    Psychiatric-psychotherapeutic treatment of mothers with preschool children: Connections between functional and relational aspects

    Die hier durchgeführte Studie hatte das Ziel, bei psychisch erkrankten Patientin- nen mit einem Kind von 2 bis 6 Jahren Zusammenhänge zwischen Beziehungspa- rametern und Funktionsaspekten der Patientin und des Kindes herzustellen und die Wirksamkeit der Mutter-Kind-Therapie anhand der Betrachtung der Funkti- onsniveaus zu untersuchen. Dazu wurden zwischen 2015 und 2020 am Psychiatri- schen Zentrum Nordbaden in Wiesloch verschiedene Fragebögen von 91 Mutter- Kind-Dyaden untersucht und eine telefonische Nachbefragung bei 20 Patientinnen durchgeführt. Ein weiterer Fokus wurde auf die Verbindung der Faktoren Resilienz, Selbstmanagement und Zufriedenheit mit den Lebensbedingungen ge- legt. In den Hauptuntersuchungen konnte mithilfe von linearer Regression dargestellt werden, dass ein hohes Funktionsniveau der Mutter-Kind-Beziehung bei Auf- nahme und eine geringe kindliche Symptomatik bei Aufnahme Prädiktoren für einen hohen Behandlungserfolg sind. Ein starker Behandlungserfolg, ein hohes psychisches, soziales und berufliches Funktionsniveau der Patientin bei Auf- nahme und eine starke Verbesserung dieses Funktionsniveaus prädizieren ein ho- hes Funktionsniveau der Patientin bei Entlassung. Mithilfe von Korrelationen konnte ein Zusammenhang zwischen einem hohen Funktionsniveau der Mutter- Kind-Beziehung und niedrigem kindlichen Problemverhalten, funktionalen Erzie- hungsparametern und starker mütterlicher Fürsorge hergestellt werden. Die Nebenuntersuchungen konnten die Wirksamkeit der Mutter-Kind-Therapie unterstützen. Anhand von Regressionsanalysen konnte dargestellt werden, dass eine hohe Resilienz ein Prädiktor sowohl für ein gutes Selbstmanagement der Pa- tientin als auch für eine hohe Zufriedenheit mit den Lebensbedingungen ist. Es konnten darüber hinaus Korrelationen von Resilienz mit sowohl Selbstmanage- ment als auch Zufriedenheit mit den Lebensbedingungen festgestellt werden. Weiterhin konnte eine Steigerung des psychischen, sozialen und beruflichen Funktionsniveaus der Patientinnen im Verlauf der Therapie dargestellt werden. Anhand der Auswertung von Zufriedenheitsskalen in der Katamnese konnten erste Hinweise für eine langfristige Wirksamkeit der Therapie festgehalten werden. Ein hohes Funktionsniveau der Patientinnen bei Entlassung zeigte sich zu- sätzlich als Prädiktor für einen hohen Zufriedenheitsscore bei Katamnese. Die festgehaltenen Ergebnisse der Haupt- und Nebenuntersuchungen lassen sich gut in die Forschungsergebnisse vergleichbarer Studien einordnen. Dennoch han- delt es sich bei diesen Studien meist um die Patientengruppe von Müttern mit Kindern im Säuglingsalter, wohingegen die hier untersuchte Patientengruppe von Müttern mit Kindern im Alter von 2 bis 6 Jahren weitgehend unerforscht ist. So- mit konnten für diese Patientengruppe Zusammenhänge zwischen Beziehungspa- rametern und Funktionsaspekten hergestellt werden und die Wirksamkeit der Mutter-Kind-Therapie nachgewiesen werden, was eine gute Grundlage für weitere Forschung an Müttern mit Kindern im Vorschulalter bildet. Im Allgemeinen wird deutlich, dass sowohl Bedarf an der Mutter-Kind-Therapie für Patientinnen mit Kindern von 2 bis 6 Jahren als auch an Forschung zu dieser Patientengruppe besteht.The study conducted here aimed to establish correlations between relationship parameters and functional aspects of the patient and the child in mentally ill female patients with a child aged 2 to 6 years and to investigate the effectiveness of mother-child therapy by looking at functional levels. For this purpose, various questionnaires of 91 mother-child dyades were examined between 2015 and 2020 at the Psychiatric Center North Baden in Wiesloch and a telephone follow-up sur- vey was conducted with 20 patients. A further focus was placed on the connection of the factors resilience, self-management and satisfaction with living conditions. In the main studies, linear regression was used to show that a high level of functioning in the mother-child relationship at admission and low infant symptomatology at admission were predictors of high treatment success. A strong treatment outcome, a high psychological, social, and occupational functioning level of the patient at admission, and a strong improvement in this functioning level predicted a high functioning level of the patient at discharge. With the help of correlations, a relationship was established between a high level of functioning in the mother-child relationship and low child problem behavior, functional parenting parameters, and strong maternal care. The ancillary studies were able to support the effectiveness of mother-child therapy. Using regression analyses, it was possible to show that high resilience is a predictor of both good patient self-management and high satisfaction with living conditions. Furthermore, correlations of resilience with both self-management and satisfaction with living conditions could be found. Also, an increase in the patients' psychological, social and occupational level of functioning could be shown in the course of therapy. Based on the evaluation of satisfaction scales in the catamnesis, first indications for a long-term effectiveness of the therapy could be recorded. A high functional level of the patients at discharge was also shown to be a predictor for a high satisfaction score at catamnesis. The recorded results of the main and secondary investigations can be well classified in the research results of comparable studies. Nevertheless, these studies mostly concern the patient group of mothers with children in infancy, whereas the 90 patient group of mothers with children aged 2 to 6 years investigated here is largely unexplored. Thus, for this patient group, correlations between relationship parameters and aspects of functioning could be established and the efficacy of mother-child therapy could be demonstrated, providing a good basis for further research on mothers with preschool-aged children. In general, it is clear that there is both a need for mother-child therapy for patients with children 2 to 6 years of age and a need for research on this patient group

