    Development of a low-cost device for photobiomodulation using 3D printing

    INTRODUCTION: Photobiomodulation therapy (PBM) is a non-pharmacological and non-invasive therapeutic strategy that has shown promising results in several health conditions. Despite important advances in knowledge of its modulatory effects at the molecular, cellular, and tissue levels, the cost of PBM equipment still greatly limits its use in the clinical setting, especially in developing countries. In this context, the use of new technologies such as 3D printing has presented several possibilities for application in the health area. OBJECTIVES: To develop a 3D printing PBM device and test its technical and operational viability. METHODS AND MATERIALS: Development of Health Technology on a Photobiomodulation Device with 3D Printing Tests of the safety, emission, and support of the light beams were carried out. RESULTS: The PBM device demonstrated operational safety and good quality in the maintenance of light beams. CONCLUSION: A three-dimensional (3D) printed LED PBM device is technically, operationally, and financially viable. However, more experimental tests and clinical validation are required before this prototype can be used in human health applications

    INTRODUCTION: Electrical and magnetic stimulation of the nervous system has been used to treat cerebral palsy, autism spectrum disorder, attention deficit hyperactivity disorder, among other childhood conditions. OBJECTIVES: This infographic was designed to elucidate the decision-making process entailed in selecting neuromodulation protocols for children. METHODS: The development of this infographic was guided by the principles of several healthcare philosophies: (1) the Biopsychosocial Model of the International Classification of Functioning, Disability, and Health;1 (2) Patient- and Family-Centered Practice;2 (3) Evidence-Based Practice;3,4 and (4) the SMART model (Specific, Measurable, Attainable, Realistic/Relevant and Timely outcome) for establishing therapeutic goals.5 RESULTS: This visual tool delineated value points that should be considered before initiating any neuromodulation treatment, conceptualized within a structured framework across various levels, including screening, feasibility and safety assessment, selection of a “SMART” outcome, evaluation of complaints, protocol definition, risks clarification, assessment of benefits and cost-effectiveness, and collaborative and empowered participation of parents/caregivers. CONCLUSION: Neuromodulation protocols’ definitions involve a complex process, encompassing choices related to techniques, targets, parameters, distribution and number of sessions, and the potential incorporation of other therapeutic modalities. This process must be tailored to specific objectives to ensure that treatment can yield optimal results within the users’ functional conditions. Our infographic can serve as a tool for comprehensively visualizing the entire process and making well-informed choices in a clear, concise, and organized manner. Nevertheless, it is essential to acknowledge that this model is not flawless and should be refined further to advance toward more humanized and responsible care

    Melhorando a caminhada por meio de feedback visual em tempo real após acidente vascular cerebral em treinamento em esteira (RE-VISIT): um protocolo para ensaio clínico randomizado

