    Mapping the methodological diversity of published drug discontinuation studies—a scoping review of study topics, objectives, and designs

    Background Trials evaluating drug discontinuation (drug discontinuation trials, DDTs) show a broad methodological spectrum. There are several specific methodological aspects in drug discontinuation trials (e.g., determination of research question; configuration of intervention; definition of outcomes). To verify this specifies, we did a scoping review about the study designs of drug discontinuation trials. Methods A systematic literature search in Medline (PubMed), The Cochrane Library, EMBASE, CINAHL, Web of Science, and PsycINFO was performed. In a two-step selection process, we identified DDTs, which evaluate the discontinuation of one or more long-term medication as the investigated intervention, by two independent reviewers. Besides bibliographic data, we extracted several parameters to describe the used study design of the included DDTs: motivation for DDT, initially treatment aim of the discontinued medication, study design, methods of discontinuation, follow-up times, number of study participants, and outcome parameter. Results Out of 12,132 records, we included 581 DDTs. The most common motivation for doing a DDT were expected side effects (48.8%), the motivation of proving the efficacy of medication (21.6%), or doubts on the expected benefit of the used medication (13.8%). The majority of the discontinued medication was initially prescribed to improve the prognosis of a chronic disease (60.4%) or to relieve symptoms (31%). The study designs of the trials showed a broad methodological spectrum. The minority of the drug discontinuation trials were randomized controlled trials (34%). Conclusion The results of this scoping review illustrates the need for an evidence-based methodological standard for planning and conducting drug discontinuation trials.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg

    Volatile, nitrogen-rich metal compounds for chemical vapor deposition

    In der vorliegenden Arbeit werden flüchtige Metallkomplexe als potentielle Präkursorenverbindungen fur die Abscheidung von metallischen Filmen aus der Gasphase entworfen, dargestellt und charakterisiert. Diese Beschichtungverfahren, chemische Gasphasenabscheidung (CVD) und Atomlagenabscheidung (ALD), sind von besonderer Bedeutung fur die Nanostrukturierung von Halbleiterelektronik. Dafür sind Präkursoren notwendig, die ihr Metallatom als vordefiniertes Funktionsmaterial auf einer Oberfläche durch eine thermisch oder chemisch induzierte Folgereaktion aus der Gasphase abscheiden. Um diesen speziellen Anforderungen gerecht zu werden, wurden potentiell flüchtige Metallkomplexe mit stickstoffreichen Chelatliganden entworfen. Diese neuen Präkursorverbindungen weisen thermisch labile N−N-Bindungen im Ligandengerüst und chemisch leicht angreifbare M−N-Bindungen auf. Der Forschungsschwerpunkt lag auf der Synthese und Charakterisierung flüchtiger Komplexe des Rutheniums und der Seltenerdmetalle in Kombination mit den Liganden N,N’-Bis(dimethylamino)methylacetamidin H(bdma), Di-tert- H(dbt) und Methyl-tert-butyltriazen H(mbt), 1,5-Di-tert-butyl-3-methylformazan H(dbmf), und 1,4-Di-iso-propyldiazadien iPrDAD. Die Verbindungen wurden in TGA/SDTA Messungen bezüglich der Verdampfbarkeit bei Standarddruck und ihrer thermischen Stabilität in einer Stickstoffatmosphäre untersucht. Basierend aus den Ergebnissen der TGA/SDTA wird die Eignung für die ALD/CVD abgeleitet. Es wurden verschiedene η6-aren-Ruthenium-Komplexe ausgehend von [Ru(η6-aren)Cl2]2 dargestellt. Die neuen Verbindungen wurden nach Ru0 - und RuII-Präkursoren kategorisiert. Die flüchtigen Ru0-Komplexe wurden in einer metallfreien Reduktion des Rutheniums mittels iPrOH und NEt3 in Anwesenheit neutraler Chelatliganden hergestellt. Die Darstellung der RuII-Komplexe erfolgte durch N−H Deprotonierung protischer Chelatliganden in Anwesenheit von NEt3. Dabei wurde selektiv nur ein Chlorido-Ligand substituiert. In Folgereaktionen wurde der verbleibende Chlorido-Ligand durch eine Methyl- oder Hydrido-Gruppe substituiert, um flüchtige RuII-Komplexe zu erhalten. Bei dem Versuch [RuII(cod)Cl2]n (cod = Cyclooktadien) mit Li(bdma) umzusetzen, wurde eine untypische, thermisch induzierte N-Demethylierung des bdma-Liganden zu einem 1,1,3,5-Tetramethylformazan im Triazadienbindungsmodus (Me,NMe2TAD) beobachtet. Der cod-Ligand wurde dabei über ein Cyclooktadienylanion (η5-cod) zu einem Cyclooctatrien (cot) dehydriert. Gleichzeitig wird RuII zu Ru0 reduziert. Die N-Demethylierung des bdma-Liganden ließ sich für aren- Komplexe mit dem Reduktionsmittel iPrOH in NEt3 reproduzieren. Die Me,NMe2TAD-Komplexe wurden auch beim Erhitzen der [RuII(aren)(bdma)H]-Komplexe über 80 °C erhalten. Sie sind daher deren erstes thermisches Abbauprodukt. Laut TGA/SDTA geht bei den meisten Ruthenium-Präkursoren die Verdampfung bei höherer Temperatur in Zersetzung über oder