Einfluss der Aortenpathologie auf das Postimplantationssyndrom nach TEVAR (Stentimplantation in der thorakalen Aorta)
Hintergrund: Die thorakale endovaskuläre Aortenreparatur (TEVAR) ist die Behandlungsoption der ersten Wahl für fast alle Pathologien der absteigenden thorakalen Aorta. Das Ziel der vorliegenden Studie ist es, den Einfluss der Aortenpathologie auf das Auftreten des Postimplantationssyndroms (PIS) nach einer TEVAR zu bestimmen.
Methoden: Vierundsiebzig Patienten, die sich einer TEVAR wegen Aortendissektion (TAD, 25), Aortenaneurysma (TAA, 26) und Aortenruptur oder perforiertem Ulkus (TAR/PAU, 23) unterzogen, wurden in diese retrospektive Studie eingeschlossen. Als klinischen Ergebnisparameter galten persistierende Entzündungen bei Entlassung und Mortalität im Krankenhaus.
Ergebnisse: Ein PIS wurde bei 22,97 % aller Patienten untersucht, überwiegend in der TAD-Gruppe (p = 0,03). Das CRP stieg nach einer TEVAR (156,6 ± 94,5, p < 0,001; 108,1 ± 57,7, p < 0,01 und 117,8 ± 70,4, p < 0,05) gegenüber dem Ausgangswert (58,1 ± 77,5, 31,94 ± 52,1 und 31,9 ± 52,1 mg/l) bei TAD, TAA bzw. TAR/PAU) an. Dieser Anstieg war in der TAD-Gruppe insgesamt stärker ausgeprägt (p < 0,05). Die Stentlänge war in allen Gruppen ähnlich (p = 0,226), korrelierte jedoch nur bei TAD mit dem postoperativen CRP-Wert (R = 0,576, p = 0,013). Die Menge an parietalem Thrombus korrelierte mit dem Anstieg des CRPs (R=0,4507, p=0,0005) und ist (OR=1,0883, p=0,0001) zusammen mit der Pathologie der Aortendissektion (OR=6,2268, p=0,0288) ein signifikanter Prädiktor für die Entwicklung eines PIS nach einer TEVAR. Die Mortalität (5,4 %) korrelierte weder mit einem PIS (p=0,38) noch sah man einen Zusammenhang zur Aortenpathologie (p=0,225). Der Krankenhausaufenthalt wurde jedoch nach einer TEVAR signifikant durch ein PIS verlängert (p=0,03).
Schlussfolgerung: Die Aortendissektion ist nach dem Eingriff einer TEVAR mit mehr Entzündungen assoziiert als es bei einem Aortenaneurysma, einer Ruptur oder einem perforierten Ulkus der Fall ist. Beim Auftreten eines PIS spielt die Menge an frischem parietalem Thrombus die wichtigste Rolle. Wir konnten signifikant nachweisen, dass ein PIS den Krankenhausaufenthalt verlängert, das Sterblichkeitsrisiko nach einer TEVAR jedoch nicht erhöht.Background: The Thoracic endovascular aortic repair (TEVAR) is the treatment option of choice for almost all pathologies of the descending thoracic aorta. The aim of the present study is to determine the impact of aortic pathology on the occurrence of post-implantation syndrome (PIS) after TEVAR.
Methods: In this study seventy-four patients were undergoing TEVAR for aortic dissection (TAD, 25), aortic aneurysm (TAA, 26), and aortic rupture or perforated ulcer (TAR/PAU, 23). The clinical outcome measurements were persistent inflammation (CRP) at hospital discharge, as well as in-hospital mortality.
Results: PIS was diagnosed in 22.97% of all patients, predominantly in the TAD group (p=0.03). CRP increased significantly after TEVAR (156.6± 94.5, p< 0.001; 108.1±57.7, p< 0.01 and 117.8±70.4, p< 0.05) vs. baseline (58.1±77.5, 31.94±52.1 and 31.9±52.1 mg/L, in TAD, TAA and TAR/PAU, respectively). This increase was more profound in the TAD group (p< 0.05). Stent-length was similar in all groups (p=0.226), but correlated with postoperative CRP only in TAD (R=0.576, p=0.013). Fresh parietal thrombus correlated with a rise in CRP (R=0.4507, p=0.0005) and is can be therefore (OR=1.0883, p=0.0001), together with the pathology of aortic dissection (OR=6.2268, p=0.0288), seen a predictor of PIS after TEVAR. Whereas mortality (5.4%) did not correlate with PIS (p=0.38) regardless of the aortic pathology (p=0.225), the hospital stay was significantly prolonged by PIS (p=0.03) after conducting TEVAR.