    BACKGROUND: After a stroke, most patients often suffer reduced walking ability and balance. Restoring walking ability and improving balance are major goals of stroke rehabilitation. Treadmills are often used in clinical setups to achieve these goals. Adding dimensions to the visual feedback in addition to the mirror for real-time frontal view is proven to enhance the gait. It is, therefore, important to design additional real-time visual feedback in treadmill training, in particular for the sagittal view involved side. OBJECTIVE: The objective of this study is to test if the real-time sagittal visual feedback during treadmill training is superior to the conventional mirror feedback treadmill training program of equivalent intensity in improving walking speed and balance after stroke. METHODS/DESIGN: The RE-VISIT trial (Real-time Visual feedback after Stroke in Treadmill training) is registered in the Clinical Trial Registry of India (CTRI/2023/10/058299). In this two-arm randomized control trial, which will be a single-blinded study, 42 eligible stroke survivors undergoing rehabilitation will be randomly allocated (1:1 ratio) to either real-time visual sagittal feedback along with front mirror (experimental) group or only front mirror treadmill training (control) group, all the participants will receive 15 sessions of treadmill training for up to 15 min at a safe self-selected speed over 5-6 weeks. The RE-VISIT (experimental) group will receive real-time, visual sagittal view feedback of the involved lower limb trajectory along with the routine front mirror view during treadmill training and will be asked to modify their gait pattern. The control group will receive treadmill walking training only with the routine front mirror view feedback. Clinical and gait assessments will be conducted at the baseline, immediately following the final session of training, and at the 9th week during follow-up. The outcome measures of interest are walking speed (primary) and balance (secondary), which will be measured prior to baseline, post 15 sessions of training, and at the 9th week following training. DISCUSSION: This REVISIT trial will provide insight and contribute to the existing innovation and modifications of incorporating real-time visual feedback during treadmill training in post-stroke gait rehabilitation. The findings will help the better designing of a gait rehabilitation program with a treadmill for post-stroke subjects to improve walking speed, and balance for those who have greater difficulties in community ambulation. We anticipate that those in the REVISIT training will demonstrate improved walking ability.CONTEXTO: Após o acidente vascular cerebral, a maioria dos pacientes frequentemente sofre redução da capacidade de caminhar e do equilíbrio. Restaurar a capacidade de caminhar e melhorar o equilíbrio são os principais objetivos da reabilitação do AVC. As esteiras são frequentemente usadas em ambientes clínicos para atingir esses objetivos. Está comprovado que adicionar dimensões ao feedback visual, além do espelho para visão frontal em tempo real, melhora a marcha. É, portanto, importante projetar feedbacks visuais adicionais em tempo real no treinamento em esteira, em particular para o lado envolvido na visão sagital. OBJETIVO: O objetivo deste estudo é testar se o feedback visual sagital em tempo real durante o treinamento em esteira é superior ao programa de treinamento em esteira com feedback de espelho convencional de intensidade equivalente na melhoria da velocidade de caminhada e equilíbrio após acidente vascular cerebral. MÉTODOS/DESENHO: O ensaio RE-VISIT (feedback visual em tempo real após acidente vascular cerebral no treinamento em esteira) está registrado no Registro de Ensaios Clínicos da Índia (CTRI/2023/10/058299). Neste ensaio de controle randomizado de dois braços, que será um estudo cego, 42 sobreviventes de AVC elegíveis em reabilitação serão alocados aleatoriamente (proporção de 1:1) para feedback sagital visual em tempo real junto com grupo de espelho frontal (experimental) ou apenas Grupo de treinamento em esteira com espelho frontal (controle), todos os participantes receberão 15 sessões de treinamento em esteira por até 15 minutos em uma velocidade segura e autosselecionada durante 5-6 semanas. O grupo RE-VISIT (experimental) receberá feedback visual em tempo real da visão sagital da trajetória dos membros inferiores envolvidos, juntamente com a visão rotineira do espelho frontal durante o treinamento em esteira e será solicitado a modificar seu padrão de marcha. O grupo de controle receberá treinamento de caminhada em esteira apenas com o feedback rotineiro da visão do espelho frontal. Avaliações clínicas e de marcha serão realizadas no início do estudo, imediatamente após a sessão final de treinamento e na 9ª semana durante o acompanhamento. As medidas de resultados de interesse são a velocidade de caminhada (primária) e o equilíbrio (secundário), que serão medidos antes da linha de base, após a 15ª sessão de treinamento e na 9ª semana após o treinamento. DISCUSSÃO: este ensaio REVISIT fornecerá insights e contribuirá para a inovação e modificações existentes na incorporação de feedbacks visuais em tempo real durante o treinamento em esteira na reabilitação da marcha pós-AVC. As descobertas ajudarão no melhor desenho de um programa de reabilitação da marcha com esteira para indivíduos pós-AVC para melhorar a velocidade de caminhada e o equilíbrio para aqueles que têm maiores dificuldades na deambulação comunitária. Prevemos que aqueles no treinamento REVISIT demonstrarão melhor capacidade de caminhada

    Influência do uso do diário de caminhada na mobilidade de pacientes em reabilitação cardíaca fase 1: ensaio clínico randomizado

