    Proinflammatory cytokines involved in the pathogenesis of rheumatoid arthritis (RA) can inhibit insulin production and cause insulin resistance of peripheral tissues. Perhaps, with RA, the risk of developing disorders of carbohydrate metabolism (NUO) increases: diabetes mellitus (DM), fasting hyperglycemia (GN), glucose tolerance disorders. Patients with a combination of RA and DM belong to the category of the most severe patients with an unfavorable prognosis of macro- and microvascular complications. The problem of rheumatoid arthritis (RA) is becoming of general medical importance, since it creates prerequisites for improving the pharmacotherapy of other chronic diseases, such as atherosclerosis, diabetes mellitus (DM) type 2, osteoporosis, the development of which is also associated with chronic inflammation. The study of the pathogenesis of RA has revealed numerous "non-rheumatological" functions of proinflammatory cytokines, for example, the ability to interfere with various stages of metabolism glucose. It has been shown that tumor necrosis factor (TNF) α and interleukin (IL) 6 disrupt the synthesis and functioning of insulin receptors and intracellular glucose transporter in muscles, adipose tissue, liver, along with IL1ß inhibit insulin secretion, cause apoptosis of beta cells of islets Langerhans of the pancreas. RA can be considered as a model for studying disorders of carbohydrate metabolism (NUO). Several studies in RA patients have demonstrated a decrease in the level of functional capabilities of β-cells and an increase in insulin resistance of peripheral tissues. Samples were collected from the clinics of the Dentistry College-University of Dhi-Qar.     Endocrine disorders are of primary importance in the occurrence and progression of rheumatic diseases. Often, endocrine restructuring that occurs during puberty, menopause, pregnancy, childbirth, and abortion serves as a background for the development of connective tissue diseases. At the same time, the change in neuroendocrine status is an integral component of the progression of rheumatic diseases. Especially distinct hormonal shifts were found on the part of the thyroid gland in rheumatoid arthritis (RA). The prevalence of thyroid pathology in general among patients with rheumatoid arthritis, according to literature data, reaches 34%, and Hashimoto's thyroiditis, according to various authors, from 4 to 13.5%. In addition, the greatest susceptibility to RA in women of reproductive age indicates a certain role of sex hormones in the pathogenesis of the disease. Metabolic and degenerative changes in connective tissue largely determine the pathogenesis and clinical picture of various diseases.     Modern Clinical and Biochemical Characteristics of Liver Cirrhosis Patients of Viral Etiology with Spontaneous Bacterial Peritonitis

    The clinical and laboratorical characteristics of spontaneous bacterial peritonitis with cirrhosis of the liver of viral etiology were studied. A total of 58 (32 (55.2%) men and 26 (40.6%) women) patients with cirrhosis of the liver of viral etiology aged 30 to 69 years were examined. Upon admission, all patients underwent diagnostic laparocentesis, followed by counting the number of neutrophils in the AF and plating on culture media. It was established in the examined patients that cirrhosis with HCV markers was most frequently encountered. An analysis of the infection factors in the examined patients with liver cirrhosis with complicated SBP showed that problems associated with parenteral interventions were noted by 67.3% and the source of infection was not established in 24.1% of patients. The most common signs of SBP were dyspeptic symptoms-60.3%, fever-37.9%, leukocytosis with the appearance of immature forms of leukocytes-48.2%

    Diabetic foot ulcers are serious complications of Diabetes Mellitus and are known to be resistant to conventional treatment; they may herald severe complications and may lead to lower limb amputation if not treated wisely. Objectives: To assess the efficacy of conservative management and LowLevel Laser Therapy (LLLT) in the treatment of diabetic foot ulcers. Setting: Baghdad Teaching Hospital. Time, from January 2015 through April 2020. Patients and methods: A prospective randamised study included (45) patients who were proved to have a diabetic foot ulcer, the ulcers were graded according to Wagner classification into six grades from (0 to 5), and patients with Wagner grades 1, 2, and 3 were treated conservativly by wound cleaning, controlling of blood sugar in the ward and send for laser therapy. Results: There were six patients with grade 1 ulcers, and all of them (100%) showed complete healing over a period of 3 weeks of conservative treatment and laser therapy. Also, 22 patients out of 25 (88%) with grade 2 ulcers and 11 patients out of 14 (78.6%) with grade 3 ulcers showed complete healing in about six weeks of treatment. Forty-five patients were treated by laser, and only six patients (13.3%) showed minor complications (itching, numbness, erythema, or even blisters), which were reported after the 2nd. Weak of treatment. Conclusion: Diabetic foot ulcers are a common problem in Baghdad Teaching Hospital; it is most commonly occure in patients with long-standing uncontrolled diabetes.