Conclusions: Aortic dissection is associated with more inflammation after TEVAR than it is the case with aortic aneurysm, rupture, or perforated ulcer. The amount of fresh parietal thrombus playing the most significant role in the occurrence of PIS. In conclusion, PIS prolongs hospital stay but not mortality after TEVAR
Comparative analysis of full-length 16s ribosomal RNA genome sequencing in human fecal samples using primer sets with different degrees of degeneracy

and greatly expanded our knowledge of complex bacterial communities.
Nanopore sequencing provides distinct advantages, combining costeffectiveness,
ease of use, high throughput, and high taxonomic resolution
through its ability to process long amplicons, such as the entire 16s rRNA
genome. We examine the performance of the conventional 27F primer (27F-I)
included in the 16S Barcoding Kit distributed by Oxford Nanopore Technologies
(ONT) and that of a more degenerate 27F primer (27F-II) in the context of highly
complex bacterial communities in 73 human fecal samples. The results show
striking differences in both taxonomic diversity and relative abundance of a
substantial number of taxa between the two primer sets. Primer 27F-I reveals a
significantly lower biodiversity and, for example, at the taxonomic level of the
phyla, a dominance of Firmicutes and Proteobacteria as determined by relative
abundances, as well as an unusually high ratio of Firmicutes/Bacteriodetes when
compared to the more degenerate primer set (27F-II). Considering the findings in
the context of the gut microbiomes common in Western industrial societies, as
reported in the American Gut Project, the more degenerate primer set (27F-II)
reflects the composition and diversity of the fecal microbiome significantly better
than the 27F-I primer. This study provides a fundamentally relevant comparative
analysis of the in situ performance of two primer sets designed for sequencing of
the entire 16s rRNA genome and suggests that the more degenerate primer set
(27F-II) should be preferred for nanopore sequencing-based analyses of the

Some Insights into the Development of Cryptocurrencies
Cryptocurrencies such as Bitcoin might revolutionize the economy through
enabling peer-to-peer based transactions by abolishing the need for a trusted
intermediary. As for now, Bitcoin remains to be the best recognized cryptocurrency, in particular in terms of market capitalization. However, as this
paper shows, there are plenty of alternatives. This paper outlines the historical roots which have led to the creation of privately emitted, cryptography based digital currencies. Additionally, this paper discusses future possible hurdles of the development of cryptocurrencies and outlines features which might influence the success of a cryptocurrency. Insights into the beginning of cryptocurrency development are gained by analysis of the publicly available DOACC dataset. The paper does so by providing an overview of the techniques and mechanisms used by cryptocurrencies. It shows that newly created cryptocurrencies tend to be very similar in some properties in the early stages but new features and more diversity developed in more recent years. Additionally, newly created cryptocurrencies tend more and more to create a fixed number of coins before the initial announcement in order to sell these in Initial Coin Offerings. Even when the amount of premining increases over years, it remains at lower levels on the aggregate

The Impact of Persistent Shocks and Concave Objective Functions on Collusive Behavior
This paper provides a theoretic model for the analysis of cartel formation in an industry that is subject to profit shocks. The competitive or collusive conduct of a firm is determined by a decision maker who maximizes the present value of utility that accrues to him by earning a share of the firm's profit. The paper assumes that, first, factors like progressive taxation, shareholders' preference for smooth profits, or risk aversion may make the utility function of the decision maker rise concavely in the profits of the firm. Second, collusion causes the decision maker a dis-utility by violating legal and, thus, ethical or social norms. This disutility is independent from the level of profits. Concavity has adverse effects on collusion by making the decision maker value the additional utility from, first, establishing a new cartel, second, deviating from an existing cartel and, third, being punished for this deviation higher when the industry is in a bad state with low profits. Under these conditions, a negative profitability shock must be rather persistent to trigger cartel formation. Persistence prevents a newly formed cartel from falling apart quickly as the intense punishment in this state would also persist for a long time