    INTRODUCTION: Inpatient rehabilitation is extremely important for patients recovering from cardiac surgery. Although a walking diary is routinely used in clinical practice, it has yet to be adequately tested and reported in the literature. OBJECTIVES: To establish whether the use of a walking diary affects the number of steps taken following cardiac surgery and whether this is related to the patient’s level of cardiac anxiety. METHODS: An open, controlled, randomized clinical trial was conducted with adult patients submitted to elective valve and/or coronary surgery, who had no motor impairment. All the participants used a pedometer to register the number of steps taken over five consecutive days in the hospital. Twenty-nine individuals were randomized to create an intervention group that used the walking diary as a treatment strategy, while twenty-three were allocated to a control group. The Mann-Whitney test was used to compare the number of steps between the two groups, while Spearman’s correlation coefficient was performed to evaluate the relationship between the number of steps and the level of cardiac anxiety. Statistical significance was defined as p<0.05. RESULTS: The groups were similar regarding their demographic, clinical and surgical characteristics. There was no difference between the groups regarding the total number of steps taken: control group=1,496 (477.5 - 2992.5) vs. intervention group=1,468.5 (494.2 - 2,678) (p=0.902). CONCLUSION: The use of the walking diary had no effect on the number of steps taken and was unassociated with the level of cardiac anxiety in inpatients following cardiac surgery.INTRODUÇÃO: A reabilitação hospitalar é extremamente importante para pacientes em recuperação de cirurgia cardíaca. Embora o diário de caminhada seja rotineiramente utilizado na prática clínica, ele ainda não foi adequadamente testado e relatado na literatura. OBJETIVOS: Estabelecer se o uso do diário de caminhada afeta o número de passos dados após cirurgia cardíaca e se isso está relacionado ao nível de ansiedade cardíaca do paciente. MÉTODOS: Foi realizado um ensaio clínico aberto, controlado e randomizado com pacientes adultos submetidos à cirurgia eletiva de valva e/ou coronária, sem comprometimento motor. Todos os participantes usaram um pedômetro para registrar o número de passos dados ao longo de cinco dias consecutivos de internação. Vinte e nove indivíduos foram randomizados para um grupo de intervenção para usar o diário de caminhada como estratégia de tratamento, enquanto vinte e três foram alocados para um grupo controle. O teste de Mann-Whitney foi utilizado para comparar o número de passos entre os dois grupos, enquanto o coeficiente de correlação de Spearman foi realizado para avaliar a relação entre o número de passos e o nível de ansiedade cardíaca. A significância estatística foi definida como p < 0,05. RESULTADOS: Os grupos foram semelhantes quanto às características demográficas, clínicas e cirúrgicas. Não houve diferença entre os grupos quanto ao número total de passos dados: grupo controle=1.496 (477,5 - 2.992,5) vs. grupo intervenção=1.468,5 (494,2 - 2.678) (p=0,902). CONCLUSÃO: O uso do diário de caminhada não teve efeito no número de passos dados e não foi associado ao nível de ansiedade cardíaca em pacientes internados após cirurgia cardíaca

    Versão hindi da escala funcional de extremidade inferior: um estudo de tradução, validação e adaptação transcultural

