    Evaluation of the Role of High Resolution Computed Tomography and Magnetic Resonance Cisternography in Preoperative Identification of Skull Base Defect in Cases of Cerebrospinal Fluid Rhinorrhea

    Introduction: The New Zealand White (NZW) rabbit model is commonly used for the research of posterolateral intertransverse lumbar arthrodesis. The standard approach reported by many researchers are a muscle splitting approach through the intermuscular plane between multifidus and longissimus muscles. Methods: In this research the fusion bed was exposed by retracting the three groups of paraspinal muscles (multifidus, longissimus and iliocostalis) medially. Results: This alternative method showed good exposure of the transverse processes and intertransverse membrane with less bleeding from the muscles observed. No death due to surgical complication was observed out of twenty-four rabbits operated. Conclusion: An easy and safe method for performing posterolateral intertransverse lumbar arthrodesis was demonstrated and recommended as an alternative approach for surgery on New Zealand White rabbits

    Primary Ewing’s Sarcoma of the Lumbosacral Spine: A Case Report

    Ewing's sarcoma is a rare tumor first discovered by James Ewing in 1921. It is more common in bone or skeletal component compared to soft tissue or extraosseous Ewing's sarcoma. Among soft tissue Ewing’s, spinal cord involvement is rarer with only nine cases reported. We report a case of nine-year-old Malay girl who presented with low back pain for two months following a fall with progressive neurological deficits of bilateral lower limb. Magnetic resonance imaging was suggestive of a well-defined margin of intradural extramedullary tumor. With nerve sheath tumor in mind, surgical excision with laminectomy L2-S1 was performed. Intraoperative finding was an extradural mass from L3-L5 with extension to bilateral neuroforamen. Histopathology report defined a round cell tumour of Ewing’s sarcoma from the mass

    Religious Personality and Smoking among Form Two Students in Rural Schools in Malacca, Malaysia

    Introduction: There is an increasing trend in smoking among rural adolescents compared to urban adolescents in Malaysia. This study was to determine the relationship between religious personality and smoking among form two students from the rural schools in Malacca. Methods: This was an unmatched case-control study conducted from May 2010 to August 2010 among form two school children from rural area in Malacca. There were 484 cases, and 444 controls selected using randomized cluster sampling. The dependent variable was smoking, and the independent factors were religious personality. Validated and reliability-tested questionnaires were used for data collection. Results: The mean age (year) at which smoking was first tried among the cases was 11.39 + 2.107. There were eleven out of twenty three significant differences between religious personality statements in the cases and control group. Those were: reciting the Quran/Holy book/Scriptures even when busy, making efforts to deepen the understanding of the religion, trying to understand the teachings of the religion in the Holy book/Quran, inviting others to perform solat/prayer/religious service, making sure all the family members are following the teachings according to the religion, referring to the learned people when feeling uncertain about the rulings/teachings of the religion, performing work duties enthusiastically because of the religion/God, making efforts to remember death and afterlife often, making efforts to internalize the ethical conduct of the religion in daily life, setting aside money every year for religious purposes and looking for opportunities to give charity. Conclusion: Religious personality is an important factor which may influence adolescents volved in smoking

    Scrotal Scintigraphy in Testicular Torsion: An Experience at a Tertiary Care Centre

    Introduction: Accurate diagnosis of testicular torsion is important to avoid serious consequences of any delay in proper management. Objective of this study was to compare performance parameters of scrotal scintigraphy and scrotal ultrasound to correctly and confidently diagnose the etiology of an acute scrotum in university hospital settings. Materials and Methods: This is an observational study with a retrospective analysis of data from July 2004 to March 2009. A total of 21 patients (age three years to 37 years) were referred from Emergency Department for scrotal scintigraphy to rule out testicular torsion. All patients went through a standard protocol for scrotal scintigraphy upon referral to Nuclear Medicine Department. Scrotal ultrasound was performed either before or immediately after the scintigraphy. Results: Confirmation of the diagnosis in all 21 patients was based either on the histopathological report of the surgical specimen (n=7) or through clinical follow up (for conservative treatment; n=14). At presentation, nine patients (43%) had left sided pain, and 12 patients (57%) had right sided pain (p>0.05). Eight patients (38%) were diagnosed to have testicular torsion on scintigraphy; one of them turned out to be false positive (testicular abscess). Sensitivity of scrotal scintigraphy to diagnose testicular torsion was 100%, specificity 93%, negative predictive value (NPV) 100% and positive predictive value (PPV) 88%. Conclusion: Scrotal scintigraphy in an acute scrotum is a feasible procedure to be performed on emergency basis, and it is a valid and reliable test to diagnose testicular torsion when US is doubtful or non conclusive

    A Rare Case of Traumatic Bilateral Sixth Cranial Nerve Palsy

    Traumatic bilateral sixth cranial nerve palsy is a rare condition which is typically associated with additional intracranial, skull, and cervical spine injuries. We describe a case of complete bilateral sixth nerve palsy in a 28-year-old female patient after an alleged motor vehicle accident. She had altered level of consciousness but no intracranial lesion or associated skull or cervical spine fracture was detected. In this case, we discussed the differential diagnoses, initial workup, and possible treatment options in cases of traumatic 6th nerve palsy