The effect of self-generated versus externally generated actions on timing, duration, and amplitude of blood oxygen level dependent response for visual feedback processing
It has been widely assumed that internal forward models use efference copies to cre-ate predictions about the sensory consequences of our own actions. While these pre-dictions have frequently been associated with a reduced blood oxygen leveldependent (BOLD) response in sensory cortices, the timing and duration of thehemodynamic response for the processing of video feedback of self-generated(active) versus externally generated (passive) movements is poorly understood. In thepresent study, we tested the hypothesis that predictive mechanisms for self-generated actions lead to early and shorter neural processing compared with exter-nally generated movements. We investigated active and passive movements using acustom-made fMRI-compatible movement device. Visual video feedback of theactive and passive movements was presented in real time or with variable delays.Participants had to judge whether the feedback was delayed. Timing and duration ofBOLD impulse response was calculated using a first (temporal derivative [TD]) andsecond-order (dispersion derivative [DD]) Taylor approximation. Our reanalysis con-firmed our previous finding of reduced BOLD response for active compared to pas-sive movements. Moreover, we found positive effects of the TD and DD in thesupplementary motor area, cerebellum, visual cortices, and subcortical structures,indicating earlier and shorter hemodynamic responses for active compared to passivemovements. Furthermore, earlier activation in the putamen for active compared topassive conditions was associated with reduced delay detection performance. Thesefindings indicate that efference copy-based predictive mechanisms enable earlierprocessing of action feedback, which might have reduced the ability to detect shortdelays between action and feedback.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg
How scale and institutional setting explain the costs of small airports? -An application of spatial regression analysis
One of the main pillars of efficient airport operations is cost-minimization. Unit costs of operation with respect to the level of passengers served are a possible proxy to measure the cost efficiency of an airport. Due to compound production framework and sophisticated political-economic environment of airports, estimation of airport costs requires detailed specifications. Airport cost functions should be able to explain the total costs with the main inputs labor, material and capital as well as by taking the airport specific characteristics into account. In this paper, we apply such an approach and focus on airport specific characteristics. We use a spatial regression methodology to explain how these drive the unit costs and analyze the spatial relationship among the dependent variables. Two separate data samples from Norwegian and French airports are used in this research to test various hypotheses.
Because a large number of regional airports in both countries cannot reach financial break-even, our first research question deals with the effects of subsidies, which often follow regional and political considerations. One must therefore find an efficient way to maintain these airports without any distortions on the incentives. When evaluating the relationship between subsidies and unit costs, we find negative effect of subsidies on airport cost efficiency. Second, we evaluate the importance of economies of scale by focusing on the relationship between airport size and unit costs. Finally, the results of spatial regression show that a denser spatial distribution of airports results in higher unit costs as a consequence of lower capacity utilization, indicating the negative effect of spatial competition on airport unit costs within an airport network
Zeitaufgelöste Kristallographie über die strukturelle Dynamik von Cryptochromen und Photolyasen
Photolyases and cryptochromes form a superfamily (PCSf) of light-directed proteins found in all areas of life. Photolyases repair UV-induced DNA lesions, namely either cyclobutane-pyrimidine dimers (CPD) or pyrimidine-(6-4)-pyrimidone photoproducts, (6-4)PP. The cryptochromes have mostly lost DNA repair functionality and act as signaling proteins coupling various biological responses to light input. Photolyases and cryptochromes have close evolutionary relationships sharing both a common topology, including the same photoactive regions (FAD, antenna cofactors). During photoactivation, FAD can capture one or two electrons and exist in four possible redox states: oxidized (FADox), anionic semiquinone FAD radical (FAD•−), radical semi-reduced (semiquinone, FADH•) and fully reduced (hydroquinone, FADH−). When blue light absorption drives first electron transfer to the inactive FADox, resulting in a short-lived FAD•− which is intermediate of long-lived protonated FADH•. Furthermore, the second electron transfer leads to the fully photoactivated state, FADH−, which catalyze DNA repair of CPD or (6-4)PP by injecting an electron from excited FADH−* onto the DNA lesion. This study focuses on the structural changes via time-resolved serial femtosecond X-ray crystallography (TR-SFX) to describe how cryptochromes perform signal transduction mechanism in animal-like cryptochrome from the green algae Chlamydomonas reinhardtii (CraCRY). However, CraCRY is not only a cryptochrome, but also has (6-4) photolyase function, meaning (6-4)PP repair. In CraCRY, during first light-driven cycle, three tryptophans and one tyrosine (Y373) act as an electron transfer chain, with Y373 donating the electron to the FAD to produce short-lived radical pair (RP) FAD•−/Y373•+, which evolves to the long-lived FADH•/Y373• RP. The tyrosyl radical is triggering a structural change in the region between the loop carrying the tyrosyl radical and the C-terminal α22- helix, and c-terminal unfolding afterwards. In the photoreduction of CraCRY, 19 snapshot structures were obtained, which showed that signal transduction occurred in three different phases in ns to ms: (1) stabilization of FAD•− and Tyr•+ radicals (ns), (2) stabilization of FAD•− via proton transfer (μs to ms), and (3) c-terminal unfolding (ms to s). These results demonstrated a detailed molecular mechanism for cryptochrome photoactivation. For (6-4)PP repair, we succeeded to preliminarily characterize via in crystallo cryo-trapping of post-illumination intermediates, immediately followed by data collection. We were able to determine three stages of complex dissociation at atomic resolution: (1) base relaxation within the active site, (2) base return towards the unpaired bubble and (3) DNA release from the (6-4)-photolyase active site. For further investigation of photoreduction, mutants of each relevant actor in the signaling state were characterized both spectroscopically and crystallographically. These mutations influenced the formation of the signaling state characterizing the role of individual amino-acids during the CraCRY photocycle. For repairing DNA lesion, (6-4)PP repair mechanism is still not clear. Future TR-SFX experiments will show the whole repair mechanism for (6-4)PP repair.Photolyasen und Cryptochrome bilden eine Superfamilie (PCSf) von lichtgesteuerten Proteinen, die in allen Bereichen des Lebens vorkommen. Die vorliegende Studie konzentriert sich auf die strukturellen Veränderungen mittels zeitaufgelöster serieller Femtosekunden-Röntgenkristallographie (TR-SFX), um zu beschreiben, wie Cryptochrome den Signalübertragungsmechanismus im tierähnlichen Cryptochrom aus der Grünalge Chlamydomonas reinhardtii (CraCRY) durchführen. CraCRY ist nicht nur ein Cryptochrom, sondern hat auch eine (6-4)-Photolyase-Funktion, d.h. ist zur Reparatur von (6-4)PP-Schäden. In CraCRY fungieren während des ersten lichtgesteuerten Zyklus drei Tryptophanreste und ein Tyrosin (Y373) als Elektronentransferkette, wobei Y373 das Elektron an das FAD abgibt, um das kurzlebige Radikalpaar (RP) FAD•−/Y373•+ zu erzeugen, das sich zum langlebigen FADH•/Y373• RP durch Protonierung stabilisiert. Das Tyrosyl-Radikal löst eine Strukturänderung in der Region zwischen der Schleife, die den Tyrosyl-Radikal trägt, und der C-terminalen α22-Helix aus, was zur anschließenden Entfaltung des C-Terminus führt. Während der Photoreduktion von CraCRY wurden 19 Schnappschussstrukturen durch TR-SFX erhalten, die zeigten, dass die Signaltransduktion in drei verschiedene Phasen vom ns- bis ms-Bereich erfolgte: (1) Stabilisierung des FAD•− und Tyr•+-Biradikals (ns), (2) Stabilisierung von FAD•− über Protonentransfer (μs zu ms) und (3) C-terminale Entfaltung (ms zu s). Diese Ergebnisse zeigten erstmals einen detaillierten molekularen Mechanismus für die Cryptochrom-Photoaktivierung. Für die (6-4)PP-Reparatur gelang es, den Prozess via in-crystallo Cryo-Trapping von Post-Illuminations-Zwischenprodukten vorläufig zu charakterisieren. Wir waren in der Lage, drei Stadien der komplexen Dissoziation mit atomarer Auflösung zu bestimmen: (1) Basenrelaxation innerhalb des aktiven Zentrums, (2) Basenrückkehr in Richtung der ungepaarten DNA-bubble und (3) DNA-Freisetzung aus dem aktiven Zentrum der (6-4)-Photolyase. Zur weiteren Untersuchung der Photoreduktion wurden Mutanten jedes relevanten Akteurs im Signalzustand sowohl spektroskopisch als auch kristallographisch charakterisiert. Diese Mutationen beeinflussten die Ausbildung des Signalzustands, wodurch die Rolle einzelner Aminosäuren während des CraCRY-Photozyklus charakterisiert wurde. Gegenwärtig ist der Reparaturmechanismus für die Reparatur von (6-4)PP DNA-Läsionen noch unklar. Zukünftige TR-SFX-Experimente werden in der Lage sein den gesamten Reparaturmechanismus für die (6-4)PP-Reparatur zeigen
Eye-Tracking-Paradigma zur Identifizierung krankheitsspezifischer Verhaltensbiomarker bei Neurodegeneration
Isolated rapid eye movement (REM) sleep behavior disorder (iRBD) has been identified as the most specific and common prodromal stage of α-synucleinopathies (αSYN) such as Parkinson's disease (PD), dementia with Lewy bodies (DLB), and the sporadic disease multiple system atrophy (MSA). Within 10 to 20 years, patients with this dream-sleep disorder convert in up to 85 % of cases to a neurodegenerative disease of the type of αSYN. Hence, iRBD is an ideal group for testing a disease-modifying therapy to postpone or even prevent phenoconversion. The latency, however, from diagnosis to phenoconversion is prolonged, lasting years to decades. Therefore, identifying iRBD patients more likely to phenoconvert needs highly sensitive and specific prodromal biomarkers and progression markers.
The goal of this study was to contribute to the identification of biomarkers in manifest and prodromal αSYNs for their future selection as participants in protection trials. Furthermore, comparing patients with αSYN and Tauopathy is the second objective of this dissertation, aimed at identifying the underlying differences between the two disorders.
To date, most of the biomarkers and progression markers for manifest αSYN relate to the motor and cognitive dysfunctions and imaging of the central nervous system but less to sensory and autonomic dysfunction. For iRBD, a recent review paper has summarized the state-of-the-art that confirms the above statement that most of the works in the field of biomarkers are performed on motor and cognitive functions and imaging.
Until 2022, little has been published on oculomotor and pupillomotor dysfunctions in manifest and prodromal αSYN, but rather on the Tauopathy; progressive supranuclear palsy (PSP). The methodologies for studying eye movements and pupillary responses are highly developed. They offer a high resolution and precision in time and space for measuring sensory, autonomic, motor, and cognitive functions.
Therefore, we systematically investigated the saccade, pupil, and blink behaviors in the manifest αSYN PD and MSA and their prodrome iRBD compared to healthy age and gender-matched controls. As a ”disease control” and for comparison, we also studied patients suffering from Tauopathy PSP.
PSP is well-known for its oculomotor abnormalities, particularly for its characteristic symptom of relative vertical gaze palsy. PSP is – like MSA – another atypical parkinsonian disorder with multiple brain tissue losses, for example, in the frontal cortex. Because the early diagnosis of PD and MSA from PSP is difficult, PSP patients have been recruited for this study.
As methods, we employed a structured saccade task that is called the Interleaved Pro/ Anti Saccade Task (IPAST) and a free viewing task (FV) to investigate oculomotor and pupillomotor function along with blink behavior in SYN and PSP.
The IPAST is a structured saccade task that requires strong cognitive control, alertness, and attention. Previous studies on the manifest SYN have shown that patients with PD have systemic abnormalities in oculomotor, pupillometric parameters, and blink behavior in the IPAST.
In order to simplify our method and broaden our ability to collect a wide range of eye movement parameters, we additionally employed another task, the unstructured free viewing of video clips (FV). Therefore, the research question is whether oculomotor and pupillomotor abnormalities and blinking during the IPAST and FV in iRBD patients differ from healthy controls, PD, MSA, and PSP.
This study represents the first use of FV for the investigation of eye movement and pupil responses in subjects suffering from prodromal and manifest SYN. It is also the first study comparing prodromal and manifest SYN (PD, MSA) with PSP in FV. This dissertation has been performed in the context of the evolving disease-modifying therapy trials for manifest SYN, which are currently ongoing in patients with Parkinson’s disease.
The next challenge will be to test these therapies in people with iRBD to slow or even prevent the full manifestation of the SYN. It will be essential to enrich prodromal populations with biomarkers of short-term conversion and to be able to monitor disease progression with serial measurements. Developing neurodegenerative disease treatments is becoming increasingly important as the population ages and the burden on families and society increases.