    BACKGROUND: Lower Extremity Function Scale (LEFS) serves as a patient-reported outcome measure, used to evaluate the functional status of lower extremity in different musculoskeletal conditions. It was originally developed in English language (E-LEFS) and numerous translations in different languages are available to measure its usability among different cultures. OBJECTIVES: To translate the English version of the Lower Extremity Function Scale (E-LEFS) into Hindi language and to evaluate its content validity, cross-cultural adaptation along with the concurrent validity and reliability. METHOD: Process of forward and backward translation was carried out by two bilingual translators and a physiotherapist after obtaining developer’s approval, for which the Beaton guidelines were used. For the content validation, Delphi method of was employed in which 12 field experts of more than 5 years of experience participated. Cultural adaptation procedures were then carried out with 12 patients, who reported in out-patient department of the hospital through convenience sampling who were asked about whether the H-LEFS was well understood by the patients. Their responses were recorded as positive and negative responses. This process was followed by concurrent validation which was conducted by comparing the scores with the SF-36 scale, involving 30 patients with activity limitation in their daily life. Lastly, 50 patients were recruited for intra-rater reliability testing. RESULT: The scale was successfully translated and verified by the bi-lingual language experts. Content validity came out to be significant with each item scoring more than 0.92. Nearly all items, excluding item no. 8, 16, 17, 18, and 19 received universal agreement with a content validity ratio of 1. The average scale-level validity stood at 0.98, indicating excellent level of validity and the scale was found to be well adapted by the native people as out of 12 patients, 10 patients provided 100% positive response. Concurrent validation using Pearson’s correlation coefficient came out excellent with the values of 0.993 for E-LEFS and 0.890 for SF-36. Cronbach\u27s alpha and ICC values were calculated as 1.00 for Intra-rater reliability. CONCLUSION: LEFS was well translated into Hindi language with an excellent content and concurrent validity and is found to be highly reliable. Hence, it is recommended for the usage of native Hindi speaking population.INTRODUÇÃO: A Escala de Função dos Membros Inferiores (LEFS, na sigla em inglês) serve como uma medida de resultados relatados pelo paciente, usada para avaliar o estado funcional dos membros inferiores em diferentes condições músculo-esqueléticas. Foi originalmente desenvolvido em língua inglesa (E-LEFS) e inúmeras traduções em diferentes idiomas estão disponíveis para medir a sua usabilidade entre diferentes culturas. OBJETIVOS: Traduzir a versão em inglês da Lower Extremity Function Scale (E-LEFS) para a língua hindi e avaliar sua validade de conteúdo, adaptação transcultural, juntamente com validade concorrente e confiabilidade. MÉTODO: O processo de tradução direta e reversa foi realizado por dois tradutores bilíngues e um fisioterapeuta após obtenção da aprovação do desenvolvedor, para a qual foram utilizadas as diretrizes de Beaton. Para a validação de conteúdo foi empregado o método Delphi, do qual participaram 12 especialistas da área com mais de 5 anos de experiência. Foram então realizados procedimentos de adaptação cultural com 12 pacientes, que compareceram ao ambulatório do hospital por meio de amostragem de conveniência e foram questionados se o H-LEFS era bem compreendido pelos pacientes. Suas respostas foram registradas como respostas positivas e negativas. Esse processo foi seguido de validação concorrente realizada por meio da comparação dos escores com a escala SF-36, envolvendo 30 pacientes com limitação de atividades de vida diária. Por fim, 50 pacientes foram recrutados para testes de confiabilidade intra-avaliador. RESULTADO: A escala foi traduzida e verificada com sucesso pelos especialistas em idiomas bilíngues. A validade de conteúdo revelou-se significativa, com cada item pontuando mais de 0,92. Quase todos os itens, excluindo os ítens no. 8, 16, 17, 18 e 19 receberam concordância universal com um índice de validade de conteúdo de 1. A validade média do nível da escala foi de 0,98, indicando excelente nível de validade e a escala foi considerada bem adaptada pelos nativos pois de 12 pacientes, 10 pacientes forneceram 100% de resposta positiva. A validação concorrente utilizando o coeficiente de correlação de Pearson obteve excelente resultado com valores de 0,993 para E-LEFS e 0,890 para SF-36. Os valores de alfa de Cronbach e ICC foram calculados como 1,00 para confiabilidade intra-avaliador. CONCLUSÃO: O LEFS foi bem traduzido para a língua hindi, com excelente conteúdo e validade concorrente e é considerado altamente confiável. Portanto, é recomendado para uso pela população nativa de língua hindi

    Impacto da reabilitação abrangente na capacidade funcional, fadiga e qualidade de vida entre sobreviventes de COVID-19 de longo prazo em ambientes com recursos limitados - um ensaio clínico randomizado

