S. Romaško master‘s thesis „ Determination of polyphenolic compounds and antioxidant activity accumulated in the flowers and stigmas of Crocus sativus L. “, supervisor of scientific work prof. dr. Liudas Ivanauskas; Department of Analytical and Toxicological Chemistrу, Lithuanian University of Health Sciences, Medical Academy, Faculty .of. Pharmacy. Kaunas, 2020 Title: Determination of antioxidant activity and polyphenolic compounds accumulated in Crocus sativus L. flowers and stigmas. Keywords: Crocus sativus, stigma, petal, flavonoids, phenolic acids, polyphenols. Purpose: To determine the polyphenolic compounds and antioxidant activity of Crocus sativus L. flowers and stigmas by spectrophotometric methods. The tasks: 1. Apply the most appropriate extraction method for the analysis of flavonoids and polyphenolic acids; 2. Determine the total amount of phenolic.compounds in Crocus sativus stigma and petal by spectrophotometric method; 3. To evaluate.the diversitу of Crocus sativus flowers and stigma flavonoids bу aluminum (III) chloride spectrophotometry; 4. To determine the total antioxidant activitу of Crocus sativus in saffron and flowers using ABTS spectrophotometric method. Research methodology: Crocus sativus flowers and stigma extracts is obtained by pouring 70 proc. ethanol on the shredded plant material and extraction in an ultrasonic bath for 20 minutes at the room temperature. The spectrophotometric method using Folin - Ciocalteu is used to determine the total amount of phenolic compounds, the results obtained are calculated from the calibration graph of the galic acid in mg/g. Diversification of flavonoid content determined spectrophotometrically using aluminum (III) chloride, results are calculated and expressed as the routine equivalent in mg / g. ABTS • + method was used for determination of antiradical activity, the results are expresed in trolox equivalent per gram of dry matter. Results and conclusions: The most suitable extraction method for phenolic compounds of Crocus sativus L. was selected. The extraction is carried out for 20 minutes in an ultrasound bath with 70%. methanol solution. Phenolic acids and flavonoids of Crocus sativus L. flowers and stigmas were evaluated spectrophotometrically. The total amount of phenolic acids in the C. sativus stigmas is 17.93 GRE mg/ g, and in the petals is 37.99 GRE mg/ g. Petals accumulate approximately twice as much phenolic acid as stigmas. In 2019 Ukrainian Chersone collected petals accumulated 10,549 GRE mg/ g, and from same place ant year collected stigmas had approximately 2.3 times less phenolic acid 4579 GRE mg/ g. The total amount of flavonoids in Crocus sativus L. stigmas is 21,813 RE mg / g and in flowers 37,244 RE mg/ g. Petals accumulate 1.7 times more flavonoids than stigmas. The highest amount of flavonoids was accumulated in Cherson Ukraine in 2019 by flowers 11,088 RE mg/ g and stigmas 10,091 RE mg / g. The least flavonoids had stigmas, which were found in Ukraine in 2017 Chersone 3.308 RE mg/ g. In the same year, stigmas collected from Cherson, Ukraine, accumulated 2.17 times less than the flowers. The ABTS method used to calculate antiradical activity. Comparing the obtained results, the Crocus sativus L. stigmas 702.5 μmol/ g had bigger antioxidant activity, than petals 641.46 μmol/ g. In 2017 Ukraine Chersone collected Crocus sativus L. petals have antiradical activity 226.08 μmol/ g and stigmas that were collected in the same year and from same place have slightly lower activity 215.1 μmol/ g. From the results obtained, it can be concluded that the flowers of Crocus sativus L., which are much cheaper than stigmas and are discarded as a by-product, accumulate approximately twice as much phenolic acids as saffron, as well as significantly more flavonoids than stigmas and similar antiradical activity. In conclusion, it is worth paying attention to Crocus sativus L. petals, as they also have a pharmacological effect
Viršutinio žandikaulio sinuso dugno pakėlimo operacijos metu įvykstančių sinuso membranos perforacijų įtaka dantų implantų komplikacijoms.