findet parallel statt. Die Sc- und Y-Präkursoren mit Ethylcyclopentadienyl-Liganden wurden mit stickstoffbasierten, monoanionischen Chelatliganden (LL) kombiniert und charakterisiert. Über Verbindungen des Typs [M(EtCp)2(LL)] (M = Sc, Y) hinaus, wurden die EtCp-Liganden sukzessive mit dem dbt-Liganden bis zum homoleptischen Komplex [M(dbt)3] substituiert. Es wurde eine neue skalierbare Synthese für H(dbmf) und Li(dbmf) mit deutlich höheren Ausbeuten entwickelt. Neue flüchtige, homo- und heteroleptische Nebengruppenkomplexe mit neutralen 5-Ring oder anionischen 6-Ring dbmf-Liganden wurden realisiert. Beide Bindungsmodi liegen in [Cu(Hdbmf)(dbmf)] an einem Metallzentrum gleichzeitig vor. Kristallstrukturen von neutralen 5-Ring-Verbindungen zeigten konsequent keine Konjugation der NHtBu-Gruppe mit dem Rückgrat. Daher ähnelt die Bindungssituation eher einem Dien, als einem klassischen Formazan. Die Bisformazenato-Komplexe wurden durch Verzahnung der tBu-Substituenten ausschließlich in verzerrter Tetraeder-Konfiguration erhalten. Mit Ausnahme von [Mo(Hdbmf)(CO)4] wurden alle dbmf-Präkursoren im Vakuum zersetzungsfrei sublimiert bzw. kondensiert. Zudem wurden leicht schmelzbare, flüchtige, dimere Zink-Komplexe durch Protolyse von ZnMe2 mit H(dbt) und H(mbt) dargestellt. Die gute Flüchtigkeit des dbt-Derivates wird auf die leichte Dissoziation der Dimere in Monomere zurückgeführt. Tris(hexamethyldisilazanide) der Lanthanoide ([Ln(hmds)3], Ln = Ce, Nd, Eu, Tb, Lu) gehen mit den Triazenido-Liganden H(dbt) und H(mbt) eine Protolyse unter Freisetzung von H(hmds) ein. Dadurch konnten die flüchtigen, homoleptischen Lanthanoid-Präkursoren [Ln(dbt)3] und [Ln(mbt)2(μ-mbt)]2 ( Ln = Ce, Nd, Eu, Tb, Lu) dargestellt werden. Die monomeren dbt-Präkursoren sind in der TGA/SDTA vollständig verdampfbar, während die dimeren mbt-Präkursoren ab 220 °C pyrolisieren. Durch Kristallstrukturen der dimeren mbt-Präkursoren wurde eine bisher unbekannte, verbrückende Koordinationsweise der Triazenido-Liganden identifiziert.In this Thesis volatile metal complexes were synthesised, characterised and discussed as potential precursors for thin metallic film deposition from the gas phase. These deposition techniques, chemical vapour deposition (CVD) and the atomic layer deposition (ALD), are applied in the fabrication of nanostructured semiconductors. Therefore precursor compounds are needed, which deposit their metal as pre-defined functional material on a surface through a thermally or chemically induced consecutive reaction from the gas phase. In order to meet these special requirements, potentially volatile metal complexes with nitrogen rich chelating ligands were developed. These new precursors show thermally weak N−N-bonds in the ligand and chemically vulnerable M−N-bonds. The research focus was to synthesise and characterise volatile complexes containing ruthenium and rare-earth metals combined with the chelating ligands N,N’-bis(dimethylamino)methylacetamidine H(bdma), di-tert- H(dbt) und methyl-tert-butyltriazene H(mbt), 1,5-di-tert-butyl-3-methylformazane H(dbmf), und 1,4-di-iso-propyldiazadiene iPrDAD. These compounds were tested in TGA/SDTA measurements regarding evaporability at standard pressure and thermal stability in a nitrogen atmosphere. Their qualification for ALD/CVD is screened by results of TGA/SDTA measurements. Different η6-arene-ruthenium complexes were synthesised and studied, starting from [RuII(η6-arene)Cl2]2-dimers. The new compounds were classified into Ru0 - and RuII precursors. The volatile Ru0 complexes were synthesised by a metal free reduction of ruthenium with iPrOH and NEt3 in presence of neutral chelating ligands. The products obtained by route A showed lower yields with sterically higher demanding arenes and high solubility of the starting material. The synthesis of the RuII complexes was accomplished by a N−H deprotonation of protic chelating ligands in NEt3. Here, just one chlorido ligand was substituted selectively. In a consecutive reaction, the remaining chlorido ligand was substituted by a methyl or hydrido ligand to obtain the highly volatile complexes. The conversion of [RuII(cod)Cl2]n (cod = cyclooctadien) and Li(bdma) showed an uncommon, thermally induced N-demethylation of the bdma ligand to a 1,1,3,5-tetramethylformazane coordinating in its triazadiene-bonding mode. Meanwhile, the neutral cod-ligand was isomerized and deprotonated first to a cyclooctadienyl anion (η5-cod) and finally dehydrogenated to cyclooctatrien (cot). RuII was reduced to Ru0 simultaneously. The N-demethylation of the bdma ligand could be reproduced for the arene complexes with the reducing agent iPrOH in NEt3. [Ru0(arene)(Me,NMe2TAD)] complexes were also obtained by heating [RuII(arene)(bdma)H] beyond 80 °C. Consequently, the latter are the first thermal decomposition products of the former. According to TGA/SDTA at higher temperatures the evaporation shifts into decomposition or happens simultaneously for most of the ruthenium