Citizen science approaches in the development of post-stroke physical activity interventions:A scoping review

    No full text
BackgroundStroke is a major cause of disability globally, with high recurrence rates despite the implementation of secondary prevention strategies. Promoting physical activity and reducing sedentary behaviour are critical to mitigate these risks. Collaborative research approaches, including citizen science, offer promising methods for developing more effective and sustainable interventions by leveraging patient insights and lived experiences across different research stages.ObjectivesThis scoping review explored the application of citizen science approaches in developing interventions targeting physical activity and sedentary behaviour for people with stroke.MethodsFollowing Arksey and O’Malley’s framework and the PRISMA-ScR checklist, five databases were searched. We included empirical studies involving stroke patients in research on physical activity or sedentary behaviour interventions. Data was extracted on terminology, collaboration methods, and participant roles and analysed using the Participation Matrix framework. Methodological rigor was assessed using the CASP qualitative checklist.ResultsFourteen studies were included, most published after 2020 and originating from diverse countries. Terms like “co-design,” “co-creation,” and “patient and public involvement” were prevalent, but “citizen science” was not explicitly mentioned. Methods for active involvement of stroke patients included focus groups, workshops, and advisory panels. Stroke patients primarily participated as advisors or partners during intervention design, with minimal involvement in early research stages, data analysis, or dissemination. Researchers predominantly held decision-making roles.ConclusionsCitizen science in stroke research is still developing, with limited patient involvement across research phases. Expanding the depth and scope of patient involvement could enhance the relevance and long-term impact of interventions

The influence of ROS1 fusion partners and resistance mechanisms in ROS1-TKI-treated non-small cell lung cancer patients

    No full text
Clinical outcomes in ROS1-fusion positive (ROS1+) non-small cell lung cancer (NSCLC) by fusion partner and resistance mechanisms are limited. This cohort study included 56 ROS1+ patients (FISH or NGS confirmed); fusion partners were identified in 27 cases, including CD74 (n = 10), EZR (n = 7), and SDC4 (n = 7). Clinical data were available for 50 patients (median age 62; 51% female; 32% never-smokers). Forty patients received tyrosine kinase inhibitors (TKIs), mostly crizotinib (n = 38). Crizotinib showed a 55% objective response rate (ORR) and a median progression-free survival (mPFS) of 5.3 months. Brain metastases (HR 2.65, 95% CI 1.06–6.60, P = 0.037) and prior chemotherapy (HR 3.17, 95% CI 1.35–7.45, P = 0.008) had a higher risk of progression. Sixteen patients received subsequent lorlatinib, with an ORR of 28% and mPFS of 3.7 months. G2032R and L2026M resistance mutations were identified in four lorlatinib non-responders, and in vitro studies confirmed resistance to lorlatinib. Fusion partners did not affect crizotinib outcomes. Lorlatinib was ineffective against on-target resistance. Real-world data showed lower TKI efficacy than clinical trials, highlighting the role of clinical and molecular factors in treatment response

Improving mental health literacy in secondary school educational professionals in the netherlands:adaptation of an existing intervention using Intervention Mapping Adapt

    No full text
BACKGROUND: Advanced interventions to enhance the mental health literacy (MHL) of educational professionals are now available, though their effectiveness varies, likely due to contextual differences. To adapt these interventions for use in other countries, a systematic and tailored approach is required to maintain the logical framework. This study describes the systematic process of selecting and adapting an intervention to improve MHL of Dutch secondary school educational professionals, using Intervention Mapping (IM) Adapt.METHODS: IM Adapt regards six steps: (1) assess needs and organizational capacity; (2) find evidence-based interventions; (3) plan adaptations; (4) make adaptations; (5) plan for implementation; and (6) plan for evaluation.RESULTS: The outcomes of a needs assessment were used to develop a logic model of the problem and of change, leading to the intervention goal of increasing mental health awareness, knowledge, attitudes, and self-efficacy among end-users. A logic model of the problem and of change visually outlines the relationship between a specific issue, its root causes, and the intended intervention to drive change. The existing evidence-based digital intervention, named LEARN, was identified as the most suitable for addressing these needs. This was done through reviewing the literature and existing intervention databases. Four main content adaptations were made to align with the distinct needs of the Dutch context and the evolving trends within the field of mental health, resulting in LEARN-NL, a digital mental health literacy intervention for Dutch educational professionals. Finally, implementers were identified, and a mixed methods feasibility study was set up.CONCLUSIONS: Our findings show that some content adaptations to LEARN were needed to address the needs and (learning) preferences of educational professionals in the Dutch education context, but that the majority of the existing intervention could be upheld. Existing evidence-based MHL interventions for educational professionals are useful as a basis for the adaptation and transfer to other countries. IM Adapt is a valuable framework for the systematic planning of adaptations to a new context while retaining its essential elements.</p

Comprehensive Needs Assessment for Enhancing Self-Management in People with Lipoedema and the Support Provided by Their Healthcare Professionals

    No full text
Background: The cause of lipoedema remains unclear, and the condition is currently incurable. Effective self-management is therefore essential for coping with its physical and psychological impacts and the necessary lifestyle adjustments. This study aimed to assess the needs, barriers, and facilitators for enhancing self-management and self-management support from the perspectives of people with lipoedema and the healthcare professionals (HCPs) involved in their care. Methods: The study used a mixed-methods approach, incorporating a narrative review focused on people with chronic conditions and their HCPs, along with focus groups involving people diagnosed with lipoedema and the HCPs involved in their care. The Core Processes of the Intervention Mapping method guided a systematic approach to address the study’s objectives. Qualitative data were analyzed using a grounded theory approach. Results: Findings revealed unique self-management barriers for people with lipoedema, including limited awareness and expertise among HCPs, as well as stigmatization from both HCPs and society. Participants identified a need for tailored lifestyle plans, guidance, and support for monitoring progress. Key facilitators included self-management skills, supportive networks, and role models. HCPs noted barriers in communication and collaboration due to a lack of specialized professionals and negative attitudes toward lipoedema. They expressed a need for multidisciplinary/interprofessional teams, accurate diagnosis, patient openness, and reliable information resources. Facilitators included fostering trust, encouraging patient participation, and setting achievable goals. Conclusion: This study underscores the need for tailored self-management interventions for people with lipoedema. The adaptation of existing self-management strategies from other chronic conditions should take into account the specific needs, barriers, and facilitators of people with lipoedema and their HCPs
