Dauerbelastungen von vorgefertigten Stegen auf einteiligen Dentalimplantaten
In der vorliegenden Arbeit wurden In vitro-Ermüdungsstests an vorgefertigten Stegen auf zwei verschiedenen Implantatsystemen durchgeführt. Getestet wurde nach den Vorgaben in EN ISO 14801. Dementsprechend wurden die vorgefertigten Stege einer intermittierenden Dauerbelastung in unterschiedlichen Höhen ausgesetzt. Ziel der Untersuchung war der Nachweis, dass die vorgefertigten Stege den im Mund vorkommenden Belastungen hinsichtlich Belastungshöhe und Belastungshäufigkeit standhalten.
Die gemessenen Stege waren SFI-Bars® der Firma Cendres+Métaux SA (Biel/Bienne, CH). Die SFI-Bars® wurden sowohl auf Thommen- als auch auf BPI-Implantaten montiert und den Ermüdungstests unterworfen. Die Stege wurden mit Implantatabständen von 8 mm und 26 mm montiert und gemessen. Dies entspricht der minimalen bzw. maximalen Einbaulänge dieses Systems.
Auf Thommen-Implantaten versagte bis zu einer Belastung von 360 N über 1*106 Belastungszyklen keiner der Prüfkörper, auf BPI-Implantaten versagte bis zu einer Belastung von 270 N über die gleiche Anzahl an Zyklen kein Prüfkörper. Die Auslenkung der Stege über die Belastungszyklen erfolgte im Wesentlichen während der ersten Zyklen. Um Daten zu Belastungen über eine höhere Anzahl an Lastzyklen zu erhalten, wurde für Stege auf BPI-Implantaten die Anzahl der Belastungszyklen bei der klinisch relevanten Lasthöhe von 180 N auf 2*106 verdoppelt. Auch hier hielten alle Prüfkörper der Belastung stand und eine deutlich stärkere Auslenkung der Stege über die höhere Zykluszahl war nicht erkennbar. SFI-Bars® erfüllen also im Bereich der klinisch relevanten Belastung bis 200 N die Anforderungen der EN ISO 14801.
Weiterhin wurden die Losdrehmomente nach erfolgter Ermüdungsprüfung sowie die Auswirkungen eines erhöhten Anzugsdrehmoments bei der Montage des SFI-Bars® auf Dauerlastverhalten und Loslösedrehmoment bei kritischer Last untersucht. Auch hier fiel keiner der Prüfkörper durch. Alle Schraubverbindungen zeigten im klinisch relevanten Lastbereich nach Ermüdungsprüfung noch mindestens 60 % des Anzugsdrehmomentes als Losdrehmoment. Ein gegenüber den Herstellerangaben deutlich erhöhtes Drehmoment führte nicht zu einer verschlechterten Haltbarkeit der Schraubverbindungen.
Ob bei erhöhtem Drehmoment eine Kaltverschweissung der Bauteile erfolgte, konnte im Rahmen dieser Arbeit nicht mit Sicherheit geklärt werden.
In den durchgeführten Ermüdungstests konnte demnach im klinisch relevanten Lastbereich kein Versagen beobachtet werden. Die gemessenen plastischen Verformungen sind in einem Bereich, der keine Einschränkungen der Funktionalität erwarten lässt. Somit kann dieses vorgefertigte Stegsystem uneingeschränkt für den klinischen Einsatz empfohlen werden

Rab requirements in HOPS-mediated late endosome-lysosome tethering and dissection of fusion factor requirements in Ca<sup>2+</sup>-dependent phagosome-lysosome fusion
Through sequential fusion with early- and late endosomes (EEs and LEs) and ultimately lysosomes (LYS), a degradative (endo-)lysosome ((E)LYS is formed. Directionality of fusion is ensured by the action of Ras-like GTPases and their effector proteins, such as tethering proteins. In LE-LYS fusion, tethering is mediated by the HOPS complex and its interaction partners. The chronological order in which these proteins are recruited and act was not established. It is also unclear how HOPS is recruited and anchored to maturing LEs. Ferrofluid (FF)-labelled, purified late endocytic compartments (FFEs) from Arl8b, Arl8a/b, Rab7a, Vps41 (subunit of HOPS), and BORC subunits myrlysin and diaskedin KO cell lines were thoroughly characterised. The examined cells still contained functional (E)LYS. However, transport of endocytosed material to LYS was significantly delayed. Thus, these proteins play a crucial role in transporting cargo to LYS. Kinetic assessment revealed that Rab7a and Vps41 were recruited to maturing endosomes simultaneously and before Arl8, lyspersin (BORC), and LAMP1. Arl8 and BORC subunit levels were reduced on FFEs from Rab7a and Vps41 KOs, indicating that both proteins are recruited first and act before Arl8 and BORC. Vps41 (HOPS) being required before Arl8 raised the question of how HOPS was recruited to LEs. Rab2a was a good candidate for recruiting HOPS, as it binds to the Vps39 subunit of HOPS. A new Rab2a KO HeLa cell line was established. The levels of Vps41 and Vps39 on Rab2a KO FFEs were reduced, as were the levels of other lysosomal proteins (Arl8, mCath D, lyspersin), indicating a role for Rab2a in recruitment of HOPS. However, loss of Rab2a did not displace Vps41 from membranes to the cytosol. Thus, Rab2a cannot be the main membrane anchor of HOPS.
In a cell-free fusion assay, requirements of ATP-and cytosol-dependent phagosome-with-LYS fusion (StaFu) can be bypassed by adding micromolar [Ca2+] (CaFu). It was unclear whether CaFu required the action of SNARE proteins or NSF (N-ethylmaleimide [NEM] sensitive factor)-dependent priming during the reconstituted fusion reaction. NEM only had moderate inhibitory effects on CaFu, indicating independence of CaFu from NSF-dependent priming during the in vitro fusion reaction. CaFu was moderately inhibited by α-SNAPL294A, a potent inhibitor of SNARE-dependent fusion. SNARE participation in CaFu was shown by the inhibitory effect of three preincubated soluble Q-SNAREs. Hence, CaFu depends on the action of SNARE proteins. Further studies must uncover which protein(s) are involved in CaFu and which physiological function this putative shortcut reaction might serve in vivo
Environmental impacts of silage production: Formation, emission and fixation of gases from field to barn
Silages are essential feeds in livestock husbandry. Losses should be minimised to close the nutrient cycle between crop and animal production. Besides, losses are generally associated with emissions of climate and environmental relevant substances, such as greenhouse gases (GHG), e.g. carbon dioxide (CO2), methane (CH4) and nitrous oxide (N2O), or volatile organic compounds (VOC), e.g. ethanol and ethyl acetate. These gases are emitted during fermentation and aerobic storage, contributing to the negative environmental impact of agricultural animal husbandry.
This thesis addresses the pathways of gas dynamics, i.e. formation, emission and fixation of the gases mentioned above, in order to derive climate impacts and mitigation options through adapted management in silage production.
Study 1 investigated the concentrations of GHGs during the 49-day fermentation of grass and lucerne silage (two varying dry matter contents in each case). The wetter silages showed earlier CO2 formation due to faster microbial activity. Grass silage had higher CH4 and N2O concentrations than lucerne silage during the local maxima (first 4 ensiling days). After a temporary drop in the concentrations, the lucerne silage showed increasing CH4 concentrations. This was due to malfermentation, i.e. the activity of clostridia. This led to the formation of butyric acid and ammonia. The rising pH value and released hydrogen facilitated methanogenesis.
The other studies investigated GHG and VOC gases’ formation, emission and fixation during 30 and 135 days of fermentation (Study 2) and 14 days of aerobic storage (Study 3). Two of the three maize silage treatments were treated with a biological (lactic acid bacteria) or chemical silage additives (organic acids). The biological additives reduced the CH4 and N2O, and the chemical reduced the ethanol and increased the N2O emissions of the fermentation (Study 2). The formation took place mainly in the first 6 ensiling days. The treatments showed decreasing gas emission quantities in the course of ensiling: microbiological fixation was assumed.
Both treated variants showed increased aerobic stability (Study 3), which reduced dry matter losses. This means that lower quantities of harvested forage are required, which can reduce indirect GHG emissions during crop production. This has a positive effect on the climate impact of silage production. At the same time, the treated variants showed increased VOC emissions during aerobic storage. The quantities of ethyl acetate emissions exceeded the original quantities in the material; microbial reformation from ethanol was suspected. Using silage additives can mitigate the negative environmental consequences of poor silage management.
Based on the studies, a structuring of gas formation and fixation phases within the silage production process chain is derived. In addition, recommendations for emissions research and management in practice are formulated in this thesis.Umwelteinflüsse der Silageproduktion: Bildung, Emission und Fixierung von Gasen zwischen Feld und Stall
Silagen sind wichtige Futtermittel in der Viehhaltung. Verluste sollen minimiert werden, um den Nährstoffkreislauf zwischen Pflanzen- und Tierproduktion zu schließen. Zudem gehen Verluste meist mit Emissionen umweltrelevanter Stoffe, wie Treibhausgase (THG), e.g. Kohlenstoffdioxid (CO2), Methan (CH4) und Lachgas (N2O), oder flüchtige organische Verbindungen (VOC), e.g. Ethanol und Ethylacetat, einher. Diese Gase emittieren während der Fermentation sowie der aeroben Lagerung und beeinflussen die Umweltfolgen der Viehhaltung.
Diese Dissertation befasst sich mit der Bildung, Emission und Fixierung der Gase, um daraus Klimaauswirkungen und Minderungsmöglichkeiten in der Silageproduktion abzuleiten.
Studie 1 untersuchte die Konzentrationen der THG während der 49-tägigen Fermentation von Gras- und Luzernesilage (jeweils zwei variierende Trockenmassegehalte). Die nasseren Silagen zeigten aufgrund schnellerer mikrobieller Aktivität eine frühere CO2-Bildung. Grassilage wies während der lokalen Maxima (erste 4 Siliertage) höhere CH4- und N2O-Konzentrationen als Luzernesilage auf. Nach einem zwischenzeitlichen Abfall der Konzentrationen zeigte die Luzernesilage ansteigende CH4-Konzentrationen, welche auf eine Fehlgärung, d.h. die Aktivität von Clostridien, zurückzuführen waren. Diese führte zur Buttersäure- und Ammoniakbildung. Die steigenden pH Werte und der freigesetzte Wasserstoff begünstigten die Methanogenese.
Die weiteren Studien untersuchten die Bildung, Emission und Fixierung der THG und VOC Gase während der 30- bzw. 135-tägigen Fermentation (Studie 2) und 14-tägigen aeroben Lagerung (Studie 3). Zwei der drei Maissilagevarianten wurden mit einem biologischen (Milchsäurebakterien) bzw. einem chemischen Siliermittel (organische Säuren) behandelt. Das biologische Siliermittel reduzierte die CH4- und N2O-, das chemische reduzierte die Ethanol- und erhöhte die N2O-Emissionen der Fermentation (Studie 2). Die Bildung fand in den ersten 6 Tagen statt. Alle Varianten zeigten abnehmende Gasmengen: eine mikrobielle Fixierung wird vermutet.
Die behandelten Varianten zeigten eine verlängerte aerobe Stabilität (Studie 3) was die Trockenmasseverluste reduziert. So werden geringere Erntemengen benötigt, was zu sinkenden indirekten THG Emissionen während des Ackerbaus führt. Dies beeinflusst die Klimafolgen der Silageproduktion positiv. So kann der Einsatz von Siliermitteln die negativen Umweltfolgen von schlechtem Silagemanagement reduzieren. Jedoch zeigten die behandelten Varianten erhöhte VOC Emissionen während der aeroben Lagerung. Die Mengen an Ethylacetatemissionen überschritten die Mengen im Material; eine mikrobielle Neubildung aus Ethanol wird vermutet.
Auf Basis der Studien wird eine Strukturierung der Phasen der Gasbildung und -fixierung innerhalb der Prozesskette der Silageproduktion hergeleitet. Zudem werden in dieser Dissertation Empfehlungen für die Emissionsforschung und das Management in der Praxis formuliert
Die Bedeutung des Paucimannose-Nachweises beim Ovarialkarzinom
Das Ziel der Dissertation bestand darin, den Nachweis von Paucimannose beim Ovarialkarzinom zu erforschen. Hierfür wurde eine Korrelation zwischen der immunhistochemischen Paucimannoseexpression und dem Gesamtüberleben sowie dem krankheitsfreien Überleben untersucht. Außerdem wurden Zusammenhänge zwischen klinischen beziehungsweise intraoperativen Parametern und der Paucimannoseexpression geprüft. Eine Tumorzellfärbung von über 50 % in 400-facher Vergrößerung (Hot Spot) war signifikant mit einer niedrigeren 5-Jahres-Gesamtüberlebensrate und folglich mit einer schlechteren Prognose assoziiert. Darüber hinaus war die Färbung von Immunzellen im Tumorgebiet signifikant mit dem 5-Jahres-Gesamtüberleben sowie dem krankheitsfreien 5-Jahres-Überleben und daher mit einer günstigeren Prognose assoziiert. Die Ergebnisse des Projekts belegen, dass Paucimannose Teil des Glykosylierungsprofils von Ovarialkarzinomzellen und Makrophagen ist und liefern Hinweise auf unterschiedliche Funktionen paucimannosidischer Glykane in diesen zwei Zellentitäten
Charakterisierung der Effekte agonistischer und antagonistischer Modulation von Immuncheckpoint-Molekülen auf die Osteoklastenbiologie
Immuncheckpoint-Inhibition hat die Therapie maligner Erkrankungen revolutioniert und stellt einen wertvollen Teil der Behandlung inflammatorischer und autoimmuner Krankheiten dar. Allerdings gehen mit der erheblichen anti-malignen Wirksamkeit auch diverse kritische auto-inflammatorische Nebenwirkungen einher, sogenannte „Immune-related adverse events“. Für diejenigen Nebenwirkungen, die den Knochenstoffwechsel betreffen, sind auf zellulärer Ebene nicht nur reife Osteoklasten, sondern auch Makrophagen als Teil der angeborenen sowie erworbenen Immunreaktion und speziell als Osteoklasten-Vorläuferzellen besonders relevant.
Zur Analyse der Effekte von therapeutischer Checkpoint-Regulation auf diese Zelltypen wurde ein in vitro Modell der Osteoklasten-Differenzierung ausgehend von humanen mononukleären Zellen des peripheren Bluts entwickelt. Die quantitative sowie qualitative Auswertung der Osteoklastogenese unter dem Einfluss agonistischer Checkpoint-Proteine und antagonistischer Antikörper erfolgte in einem umfassenden Screening-Ansatz mittels Immunfluoreszenzfärbung, Messung der Zellgröße, manueller Zellzählung und Resorptionsaktivitätsbestimmung in großer Probenzahl.
Diese Untersuchungen ergaben reproduzierbare und vergleichbare Ergebnisse, welche die Relevanz der Checkpoint-Regulation für die Knochenhomöostase trotz der Osteoklasten-typischen Heterogenität bestätigen. Diese erleichtern auch die weitere Entwicklung von individualisierten diagnostischen und therapeutischen Optionen für Erkrankungen, die durch eine Fehlregulation des Knochenstoffwechsels charakterisiert sind. Zelltyp- und Checkpoint-spezifisch sollte hierfür nicht nur Immuncheckpoint-Inhibition, sondern insbesondere auch die Stimulation von Checkpoint-Regulatoren in Betracht gezogen werden
Biologically plausible learning and dynamics in neural networks
The plasticity and dynamics of biological neural networks enable remarkable computations despite physiological constraints. Here we study plasticity and dynamics that satisfy such constraints using analytically tractable linear rate networks and numerical simulations.
Biologically plausible models of reinforcement learning must be local and able to learn from sparse, delayed rewards. Weight (WP) and node perturbation (NP) achieve this by correlating fluctuations in synaptic strength or neuronal activity, respectively, with reward changes. Because the number of weights massively exceeds the number of neurons, NP was believed to be far superior to WP and more likely neurobiologically realized. We develop a clear, mathematically-grounded understanding of these versatile learning rules applied to linear rate networks learning a student-teacher task. Our analytical results show that WP can perform similarly to or even better than NP for many temporally extended and low-dimensional tasks, which we confirm in simulations of more complex networks and tasks. We further find qualitative differences in the weight and learning dynamics of WP and NP that might allow to distinguish them experimentally. The generated insights allow us to formulate modified learning rules that in certain situations combine the advantages of WP and NP. Together, our findings indicate WP as competitive or even preferable to NP for many relevant biological and machine learning tasks, suggesting it as a useful benchmark and plausible candidate for learning in the brain.
