Management of Coalescence of the Little Lips in Girls at the Pediatric Surgery Department of the Donka National Hospital (HND) of the Conakry Chu
Introduction: Coalescence of the labia minora, also called coalescence of the nymphs, is an acquired pathology of prepubescent little girls. It is relatively common but poorly understood by practitioners. The objective of this study is to report the management of this pathology in our African social conditions. Patients and methods : We carried out a retrospective descriptive study of the files of child patients aged less than 15 years treated for coalescence of the labia minora in the pediatric surgery department over a period of 5 years. The epidemiology, diagnosis, therapeutic and evolutionary aspects were studied. Results: We collected 28 files of coalescence of the labia minora and the annual incidence was 5.6 cases per year. The average age of our patients was 13 months with a range of 4 months and 2 years 9 months. Twenty-four (85.72%) were less than 2 years old. The circumstances of discovery were made by the mother at home by chance in 24%. The main reason for consultation was spontaneous vaginal closure. In the history we noted a case of perineal erythema and a urinary infection. In this study there were 20 cases of total form coalescence and 8 cases of partial form. All patients were asymptomatic. Surgical detachment was done with the Halsted forceps (22 cases) and by the thermometer (6 cases). Local antiseptic care and 72 hours of oral antibiotic therapy were prescribed for all patients. Five patients were given an adjunct to a prescription for estrogen-based colpotrophin ointment. The postoperative course was simple. After a 1-year follow-up, no recurrence was found in our study. Conclusion: Coalescence of the labia minora remains an acquired and benign pathology in little girls. Surgical detachment under local anesthesia in a single attempt in our social context remains the only effective treatment
Treatment of Recurrent Basilar Artery Aneurysms at Fenestration After Coils using Flow Diverter Combined with Coils: Case Report
An aneurysm at the fenestration position of the basilar artery is an extremely rare condition that presents challenges in treatment. Aneurysm recurrence is a complication that can occur after interventions for aneurysms in this location. We report the case of a 43-year-old male patient with a ruptured basilar artery aneurysm treated with coils. After 30 months of follow-up, the aneurysm recurred and was treated with a flow diverter stent combined with coils
Impact of the Ebola Virus Outbreak on Tuberculosis Treatment Adherence and Outcomes in a Military Hospital in Freetown, Sierra Leone
Introduction: The impact and disruption of infectious disease outbreaks stretch far beyond their direct death toll, as they often overburden health systems, reduce treatment seeking behaviors, and interrupt treatment� regimens. This study examines the impact of the 2014�2016 Ebola virus outbreak on tuberculosis (TB) treatment outcomes at the 34 Military Hospital in Freetown, Sierra Leone.
Methods: We used retrospective data from 1,085 TB patient outcome data registers to build a multinomial logistic regression model to evaluate the change in TB treatment outcomes before and after the Public Health Emergency of International Concern (PHEIC) declaration in August 2014.
Results: These results showed that HIV status, patient age, whether patients had active versus latent TB, and the time since the start of the outbreak were significantly associated with TB treatment outcomes.
Discussion: The model showed an increase in probability of unknown and unsuccessful (died or treatment failed) treatment outcomes with each month after the PHEIC declaration, across age groups, TB status, and HIV status
Body Weight Changes, Histological Features and Anti-Hyperglycemic Effects of Cocoyam-Soya Bean-Bambara Groundnut Flour Blend-Fed Streptozotocin (STZ)-Induced Diabetic Rats.
Background: Diabetes mellitus (DM) has a global prevalence of 536.6 million people, which an estimated rise of 12.2% by 2045. Diabetes management is expensive, hence alternate management options are being explored. The World Health Organization recognizes some plants and plant-based meals as excellent diabetes treatment agents. These include cocoyam (CYN), soya bean (SB), and Bambara groundnut (BGN). The purpose of this study was to evaluate these extracts\u27 hypoglycemic, weight changes and histological effects.
Methodology: CYN, SB, and BGN were sourced and processed to generate high-quality flour, which were pelletized and oven-dried at 60�C, before storage. The eighty-two male albino rats weighing between 134 and 247 g, were administered with low-dose fructose to induce insulin resistance. Type 2 diabetes mellitus (T2DM) was induced with �intraperitoneal injection of streptozotocin, following which a 28 days intervention formulations were administered. Throughout the investigation, the weights of the rats were recorded while on the 28th day, their organs and blood samples were collected from killed rats for histological examination and blood glucose analysis respectively.
Results & Discussion: The investigation revealed an average random blood glucose (RBG) levels that varied significantly over time for each group, displaying a consistent pattern of changes across the entire group from week 1 to week 4 (F=79.106, p<0.01) (F=76.755, p<0.001), with significant differences between groups (F= 13.963, p<0.001). Group A diabetic rats exceeded the normal control group in terms of anti-diabetic efficacy, rat body weight gain of 8.85% over four weeks period, an increase in liver weight in the intervention formulation groups and an unchanged pancreatic weight. Histological examinations revealed varied levels of tissue regeneration and intra-hepatic inflammation in pancreatic and hepatic cells between groups treated with different intervention formulations.
Conclusion: Group A rats, treated with formulation 1 (16.6% CY+16.6%SB+16.6%BGN) had histological characteristics identical to the normal control group, exceeding the standard control groups. This showed that these formulations successfully stopped STZ-induced organ damage and resulted in successful organ healing. Furthermore, the Cocoyam-Soya bean-Bambara groundnut flour blend demonstrated hypoglycemic, tissue regeneration, and hepato-pancreatic protective effects in STZ-induced diabetic rats, showing its potential as a therapeutic supplement in the treatment of diabetes mellitus

The Origin of Life on Earth: An Economic Interpretation
Economists believe that most theories in economics, if not all, can be formulated in terms of minimization or maximization of a suitable economic quantity. Recent studies in geophysics and biophysics have found convincing evidence that the principle driving the origin of life is energy-dissipation-driven adaptation of matter. In this view, life is a result of maximizing entropy production through a maximum dissipation of sunlight energy efficiently transforming it into heat. If this is true, it could be said that in a fundamental sense it is the economic forces that are responsible for the creation of life on earth. To be more accurate, it is the physical forces based on the second law of thermo-dynamics driven by the fundamental economic forces of maximizing entropy along the evolutionary path of the universe that are responsible for the origin of life on Earth.
This is not surprising considering the fact that the behaviors of most animal and plant species including their genome itself can be explained in terms of constrained dynamic optimization producing efficient outcomes through decentralized decision making. It is to be recognized that the similarities between the efficient biological system and the efficient economic system, both with efficient decentralized signaling and responses, are more than just coincidental.