In summary, we identified potential prodromal biomarkers in iRBD and differences between αSYN and the Tauopathy PSP, suggesting that the IPAST and especially FV task may be a tool to identify prodromal SYN and help to distinguish early manifest αSYN from early PSP. The future goal is intra-individual follow-up studies in iRBD patients to determine whether the so far observed subtle changes in oculo/pupillo-motor measures will progressively increase over time and allow the prediction of the phenoconversion of iRBD into manifest αSYN. These longitudinal studies will show whether oculo/pupillo-motor parameters can reliably distinguish the different neurodegenerative movement disorders in the manifest stages, and even more challenging, during their prodromal progression towards phenoconversion.Die isolierte Rapid-Eye-Movement-(REM)-Schlafverhaltensstörung (iRBD) wurde als das spezifischste und häufigste Prodromalstadium von α-Synucleinopathien (αSYN) wie der Parkinson-Krankheit (PK), der Demenz mit Lewy-Körperchen (DLB) und der sporadischen Erkrankung Multisystematrophie (MSA) identifiziert. Innerhalb von 10 bis 20 Jahren entwickeln Patienten mit dieser Traumschlafstörung in bis zu 85 % der Fälle eine neurodegenerative Erkrankung vom Typ der αSYN (sogenannte Phänokonversion). Daher stellen Patienten mit iRBD eine ideale Gruppe für die Untersuchung einer krankheitsmodifizierenden Therapie dar, die diese Phänokonversion verzögern oder sogar verhindern soll. Die Zeitspanne von der Diagnose bis zur Phänokonversion ist jedoch sehr lang und kann Jahre bis Jahrzehnte dauern. Zur Identifizierung von iRBD-Patienten, bei denen die Wahrscheinlichkeit einer Phänokonversion höher ist, werden daher hochempfindliche und spezifische Biomarker für das Prodromalstadium und die Messung der prodromalen Krankheitsprogression benötigt.
Ziel dieser Studie war es, einen Beitrag zur Identifizierung von Biomarkern bei manifesten und prodromalen αSYN-Patienten zu leisten, damit diese künftig als Teilnehmer an neuroprotektiven Studien ausgewählt werden können. Darüber hinaus war der Vergleich von Patienten mit αSYN und Tauopathien das zweite Ziel dieser Dissertation, um die zugrundeliegenden Unterschiede zwischen den beiden Erkrankungen zu identifizieren.
Bisher beziehen sich die meisten Biomarker und Progressionsmarker für manifeste αSYN auf die motorischen und kognitiven Störungen und die Bildgebung des zentralen Nervensystems, aber weniger auf sensorische und autonome Funktionsstörungen. Für iRBD hat eine kürzlich erschienene Übersichtsarbeit den Stand der Forschung zusammengefasst und bestätigt, dass sich die meisten Arbeiten im Bereich der Biomarker auf motorische und kognitive Funktionen und die Bildgebung beziehen.
Bis 2022 wurden vor allem Arbeiten zu Störungen der Okulo- und Pupillomotorik bei der Tauopathie, progressive supranukleäre Blickparese (PSP), veröffentlicht. Nur wenige haben sich mit Auffälligkeiten der Okulo- und Pupillomotorik bei manifesten und prodromalen αSYN beschäftigt. Die Methoden zur Untersuchung von Augenbewegungen und Pupillenreaktionen sind hoch entwickelt. Sie bieten eine hohe zeitliche und räumliche Auflösung und Präzision für die Messung sensorischer, autonomer, motorischer und kognitiver Funktionen.
Daher untersuchten wir systematisch das Sakkaden-, Pupillen- und Blinzelverhalten bei den manifesten αSYN, PK und MSA, sowie deren Prodromalstadium, der iRBD, im Vergleich zu gesunden, altersgleichen Kontrollen. Als "Krankheitskontrolle" und zum Vergleich untersuchten wir auch Patienten, die an der Tauopathie PSP leiden.