precursors. The Sc and Y precursors containing ethyl cyclopentadienyl ligands were combined with nitrogen-rich, mono-anionic, chelating ligands and characterisised. Additionally to compounds of type [M(EtCp)2(LL)] (M = Sc, Y), EtCp ligands were successively substituted by dbt ligands up to the homoleptic complexes [M(dbt)3]. A new synthesis to H(dbmf) and Li(dbmf) with higher yields has been developed. Novel volatile homo- and heteroleptic complexes of the transition metals featuring the neutral 5-ring or anionic 6-ring structure of the dbmf ligand have been realised. Both bonding modes are combined at one metal centre in [Cu(Hdbmf)(dbmf)]. The molecular structures of neutral 5-membered ring complexes show no conjugation of the NHtBu group with the ligand backbone. Therefore, the bonding situation is closer to a diene than a formazane. The bisformazantes were obtained as tetrahedral complexes exclusively, because of the interlocking position of the tert-butyl substituents. All dbmf precursors, with exception of [Mo(Hdbmf)(CO)4], could be sublimed in the vacuum without decomposition. Addionally, volatile, dimeric zinc complexes were synthesised. The high volatility of the dbt derivate can be traced back on the easy dissociation of the dimers to monomers. Tris(hexamethyldisilazanides) of lanthanide metals ([Ln(hmds)3], Ln = Ce, Nd, Eu, Tb, Lu) react with triazanido ligands H(dbt) and H(mbt) in a protolysis reaction under release of H(hmds). Thereby, the volatile, homoleptic lanthanide precursors [Ln(dbt)3] and [Ln(mbt)2(μ-mbt)]2 (Ln = Ce, Nd, Eu, Tb, Lu) could be isolated. The monomeric dbt precursors evaporate according to TGD/SDTA, while the dimeric mbt precursors decompose beyond 220 °C.     Integration of optic flow into the sky compass network in the brain of the desert locust

    Flexible orientation through any environment requires a sense of current relative heading that is updated based on self-motion. Global external cues originating from the sky or the earth‘s magnetic field and local cues provide a reference frame for the sense of direction. Locally, optic flow may inform about turning maneuvers, travel speed and covered distance. The central complex in the insect brain is associated with orientation behavior and largely acts as a navigation center. Visual information from global celestial cues and local landmarks are integrated in the central complex to form an internal representation of current heading. However, it is less clear how optic flow is integrated into the central-complex network. We recorded intracellularly from neurons in the locust central complex while presenting lateral grating patterns that simulated translational and rotational motion to identify these sites of integration. Certain types of central-complex neurons were sensitive to optic-flow stimulation independent of the type and direction of simulated motion. Columnar neurons innervating the noduli, paired central-complex substructures, were tuned to the direction of simulated horizontal turns. Modeling the connectivity of these neurons with a system of proposed compass neurons can account for rotation-direction specific shifts in the activity profile in the central complex corresponding to turn direction. Our model is similar but not identical to the mechanisms proposed for angular velocity integration in the navigation compass of the fly Drosophila.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg

    How to Enable Participation until the End of Life? A Survey of German Occupational Therapists Working in Palliative Care

    Background: In palliative care, the needs of people with life-limiting illnesses must be addressed with the support of a multidisciplinary team. Occupational therapy is a profession that focuses on activity and participation. In Germany, there are no surveys to date that demonstrate the work of occupational therapists in palliative care and which problems can occur in this field. Aim: The aim of this study is to describe the work and problems of occupational therapists in German palliative care. Design: The survey consists of 17 questions and could be conducted anonymously. Descriptive statistics and a thematic analysis of the free text responses were used to analyze data. Setting/participants: Respondents were German occupational therapists who are members of the “German Association of Occupational Therapy”. Results: A total of 281 valid responses were evaluated, of which 120 respondents work in the context of palliative care. Most of them provide needs-based therapies (74%), train relatives (69%), or help patients with positioning (69%) or relaxation therapy (66%). Four themes were developed in the free text responses that describe problems in the utilization of occupational therapy in palliative care. Conclusions: The evaluation shows that occupational therapy in palliative care offers a variety of possible applications and approaches. The findings urgently need to be implemented in daily practice to improve the status of occupational therapists. This requires appropriate billing modalities and clear guidelines.     MUSE - Musik und Stresserleben Eine fMRT-Studie zur Stressantwort und dem stressreduzierenden Effekt von Musik - eine Analyse anhand physiologischer und neuronaler Daten

    In der fMRT-Pilotstudie ”MUSE - Musik und Stresserleben” werden die akustischen Stimuli Musik und Naturgeräusche als Mittel zur Stressreduktion evaluiert. Somit konnten Ergebnisse früherer Arbeiten zum stressreduzierenden Einfluss von Musik und Naturgeräuschen durch neuronale Daten flankiert werden. Da Stress als wichtiger Risikofaktor für organische und psychische Erkrankungen bekannt ist, entsteht mit der Erforschung von Möglichkeiten der Stressreduktion ein wichtiger Beitrag zur Erschließung neuer Therapieansätze. Um eine Stressreduktion näher untersuchen zu können, musste zunächst eine Stressinduktion im MRT-Scanner stattfinden. Als Forschungsmethode wurde dafür der MR-Stresstest erarbeitet. Dieser beruht auf dem Trier Social Stresstest und wurde auf Basis des ScanSTRESS-Paradigmas entwickelt. Im Paradigma wechseln sich Stress- und Ruhebedingungen ab. Stress wird durch die Präsentation von Mathe- und Rotationsaufgaben, Zeitdruck und die negative Bewertung eines zugeschalteten Gremiums verursacht. Neben den neuronalen Daten werden die Hormone Cortisol und Alpha-Amylase abgenommen, physiologische Messgrößen wie die Hautleitfähigkeit, Atem- und Herzfrequenz bestimmt und Fragebögen zum subjektiven Stresserleben erhoben. Der MR-Stresstest kombiniert somit verschiedene Ebenen der Stressinduktion und liefert umfangreiche Einblicke in das Zusammenspiel hormoneller Regelkreise, physiologischer Körperreaktionen und den psycho-sozialen Aspekten von Stress. Die Stressinduktion konnte auf allen Ebenen erfolgreich umgesetzt werden. Dies zeigte sich in einem Anstieg des subjektiv empfundenen Stresslevels, der Cortisol- und Alpha-Amylase-Spiegel, der Herz- bzw. Atemfrequenz und der Hautleitfähigkeit unter Stressbedingungen des Experiments. Dies unterstreicht die Rolle des autonomen Nervensystems und der Hypothalamus-Hypophysen-Nebennierenrinden-Achse in der kurzfristigen und langanhaltenden Stressantwort. Da eine Abschwächung der Stressreaktion über den Versuchsablauf beobachtet werden konnte, ist eine Weiterentwicklung desTestdesigns zu empfehlen, um Habituationseffekte zu vermeiden. Auf neuronaler Ebene finden sich signifikante Cluster im Thalamus, Gyrus lingualis, Gyrus cinguli, Gyrus frontalis inferior und dem Gyrus temporalis. Eine Erhöhung der zentralen Aktivität steht demnach im Vordergrund. Als Reaktion auf die stressige Situation, wird die Aufmerksamkeit gesteigert und die Fähigkeit verbessert, sich schnell zu orientieren. Das Studiendesign ist so konzipiert, dass die Stressantwort zwischen den zwei Untersuchungsgruppen (Musik/Naturgeräusche) verglichen werden kann. Die Annahme, dass das Hören von Musik im verstärkten Maße zu einer Stressreduktion beiträgt, im Vergleich zum Hören von Naturgeräuschen, konnte nicht bestätigt werden. In beiden Subgruppen zeigte sich nach dem Hören von Musik oder Naturgeräuschen in gleicher Weise eine Reduktion der Stressantwort. Es bestand auf keiner Ebene ein signifikanter Unterschied zwischen den Gruppen. Um diese Ergebnisse besser einordnen zu können, wird auch auf Störfaktoren und Grenzen der Untersuchungsmethode eingegangen. Dabei wird der besondere Einfluss des MRT-Scanners als zusätzlicher Stressor auf die Hormonausschüttung und die Bewertung akustischer Stimuli thematisiert. Zusammenfassend lässt sich feststellen, dass die Funktionalität des MR-Stresstestes zur Induktion von Stress unter fMRT-Bedingungen gegeben ist. Musik und Naturgeräusche eignen sich gleichermaßen zur Stressreduktion. Dieses Potenzial sollte in den Blickpunkt weiterführender klinischer Studien rücken.In the fMRI pilot study MUSE - music and stress the acoustic stimuli music and nature sounds are evaluated as a means of stress reduction. This neural data can be used to extend the results of previous work on the stress-reducing influence of music and nature sounds. Since stress is known to be an important risk factor for organic and mental diseases, research into new ways of reducing stress will make an important contribution to the development of new therapeutic approaches. To investigate stress reduction in more detail, stress induction first had to take place in the MRI scanner. The MR-stresstest was developed as a research method for this purpose and is based on the Trier Social Stress Test. It was developed on the basis of the ScanSTRESS paradigm in which stress and rest conditions alternate. During the experiment stress was induced by the presentation of math and rotation tasks, time pressure and the negative evaluation of a filmed jury. In addition to neural data, the hormones cortisol and alpha-amylase are sampled. Physiological parameters, such as skin conductance, respiratory rate, and heart