Assesment of the Effects of İzmit Gulf Sediment on the Microstructural and Technical Characteristics of Porcelain Tiles
Enhanced vs. standard low dose dexamethasone treatment on respiratory outcomes of preterm infants with bronchopulmonary dysplasia
Background: Premature infants are particularly vulnerable to complications such as bronchopulmonary dysplasia (BPD). Although systemic steroids have been used for some time, no effective alternative treatments have emerged. This has led clinicians to investigate various dosing regimens and discuss the necessity of revising systemic steroid dosages. This study aims to evaluate the effectiveness and safety of two different doses of systemic dexamethasone use in treating BPD in premature infants born at or below 30 weeks of gestation. Methods: A retrospective review was conducted on hospital records of premature infants admitted to the Neonatal Intensive Care Unit of Izmir Private Medicalpark Hospital for a 6-year period. In the first 2 years of these 6 years, as historical controls, infants with evolving BPD had received dexamethasone treatment according to the DART protocol, while the ones born in the last 4 years received enhanced low-dose treatment. Data collected included demographic characteristics, risk factors for preterm birth, duration and way of oxygen support, prematurity-related complications, systemic dexamethasone doses, and mortality. Results: Of the 368 infants analyzed in the study, 265 had received systemic enhanced low dose dexamethasone, while 103 had received low dose dexamethasone treatment. Baseline characteristics including gestational age, birth weights, and RDS severity were similar among groups The study found that a cumulative dose of 1.35 mg/kg of systemic dexamethasone (enhanced low dose) is both safe and more effective than standard low dose dexamethasone (DART protocole) therapy in the management of BPD. The DART group required significantly longer invasive and non-invasive ventilatory support compared to the enhanced low-dose group. By the 36th gestational week, a higher proportion of infants in the enhanced low-dose group were free from oxygen support. The enhanced low-dose group also had a higher extubation success rate. Dexamethasone -related side effects were minimal, with only two cases of hypertension and one of interventricular septal hypertrophy in the enhanced low dose group, and one case of hypertension in the DART group, all resolving over time. Conclusions: Our study suggests that an enhanced low-dose dexamethasone regimen (1.35 mg/kg) may offer superior respiratory outcomes compared to the conventional DART protocol without a significant increase in short-term adverse effects
A cross-country comparison of health promotion for the unemployed people in Germany and Türkiye
Objective: This study examines health promotion initiatives for unemployed individuals, comparing practices in Germany and Türkiye. The cross-country comparison based upon a literature review considers the psychosocial impacts of unemployment-such as depression, anxiety, and low self-esteem-and evaluates national approaches in relation to welfare regime characteristics, unemployment rates, and available institutional resources. Germany’s comprehensive health and psychosocial support programs for the unemployed are contrasted with the more limited measures found in Türkiye. The German model demonstrates a more structured and wide-ranging implementation of psychosocial interventions, while Türkiye's approach remains in an early development phase with constrained reach and resources. The comparative evaluation highlights the significance of welfare regime type and institutional capacity in shaping health promotion efforts. The study underscores the need for Türkiye to expand and systematize health promotion programs for the unemployed. Policy recommendations are offered to improve psychosocial health outcomes through more inclusive and resource-supported interventions. Keywords: Unemployment, Health promotion, Public health</p

Clinical characteristics of AATD-related COPD patients vary with age at diagnosis: data from the EARCO international registry
