    Facteurs périopératoires d’une transplantation rénale comme facteur de risque de reprise retardée de la fonction du greffon : étude rétrospective monocentrique

    Background. The quality of renal transplantation and long-term outcomes are influenced by multiple factors, many of which remain unknown, particularly during the perioperative period. Methods. We conducted a retrospective study between January 1, 2022, and December 31, 2022, at Bordeaux University Hospital, France. The aim was to analyze the risk factors associated with delayed graft function (DGF), with a specific focus on blood pressure in both the donor and recipient before and during transplantation surgery. Results . Among 164 patients, 55 experienced DGF. Multivariate analysis showed that higher blood pressure at the time of entering the operating room was a protective factor against DGF (median blood pressure (MBP): OR 0.671 [CI 0.499–0.884] per 10 mmHg, p < 0.01). Conversely, a history of treated hypertension in the donor (OR 2.62 [CI 1.2–5.89], p < 0.01), longer cold ischemia time (OR 1.03 [CI 1.01–1.05] per additional 15 minutes, p < 0.01), and higher blood pressure at the time of unclamping (MBP: OR 2.44 [CI 1.53–4.07] per additional 10 mmHg, p < 0.01) were predictive factors for DGF. Conclusion . Our findings suggest that higher blood pressure before transplantation surgery is protective against DGF. However, a history of hypertension, particularly in elderly donors, is a significant risk factor for DGF.Contexte La qualité de la transplantation rénale et les résultats à long terme sont influencés par de multiples facteurs, dont beaucoup restent méconnus, notamment pendant la période péri-opératoire. Méthodes Nous avons mené une étude rétrospective entre le 1er janvier 2022 et le 31 décembre 2022 au CHU de Bordeaux. L'objectif était d'analyser les facteurs de risque associés à la reprise retardée de la fonction du greffon, avec un intérêt particulier pour la pression artérielle chez le donneur et le receveur avant et pendant la chirurgie de transplantation. Résultats Parmi les 164 patients, 55 ont présenté une reprise retardée de la fonction du greffon. L'analyse multivariée a montré qu'une pression artérielle plus élevée au moment de l'entrée en salle d'opération était un facteur protecteur contre la reprise retardée de la fonction du greffon (pression artérielle moyenne (PAM) : OR 0,671 [IC 0,499–0,884] par 10 mmHg, p < 0,01). En revanche, des antécédents d'hypertension traitée chez le donneur (OR 2,62 [IC 1,2–5,89], p < 0,01), un temps d'ischémie froide plus long (OR 1,03 [IC 1,01–1,05] par 15 minutes supplémentaires, p < 0,01) et une pression artérielle plus élevée au moment du déclampage (PAM : OR 2,44 [IC 1,53–4,07] par 10 mmHg supplémentaires, p < 0,01) étaient des facteurs prédictifs de la reprise retardée. Conclusion Nos résultats suggèrent qu'une pression artérielle plus élevée avant la chirurgie de transplantation est protectrice contre la reprise retardée de la fonction du greffon. Cependant, des antécédents d'hypertension, notamment chez les donneurs âgés, constituent un facteur de risque significatif pour la reprise retardée de la fonction du greffon

    International audienceThe study of a new scorpion specimen from Cretaceous Burmite leads to the description of asecond new species belonging to the remarkable genus Palaeobutheolus Lourenço, 2025, and confirms at the same time its association to a buthoid lineage and to the extant family Buthidae C. L. Koch, 1837. The new species shows some clear distinct characters, particularly a telson with a pear-like-shape, without a sub- aculear tooth but with the presence of a pseudo annular ring, the anterior edge of the carapace almost convex, spiracles oval to slit-like, distinct morphometric values and marked differences in the number and position oftrichobothria. The discovery of this new species comforts the validity of the genus Palaeobutheolus and the existence of buthid elements within the Burmite fauna.L’étude d’un nouveau spécimen de scorpions de l’ambre du Crétacé inférieur du Myanmar conduit à la description d’une nouvelle espèce appartenant au remarquable genre Palaeobutheolus Lourenço, 2025, confirmant ainsi son association à la lignée des Buthoidea ainsi qu’à la famille actuelle des Buthidae C. L. Koch, 1837. La nouvelle espèce présente des caractères très distincts, notamment un telson plutôt en forme de poire, avec absence d’épine sous-aiguillonaire mais présence d’une pseudo constriction à la base de l’aiguillon, le bord antérieur de la carapace de forme presque convexe, des stigmates de forme ovale ou allongée, des valeurs morphométriques distinctes et des différences notables dans le nombre et la position des trichobothries. La découverte de cette nouvelle espèce confirme la validité du genre Palaeobutheolus et conforte une fois de plus l’existence d’élémentsappartenant aux Buthidae au sein de la faune scorpionique de l’ambre Crétacé de Myanmar