    The role of the Notch signalling pathway in regulating the balance between neuronal and nonneuronal cells in sympathetic ganglia and the adrenal gland

    Sympathetic neurons and endocrine chromaffin cells of the adrenal medulla are catecholaminergic cells that derive from the neural crest. According to the classic model, they develop from a common sympathoadrenal (SA) progenitor that has the ability to differentiate into both sympathetic neurons and chromaffin cells depending on signals provided by their final environment. Our previous data revealed that a single premigratory neural crest cell can give rise to both sympathetic neurons and chromaffin cells, indicating that the fate decision between these cell types occurs after delamination. A more recent study demonstrated that at least half of chromaffin cells arise from a later contribution by Schwann cell precursors. Since Notch signalling is known to be implicated in the regulation of cell fate decisions, we investigated the early role of Notch signalling in regulating the development of neuronal and non-neuronal SA cells within sympathetic ganglia and the adrenal gland. To this end, we implemented both gain and loss of function approaches. Electroporation of premigratory neural crest cells with plasmids encoding Notch inhibitors revealed an elevation in the number of SA cells expressing the catecholaminergic enzyme tyrosine-hydroxylase, with a concomitant reduction in the number of cells expressing the glial marker P0 in both sympathetic ganglia and adrenal gland. As expected, gain of Notch function had the opposite effect. Numbers of neuronal and non-neuronal SA cells were affected differently by Notch inhibition depending on the time of its onset. Together our data show that Notch signalling can regulate the ratio of glial cells, neuronal SA cells and nonneuronal SA cells in both sympathetic ganglia and the adrenal gland.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg

    Rights on Data: The EU Communication ‘Building a European Data Economy’ From an Economic Perspective

    In its Communication "Building a European data economy" the EU Commission discusses the introduction of a new exclusive property right on data ("data producer right") for non-personal (or anonymised) machine-generated data, and mandatory access rights to privately held data for achieving more access, transfer and reuse of data, esp. in the context of "Internet of Things" applications. This article analyzes the problem of "rights on data" from an economic perspective (incentive problem, data markets, bargaining power problems, access problems in multi-stakeholder situations) and the reasonings and proposals in the Communication from an economic perspective. Important results are that a "data producer right" cannot be recom-mended but that access rights to data can be part of specifically tailored data governance solu-tions in certain sectors

    Data Governance in Connected Cars: The Problem of Access to In-vehicle Data

    Through the application of the technological solution of the “extended vehicle” con-cept the car manufacturers can capture exclusive control of the data of connected cars lead-ing to serious concerns about negative effects on competition, innovation and consumer choice on the markets for aftermarket and other complementary services in the ecosystem of connected and automated driving. Therefore a controversial policy discussion has emerged in the EU about access to in-vehicle data and the connected car for independent service pro-viders in the automotive industry. This paper claims that this problem should be seen as part of the general question of the optimal governance of data in the ecosystem of connected and automated mobility. The paper offers an overview about this policy discussion and analyzes this problem from an economic perspective by using a market failure analysis. Besides com-petition problems (esp. on markets for aftermarket and other services in the connected car) also market failures in regard to technological choice (extended vehicle vs. interoperable on-board application platform) and information and privacy problems (“notice and consent” solu-tions) can emerge, leading to the question of appropriate regulatory solutions. The paper discusses solutions through data portability, data rights, competition law, and recommends a sector-specific regulatory approach

    Cohort size and youth labour-market outcomes: the role of measurement

    Using data from 49 European regions covering 2005-2012, this paper finds that the estimated effect of cohort size on employment and unemployment outcomes is very sensitive to the age range of the sample. We argue that this is because the identification strategy commonly used in this literature is unable to eliminate the bias caused by measurement error in the cohort-size variable. The latter arises because large shares of the young choose to acquire education and consequently the size of an age group provides a poor measure of age-specific labour supply. In our view older age groups provide a more suitable sample to test the implications of cohort crowding since the former will have largely entered the labour market. Using a sample aged 25-29, which has relatively low rates of participation in education, we find robust evidence that an increase in cohort size increases employment and reduces unemployment

    Wirkung der Tiefen Hirnstimulation des Nucleus subthalamicus auf nicht-motorische Symptome, mit Fokus auf schlafbezogene Symptome, beim idiopathischen Parkinson-Syndrom

    Ziel unserer Studie war es, die Wirkung der Tiefen Hirnstimulation sechs Monate post Implantation der STN-THS vorrangig auf die nicht-motorischen Symptome des idiopathischen Parkinson-Syndroms an 17 Probanden darzustellen. Es ergab sich eine signifikante Reduktion der NMS um annähernd 47 %. Dabei zeigte sich die größte Wirkung der THS auf Schlaf und Müdigkeit sowie auf Aufmerksamkeit und Gedächtnis. Der Faktor Psyche wurde in Hinblick auf die Angst signifikant gebessert um nahezu 37 %. Die Depressionsskala besserte sich nicht signifikant. Die motorischen Symptome zeigten sich um über 1/3 gebessert und der Medikamentenbedarf konnte erheblich gesenkt werden. Mit genannter Entwicklung stieg auch die Lebensqualität. Die THS hat demzufolge eine umfassende Wirkung sowohl auf motorische als auch auf nicht-motorische Symptome des idiopathischen Parkinson-Syndroms und auf die Lebensqualität. Unser Hauptaugenmerk galt der Wirkung der Tiefen Hirnstimulation auf den Schlaf. Nachweislich besserten sich die schlafbezogenen Symptome unter THS um annähernd 19 %. Korrelationsanalysen zeigten keinen Zusammenhang zwischen der Entwicklung des Schlafes mit der Reduktion der Medikation oder der Verbesserung der Motorik. Hingegen wirkte die THS gleichermaßen positiv auf die Entwicklung des Schlafes wie auf die Angst und depressive Symptome. Weiterhin korrelierte die Reduktion der Angst mit der Abnahme von Bewegungsdrang, der am Schlafen hindert, und mit der Reduktion der Nykturie. Die Besserung depressiver Symptome korrelierte mit der Abnahme von Durchschlafstörungen. Des Weiteren zeigte sich eine abhängige Entwicklung der Verbesserung des Schlafes und depressiver Symptome mit der Steigerung der Lebensqualität. Da aktuell keine Heilung für Patienten mit idiopathischem Parkinson-Syndrom in Aussicht steht, rückt die Verbesserung der Lebensqualität für die Betroffenen in den Vordergrund. Dabei ist die Tiefe Hirnstimulation der bestmöglichen medikamentösen Therapie überlegen. Die STN-THS wirkt durch elektrische Impulse modulierend auf die STN-Funktion, auf verbundene neuronale Netzwerke und angrenzende Strukturen. Durch gezielte Elektrodenplatzierung kann eine Verbesserung spezifischer NMS erreicht werden. Bei ventralerer Stimulation im STN und dadurch vermeintlicher Einflussnahme auf den angrenzenden pedunkulopontinen Kern kann laut Studien der Schlaf-Wach-Zyklus beeinflusst werden. Eine gleichförmige Wirkung der THS auf den Schlaf und psychische Symptome wäre bei ventraler Elektrodenplatzierung mit Stimulation im limbischen Anteil des STN denkbar. Unsere Ergebnisse sind vergleichbar mit zahlreichen Studien zur Wirkung der THS auf idiopathische Parkinson-Symptome und die Lebensqualität. Wir konnten die positive Wirkung der THS, insbesondere auf den Schlaf, bestätigen und neue Erkenntnisse zur Korrelation von Angst und Depression mit schlafbezogenen Symptomen gewinnen. Die NMS spielen derzeit noch eine untergeordnete Rolle beim Einsatz der Tiefen Hirnstimulation. Nach wie vor richtet sich die Platzierung der Elektroden der Tiefen Hirnstimulation nach dem Ansprechen der motorischen Symptome auf die Stimulation. Die nicht-motorischen Symptome tragen jedoch einen erheblichen Teil zur Schwere der Krankheitslast bei und können größere Einbußen an Lebensqualität als die motorischen Symptome verursachen. Daher sollte die Optimierung der Parkinsontherapie mit Anpassung der Operationstechniken vorangetrieben werden, um die THS präzise auf das individuelle Symptomprofil mit Einbezug der NMS einsetzen zu können.The aim of our study was to demonstrate the effect of deep brain stimulation 6 months after STN-DBS on non-motor symptoms of 17 patients with Parkinson's disease. There was a significant reduction in NMS of approximately 47 %. DBS had the greatest effect on sleep and fatigue, as well as attention and memory. Furthermore, anxiety was significantly improved by almost 37 %, whereas depressive symptoms did not improve significantly. The motor symptoms improved by more than 33 % and the need for medication could be significantly reduced. With this development, the quality of life also increased. Consequently, DBS has a comprehensive effect on both motor and non-motor symptoms of Parkinson’s disease and on quality of life. Our main focus was the effect of deep brain stimulation on sleep, which improved by approximately 19 % with DBS. Correlation analyses showed no connection between the development of sleep with the reduction in medication or the improvement in motor skills. On the other hand, DBS had an equally positive effect on the development of sleep as well as on anxiety and depressive symptoms. Furthermore, the reduction in anxiety correlated with a decrease in the urge to move that prevents sleep and with a reduction in nocturia. The improvement in depressive symptoms correlated with the decrease in sleep maintenance insomnia. Furthermore, a dependent development of the improvement in sleep and depressive symptoms with the increase in quality of life was shown. Since there is currently no prospect of a cure for patients with Parkinson’s disease, improving the quality of life for those affected is becoming a priority. Therefore, deep brain stimulation is superior to best medical therapy. The STN-DBS has a modulating effect on STN function, on connected neural networks, and adjacent structures through electrical impulses. Targeted electrode placement can improve specific NMS. According to studies, the sleep-wake cycle can be regulated with ventral stimulation in the STN, probably by influences on the adjacent pedunculopontine nucleus. A uniform effect of DBS on sleep and neuropsychiatric symptoms would be conceivable with a ventral electrode placement with stimulation in the limbic part of the STN. Our results are in line with numerous studies on the effect of DBS on Parkinson’s disease symptoms and quality of life. We could confirm the positive effect of DBS, especially on sleep and gain new insights into the correlation between anxiety, depression and sleep-related symptoms. The NMS currently play a subordinate role in the decision making for indication for deep brain stimulation and electrode placement is still solely based on the response of motor symptoms to stimulation. However, the non-motor symptoms contribute a significant part to the severity of the disease burden and can cause a greater reduction in quality of life than motor symptoms. Therefore, individualized Parkinson's disease therapy should be promoted in order to use the DBS precisely on the individual patients’ symptom profile including the non-motor symptoms

    We conducted an experimental study on social preferences using dictator games similar to Fehr et al. (2008). Our results show that social preferences differ between participants who receive low-stakes monetary rewards for their decisions and participants who consider hypothetical stakes. The results were robust when we controlled for socio-demographic characteristics and participants’ risk attitudes. Our findings indicate that, apart from incentives, gender plays an important role for the categorization of different social preferences

    TLR7 and IgM: Dangerous Partners in Autoimmunity

    The B cell antigen receptor (BCR)-repertoire is capable of recognizing a nearly unlimited number of antigens. Inevitably, the random nature of antibody gene segment rearrangement, needed in order to provide mature B cells, will generate autoreactive specificities. Once tolerance mechanisms fail to block the activation and differentiation of autoreactive B cells, harmful autoantibodies may get secreted establishing autoimmune diseases. Besides the hallmark of autoimmunity, namely IgG autoantibodies, IgM autoantibodies are also found in many autoimmune diseases. In addition to pathogenic functions of secreted IgM the IgM-BCR expressing B cell might be the initial check-point where, in conjunction with innate receptor signals, B cell mediated autoimmunity starts it fateful course. Recently, pentameric IgM autoantibodies have been shown to contribute significantly to the pathogenesis of various autoimmune diseases, such as rheumatoid arthritis (RA), autoimmune hemolytic anemia (AIHA), pemphigus or autoimmune neuropathy. Further, recent studies suggest differences in the recognition of autoantigen by IgG and IgM autoantibodies, or propose a central role of anti-ACE2-IgM autoantibodies in severe COVID-19. However, exact mechanisms still remain to be uncovered in detail.     Nach dem Versuch unzähliger Kryptowährungen das Finanzwesen zu revolutionieren und den ersten Schritten nationaler Notenbanken in Richtung einer Digitalisierung des Geldwesens, folgen nun privatwirtschaftliche Großprojekte. Das maßgeblich von Facebook vorangetriebene Libra will den Zugang zu Finanzdienstleistungen vereinfachen. Der vorliegende Beitrag untersucht die währungspolitische Dimension des Corporate Cryptocurrency Projekts. Mit den klassischen Kryptowährungen hat Libra nur die kryptographische Grundlage gemein. Durch vollständige Besicherung der Libra-Tokens durch eine Reserve aus niedrig-volatilen Wertpapieren soll ein " Stable Coin\ entstehen, dessen Wechselkursvolatilität deutlich unter dem typischer Kryptowährungen liegen dürfte. Obwohl Libra keine eigene Geldpolitik verfolgt, wird es mit den bestehenden Zentralbanken interagieren, indem es einen von diesen nicht kontrollierten Transmissionskanal schafft. Damit erodiert der Wirkungskreis klassischer Geldpolitik, was letztlich in Versuchen münden wird, den neuen Marktakteur zu regulieren

    Facilitation of sensorimotor temporal recalibration mechanisms by cerebellar tDCS in patients with schizophrenia spectrum disorders and healthy individuals

    Core symptoms in patients with schizophrenia spectrum disorders (SSD), like hallucinations or ego-disturbances, have been associated with a failure of internal forward models to predict the sensory outcomes of self-generated actions. Importantly, forward model predictions must also be able to flexibly recalibrate to changing environmental conditions, for example to account for additional delays between action and outcome. We investigated whether transcranial direct current stimulation (tDCS) can be used to improve these sensorimotor temporal recalibration mechanisms in patients and healthy individuals. While receiving tDCS on the cerebellum, temporo-parietal junction, supplementary motor area, or sham stimulation, patients with SSD and healthy control participants were repeatedly exposed to delays between actively or passively elicited button presses and auditory outcomes. Effects of this procedure on temporal perception were assessed with a delay detection task. Similar recalibration outcomes and faciliatory effects of cerebellar tDCS on recalibration were observed in SSD and healthy individuals. Our findings indicate that sensorimotor recalibration mechanisms may be preserved in SSD and highlight the importance of the cerebellum in both patients and healthy individuals for this process. They further suggest that cerebellar tDCS could be a promising tool for addressing deficits in action-outcome monitoring and related adaptive sensorimotor processes in SSD.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg
