    Indicators and effectiveness of sensory and motor blocking unilateral spinal anesthesia combined with unilateral epidural analgesia

    The relevance of this issue is due to the intensive development and introduction of the latest technologies in modern surgical interventions, including joint replacement, and a significant expansion of indications for surgical treatment. We have already pointed out that in the vast majority of patients of these operations, elderly and senile people with various comorbidities and, in some cases, severely disabled, burdened with severe pain for many months and years, which leads to whole complexes of somatic reactions. and deviations in the clinical status of patients. All this greatly complicates the choice of the method of anesthesia and postoperative analgesia. After all, postoperative pain is the result of surgery, but its treatment is not just a humane requirement, but a key aspect of postoperative therapy. Endoprosthetics of the joints of the lower limb is also a very traumatic intervention and, in some cases, is accompanied by significant blood loss. In the postoperative period, venous thrombosis and PE are the most dangerous [152; pp.101-106, 151; pp. 1018-1025, 72; pp.455-461]. In connection with the above, anesthesia should correspond to the nature of this intervention: completely block nociceptive impulses, maximally reduce perioperative blood loss and the need for donor blood components, the likelihood of developing thrombotic complications in the postoperative period, and the severity of postoperative pain syndrome [18; pp.210-212]. All these requirements are met to the maximum extent by the methods of central segmental blockades [92; pp. 810-814, 188; pp.193-199]

    This study uses annual time series data of HIV prevalence among individuals aged 15-49 years for Cambodia from 1990 to 2020 to predict future trends of HIV prevalence over the period 2021 to 2030. The study utilizes Holt’s double exponential smoothing model. The optimal values of smoothing constants α and β are 0.9 and 0.6 respectively based on minimum MSE. The results of the study indicate that annual HIV prevalence among people aged 15-49 years will continue to decline significantly over the out of sample period. Therefore, we encourage authorities to strengthen preventive measures among key populations who are experiencing an increase in HIV seroprevalenc

The study utilizes Holt’s linear method (HLM). The optimal values of smoothing constants α and β are 0.9 and 0.1 respectively based on minimum MSE. The results of the study indicate that annual HIV prevalence among individuals aged 15-49 years will continue to decline over the out of sample period but still remain high. Therefore, policymakers must scale up HIV case detection, increase ART coverage among people living with HIV and improve coverage of HIV prevention interventions particularly among key populations

    Evaluating The Effectiveness Of Laparoscopic Longitudinal Gastric Resection Surgery On Early Postoperative Complications And Obesity-Related Comorbidities

    Studying of the corrective effect of longitudinal gastrectomy on concomitant diseases in patients with morbid obesity. This scientific work is based on the results of a comprehensive examination and treatment of 187 patients with MO who were treated in the our clinical bases from 2021 to 2023. The analysis of comorbidities in the compared groups showed that one or more comorbidities were detected in 72% of the control group, 77% of the main group. The main part of these patients was made up of patients with obesity of the III degree. The proposed modification of SG in morbidly obese patients improved the cardioesophageal function and the strength of the gastroduodenal staple line, eliminating the observed complication of staple line failure of 2.2%. It also led to a decrease in the symptoms of gastroesophageal reflux disease from 14% to 6.1%. In the longitudinal gastric resection procedure, the use of local hemostatic powder "HEMOBEN" along the gastric staple line reduced the bleeding complication from 3.2% to 1.1% and the duration of the procedure by an average of 15±3 minutes.In particular, if in the control and main groups, arterial hypertension was detected in 50 and 59.3% of patients before the surgical procedure, then 3 months after the surgical procedure, an increase in blood pressure was observed in 45.4 and 37% of the patients. After surgery, the clinical signs of arterial hypertension and arthralgia in patients with morbid obesity gradually disappeared with the elimination of obesity. In conclusion, these clinical signs were caused by obesity, and its elimination led to the disappearance of these clinical signs

    Over the past years and decades, in the world in general and particularly in Russia, bariatric surgery is becoming more widespread for the treatment of severe forms of obesity. There is an increase in the number of operations performed along with the effectiveness and minimising risks of the operations held due to the improvement of surgical techniques. In this lecture, there are clearly stated indications and contraindications for the surgical treatment of obesity. Here are presented various types of bariatric surgeries and their pathophysiological mechanisms which affect on body weight, carbohydrate and lipid metabolism. There is also presented their effectiveness in comorbid obesity pathology, primarily among patients with type 2 diabetes. Here was held an analysis of possible negative effects after surgical treatment for obesity, including secondary hyperparathyroidism, post-bariatric hypoglycemia, etc., as well as predictors of postoperative prognosis in relation on metabolic control among patients with obesity and type 2 diabete