    International audienceThe Col de Dio palaeontological site yields the first swimming trails of fishes from the continental Permian deposits of the Lodève Basin (southern France). Several morphologies of trails were distinguished, each of them including one, two or four thin, horizontal and sinusoidal grooves/ridges. Trails are ascribed to Undichna bina , Undichna britannica and Undichna unisulca. They co-occur with arthropod body remains (e.g. “conchostracans”, triopsids), invertebrate traces (cf. Cochlichnus , Diplichnites , and Diplopodichnus ) and swimming tracks of tetrapods ( Characichnos ). The sedimentological analysis conducted at the Col de Dio tracksite indicates the presence of two orders of stratigraphic sequences that rule the alternation of humid and arid cycles in an endorheic terminal mud-dominated floodplain and playa-lake environment. Although body fossils of fishes remain unknown in the Salagou Formation, the presence of Undichna shows that ichthyofauna was an abundant component of the palaeoecosystem related to this formation. The diverse shapes of Undichna suggest a great diversity of fish morphologies and/or behaviours. This new discovery of fish trails co-occurring with tetrapod and arthropod tracks complements other coeval French tracksites, suggesting that such ichnofaunas were common in Permian terrestrial ecosystems from the Eastern Pangean intertropical zone

    Five-Year Results With Patisiran for Hereditary Transthyretin Amyloidosis With Polyneuropathy

    International audienceImportance There is a lack of long-term efficacy and safety data on hereditary transthyretin amyloidosis with polyneuropathy (hATTR-PN) and on RNA interference (RNAi) therapeutics in general. This study presents the longest-term data to date on patisiran for hATTR-PN. Objective To present the long-term efficacy and safety of patisiran in adults with hATTR-PN. Design, Setting, and Participants This global open-label extension (OLE) of the APOLLO randomized clinical trial and phase 2 OLE study enrolled patients from 43 hospitals or clinical centers across 19 countries between July 2015 and August 2017, with follow-up until November 2022. Of 212 eligible patients with hATTR who completed the phase 3 APOLLO or phase 2 OLE parent studies, 211 enrolled in and 138 completed the global OLE. Intervention Patisiran, 0.3 mg/kg, intravenously once every 3 weeks for up to 5 years. Main Outcomes and Measures Outcomes evaluated at year 5 of the global OLE included disability (polyneuropathy disability [PND] score); polyneuropathy severity (Neuropathy Impairment Score [NIS]), nutritional status (modified body mass index [mBMI]), quality of life (Norfolk Quality of Life–Diabetic Neuropathy [Norfolk QOL-DN]), and Rasch-Built Overall Disability Scale (R-ODS), with no statistical hierarchy. Safety, survival probability, and mortality were also assessed. Results At the global OLE baseline, the mean (SD) age was 61.3 (12.3) years, and 156 patients (73.9%) were male. In 138 patients completing the study, PND scores remained stable or improved in 89 patients (65.0%), NISs showed a mean (SD) change of 10.9 (14.7), and mean (SD) mBMI (calculated as weight in kilograms divided by height in meters squared times serum albumin in grams per liter) increased by 46.4 (120.7) over 5 years from baseline. Norfolk QOL-DN and R-ODS scores showed mean (SD) changes of 4.1 (16.7) and –3.7 (6.2), respectively. Adverse events (AEs) leading to study withdrawal occurred in 47 patients (22.3%). Infusion-related reactions were the most common treatment-related AE (n = 34 [16.1%]). Overall, 41 patients (19.4%) died during the study. Patisiran treatment in the parent study and low familial amyloid polyneuropathy score at parent study baseline were associated with significantly improved survival. Conclusions and Relevance In the longest study of an RNAi therapeutic for any disease, patisiran treatment resulted in modest changes for patients with hATTR-PN with an acceptable safety profile. These results highlight the importance of initiating early treatment for hATTR and the potential of RNAi therapeutics in medicine.
