was performed for a chosen clinical strain (MIC = 1 mg/L) in Mueller-Hinton broth also supplemented with glucose-6-phosphate in the presence of various concentrations of fosfomycin (0.5-64 mg/L). The bacterial population density was expressed as log CFU (colony forming units)/mL. The decrease of the bacterial population viability after 6h incubation was found with ∆log CFU/mL = 1.05-5.02, depending on fosfomycin concentration. However, bactericidal effect of this antibiotic (∆log CFU/mL >3) was observed only at the highest concentrations (32-64 mg/L). Moreover, after prolonged incubation (24 h), the re-growth of bacterial population revealed as the increase of its viability (∆log CFU/mL = 1.03-3.03) was observed at fosfomycin concentration in the range 0.5-32 mg/L, but not at 64 mg/L. This phenomenon may be due the presence of subpopulations of spontaneous mutants sensitive only to higher concentrations of this antibiotic comparing to MIC.     Humoral response after breakthrough SARS-CoV-2 infection in type 2 diabetes mellitus patients

    Type 2 diabetes mellitus (T2DM) remains an important public health problem in both developing and developed countries. In addition, the recent COVID-19 pandemic has revealed furthel risks for diabetes patients in terms of symptoms and disease progression. Higher mortality and morbidity are related to the complexity of the pathology of this chronic underlying disease, which negatively affects the immune response to the SARSCoV-2 virus. The humoral response plays an important role in the eradication of the virus; thus, it was analyzed in vaccinated diabetics who underwent COVID-19, as well as in the control group. The aim of this study was to assess the prevalence and level of IgG antibodies raised against the nucleocapsid protein (NCP), S1 subunit receptor binding domain (RBD) and subunit Spike 2 (S2) subunit of the virus’s S protein using the Microblot Array test. The results demonstrated significantly lower prevalence and titers of anti-SARS antibodies in diabetic patients compared to the control group. In addition, antibody titers were negatively related to the duration of this chronic disease, body mass index (BMI), comorbidities and HbA1c concentration. Further research is needed to develop the best strategy for specific prevention of SARS-CoV-2 infection in diabetic patients

    Safety asssessment of iopromide contrast media: a narrative review focusing on adverse events

    Many clinical contexts require radiological exams based on contrast media administration. Iodinated contrast media (ICM) represents one of the most studied contrast agents often used in radiological examinations. ICM vary widely in their physicochemical properties, clinical uses, as well as in the incidence of adverse reactions (ARs). Therefore, a basic understanding of ARs occurrence, risk factors, clinical features, and management of ICM is increasingly important in clinical practice. Iopromide is a nonionic ICM widely used in clinical practice due to its favourable safety profile and numerous applications. This narrative review provides a comprehensive report of the available data concerning iopromide ARs. It also analyses iopromide ARs occurrence and frequency with diverse potential risk factors such as age, sex and pre-existing conditions

    Adiponectin as a novel predictive biomarker of multiple sclerosis course

    Multiple sclerosis (MS) is a serious neurological disease, the actual worldwide prevalence of which is estimated to be 2,8 million people (35,9 per 100,000). During the course of MS, various neurological symptoms and its complications result in raising patient disability, which range from skeletal muscles impairment, to losses in cognitive functions. Achieving control over course of MS progression appears to be crucial in its treatment. This enforces the need for recognizing novel predictive factors so as to allow prognosis of future remissions and/or progressions. Adiponectin, hormone secreted by adipose tissue, currently is considered as a possible candidate for such a biomarker. The aim of this review is to summarise present knowledge and to assess possible clinical usage.According to collected data, adiponectin measurements in serum and cerebrospinal fluid appear to provide plausible and useful biomarkers in predicting the course of MS. Further studies are, however, needed, especially using non-invasive, but promising sources such as saliva

    Effect of the inferior alveolar nerve block injection versus maxillary infiltration with two local anesthetic agents on cardiovascular parameters: A randomized comparative double-blind clinical trial

    This study aimed to assess the effect of inferior alveolar nerve (IAN) block of the mandible and infiltration anesthesia of the maxilla using lidocaine plus epinephrine or prilocaine with felypressin anesthetic agents on systolic and diastolic blood pressures, heart rate (HR) and peripheral oxygen saturation rate of patients. This randomized double-blind clinical trial evaluated 112 patients in four groups (n=28) based on the jaw (maxilla or mandible) and type of anesthetic agent (lidocaine with epinephrine or prilocaine with felypressin). Groups 1 and 2 received IAN block of the mandible or infiltration anesthesia of the maxilla, respectively, with 2% lidocaine plus 1:80.000 epinephrine. Groups 3 and 4 received IAN block of the mandible or infiltration anesthesia of the maxilla, respectively, using prilocaine with felypressin. Systolic and diastolic blood pressures and HR were measured by an automatic upper arm blood pressure monitor and the percentage of peripheral oxygen saturation rate was measured by a finger pulse oximeter 10 minutes before the injection, immediately after injection, 10 minutes after injection and 20 minutes after injection.The mean diastolic blood pressure showed a greater reduction following the administration of lidocaine with epinephrine, as compared with prilocaine with felypressin (p<0.001). The mean HR showed a significantly greater increase following the administration of lidocaine with epinephrine, as compared with prilocaine with felypressin (p<0.001). The effect of type of anesthetic agent on diastolic blood pressure depended on the type of injection (p=0.033).Administration of 2% lidocaine with 1:80.000 epinephrine, in comparison to prilocaine with felypressin, can cause changes in cardiovascular parameters such as heart rate and diastolic blood pressure in patients undergoing restoration of a maxillary or mandibular molar tooth. Type of injection has no significant effect on cardiovascular parameters