Background This study aims to evaluate the clinical characteristics of COPD patients with AATD according to their age at diagnosis. Methods Data was obtained from the European Alpha-1 Research Collaboration (EARCO) registry, an international prospective cohort study. AATD patients with COPD registered between February 2020 and October 2024 were analysed. Clinical charateristics were compared between groups, stratified by age of AATD diagnosis as follows; = 65 years. A multivariable logistic regression model explored factors associated with age at diagnosis. Results A total of 1,565 AATD-COPD patients were included, with 18.2% receiving an AATD diagnosis at age >= 65. In univariate comparisons according to diagnosis age revealed that the prevalence of patients with Pi*ZZ mutation was lower in the >= 65 age group (47.1%) compared to the 45-65 (65.5%) and = 65 group compared to the younger age groups. The proportion of never-smokers was highest in the >= 65 group (39.5%), whereas only 15.3% of patients under 45 were never-smokers. Multivariate analysis showed that; compared to never-smokers, former smoking (OR: 0.08; 95% CI: 0.03-0.23) and current smoking (OR: 0.43; 95% CI: 0.27-0.70) were negatively associated with a diagnosis at age >= 65 in all sample. Compared to the Pi*ZZ genotype, among all sample, Pi*SS was associated with more than a 3-fold increased likelihood of diagnosis at age >= 65 and when considering only index cases Pi*SZ was associated with diagnosis age of >= 65 (OR: 2.01, 95%CI: 1.01-4.04). Among patients with the Pi*ZZ, current smoking was negatively associated (OR:0.24,95%CI: 0.13-0.47) with a diagnosis at age >= 65, whereas higher FEV1% and serum AAT levels were positively associated with later diagnosis. Conclusion Patients diagnosed at an older age had lower tobacco exposure and less severe disease. This suggests that subclinical symptoms may contribute to delays in diagnosis, as COPD features can be subtle or under-recognised until later in life. Predictors of Transphobia and Attitudes Toward Transgender Individuals Among Nurses in Türkiye: A Cross-Sectional Study
Background/Objectives: Transgender individuals face significant healthcare disparities, often exacerbated by provider prejudice and knowledge gaps. Nurses, as frontline providers, play a critical role in promoting inclusive care. This study aimed to evaluate the levels and predictors of transphobia and attitudes toward transgender individuals among nurses in T & uuml;rkiye. Methods: A cross-sectional survey was conducted with 100 registered nurses. Participants completed the Transphobia Scale, the Attitudes Toward Transgender Individuals Scale, and the Hudson & Ricketts Homophobia Scale. Additional measures included perceived knowledge, prior education on transgender health, familial attitudes, and comfort levels when working with transgender patients. Multiple linear regression analyses identified predictors of transphobia and attitudes. Results: The mean Transphobia Scale score was 41.53 (SD = 12.67), and the mean Attitudes Toward Transgender Individuals Scale score was 57.45 (SD = 16.42). Greater homophobia, negative parental attitudes, and higher perceived knowledge significantly predicted higher transphobia scores (Adjusted R2= 0.327). Prior education on transgender health was also associated with lower transphobia. Lower comfort with transgender patients was marginally associated with higher transphobia. More positive attitudes toward transgender individuals were predicted by lower transphobia, lower homophobia, and prior education (Adjusted R2 = 0.526). Conclusions: Findings underscore the urgent need for structured transgender health education and culturally sensitive interventions among nurses. Addressing sociocultural factors and training gaps could enhance equitable healthcare delivery for transgender populations, particularly in culturally conservative settings like T & uuml;rkiye

Optimizing Solar Energy Harvesting: A K-Means Clustering Approach for Enhanced Efficiency and Viability
In recent years, photovoltaic (PV) solar energy has played a crucial role in the global transition toward renewable energy, contributing to 46% of the electric capacity. It has emerged as a primary source; however, optimizing energy utilization and solar panel efficiency to maximize absorbed solar radiation remains a significant challenge. Additionally, it addresses the optimization of solar energy generation and the mitigation of potential overheating issues in dual-axis solar tracking systems. Despite its importance, PV power generation is hindered by uncertainty and intermittency, posing obstacles to achieving a stable and reliable power supply. This research introduces an innovative synthesis method for a typical solar radiation year (TSRY) based on K-means clustering to maximize energy harvest. The K-means algorithm, a fundamental image processing technique, is utilized to classify images into distinct groups. This approach enhances energy generation potential, panel efficiency, and the long-term sustainability of solar energy systems compared to conventional methods