    INTRODUCTION: Months or even year after COVID 19 infection, patients suffered from persistent symptoms. The present study aimed to evaluate the impact of comprehensive rehabilitation on functional capacity, fatigue, and quality of life in long-term COVID-19 survivors in resource limited settings. MATERIALS AND METHODS: Comprehensive rehabilitation comprising of aerobic, strength, balance, flexibility, breathing exercise and education was administered in low-resource settings to patients aged 25-60 years diagnosed with long COVID in this randomised controlled trial. Functional capacity (assessed by six-minute walk test), fatigue (assessed by fatigue severity scale), and quality of life (assessed by EQ-5D-5L) were documented at the baseline and after six weeks in intervention (n=19) and control (n=19) groups. Categorical variables were reported as percentages and continuous variables as mean and standard deviation (SD). Independent t-test was used for the between group comparison and paired t test for within group comparison to establish the statistical significance. A p-value of < 0.05 was considered as statistically significant. Effect size and post hoc power analysis were performed. Effect sizes were interpreted according to the Cohen\u27s classification. RESULTS:  Significant improvements were observed in fatigue (p = 0.006, effect size = -1.23), EQ-5D-5L index scores (p = 0.007, effect size = -0.73), EQ-5D visual analog scale scores (p = 0.002, effect size = 1.09), and six-minute walk distance (p < 0.0001, effect size = 1.37) following the rehabilitation program. CONCLUSION: Comprehensive rehabilitation in low-resource settings effectively enhances quality of life, reduces fatigue, and improves functional capacity among long-term COVID-19 survivors.INTRODUÇÃO: Meses ou mesmo anos após a infecção por COVID 19, os pacientes sofriam de sintomas persistentes. O presente estudo teve como objetivo avaliar o impacto da reabilitação abrangente na capacidade funcional, fadiga e qualidade de vida em sobreviventes de longo prazo da COVID-19 em ambientes com recursos limitados. MATERIAIS E MÉTODOS: Reabilitação abrangente composta por exercícios aeróbicos, de força, de equilíbrio, de flexibilidade, de exercícios respiratórios e de educação foi administrada em ambientes de poucos recursos a pacientes com idade entre 25 e 60 anos com diagnóstico de COVID longa neste ensaio clínico randomizado. Capacidade funcional (avaliada pelo teste de caminhada de seis minutos), fadiga (avaliada pela escala de gravidade da fadiga) e qualidade de vida (avaliada pelo EQ-5D-5L) foram documentadas no início do estudo e após seis semanas de intervenção (n=19) e grupos controle (n=19). As variáveis ​​categóricas foram relatadas como porcentagens e as variáveis ​​contínuas como média e desvio padrão (DP). O teste t independente foi utilizado para comparação entre grupos e o teste t pareado para comparação dentro do grupo para estabelecer a significância estatística. Um valor de p < 0,05 foi considerado estatisticamente significativo. Foram realizadas análises de tamanho de efeito e poder post hoc. Os tamanhos de efeito foram interpretados de acordo com a classificação de Cohen. RESULTADOS: Melhorias significativas foram observadas na fadiga (p = 0,006, tamanho do efeito = -1,23), pontuações do índice EQ-5D-5L (p = 0,007, tamanho do efeito = -0,73), pontuações da escala visual analógica EQ-5D (p = 0,002, tamanho do efeito = 1,09) e distância percorrida no teste de caminhada de seis minutos (p < 0,0001, tamanho do efeito = 1,37) após o programa de reabilitação. CONCLUSÃO: A reabilitação abrangente em ambientes com poucos recursos melhora efetivamente a qualidade de vida, reduz a fadiga e melhora a capacidade funcional entre os sobreviventes de longa prazo da COVID-19

    Percepção de fisioterapeutas sobre alterações do sistema autônomo e do estado comportamental de recém-nascidos submetidos a procedimentos de estimulação sensório-motora: estudo transversal

    INTRODUCTION: Sensory motor stimulation (SMS) is an early intervention used in newborns (NB) to organize their systems. The physiotherapist who uses SMS must pay careful attention to the signs presented by NB after the procedures. OBJECTIVE: To analyze the perception of physiotherapists about changes in the autonomic system and behavioral state of NB after SMS procedures. METHODS: Cross-sectional study performed with physiotherapists working in neonatal units. Data were collected by electronic questionnaire, processed in the SPSS program (version 21.0) and presented in absolute (n) and relative (%) frequency. RESULTS: 72 physiotherapists participated in this study, 48.6% had a specialist degree, 63.9% worked in Neonatal Intensive Care Unit, 38.8% lived in the southern region and 38.8% in the southeastern region of Brazil. The perception of most physiotherapists after tactile procedures was stability or decrease in heart rate (HR) and respiratory rate (RR), and stability of oxygen saturation (SpO2), and after vestibular procedures it was stability of HR, RR and SpO2. There was a perception of active sleep, drowsiness, and active alertness after all tactile and vestibular procedures. Active alertness was noticed after all procedures and intense crying was not noticed.  CONCLUSION: Regarding the autonomic system, most physiotherapists noticed stable or decreased HR and RR, and stable or increased SpO2 after SMS procedures. Regarding the behavioral state, active alert was noticed more frequently among physiotherapists.INTRODUÇÃO: A estimulação sensório-motora (ESM) é uma intervenção precoce utilizada em recém-nascidos (RN) para a organização de seus sistemas. O fisioterapeuta que faz uso da ESM deve ter um olhar cuidadoso para os sinais apresentados pelos RN após os procedimentos. OBJETIVO:  Analisar a percepção de fisioterapeutas sobre alterações do sistema autônomo e do estado comportamental de RN após procedimentos de ESM. MÉTODOS: Estudo transversal realizado com fisioterapeutas atuantes em unidades neonatais. Os dados foram coletados por questionário eletrônico, processados no programa SPSS (versão 21.0) e apresentados em frequência absoluta (n) e relativa (%). RESULTADOS: Participaram deste estudo 72 fisioterapeutas, 48,6% possuíam título de especialista, 63,9% atuavam em Unidade de Terapia Intensiva Neonatal, 38,8% residiam na região sul e 38,8% na região sudeste do Brasil. A percepção da maioria dos fisioterapeutas após procedimentos de ordem tátil foi estabilidade ou diminuição da frequência cardíaca (FC) e respiratória (FR), e estabilidade da saturação de oxigênio (SpO2), e após procedimentos de ordem vestibular foi estabilidade da FC, FR e SpO2. Houve percepção de sono ativo, sonolência e alerta ativo após todos os procedimentos de ordem tátil e vestibular. O alerta ativo foi percebido após todos os procedimentos e o choro intenso não foi percebido. CONCLUSÃO: Do sistema autônomo, a maioria dos fisioterapeutas perceberam a FC e FR estáveis ou diminuídas e SpO2 estável ou aumentada após os procedimentos de ESM.  Do estado comportamental, o alerta ativo foi percebido com maior frequência entre os fisioterapeutas

