The Current Review on Treatment of Necrotizing Fasciitis in Extremities

Necrotizing Fasciitis (NF) is a limb-threatening disease as well as a lethal condition. The progression of NF is rapid. The purpose of this review is to present updated concepts in the management of necrotizing fasciitis. The common risk factors for NF are diabetes mellitus, elderly, and immunosuppression conditions. It usually results from surgery, animal bites, and open injuries. It is common in intravenous drug users. The severity of the disease is based on bacterial virulence factors and defects in the immune defence of hosts. Often the infection begins in the hypodermis and in the superficial fascia. Proliferation of bacteria produces various toxins and enzymes leads to a cascade of endogenous cytokine reactions, resulting in widespread tissue destruction. The infection spreads along the fascial plane, undermining of the overlying skin. Erythema, pain, and oedema constitute the triad of early features of NF. It is followed by skin “hard signs” and systemic manifestations of sepsis. The diagnosis of necrotizing soft tissue infection is based on clinical assessment. Imaging and laboratory assessments are advisable only with a dubious clinical picture. High clinical suspicion, prompt diagnosis, broad-spectrum empirical antimicrobial therapy, and immediate radical surgical debridement are the keys in the treatment of NF. Early diagnosis, empirical antibiotic therapy, and prompt radical surgical debridement are crucial for saving limbs and lives

The incidence of myopia keeps increasing which leads to the risk for many eye diseases. The routine myopia assessment will not reveal changes in the retina thus investigation of its pathophysiology is becoming more important. The study investigated the correlation between ocular parameters and the degree of myopia in the adult Malay population in Shah Alam, Malaysia. Participants with myopia (Spherical Equivalence= -0.50DS) aged between 18 years to 39 years old who attended the eye examinations at MSU Eye Centre were recruited for the study. The ocular parameters investigated include macular thickness, RNFL thickness, axial length, and degree of myopia. The axial length was measured using the ZEISS IOLMaster 700, and the retinal layer and the macula thickness were measured using the ZEISS Cirrus HD-OCT 5000. A total of 41 participants have completed the study. The mean age of the participants was 26.98 ± 5.37 years. For the low, moderate and high myope, the mean axial length was 24.19mm, 25.65mm, and 26.33mm and RNFL thickness was 94.92 ± 8.90mm and 95.95 ± 18.54mm, and macula thickness was 278.73mm; 279.24mm; 271.75mm and 277.48mm, respectively. There was a significant difference between the axial length of myopia and the control group (p<0.01) and RNFL and macular thickness were thinner in eyes with moderate to high myopia than in normal eyes (p-<0.01). A significant RNFL and macula thinning were observed in high myope suggesting that retinal assessment is needed for patients with high myope for early detection of retinal diseases due to myopia complication

Fingerprints have long been utilised as a form of evidence in forensic trials. This study centred on the association between fingerprint patterns and skin disease eczema, which can subsequently be utilised to narrow down a suspect list in an inquiry. All ten fingerprint patterns were collected from Klang Valley Malaysians aged 18 to 50. 100 samples were collected from 82 healthy individuals and 18 eczema cases. Following that, the fingerprint patterns were divided into four categories: ulnar loop, radial loop, whorl, and arch. The collected data were statistically analysed. The Chi-square test was used to determine the link between eczema and fingerprint patterns. As a result, there was a satisfactory correspondence between eczema and fingerprint patterns. A positive correlation exists between the ulnar loop pattern and eczema with p<0.05. The dominant fingerprint pattern in the study group was ulnar loop (9.9%), followed by whorl (5.8%), radial loop (4.7%) and arch (1.5%). Among healthy individuals, the dominant fingerprint pattern was whorl (42.6%), followed by radial loop (25.5%), ulnar loop (24.5%) and arch (7.2%). Thus, it is suggested that fingerprint patterns might be applicable as a screening tool for eczema. When it is done from a forensic standpoint, other research may validate the trustworthiness of this study even more

Statistics by the Ministry of Health Malaysia indicated that smoking is the primary risk factor for chronic diseases that accounts for 10% - 12% of deaths in this country. In response to this issue, the Malaysian government introduced the Generation Endgame (GEG) smoking policy, which aims to reduce the prevalence of smoking in the country to less than 5% by 2045. However, there is still lack of report to explore the awareness and support among Malaysians on this policy. A cross-sectional online survey was conducted among adult Malaysian population using a set of questionnaires assessing the sociodemographic characteristics as well as the awareness and perception towards the newly implemented tobacco generational endgame policy. Only 41.5% out of 402 respondents were aware of the tobacco generational endgame policy. Despite that, more than 90% of the respondents strongly support the ban on the sale of cigarettes and tobacco to those born after 2007, restrict the sale of cigarette and tobacco near schools, raising the legal age of sale of cigarettes and tobacco to 21 years old, reducing the number of retailers selling cigarettes and tobacco and requiring retailers to have a license which can be removed if they sell to those under-age. The present study highlights the positivity among Malaysian in supporting the tobacco generational endgame policy while collaborative efforts from the government and private sectors are essential to further promote effective health messaging and increase the awareness level on this issu

Healthy weight management is very important to decrease prevalence of obesity. Weight reduction plan and controlling calorie diet intake are common weight loss strategy among obese people to achieve ideal body weight. The purpose of this review is to observe different type of dietary regimen affecting weight loss management. This review examines 30 PubMed articles published between 2018 and 2022. The following keywords such as “obesity”, "dietary” and "weight loss" were used to search in PubMed. This review focusing on adult studies involving meal replacement, micro nutrient-focused diet and specific dietary practices excluding abstracts and eating disorders or disease morbidity. Lastly, we evaluate nine articles for the present review and the relationship of dietary regimen associated with weight loss goals were observed. Diets like meal replacement, low glycemic index – High protein diet, low carbohydrate diet, moderate fat diet have shown significant effects on weight loss including reduced body fat and increased lean body mass. However, these diets also have negative effects like tiredness due to lack of calorie intake and increase of hunger level due to small portion size of meal intake. In addition, these diet findings are contentious. A Mediterranean diet may be beneficial for faster weight loss, but further research is required. Increased compliance in dietary regimen were associated with greater weight loss such as consuming nutritious foods, meal timing compliance and reduce food craving. Further research is required to determine effective obesity management for weight loss maintenance. Moderate fat foods, low carbohydrate and low glycemic index are recommended in healthy diets but effectiveness is unclear due to compliance issues in dietary regimen and some of individual not complete the weight loss program. However, it appears that eating less calories aids individuals in maintaining weight reduction. A balanced low-calorie diet that incorporating various food groups and appropriate portion sizes can control calorie intake thus meet the micro nutrients content requirement and promote greater weight loss. As evidence, a study conducted in Ireland have shown that milk-based meal replacements for bariatric patient had successful effect on weight loss. A study conducted among adult women in Iran showed that high protein diet was highly promotes weight reduction compared to control group. Mediterranean diet conducted in Italy among men and women showed that the diet promotes greater weight loss. Therefore, milk-based meal replacement, high protein diets and Mediterranean diets can effectively reduce prevalence of obesity among adults. These diet strategies can prevent weight gain and improve overall health
