Síndrome Miller Fisher: un reporte de caso y diagnósticos diferenciales
Polyneuropathies of immunological origin such as Guillain-Barré syndrome (GBS) and its variant Miller-Fisher syndrome (SMF) have an incidence of 0.6 to 2.4 cases per 100,000 inhabitants per year, more frequent in men and with a tendency increased in Colombia by infection with the Zika virus. Here is a clinical case of a 66-year-old patient who is taken by his relative to the emergency department for presenting a clinical course consisting of dysarthria, diadochokinesias,weakness and paresthesia in lower limbs, does not tolerate standing, and associates antecedent of infl uenza.It is decided to hospitalize, with probable diagnoses of a Cerebellar Syndrome, Hemiplegia alternating or Cerebral Cerebrovascular Event in progress. It enters the UCI when it worsens neurologically and enters respiratory failure; associates hypertension, taquibradi syndrome, hyperglycemia, alteration of oxygenation and ventilation; diagnostic tests are available to rule out neuroinfection or neurodegenerative disease, associated pathologies were monitored in parallel to the recovery of their initial neurological state, and on day 19, they were again transferred to hospital inpatient rooms with tracheostomy and percutaneous gastrostomy. Being on fl oors, Magnetic Nuclear Resonance (MRI) is used whose results and the neurological evolution of the patient lead to electromyography of upper limbs with normal fi ndings, so it is decided to carry out the diagnostic management for diseases of immunological origin. An immunological profi le is requested, obtaining positive results for ANAS and Parietal anti-cell antibodies. Given these fi ndings and given the patient’s favorable response to corticosteroid management, an acute infl ammatory neuropathic disease of immunologic origin of the SMF type or Myasthenia Gravis type. Management with intravenous immunoglobulin is initiated and the patient responds with adequate evolution. With a negative report for anti-acetylcholine receptor antibodies, myasthenia gravis is ruled out and, in view of the stable condition and favorable neurological outcome of the patient, the patient is given a home hospitalization with a diagnosis of acute infl ammatory neuropathy of the SMF type. Conclusion: the diagnosis of polyneuropathy is confusing and requires a doctor to know the differential diagnoses associated with clinical symptoms of acute polyneuropathy, to make the correct diagnosis and therapeutic treatment of the patient and to favor their evolution with favorable outcomes.Las polineuropatías de origen inmunológico como el Síndrome de Guillain-Barré(SGB) y su variante, el Síndrome Miller-Fisher (SMF), tienen una incidencia de 0.6 a 2.4 casos por cada 100.000 habitantes al año, mayor frecuencia en hombres y con tendencia incrementarse en Colombia por infección con el virus del Zika. A continuación, se presenta un caso clínico de un paciente de 66 años que es llevado por su familiar al servicio de urgencias por presentar un cuadro clínico consistente en disartria, adiadococinesia, debilidad y parestesia en miembros inferiores, no tolera la bipedestación, y asocia antecedente de cuadro gripal. Se decide hospitalizar, con diagnósticos probables de un Síndrome cerebeloso, Hemiplejia alterna o Evento Cerebro vascular cerebeloso isquémico en curso. Ingresa a UCI al empeorar neurológicamente y entrar en falla respiratoria; asocia hipertensión arterial, síndrome taquibradi, hiperglicemia, alteración de oxigenación y ventilación; se reciben pruebas diagnósticas que permiten descartar neroinfección o enfermedad neurodegenerativa, se controlaron patologías asociadas en paralelo a la recuperación de su estado neurológico inicial, y en el día 19 se traslada nuevamente a hospitalización en pisos de medicina interna con traqueostomía y gastrostomía percutánea. Estando en pisos, se toma Resonancia Nuclear Magnética (RNM) cuyos resultados y la evolución neurológica del paciente llevan a solicitar electromiografía de miembros superiores con hallazgos normales por lo que se decide realizar el manejo diagnóstico para enfermedades de origen inmunológico. Se solicita perfi l inmunológico obteniéndose resultados positivos para ANAS y para Anticuerpos anticélulas parietales. Ante estos hallazgos y dada la respuesta favorable del paciente al manejo con