    Desafios enfrentados pelo enfermeiro em relação à sua autonomia profissional: uma revisão da literatura

    OBJECTIVE: To highlight the main challenges faced by nurses in relation to autonomy and leadership in Nursing. METHOD: Integrative literature review based on studies published between 2018 and 2023. The PICO strategy was used, with the following question: “What are the main challenges faced by nurses in relation to decision-making and leadership capacity?”. Data collection took place in the electronic databases BVS, SciELO and PubMed, and the articles that answered the question were in English and Portuguese. The excluded studies were those that did not fit the theme, theses, dissertations, monographs, and studies repeated in the databases. The selected articles totaled 12. RESULTS: After analyzing the studies, it became clear that the main challenges related to leadership and autonomy are linked to subordination to medical work, insufficient technical-scientific knowledge, deficits in understanding the scope of action, administrative errors, work overload and ineffective communication, thus making it difficult for professionals to make decisions. CONCLUSION: Therefore, the scientific evidence from the present study shows that despite the challenges encountered, it is possible for nurses to have autonomy, as long as they are able to overcome the difficulties that will arise.OBJETIVO: Evidenciar os principais desafios enfrentados pelo enfermeiro em relação a autonomia e liderança na Enfermagem. MÉTODO: Revisão integrativa da literatura tendo por base estudos publicados entre os anos de 2018 e 2023. Foi utilizada a estratégia PICO, com a seguinte indagação: “Quais os principais desafios enfrentados pelo Enfermeiro em relação à capacidade decisória e de liderança?”. A coleta de dados se deu nas bases de dados eletrônicas BVS, SciELO e PubMed, e os artigos que responderam questão estavam nos idiomas inglês e português. Os estudos excluídos foram aqueles que não se adequaram ao tema, teses, dissertações, monografias e estudos repetidos nas bases de dados. Os artigos selecionados totalizaram 12. RESULTADOS: Após análise dos estudos, evidenciou-se que os principais desafios relacionados à liderança e autonomia estão ligados à subordinação ao trabalho médico, insuficiência de conhecimento técnico-científico, déficit na compreensão do espaço de atuação, erros administrativos, sobrecarga de trabalho e comunicação ineficaz, dificultando assim a tomada de decisão do profissional. CONCLUSÃO: Desse modo, as evidências científicas do presente estudo mostram que apesar dos desafios encontrados, é possível que o enfermeiro possua autonomia, desde que consiga superar as dificuldades que surgirão

    ABSTRACT | OBJECTIVE: To verify the prevalence of infection, sepsis and septic shock and factors associated with these conditions in patients admitted to an Adult ICU of a hospital in the interior of Brazil. METHODS AND MATERIALS: Cross-sectional, retrospective and documentary study, in an Adult ICU in the interior of Rio Grande do Sul, Brazil. Sample of 259 medical records from 2016 to 2018. Data collection using a structured instrument and descriptive and multivariable analyses. RESULTS: It was shown that 19.3% of patients had infection, 17% had sepsis and 10.8% had septic shock. Most women (59.1%), between 51 and 64 years old (27.3%), with hypertension (36.4%) and diabetes (26.1%). 96.6% received antibiotic therapy, but only 50% collected cultures. CONCLUSIONS: The prevalence of the sepsis continuum in the ICU was 33.9%; and the associated factors are the use of enteral tubes, central venous catheters, mechanical ventilation and cardiology and pulmonology specialties.RESUMO | OBJETIVO: Verificar a prevalência de infecção, sepse e choque séptico e fatores associados a estes agravos em pacientes internados em uma unidade de terapia intensiva (UTI) adulto de um hospital do interior do Brasil. MÉTODOS E MATERIAIS: Estudo de corte transversal, retrospectivo e documental, em UTI Adulto do interior do Rio Grande do Sul, Brasil. Amostra de 259 prontuários no período de 2016 a 2018. Coleta de dados através de instrumento estruturado e análise descritiva e multivariável. RESULTADOS: Evidenciou-se que 19,3% dos pacientes apresentaram infecção, 17% sepse e 10,8% choque séptico. Maioria mulheres (59,1%), entre 51 a 64 anos (27,3%), com hipertensão (36,4%) e diabetes (26,1%). 96,6% receberam antibioticoterapia, porém apenas 50% coletaram culturas. CONCLUSÕES: A prevalência do continuum da sepse na UTI foi 33,9%; e os fatores associados são a utilização de sonda enteral, cateter venoso central, ventilação mecânica e especialidades de cardiologia e pneumologia

    Percepções de enfermeiros de Centros de Atenção Psicossocial (CAPS) sobre as práticas desempenhadas em visitas domiciliares

    OBJECTIVE: Investigate perceptions about the practices performed by nurses during home visits carried out with patients treated at Psychosocial Care Centers (CAPS). METHOD: This exploratory research with a qualitative approach was carried out with six nurses at CAPS in a countryside city of Southern Brazil. Sociodemographic questionnaires and a semi-structured interview were applied, and the data were analyzed through Content Analysis. RESULTS: Results were organized into three categories: 1) Insufficient training of nursing professionals to work in CAPS; 2) Challenges faced by the nursing team during home visits; and 3) Impacts of the support network on the recovery process of patients. FINAL CONSIDERATIONS: The nurses participating in the study reported not having received adequate training during their undergraduate studies, making it necessary to seek knowledge by their own means. In addition, several challenges were noted during visits, such as patients\u27 resistance to receiving the multidisciplinary team, incomplete or incorrect addresses, patients\u27 absence during home visits, and the difficulty in reaching patients\u27 families. Finally, the importance of family assistance as a support network for users with mental disorders was noted.OBJETIVO: Investigar as percepções sobre as práticas desempenhadas pelo enfermeiro nas visitas domiciliares realizadas com pacientes atendidos em Centros de Atenção Psicossocial (CAPS). MÉTODO: Trata-se de uma pesquisa exploratória de abordagem qualitativa realizada com seis enfermeiros nos CAPS de um município do interior do Rio Grande do Sul. Foram aplicados questionários sociodemográficos e roteiro de entrevista semiestruturada, cujos dados foram submetidos à análise de conteúdo. RESULTADOS: Os resultados foram organizados em três temas, sendo eles: 1) Insuficiência da formação do profissional enfermeiro para atuação em CAPS; 2) Desafios enfrentados pela equipe de enfermagem durante a visita domiciliar; e 3) Impactos da rede de apoio no processo de recuperação dos pacientes. CONSIDERAÇÕES FINAIS: As enfermeiras participantes do estudo relatam não ter recebido treinamento adequado durante a graduação, sendo necessária a busca de conhecimento por meios próprios. Além disso, são destacados diversos desafios nas visitas, como a resistência dos pacientes em receber a equipe multidisciplinar, endereços incompletos ou errados, ausência dos pacientes durante visitas domiciliares e a dificuldade de alcançar as famílias dos pacientes. E, por fim, constatou-se a importância da assistência familiar como rede de apoio aos usuários com transtornos mentais.