rate are determined and questionnaires on subjective stress experience are conducted. The MR-stresstest thus combines different levels of stress induction and provides comprehensive insights into the interplay of hormonal regulatory circuits, physiological body responses and the psycho-social aspects of stress. Stress induction was successfully implemented at all levels. This was shown by an increase in the subjectively perceived stress level, cortisol and alpha-amylase levels, heart rate and respiratory rate and skin conductance under stress conditions of the experiment. This highlights the role of the autonomic nervous system and hypothalamic-pituitary-adrenocortical axis in the short-term and long-term stress response. Attenuation of the stress response was observed over the course of the experiment, therefore further development of the test design is recommended to avoid habituation effects. At the neural level, significant clusters are found in the thalamus, lingual gyrus, cingulate gyrus, inferior frontal gyrus, and temporal gyrus. Accordingly, an increase in central activity is prominent. In response to the stressful situation, attention is increased and the ability to orientate quickly is improved. The study was designed to compare the stress response between the two study groups (music/nature sounds). The assumption that listening to music would contribute to stress reduction to a greater extent than listening to nature sounds could not be confirmed. Both subgroups showed a reduction of the stress response after listening to music or nature sounds. To better classify these results, confounding factors and limitations of the study method will be discussed. The particular influence of the MRI scanner as an additional stressor on hormone release and the evaluation of acoustic stimuli will be addressed. In summary, the functionality of the MR-stresstest for the induction of stress under fMRI conditions was demonstrated. Music and natural sounds are equally suitable for stress reduction. This potential should be the focus of further clinical studies

    Hintergrund Der Cobalamin Defekt Typ C (CblC) stellt die häufigste angeborene Störung des intrazellulären Cobalaminstoffwechsels dar. Ursächlich für die autosomal-rezessiv vererbte Erkrankung sind Mutationen im MMACHC-Gen. Biochemisch ist die Multisystemerkrankung gekennzeichnet durch das kombinierte Auftreten einer Methylmalonazidämie und Homocystinurie. Bestimmte Mutationen im MMACHC- Gen führen zu einem Herausspleißen des 2. Exons. Unter Erhalt des Leserasters entsteht ein trunkiertes Protein, das ΔEx2-MMACHC-Protein. CblC-Patienten mit ΔEx2-Spleißmutation zeigen phänotypisch eine auffällige Kombination aus einer renalen thrombotischen Mikroangiopathie (rTMA), bzw. eines atypischen hämolytisch-urämischen Syndroms (aHUS) und einer pulmonal-arteriellen Hypertonie (PAH). Ziel dieser Arbeit war es 1) die Stärke des Zusammenhangs zwischen ΔEx2-Spleißmutationen und dem Auftreten renaler und pulmonaler Vaskulopathien zu ermitteln, 2) durch die Charakterisierung des trunkierten Proteins Rückschlüsse auf dessen Pathophysiologie zu erlangen und 3) neue therapeutische Ansätze zu erproben. Methoden Mithilfe einer PubMed-Literaturrecherche wurde die Genotyp-Phänotyp-Korrelation von ΔEx2-MMACHC-Spleißmutationen und dem Auftreten einer rTMA und PAH herausgearbeitet. Die Charakterisierung des trunkierten Proteins erfolgte mittels Western Blot Untersuchungen und immunozytologischen Färbungen. Halbwertszeiten wurden mittels Cycloheximide Chase Assay bestimmt und Interaktionspartner über das Proximity Ligation Assay identifiziert. Zudem wurden Effekte einer Cobalamin- sowie Homocysteinexposition in HEK293- und HepG2- Zelllinien analysiert. Ergebnisse Es konnten 84 CblC-Patienten mit aHUS/rTMA und/oder PAH und berichteten Genotyp der Literatur entnommen werden. Dabei wiesen 65,4 % der 52 ΔEx2-Spleißmutationsträger einen renal-pulmonalen Phänotyp auf. Demgegenüber standen 5 Fallberichte von CblC-Patienten ohne Spleißmutationen mit renalen Auffälligkeiten und PAH. Laborexperimentell wies das ΔEx2-MMACHC-Protein im Vergleich zum wt-MMACHC-Protein eine reduzierte Stabilität, einen erhöhten proteasomalen Abbau sowie eine verringerte Interaktion mit MMADHC und MSR auf. Zudem zeichnete sich das trunkierte Protein durch die Formation von Disulfiden aus. Die Halbwertszeit des ΔEx2-MMACHC-Proteins konnte durch Cyanocobalamin (CN- Cbl) von 6,4 h auf 9,2 h erhöht werden (Monomere), bzw. von 7,4 h auf 10,6 h (Dimere). Die Anreicherung des Zellnährmediums mit 100 μM Homocystein führte beim ΔEx2-MMACHC-Protein zu einer signifikanten Zunahme an exprimierten Protein, wohingegen das wt-MMACHC-Protein mit einer signifikanten Abnahme der Proteinexpression reagierte. Diskussion Die enge Assoziation zwischen ΔEx2-Spleißmutationen im MMACHC-Gen und dem Auftreten renaler und pulmonaler Vaskulopathien kristallisierte sich in der Literaturrecherche deutlich heraus. Insbesondere der kombinierte