Biologically plausible models of neural computation must reflect experimentally observed network characteristics. One such characteristic is that principal neurons in sensory cortices encode continuous variables with overlapping responses, featuring predominant excitation between neurons with strongly overlapping responses. The reasons underlying such connectivity are still unclear, and there are even known disadvantages to it. To address this knowledge gap, we develop a novel cooperative coding scheme that relies on like-to-like excitation to implement a desired response. Neurons cooperatively share their computations and access to feedforward input with similarly-tuned neurons that also need them. This allows to exchange many feedforward and less specific recurrent connections for few specific recurrent ones, thereby minimizing the total number of synapses. By comparing cooperatively coding and feedforward networks achieving the same network response, we find that synaptic savings come at the cost of increased network response times. This trade-off improves in magnitude and scaling when introducing delayed, balancing inhibition or spike frequency adaptation, or when encoding higher-dimensional stimuli. Our results suggest the number of synapses as an important constraint that can explain observed connectivity patterns in a novel cooperative coding scheme, possibly enabled by balancing inhibition
Map Generalization and Set Visualization Through Spatial Unit Allocation
Data visualization is an important aspect of making complex data accessible to a wide audience. Capturing key information in visualized data is critical for users to understand the data and make informed decisions. On the one hand, the data may contain geospatial information, which is often visualized using maps. When navigating maps that visualize spatial data, changing the zoom level is a common strategy for controlling the level of detail. Reducing the level of detail of a map is a key challenge in cartography and can be addressed with map generalization. These changes in level of detail while zooming should be monotonous. On the other hand, the visualization of abstract data, such as set systems, is a well-established field of research in information visualization. Set systems contain information about set elements and their relationships, e.g. working groups and their research projects within a faculty. Set visualization have proven to be a powerful tool for presenting large amounts of data to a user in a compact way. Seeing the interactions between sets in a visualization is easier to understand than reading a description of those interactions. However, both types of data, geospatial and abstract, can become overwhelming for the user if every detail is visualized.
In this thesis, we present optimization approaches for creating generalizations for both geospatial data and set systems. In our algorithms, we adapt some approaches from the research field of spatial unit allocation. When generalizing geospatial data, we aggregate small polygons into larger areas, aiming for a set of representative output polygons. We use our approach to compute a set of solutions that contains an optimal solution for each value of the parameter which controls the level of generalization. For set systems, we present the first approach that is able to determine whether a given set system can be completely displayed as an Euler diagram that satisfies certain wellformedness conditions. If a complete visualization is not possible or required, we aim to create a visualization that either shows as much information as possible or improves readability by losing minimal information. Such a resulting set system can be visualized with our novel visualization technique called MosaicSets. The novel technique builds on an underlying grid and creates a mosaic-like visualization of the set system. We optimize each set to form a compact region to improve the readability of the diagram. For all our approaches, we provide theoretical analysis and evaluate the performance of our algorithms on numerous real-world datasets.Datenvisualisierung ist ein wichtiger Aspekt, um komplexe Daten einem breiten Publikum zugänglich zu machen. Dabei ist die Erfassung von Schlüsselinformationen entscheidend, um die Daten zu verstehen und fundierte Entscheidungen treffen zu können. Einerseits können die Daten raumbezogene Informationen enthalten, die oft als Karten dargestellt werden. Bei der Handhabung dieser Karten ist die Änderung der Zoomstufe eine gängige Vorgehensweise zur Steuerung des Detailgrads. Diese Änderungen sollten monoton sein. Die Verringerung des Detaillierungsgrads ist eine zentrale Herausforderung in der Kartografie und kann durch Generalisierung der Karten gelöst werden. Andererseits ist die Visualisierung abstrakter Daten, wie z.B. von Mengensystemen, ein etabliertes Forschungsgebiet in der Informationsvisualisierung. Mengensysteme enthalten Informationen über Mengenelemente und deren Beziehungen, z.B. Arbeitsgruppen und deren gemeinsame Forschungsprojekte. Die Visualisierung von Mengen hat sich als ein leistungsfähiges Werkzeug herausgestellt, um dem Nutzer große Datenmengen in kompakter Form darzustellen. Visualisierte Beziehungen zwischen Mengen lassen sich leichter verstehen als textuelle Beschreibungen. Allerdings vereint raumbezogene und abstrakte Daten, dass es für den Nutzer überfordernd sein kann, wenn jedes Detail dargestellt wird.
In dieser Arbeit werden Optimierungsansätze für die Erstellung von Generalisierungen für Geodaten und für Mengensysteme vorgestellt. In den Algorithmen werden Ansätze aus dem Forschungsbereich der Zuweisung räumlicher Flächeneinheiten adaptiert. Bei der Generalisierung von Geodaten werden kleine Polygone zu größeren Flächen zusammengefasst. Dieser Algorithmus wird verwendet, um eine Lösungsmenge zu berechnen, welche eine optimale Lösung für jeden Parameterwert, welcher den Grad der Generalisierung bestimmt, enthält. Für Mengensysteme wird der erste Ansatz vorgestellt, welcher bestimmt, ob ein gegebenes Mengensystem unter Berücksichtigung von Wohlgeformtheitsbedingungen vollständig als Euler-Diagramm dargestellt werden kann. Wenn eine vollständige Darstellung nicht möglich oder erforderlich ist, ist es das Ziel, eine Darstellung zu erzeugen, die entweder so viele Informationen wie möglich zeigt oder die Lesbarkeit durch das Entfernen minimaler Informationen verbessert. Ein solches Mengensystem kann mit der neuen eingeführten Visualisierungstechnik MosaicSets visualisiert werden. Diese baut auf einem zugrunde liegenden Raster auf und erzeugt eine mosaikartige Visualisierung des Mengensystems. Jede Menge wird so optimiert, dass sie eine kompakte Region bildet, um die Lesbarkeit des Diagramms zu verbessern. Für alle gezeigten Ansätze werden theoretische Analysen erstellt und die Leistung der Algorithmen anhand zahlreicher realer Datensätze bewertet
Bridging <em>in-vitro</em>, <em>in-silico</em> and corporate realms for pharmaceutical drug discovery
The onset of the COVID-19 pandemic in 2020 highlighted the urgent need for efficient navigation of big data. In response, numerous workflows and algorithms for data processing, aggregation, and analysis were developed and widely shared within the scientific community to address these emerging health challenges. However, these workflows have limitations, which can compromise their effectiveness and reliability. First, many workflows suffer from insufficient documentation and poor version control, making them difficult to reproduce, validate, and adapt for broader use. Second, they are often tailored to specific communities or domains, limiting their robustness and generalizability across diverse applications. Third, these workflows prioritize decisions on scientific data while overlooking critical aspects such as market applicability. Fourth, integration challenges stem from an overreliance on single-modality data analysis, neglecting the incorporation of heterogeneous data types. This limitation undermines the ability to make comprehensive go/no-go decisions based on a more holistic understanding of the problem. Finally, most existing workflows remain predominantly in silico, with minimal or no in vitro validation. This lack of experimental translation raises concerns about their applicability in real-world scenarios. In our work, we developed reproducible and well-documented pipelines to integrate and consolidate knowledge across various domains to enhance pandemic preparedness. These pipelines contextualize knowledge through graphs constructed from data extracted via manual curation of scientific publications and experimental results. Designed with flexibility in mind, the pipelines are agnostic, allowing their application across multiple domains, including diverse therapeutic indication areas. Next, to expand the scope of the underlying data beyond research, we mined patent literature, a valuable resource capturing the marketing and commercial landscape of drug discovery. Using a tool we developed called PEMT, we identified patterns in compound and target-agnostic strategies employed in the commercial sector. Moreover, to address integration challenges between knowledge graphs and omics-based technologies, we merged knowledge graphs with transcriptomics data. This enabled us to decipher the mechanisms of action of key drug and drug-like candidates. Furthermore, we demonstrated the successful translation of in silico work into biological experiments. Specifically, we built a machine learning model to predict the antibacterial activity of compounds and validated it by testing an external library for antibacterial activity in both in silico and in vitro. Our work highlights the importance of combining diverse data modalities with biological networks to gain profound insights into the mechanisms driving drug discovery. These workflows and approaches lay a strong foundation for identifying and prioritizing optimal drug candidates, facilitating their transition from preclinical studies to clinical trials and, ultimately, to the market
Passive Blood-Flow-Restriction Exercise's Impact on Muscle Atrophy Post-Total Knee Replacement : A Randomized Trial
Background/Objectives: Total knee arthroplasty (TKA) is commonly associated with postoperative muscle atrophy and weakness, while traditional rehabilitation is often limited by pain and patient compliance. Passive blood flow restriction (pBFR) training may offer a safe, low-threshold method to attenuate muscle loss in this early phase. This pilot study examined the feasibility, safety, and early effects of pBFR initiated during hospitalization on muscle mass, swelling, and functional recovery after TKA. Methods: In a prospective, single-blinded trial, 26 patients undergoing primary or aseptic revision TKA were randomized to either a control group (CON: sham BFR at 20 mmHg) or intervention group (INT: pBFR at 80% limb occlusion pressure). Both groups received 50 min daily in-hospital rehabilitation sessions for five consecutive days. Outcomes, including lean muscle mass (DXA), thigh/knee circumference, 6 min walk test (6 MWT), handgrip strength, and patientreported outcomes, were assessed preoperatively and at discharge, six weeks, and three months postoperatively. Linear mixed models with Bonferroni correction were applied. Results: The INT group showed significant preservation of thigh circumference (p = 0.002), reduced knee swelling (p p p p Conclusions: Initiating pBFR training on the first postoperative day is feasible, safe, and effective in preserving muscle mass and reducing swelling after TKA.