corticoide, se considera, como diagnóstico posible, una enfermedad neuropática infl amatoria aguda de origen inmunológico del tipo SMF o Miastenia Gravis. Se inicia manejo con inmunoglobulina intravenosa a lo cual el paciente responde con evolución adecuada. Con reporte negativo para anticuerpos anti receptor de acetilcolina, se descarta Miastenia gravis y ante la condición estable y evolución neurológica favorable del paciente se da salida para hospitalización domiciliaria con diagnóstico de neuropatía infl amatoria aguda del tipo SMF. Conclusión: el diagnóstico de las polineuropatía resulta confuso y exige médico tratante el conocer los diagnósticos diferenciales asociados a sintomatología clínica de polineuropatía aguda, para hacer el abordaje diagnóstico y terapéutico correcto del paciente y favorecer su evolución con desenlaces favorables

This article aims to show the reader how the law 1616/2013 has been conceived within the framework of the experiences of three Latin American countries (Argentina, Brazil and, Chile). In 1990 PAHO/WHO made a critical review and noted that the process of internalization, institutionalization and, psychiatric custody was not decentralized, nor was it participatory, integrated, continuous and, preventive. Hence, mental health has come up with a new conception of the services under the community approach. This implies the development and implementation of programs for a certain group of people in order to promote, protect and treat mental health and mental health problems.El presente artículo tiene como objetivo mostrar al lector cómo ha sido concebida la Ley 1616 de 2013 en el marco de las experiencias de tres países latinoamericanos (Argentina, Brasil y Chile). En 1990 la OPS/OMS hace una revisión crítica y nota que el proceso de internalización, institucionalización y custodia en psiquiátricos no es descentralizado, participativo, integral, continuo y preventivo. De allí que la salud mental bajo un enfoque comunitario estructure una nueva concepción de los servicios, lo que implica el desarrollo y ejecución de programas para un grupo determinado de personas con el fin de promover, proteger y tratar la salud mental y sus problemas
Efectividad de terapéuticas alternativas para el manejo del dolor lumbar
BACKGROUND: Low back pain due to discopathy has become one of the most expensive and highest impact problems of global public health at present.AIM: To evaluate the effectiveness of alternative therapies (AT) and related factors in lumbar disk disease (LDD).METHODS: Information was collected by using a retrospective descriptive design from medical records of patients treated from January to December 2013 with LDD who were attended in a clinic specialized in AT. Sociodemographic and clinical variables included clinical diagnostic time, Visual Analog Scale at admission (V.A.S.) and post-treatment pain (0-100 points) were identifi ed. General and specifi c infl uence of the AT was measured.RESULTS: 453 medical records were identifi ed, at admission V.A.S. was 8.2 (CI95%, 7,9 to 8,3) and showed pain relief (scale 0-100) of 54,5 points (CI95%, 52.2 to 56.8). In overall, 13 types of ATs were implemented with LDD patients. The median of pain relief (p25-p75) in the general population was 60 (40-70); 8 of 13 interventions provided an estimated median of pain relief lower than the one evidenced in the general population .CONCLUSION: The AT treatment in our study showed a reduction in pain of 50-60 points and an absolute reduction of 35 points after two months of treatment, which remained over 6 or more months of follow-up. Our results suggest that the use of AT has signifi cant effect on chronic pain in patients with LDD with no response to conventional treatmentINTRODUCCIÓN. El dolor lumbar debido a la discopatía se ha convertido en uno de los problemas más caros y de mayor impacto de la salud pública mundial en la actualidad.OBJETIVO: Evaluar la efi cacia de las terapias alternativas (TA) y los factores relacionados en la enfermedad del disco lumbar (EDL).METODOLOGÍA: La información fue recolectada utilizando un diseño descriptivo retrospectivo de los registros médicosde los pacientes tratados de enero a diciembre de 2013 con EDL que asistieron a una clínica especializada en TA. Las variables sociodemográfi cas y clínicas incluyeron tiempo de diagnóstico clínico, escala visual analóga al ingreso(V.A.S.) y dolor post-tratamiento (0-100 puntos). Se midió la infl uencia general y específi ca de las TA.RESULTADOS: Se identifi caron 453 expedientes médicos, al ingreso el V.A.S. fue de 8,2 (IC95%, 7,9 a 8,3) y mostró alivio del dolor (escala 0-100) de 54,5 puntos (IC95%, 52,2 a 56,8). En general, 13 tipos de TA se implementaron conpacientes EDL. La mediana del alivio del dolor en la población general fue de 60 puntos (40-70) y 8 de 13 intervenciones proporcionaron una mediana estimada de alivio del dolor menor que la evidenciada en la población general.CONCLUSIÓN: En nuestro estudio el tratamiento con TA mostró una reducción en el dolor de 50-60 puntos y una reducción absoluta de 35 puntos después de dos meses de tratamiento, que se mantuvo durante 6 o más meses de seguimiento. Nuestros resultados sugieren que el uso de TA tienen un efecto signifi cativo sobre el dolor crónico en pacientes con EDL sin respuesta al tratamiento convencional
Modelos predictivos de riesgo cardiovascular
Predictive models estimate the likelihood of developing a cardiovascular event (CVD), are developed based on specific population factors and have been internally validated by measuring their discrimination and calibration, in order to support primary and secondary intervention, nonetheless, if it is necessary to make them in a different population, an external validation must be performed.Several controversies surround these models, on the use of diabetes and the use in the Colombian population. Postures have been considered about considering diabetes as a cardiovascular risk equivalent, assessing these patients as well as providing a separate approach; and there are even studies that defend different positions, is an issue that is still controversial and among the international guides.On the other hand, studies have been carried out in the Colombian population using the same historical cohort, one comparing Framingham and PROCAM and another SCORE, ACC / AHA and adjusted Framingham; The fi rst study determined that there was better calibration and discrimination while Framingham overestimated the risk, and the latter concluded that ACC / AHA is better than SCORE, but estimates the risk and in fact not one correctly estimates it. In spite of this, the guides favor the use of the adjusted Framingham.This refl ects the need to delve into research on cardiovascular risk, especially in our environment, to provide a successful approach to the Colombian population, reducing CVD, improving the quality of life of the patient and reducing the costs of the health system.Los modelos predictores estiman la probabilidad de desarrollar un evento cardiovascular (ECV) a futuro, son desarrollados con base en factores de poblaciones específi cas y han sido validados internamente midiendo su discriminación y calibración, con el objeto de apoyar una intervención primaria y secundaria, no obstante, si se desea aplicarlos en una población distinta, se debe realizar una validación externa.Diversas controversias rondan entorno a estos modelos, especialmente acerca de cuál aplicar en pacientes con diabetes y cuál usar en población colombiana. Se han planteado posturas sobre considerar la diabetes como equivalente de riesgo cardiovascular, evaluar estos pacientes igual que personas que carecen de ésta o brindar un enfoque separado; y aunque hay estudios que defi enden diferentes posturas, es un tema que aún es controvertido y entre las guías internacionales aún no hay consenso sobre un modelo específi co.Por otra parte, se han llevado a cabo dos estudios en población colombiana usando la misma cohorte histórica, uno donde compara Framingham y PROCAM y otro SCORE, ACC/AHA y Framingham ajustado; el primer estudio determinó que PROCAM tenía mejor calibracióny discriminación mientras que Framingham sobreestima el riesgo, y el segundo concluyó que ACC/AHA es mejor que SCORE aunque subestima el riesgo y en realidad ninguno lo estima correctamente. Pese a ello, las guías recomiendan el uso del Framingham ajustado.Lo anterior refl eja la necesidad de ahondar en la investigación sobre la estimación del riesgo cardiovascular especialmente en nuestro medio para brindar un enfoque acertado a la población colombiana,disminuyendo los ECV, mejorando la calidad de vida del paciente y dando una reducción en los costos al sistema de salud