Phänotyp scheint nahezu pathognomonisch für CblC-Patienten mit ΔEx2-Spleißmutationen zu sein. Pathophysiologisch ist die herabgesetzte Stabilität des trunkierten Proteins hervorzuheben, die durch ein Überangebot an CN-Cbl deutlich angehoben werden konnte. Die experimentellen Erkenntnisse decken sich mit der beobachteten Wirksamkeit einer CN-Cbl-Therapie an sechs CblC-Patienten mit ΔEx2- Spleißmutationen und eröffnen eine neue Behandlungsoption für diese Patientengruppe. Die starke Tendenz zur Formation von Disulfiden könnte für die gestörte Interaktion mit den Partnerproteinen MMADHC und MSR verantwortlich sein. Zudem deutet die ausgeprägte Disulfidbildung des ΔEx2-MMACHC-Proteins zusammen mit den stabilisierenden Effekten einer Homocysteinexposition auf einen fehlenden Oxidationsschutz des Cbls durch das trunkierte Protein hin. Der damit einhergehende Anstieg an oxidativem Zellstress bietet einen schlüssigen Erklärungsansatz für die Genese von Vaskulopathien bei ΔEx2-Spleißmutationsträgern.Introduction Cobalamin deficiency type C (CblC) is the most common inborn intracellular cobalamin metabolism error. It is inherited in an autosomal-recessive manner, caused by mutations in the MMACHC gene. Biochemically the multi-systemic disease is characterized by a combined occurrence of methylmalonic aciduria and homocystinuria. Specific MMACHC gene mutations result in erroneous splicing with in-frame exon 2 deletion. Patients with this truncated ΔEx2-MMACHC protein show a phenotypically notable combination of renal thrombotic microangiopathy (rTMA) or an atypical hemolytic uremic syndrome (aHUS) and pulmonary-arterial hypertension (PAH). The objective of this study was 1) to provide a detailed analysis of the relationship between ΔEx2-splice site mutations and the occurance of renal and pulmonary vasculopathies, 2) to characterize the truncated protein in order to learn more about its pathophysiology 3) to test new therapeutical approaches. Methods Literature research was conducted using PubMed to identify the genotype- phenotype correlation between ΔEx2-splice site mutations and renal and pulmonary vasculopathies. The truncated protein was characterized on the basis of protein biochemical examinations using Western Blot techniques and immuncytological stainings. The half-life periods of the truncated protein were determined by way of the cycloheximide chase method and a proximity ligation assay was used to identify interaction partners. Effects of cobalamin and homocysteine supplementation were also analysed in HEK293 and HepG2 cell lines. Results Eighty-four CblC patients with aHUS/ rTMA and/or PAH and the reported genotype were identified, with more than 65 % of the 37 patients carrying ΔEx2-splice site mutations showing a combined renal and pulmonary phenotype. This was contrasted by five CblC patients who showed no ΔEx2-splice site mutations and renal abnormalities and PAH. In a comparison between the ΔEx2-MMACHC protein and the wild-type protein, laboratory tests revealed reduced stability, increased proteasomal degradation and a reduced interaction with the partner proteins MMADHC and MSR. The truncated protein was furthermore characterized by the formation of disulfide bonds. The truncated protein half-life period was increased from 6.4 h to 9.2 h (monomers) or 7.4 h to 10.6 h (dimers) with cyanocobalamine (CN-Cbl) treatment. Adding 100 μM of homocysteine to the cell culture medium resulted in a significant increase in expressed ΔEx2-MMACHC protein, while the wt-MMACHC protein reacted with a significant decrease in protein expression. Discussion Literature research clearly revealed a strong genotype-phenotype correlation between ΔEx2-splice site mutations and renal or pulmonary vasculopathies. The combined phenotype in particular seems to be almost pathognomonic for CblC patients with ΔEx2-splice site mutations. The reduced stability of the truncated protein is most probable of pathophysiological relevance. This was considerably increased by an excessive supply of CN-Cbl. The experimental findings reflect the observed efficacy of treatment with CN-Cbl in six CblC patients with ΔEx2-splice site mutations, thus opening up new treatment options for this group of patients. The strong tendency towards a formation of disulfide bonds between two ΔEx2- MMACHC proteins might be responsible for the disturbed interaction of the truncated protein with MMADHC and MSR. Furthermore, this distinct disulfide bond formation and the stabilizing effects of homocysteine are an indication that protection against oxidation of Cbl by the truncated protein is impaired.     Antimicrobial resistance (AMR) has become one of the significant global threats to both human and animal health, intensifying the need for rapid and precise AMR diagnostic methods. Traditional antimicrobial susceptibility testing (AST) is time-consuming, low throughput, and limited to cultivable bacteria. Machine learning offers a promising avenue for automated AMR prediction. However, most existing models emphasize features related only to known resistance genes and variants, relying heavily on AMR reference databases, and thus may overlook new AMR-related features. To address the above challenges, my first study introduces genome-wide machine learning models to detect AMR without dependence on prior AMR knowledge efficiently. Specifically, I assessed various models, including logistic regression (LR), support vector machine (SVM), random forest (RF), and convolutional neural network (CNN), for predicting resistance against four antibiotics. The findings illustrated that these models can effectively predict AMR with label encoding, one-hot encoding, and frequency matrix chaos game representation (FCGR) encoding on whole-genome sequencing data. Generally, RF and CNN outperformed LR and SVM. Importantly, I identified specific mutations associated with AMR for each antibiotic. Moreover, current AMR studies focus on single-drug resistance prediction, ignoring the cumulative nature of antimicrobial resistance over time, which makes rapid identification of multi-drug resistance (MDR) a challenge. Therefore, in my second study, in order to overcome these limitations, I constructed five multi-label classification (MLC) models for MDR problems. The findings revealed that the ECC (Ensemble Classifier Chains) model surpassed the other MLC methods, demonstrating marked effectiveness in predicting MDR. Furthermore, the constraints of limited training samples and data imbalances present significant barriers to the generalization and accuracy of AMR models. To overcome these challenges, in my third study, I have proposed a deep transfer learning model based on a CNN architecture. First, I pre-train the model on four datasets, then the best-performing model is used as the source model for transfer learning, and the model is retrained on small datasets by transferring the architecture and weights from the source model. The results showed that the deep transfer learning model improves model performance for AMR prediction on small and imbalanced datasets. In an era where data security and privacy are crucial, federated learning (FL) and swarm learning (SL) present solutions by maintaining data locally during training, which reduces the necessity to transfer sensitive information to a centralized server and improves efficiency by distributing computational load. Moreover, swarm learning achieves decentralization by not requiring a central server to manage the parameters compared to federated learning, which further improves the security of the data. Thus, in my fourth study, I delve into the application of swarm learning specifically within the context of AMR.Die Antibiotikaresistenz (AMR) ist zu einer der größten globalen Bedrohungen für die Gesundheit von Mensch und Tier geworden, was den Bedarf an schnellen und präzisen AMR-Diagnoseverfahren erhöht. Traditionelle antimikrobielle Empfindlichkeitstests (AST) sind zeitaufwändig, haben einen geringen Durchsatz und sind auf kultivierbare Bakterien beschränkt. Maschinelles Lernen bietet einen vielversprechenden Weg für die automatische AMR Vorhersage. Die meisten bestehenden Modelle legen jedoch den Schwerpunkt auf Merkmale, die sich nur auf bekannte Resistenzgene und -varianten beziehen, und stützen sich stark auf AMR-Referenzdatenbanken, wodurch neue AMR-bezogene Merkmale übersehen werden können. Um die oben genannten Herausforderungen zu bewältigen, werden in unserer ersten Studie genomweite maschinelle Lernmodelle zur effizienten Erkennung von AMR ohne Abhängigkeit von vorherigem AMR-Wissen eingeführt. Konkret haben wir verschiedene Modelle, darunter logistische Regression (LR), Support Vector Machine (SVM), Random Forest (RF) und Convolutional Neural Network (CNN), zur Vorhersage von Resistenzen gegen vier Antibiotika untersucht. Unsere Ergebnisse zeigen, dass diese Modelle AMR mit Label-Codierung, One-Hot-Codierung und `Frequency Matrix Chaos Game Representation' (FCGR) auf ganze Genom-Sequenzierungsdaten effektiv vorhersagen können. Im Allgemeinen übertrafen RF und CNN die LR und SVM Modelle. Wichtig ist, dass wir für jedes Antibiotikum spezifische Mutationen identifiziert haben, die mit AMR in Verbindung stehen. Darüber hinaus konzentrieren sich aktuelle AMR-Studien auf die Vorhersage der Resistenz gegen ein einzelnes Medikament und ignorieren die kumulative Natur der antimikrobiellen Resistenz im Laufe der Zeit, was die schnelle Identifizierung von Multiresistenzen (MDR) zu einer Herausforderung macht. Um diese Einschränkungen zu überwinden, haben wir in unserer zweiten Studie fünf Multi-Label-Klassifikationsmodelle (MLC) für MDR-Probleme entwickelt. Unsere Ergebnisse zeigten, dass das ECC-Modell (Ensemble Classifier Chains) die anderen MLC-Methoden übertraf und eine deutliche Wirksamkeit bei der Vorhersage von MDR zeigte. Darüber hinaus stellen begrenzte Trainingsstichproben und unausgewogene Daten erhebliche Hindernisse für