Introduction: This systematic review aims to evaluate whether a correlation between Schneiderian membrane perforations and implant failure is confirmed in scientific research studies. Materials and methods: Electronic databases PubMed, Embase, Ovid Medline, Springer Link and Wiley online library were searched. Eligibility criteria enclosed prospective and retrospective cohort clinical trials and randomized clinical trials, concerning patients subjected to maxillary sinus floor elevation, in which membrane perforation occurred. Results: Two prospective clinical studies and seven retrospective clinical trials were included for the final review. A total of 1628 patients (3253 implants) were treated. The follow-up period of the studies included ranged from 6 to 98 months. Studies evaluated had an unclear risk of bias. Most of the investigated studies reported that, the occurrence of a perforated maxillary sinus membrane did not have a significant effect on implant failure rate compared to intact sinus membranes during maxillary sinus augmentation procedure. Conclusion: From the accessible data it is proposed that sinus membrane perforation did not have an unfavorable effect on implant survival rates if the membrane is repaired appropriately. Long term studies are essential in order to confirm these results

Daugiadisciplininio požiūrio į neuromuskulinių sutrikimų valdymą principai
Standards of care for hereditary muscular disorders (HMD) involve a multidisciplinary team approach for optimal clinical outcomes. The standardized care has improved life expectancy of HMD patients and especially Duchenne muscular dystrophy (DMD) by more than 10 years, which underscores the importance of a regular, multidisciplinary approach to improve patient-related outcomes. Identifying HMD is challenging as there is significant overlap of symptoms and signs between HMD and other neuromuscular disorders (NMD). Narrowing the differential diagnosis is extremely aided by pattern recognition approach of the myopathy. The 10 muscle pattern approach guides subsequent laboratory testing, histological studies and genetic testing needed to confirm the diagnosis. After accurate diagnosis, disease related complications can be predicted, and that shifted multiple subspecialties within the framework of multidisciplinary management to more anticipatory diagnostic and therapeutic strategies, to achieve prevention, early identification, and treatment of potentially modifiable disease complications. Complications are widespread across many specialties such as gastroenterology and nutrition, rehabilitation, and neuromuscular, cardiovascular, respiratory, orthopedic, and surgical aspects, however the majority of NMD patients die from cardiorespiratory complications and management of these risk organs prolongs greatly the survival of neuromuscular patients. The cornerstones of respiratory care in neuromuscular disorders are airway clearance, infection control, prevention of reflux and aspiration, and assisted ventilation. Cardiac impairment represents a major obstacle to further improvements in the management of NMDs, timing and modes of cardiovascular assessment and follow-up are determined after definitive diagnosis. Regular monitoring for biomarkers of cardiac involvement and early management have increased the life of NMD patients. The purpose of this literature review is to provide a framework for recognizing the primary manifestations and possible complications and for planning optimum treatment across different specialties with a coordinated multidisciplinary team. Other identified concerns in the field of multidisciplinary management of neuromuscular disorders include public awareness towards NMD, education of the therapeutic team, the absence of a standardized referral pathway, the presence of expertise centers, the presence of a unified database and the low numbers of patients that qualify for participation in clinical trials. Certain European organizations are tackling these concerns by constantly updating standards of care facilitating planning for multicenter trials and promoting disease natural history studies through the optimization of patient recruitment

Autoimmune bullous diseases rarely affect children and literature data on this topic is scarce. These diseases develop more frequently in male children than in females and usually take a generalized course. The extremities are most often affected. DIF represents the diagnostic tool of choice. Dapsone and systemic steroids are the drugs of choice and the treatment can take years to achieve remission
Asimptominė mobili plokščiapėdytė
In childhood, the most commonly diagnosed foot deflection – asymptomatic flexible flatfoot. It is known that all children with normal development are born with a flexible flat-foot, but During the first decade of life the foot develops - the longitudinal foot arch develops, and its convex form develops. Although it is understood that all born children are flat-footed – there is no consensus in the literature as to how flat the foot should be flat. Also, age stages, leg axial deformities, obesity and flat foot development stages of a particular age are not taken into account. Different dimensions are used to define a literary flat-foot, but the common, well-defined, flat-denominator in clinical practice is unknown. Therefore, there is a debate in the literature about what the child "s feet" are considered to be "normal" in a different age of the child, and how the "normal" steps of the foot passes as a flat foot. Also, increasing associations between increased body mass index and a flatter foot have been demonstrated in the literature
Peculiarities of Generalized Anxiety Disorder Treatment in General Medical Practice
Author: Adas Gudaitis Title: Peculiarities of Generalized Anxiety Disorder Treatment in General Medical Practice Aim of the review: To select and review scientific publications that deal with the peculiarities of generalized anxiety disorder treatment. Objectives: 1. To review the pharmacological treatment options for generalized anxiety disorder by selecting first and second choice drugs. 2. To review the most effective psychotherapy options for generalized anxiety disorder treatment. 3. To review the most effective alternative and complementary measures for generalized anxiety disorder treatment. Methods: Research type - a literature review. Electronic database Google Scholar was used to search for information. The keywords used were “generalized anxiety disorder” OR “GAD” AND “primary care” OR “general practice” OR “family medicine” AND “treatment”. Search period was 2009-2019. Articles that review peculiarities in either pharmacological or psychotherapy based treatment and alternative treatment options have been selected. After the selection process, a total of 27 articles have been selected and included in the literature review. Conclusions: 1. A review of the pharmacological options for generalized anxiety disorder treatment suggests that the first-line drugs should be SSRIs or SNRIs, with the addition of benzodiazepines, until the interactions of SSRIs or SNRIs become effective. The second-line treatment for generalized anxiety disorder should be pregabalin, buspirone, or tricyclic antidepressants. 2. After reviewing psychotherapy based generalized anxiety disorder treatment options, it can be stated that cognitive-behavioral therapy and mindfulness-based stress reduction are the most effective types of psychotherapy for the treatment of this disorder. There is currently not enough evidence to say that they can completely replace a pharmacological treatment, so it is recommended to combine a psychotherapy and pharmacology based treatments. 3. After reviewing alternative therapies for generalized anxiety disorder, cannabidiol and physical activity have been shown to be effective. Cannabidiol is a new drug that, given its efficacy, may replace first-line pharmacological agents in the future, but current studies provide only short-term treatment results, with long-term treatment results yet to be seen. At present, both cannabidiol and physical activity are recommended as adjunctive therapy in pharmacological or psychotherapeutic treatment
The value added curd cheese production by using Prunus dulcis by-products and their impact on quality parameters
In recent years, a strong focus has been placed on the use of by-products, reducing production waste, re-use of food ingredients and increased value of food ingredients and products with health-beneficial properties for the development of. The purpose of this work is to make curd cheeses using Prunus dulcis press cake and to assess their impact on quality parameters. Curd cheeses with different amounts of almond press cake were produced during the experiment (5, 10, 15 and 20 %). One part of the almond press cake was treated with ultrasound; the other part was fermented with Lactobacillus casei (LUHS210). The dry matter content, total number of microorganisms, total number of yeasts and molds, colour coordinates (L*, a*, b*) has been determined for Prunus dulcis press cake. The total titratable acidity, the content of lactic acid bacteria and pH has been determined in fermented almond press cake. The sensory properties analysis and overall acceptability was used to assess the produced curd cheeses. The quality parameters such us pH, the total titrable acidity, dry matter content, lactic acid bacteria content, total number of microorganisms, colour coordinates (L*, a*, b*), texture, total content of phenolic compounds, DPPH free radical scavenging activity have been defined to assess the produced curd cheeses. The results obtained were compared with the control samples. The ultrasound had a positive effect on the overall reduction of the number of microorganisms, yeasts and molds. The fermentation process reduced the colour coordinates, the number of yeasts and molds. In curd cheeses, the smell and taste of almond press cake of fermented with Lactobacillus casei (LUHS210) were most noticeable, and the intensity increased with increasing amount. The most acceptable curd cheeses with 10 % added fermented press cake. The Prunus dulcis press cake used in the production of curd cheeses increased the pH value, the total titratable acidity, the content of lactic acid bacteria, colour coordinates, total content of phenolic compounds, and DPPH free radical scavenging activity, and reduced dry matter content, total number of microorganisms, texture strength