Typisch für die PSP sind Störungen der Okulomotorik, insbesondere das charakteristische Symptom der relativen vertikalen Blickparese. Die PSP gehört - wie die MSA – zu den atypischen Parkinsonsyndromen und ist durch multiple Hirngewebsverluste, zum Beispiel im frontalen Kortex, gekennzeichnet. Da die differentialdiagnostische Abgrenzung von PK, MSA und PSP in der Frühphase schwierig ist, wurden für diese Studie auch PSP-Patienten rekrutiert.
Als Methoden verwendeten wir eine strukturierte Sakkadenaufgabe, die Interleaved Pro/Anti Saccade Task (IPAST) genannt wird, und eine „FreeViewing“-Aufgabe (FV), um die okulomotorische und pupillomotorische Funktion sowie das Blinzelverhalten bei αSYN und PSP zu untersuchen.
Die IPAST ist eine strukturierte Sakkadenaufgabe, die eine starke kognitive Kontrolle, Wachsamkeit und Aufmerksamkeit erfordert. Frühere Studien zu manifesten αSYN haben gezeigt, dass PK-Patienten systemische Störungen der Okulomotorik, der pupillometrischen Parameter und des Blinzelverhaltens im IPAST aufweisen.
Um unsere Methode zu vereinfachen und unsere Möglichkeiten zur Erfassung eines breiten Spektrums von Augenbewegungsparametern zu erweitern, haben wir zusätzlich eine weitere Aufgabe eingesetzt: das unstrukturierte freie Betrachten von Videoclips (free viewing (FV)). Die Forschungsfrage lautete daher, ob sich okulomotorische und pupillomotorische Störungen und das Blinzelverhalten während der IPAST und des FV bei iRBD-Patienten von gesunden Kontrollen, PK-, MSA- und PSP-Patienten unterscheiden.
In dieser Studie wurde zum ersten Mal die FV zur Untersuchung von Augenbewegungen und Pupillenreaktionen bei Patienten mit prodromaler und manifester αSYN eingesetzt. Es war auch die erste Studie, die prodromale und manifeste αSYN (PK, MSA) mit der PSP in FV vergleicht. Diese Dissertation wurde im Kontext der sich entwickelnden krankheitsmodifizierenden Therapiestudien für manifeste αSYN durchgeführt, die derzeit bei Patienten mit PK durchgeführt werden.
Die nächste Herausforderung wird darin bestehen, diese Therapien bei Menschen mit iRBD zu testen, um die vollständige Manifestation der αSYN zu verlangsamen oder sogar zu verhindern. Es wird von entscheidender Bedeutung sein, Biomarker für die Prodromalphase zu haben, die Risikopatienten mit einem hohen Risiko der kurzfristigen Phänokonversion identifizieren und das Fortschreiten der Krankheit mit seriellen Messungen erfassen. Die Entwicklung von krankheitsmodifizierenden Therapien für neurodegenerative Erkrankungen wird immer wichtiger, da die Bevölkerung altert und die Belastung für Familien und Gesellschaft zunimmt.
Zusammenfassend haben wir potenzielle Biomarker für das Prodromalstadium der αSYN, der iRBD, und zur Messung von Unterschieden zwischen αSYN und der Tauopathie PSP identifiziert. Unsere Ergebnisse deuten daraufhin, dass die IPAST- und insbesondere die FV-Aufgabe ein Instrument zur Identifizierung von αSYN im Prodromalstadium und zur Unterscheidung zwischen früher, manifester αSYN und früher PSP sein könnte. Das zukünftige Ziel sind intra-individuelle Verlaufsstudien bei iRBD-Patienten, um festzustellen, ob die bisher beobachteten subtilen Veränderungen der okulo- und pupillomotorischen Messungen im Laufe der Zeit zunehmen und die Vorhersage der Phänokonversion von iRBD in manifeste αSYN ermöglichen
Liberal Coercion? - Prostitution, Human Trafficking and Policy
Liberal prostitution policy aims at improving labour conditions for prostitutes and protecting victims of forced prostitution. Its policy orientation predicts that the policy choice of liberalizing prostitution is positively associated with better protection policy for trafficking victims and enhanced anti-trafficking measures. In this paper, I investigate empirically whether the legalization of prostitution improves protection policy for victims, as it is presumed. The results of my analysis – using data from 149 countries for the period of 2001-2011 – suggest that the liberalisation of prostitution does not lead to better protection and, in some cases, legalized prostitution can be detrimental to protecting victims of human trafficking