die Generalisierung und Genauigkeit von AMR-Modellen dar. Um diese Herausforderungen zu überwinden, haben wir in unserer dritten Studie ein Deep-Transfer-Learning-Modell auf der Grundlage einer CNN-Architektur vorgeschlagen. Zunächst trainieren wir das Modell auf vier Datensätzen, dann wird das beste Modell als Ausgangsmodell für das `Transfer Learning' verwendet, und das Modell wird auf kleinen Datensätzen neu trainiert, indem die Architektur und Gewichte vom Ausgangsmodell übertragen werden. Unsere Ergebnisse zeigen, dass unser Deep-Transfer-Learning-Modell die Modellleistung für AMR-Vorhersagen auf kleinen, unausgewogenen Datensätzen verbessert. In einer Zeit, in der Datensicherheit und Datenschutz von entscheidender Bedeutung sind, bieten `Federated Learning' (FL) und `Swarm Learning' (SL) Lösungen, indem sie Daten während des Trainings lokal halten. Dieser Ansatz reduziert die Notwendigkeit, sensible Informationen an einen zentralen Server zu übertragen und verbessert die Effizienz durch die Verteilung der Rechenlast. Darüber hinaus wird beim Schwarmlernen eine Dezentralisierung erreicht, da im Vergleich zum föderierten Lernen kein zentraler Server zur Verwaltung der Parameter erforderlich ist, was die Sicherheit der Daten weiter verbessert.     Die Philipps-Universität Marburg entwickelt derzeit ein Konzept zur digitalen Präsentation ihrer wissenschaftlichen Sammlungen. Hierbei verfolgt im Rahmen der Landesinitiative HeFDI ein vom Hessischen Ministerium für Wissenschaft und Forschung, Kunst und Kultur (HMWK) gefördertes Projekt den Auftrag, Datenbankmanagementsysteme zur Integration wissenschaftlicher Sammlungen anzupassen. Neben der Konzeptionierung und Implementierung einer langfristig tragfähigen Datenbank hat das Projekt die Pilotierung der Sammlungsdigitalisierung an der Philipps-Universität zur Aufgabe.Der Datensatz enthält einen Fragebogen und das zugehörige Codebook einer im Jahr 2023 durchgeführten Umfrage unter den Sammlungen der Philipps-Universität zum Stand der Digitalisierung. Der Fragebogen enthält hierbei Punkte zu folgenden Themen: 1. Stammdaten der Sammlung, 2. physische Inventarbücher und elektronische Bestandsverzeichnisse, 3. digitale Objekte, 4. aktuelle Digitalisierungsvorhaben, 5. sammlungsspezifische Besonderheiten.1.

    Leapfrogging: Time of Entry and Firm Productivity

    We develop a model in which ex ante identical firms make endogenous entry and technology adoption decisions. We show that this model is capable of matching the stylized facts in which entry and adoption are dispersed over time and that, in many industries, it is the newest firms which are the most likely to exhibit high productivity growth and adopt new innovations (i.e., leapfrogging). We then derive the characteristics of those industries where such leapfrogging is likely to occur and show that leapfrogging can induce reverse preemption (i.e., forward-looking incumbent firms delaying entry and adoption due to leapfrogging behavior). As an application, we demonstrate how, in an industry conducive to leapfrogging, research subsidies can actually reduce short-run consumer welfare by discouraging firms from entering the market with a basic technology

    Verbal memory depends on structural hippocampal subfield volume

    Objective: To investigate correlates in hippocampal subfield volume and verbal and visual memory function in patients with temporal lobe epilepsy (TLE), mild amnestic cognitive impairment (MCI) and heathy participants (HP). Methods: 50 right-handed participants were included in this study; 11 patients with temporal lobe epilepsy (TLE), 18 patients with mild amnestic cognitive impairment (MCI) and 21 healthy participants (HP). Verbal memory performance was evaluated via the verbal memory test (VLMT) and visual memory performance via the diagnosticum for cerebral damage (DCM). Hippocampal subfield volumes of T1-weighted Magnetic Resonance Imaging (MRI) scans were computed with FreeSurfer version 7.1. Stepwise correlation analyses were performed between the left hippocampal subfield volumes and learning, free recall, consolidation and recognition performance scores of the VLMT as well as between right hippocampal subfield volumes and visual memory performance. Results: The volume of the left subicular complex was highly correlated to learning performance (β = 0.284; p = 0.042) and free recall performance in the VLMT (β = 0.434; p = 0.001). The volume of the left CA3 subfield showed a significant correlation to the consolidation performance in the VLMT (β = 0.378; p = 0.006) and recognition performance in the VLMT (β = 0.290; p = 0.037). There was no significant correlation identified between the right hippocampal subfields and the visual memory performance. Conclusion: The results of this study show verbal memory correlates with hippocampal subfields and support the role of left subiculum and left CA2/CA3 in verbal memory performance.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg
