Exploring Behaviours Related to Pain Indication for Residents in Long-Term Care with Dementia
Aim: The aim of this article is to explore the behaviours related to pain for residents of long-term care with dementia.
Background: Nurse practitioners caring for residents with dementia face the complex task of assessing and managing pain. Residents in long-term care who live with dementia may express pain differently than those without cognitive or communication impairments. Responsive behaviours that may occur in advancing dementia may also be indicators of pain. Tools that consider behaviours related to pain for cognitively impaired residents such as the PACSLAC, should be considered.
Methods: This secondary analysis is a retrospective population-based descriptive study of Resident Assessment Instrument-Minimum Data Set version 2.0 assessments conducted in long-term care homes across Ontario.
Findings: Results show that, in many circumstances when a resident with dementia exhibits responsive behaviors that may be related to both dementia and pain, no pain is reported. These items include wandering, resisting care, and repetitive verbalizations. The findings suggest that pain may not be identified or treated in people with dementia.
Conclusions: Nurse practitioners must provide an individualized approach in order to accurately assess and treat pain for residents with dementia. By acknowledging deficits and improving practice guidelines, the hope is to improve pain management and quality of life for residents with dementia and pain
The Impact of COVID-19 on Nurse Practitioner Practice and Patient Presentation in Ontario: A Qualitative Study
Aim: To highlight the impact of the COVID-19 pandemic on nurse practitioner (NP) practice and their patient populations.
Background: Across Canada, the pandemic has caused strain on the health care system, health care providers, and patients. Many NPs continued to provide care to their patients via virtual methods as they were not permitted to assess patient in office. Lack of direct physical care was detrimental to both NPs and their patients.
Methods: A survey was distributed to 2,094 NPs practicing in Ontario between May and August 2020. After quantitative analysis a qualitative phase was conducted that involved one-on-one semi-structured interviews with 14 NPs who completed the survey and agreed to follow up.
Findings: The COVID-19 pandemic has had many impacts on patients and health care workers. Specifically, this study has found that NPs and patients have encountered many obstacles, such as isolation. Additionally, patients have delayed seeking care resulting in various disease progression due to concerns regarding contracting COVID-19. Finally, patients have experienced a decline in their mental health during the pandemic due to factors such as isolation.
Conclusion: The effects of the pandemic have been detrimental for both patients and providers as demonstrated in the interviews with NPs in this study. Follow up studies should explore the long-term effects of the pandemic
Deleuze et le Yi Jing: une étude comparative du devenir
Dans cet article, nous soutiendrons que le Yi Jing présente une conception d’un devenir qui n’est pas seulement constant, mais différentiel, tout comme chez Gilles Deleuze dans ses livres Différence et répétition et Le bergsonisme. Nous trouvons particulièrement intéressant que Deleuze et les auteurs du Yi Jing mettent le concept du devenir (ou du changement) au premier plan de leurs spéculations et au fondement de leurs ontologies. Bien qu’il y ait de différences marquantes entre la philosophie deleuzienne et la philosophie sous-jacente dans le Yi Jing, fondamentalement, la « répétition de la différence » chez Deleuze et le « changement perpétuel » dans le Yi Jing reviennent au même. Par cet article, nous espérons pouvoir combler quelques lacunes quant aux connexions entre la philosophie occidentale et la philosophie orientale. Un autre objectif consiste à apporter quelque chose de nouveau à la littérature secondaire. Il y a peu de recherches consacrées à la comparaison Philosophie Deleuzienne / Philosophie Chinoise, alors nous espérons apporter une contribution à la littérature comparative en philosophie, ainsi qu’à la littérature secondaire sur Deleuze et le Yi Jing respectivement
Patient Experience with an NP-Led Community Wound Care Service
Aim: This study examined patient satisfaction with an NP-Led community wound care program.
Background: Examining patient experience provides information to guide nurse practitioner (NP) practice regarding patient experience with wound care provided by NPs in Canada.
Methods: This quantitative study utilized a Likert scale questionnaire to examine patient experience with a 12-week pilot NP-Led wound care service. The population included patients registered to receive community wound management through Southwestern Ontario LHIN.
Findings: Forty-nine surveys were returned (response rate-42%). Participants reported positive experiences with NP care. Bivariate analysis (ANOVA) found no statistical difference between group means of the independent variables age, education, income level, marital status, and the dependent variable patient experience (p = >.05). Mann-Whitney U demonstrated gender to be statistically significant (p = 0.14); females had higher median scores than males, 4.8 and 4.6 respectively. Linear regression determined that Males were less likely to have a positive experience (F (1, 45) = 7.7, p =.008).
Conclusion: Patients had a positive experience with the NP-Led wound care service. This study provides information that is useful when designing an NP-Led wound care program.
Objectif : Cette étude a examiné la satisfaction des patients à l\u27égard d\u27un programme communautaire de soins des plaies dirigé par une IP.
Contexte : L\u27examen de l\u27expérience des patients fournit des renseignements pour guider la pratique des infirmières praticiennes (IP) concernant l\u27expérience des patients en matière de soins des plaies fournis par les IP au Canada.
Méthodes : Cette étude quantitative a utilisé un questionnaire à l\u27échelle de Likert pour examiner l\u27expérience des patients avec un service pilote de soins des plaies dirigé par une NP pendant 12 semaines. La population comprenait des patients inscrits pour recevoir la gestion communautaire des plaies par l\u27entremise du RLISS du Sud-Ouest de l\u27Ontario.
Résultats : Quarante-neuf sondages ont été retournés (taux de réponse - 42 %). Les participants ont rapporté des expériences positives avec les soins des IP. L\u27analyse bivariée (ANOVA) n\u27a trouvé aucune différence statistique entre les moyennes de groupe des variables indépendantes âge, éducation, niveau de revenu, état matrimonial et la variable dépendante expérience du patient (p => 0,05). Mann-Whitney U a démontré que le sexe était statistiquement significatif (p = 0,14) ; les femmes avaient des scores médians plus élevés que les hommes, 4,8 et 4,6 respectivement. La régression linéaire a déterminé que les hommes étaient moins susceptibles d\u27avoir une expérience positive (F (1, 45) = 7,7, p = 0,008).
Conclusion : Les patients ont eu une expérience positive avec le service de soins des plaies dirigé par une IP. Cette étude fournit des informations utiles lors de la conception d\u27un programme de soins des plaies dirigé par une IP
Reading Comprehension Abilities in the Anglophone Aging Population with Post-Secondary Education
Literacy skills such as reading comprehension are essential in order to understand written information and complete daily tasks. Low literacy skills are common in the aging population thus affecting quality of life, independence and social integration. Reading comprehension is a complex activity that requires processing at many different levels. According to Kintsch’s construction-integration model, three levels of representations of a text must be achieved in order to fully comprehend a text. Previous studies often considered the aging population as a single group and compared this group to young adults. However, few studies examined the variability in reading comprehension abilities within the aging population. Studies examining the variability in the construction of a situation model in older adults are even more scarce. The main objective of this study is to examine the effect of age on reading comprehension abilities at the surface model, text-based model and situation model in the aging population with high education level.
Participants were recruited in northern Ontario and were grouped into three age intervals (60-69 years; 70-79 years; ≥ 80 years). Participants’ cognitive functions were assessed using the Montreal Cognitive Assessment (MoCA) and health literacy was assessed using the Short Test of Functional Health Literacy in Adults (STOFHLA). Reading comprehension abilities were evaluated using the Woodcock Reading Mastery Tests - third edition (WRMT-III) as well as the Gray Oral Reading Test – fifth edition (GORT5).
Results indicated no significant differences in performance of reading comprehension abilities among the three age groups at the surface level, text-based level, and situation model level of reading comprehension. These findings suggest no age-related effect on reading comprehension abilities in adults with high levels of education. The lack of differences observed may be explained by the absence of differences in cognitive functions between the three age groups as well as the high level of education which may have acted as a protective factor. Future studies should examine reading comprehension in Anglophone adults with lower levels of education to better understand whether education is a protective factor against a decline in reading comprehension abilities in the aging population
“The calm in the storm”: A scoping review of hospital-based peer support breastfeeding interventions
International health bodies have called for nations to invest more in the promotion and support of breastfeeding. Peer models of breastfeeding support have shown to be an effective modality for a diversity of populations. A synthesis of in-person, peer breastfeeding support interventions in-hospital has not been done. This scoping review aimed to describe the nature and outcomes of hospital-based breastfeeding peer support reported in academic publications. The process included identifying research questions and relevant studies, selecting studies for analysis, charting data, and collating and summarizing results. We identified 24 articles for analysis, with descriptions of 12 peer breastfeeding interventions from the US and the UK. Through a qualitative content analysis of the studies, six categories emerged related to the interventions: intervention goals, theory, components, role of peers, program development and sustainability and evaluation. Most interventions were designed and implemented with a top-down approach, and utilized psychological theories of peer support. Findings from the quantitative and qualitative evaluations indicated the interventions demonstrated the capability to increase initiation, duration and exclusivity of breastfeeding. Positive psychosocial benefits were reported by mothers and positive health impacts for infants were detected. Hospital-based peer model is a promising practice that merits further implementation and study, in particular for families facing barriers to breastfeeding.Les organismes internationaux de santé ont demandé aux nations d\u27investir davantage dans la promotion et le soutien de l\u27allaitement maternel. Les modèles de soutien à l\u27allaitement par les pairs se sont révélés être une modalité efficace pour une diversité de populations. Une synthèse des interventions de soutien à l\u27allaitement en personne et par les pairs dans les hôpitaux n\u27a pas été réalisée. Cette étude de cadrage visait à décrire la nature et les résultats du soutien par les pairs à l\u27allaitement en milieu hospitalier, tels qu\u27ils sont présentés dans les publications universitaires. Le processus a consisté à identifier les questions de recherche et les études pertinentes, à sélectionner les études à analyser, à consigner les données dans des graphiques et à rassembler et résumer les résultats. Nous avons identifié 24 articles à analyser, avec des descriptions de 12 interventions d\u27allaitement par les pairs aux États-Unis et au Royaume-Uni. Une analyse qualitative du contenu des études a permis de dégager six catégories liées aux interventions : objectifs de l\u27intervention, théorie, composantes, rôle des pairs, élaboration et durabilité du programme et évaluation. La plupart des interventions ont été conçues et mises en œuvre selon une approche descendante, et ont utilisé les théories psychologiques du soutien par les pairs. Les résultats des évaluations quantitatives et qualitatives ont indiqué que les interventions ont démontré la capacité d\u27augmenter l\u27initiation, la durée et l\u27exclusivité de l\u27allaitement. Des avantages psychosociaux positifs ont été signalés par les mères et des effets positifs sur la santé des nourrissons ont été détectés. Le modèle des pairs en milieu hospitalier est une pratique prometteuse qui mérite d\u27être mise en œuvre et étudiée plus avant, en particulier pour les familles confrontées à des obstacles à l\u27allaitement.
 
L’enseignement en mode synchrone des activités physiques et sportives dans un contexte de confinement.
In a context of confinement, to what extent is it possible to teach physical, motor or emotional learning in synchronous mode? A closed question, the responses of which were encoded on a four-point Lickert scale, made it possible to measure the perception of social actors involved in the physical, motor and emotional development of the participants. The sample was made up of 102 sports social actors from Morocco or Algeria aged between 9 and 64 years old. The actors were trainers, administrators, practitioners or parents. The question was subjected to an analysis of variance according to three nominal variables (gender, status, country) and a Pearson correlation between the age of social actors and the three desired types of development. It appears that physical and emotional development are perceived to be significantly more difficult to assess by Moroccan social actors than by their Algerian counterparts. The gender effect is only significant for the assessment of emotional development in favor of females. In contrast, the effect of social actor status is not significant for physical development, motor development, or emotional development. Finally, \u27young\u27 social actors perceive it less difficult to teach APS synchronously, but the correlation is rather weak (R = - 0.22 *). Some hypotheses of action are considered to face the didactic challenges posed by this particular type of teaching in a confinement situation.Dans un contexte de confinement, jusqu’à quel point est-il possible de développer en mode synchrone des apprentissages physiques, des apprentissages moteurs ou des apprentissages affectifs ? Une question fermée, dont les réponses étaient encodées sur une échelle de type Likert en quatre points, a permis de mesurer la perception des acteurs sociaux engagés dans le développement physique, moteur et affectif des participants. L’échantillon était composé de 102 acteurs sociaux sportifs originaires du Maroc et de l’Algérie et âgés de 9 à 64 ans. Les acteurs étaient des formateurs, des administrateurs, des sportifs ou des parents. La question a fait l’objet d’une analyse de variance selon trois variables nominales (genre, statut, pays) et d’une corrélation de Pearson entre l’âge et les trois types de développement souhaités. L\u27analyse de livres gradués (Escalire) par entremise du système d’analyse de texte par ordinateur (SATO)
A series of French leveled readers called Escalire was created to allow children to have access to books corresponding to their own reading skills (Éditions Passe-Temps, 2014). This series contains a total of 156 books, divided into 15 levels, from A to O. This scale corresponds to the levels established by Fountas and Pinnell (2012). This series of leveled readers is a reading intervention program targeting elementary students grades 1 to 4. Reading recovery programs such as this can lead students to acquire and improve reading abilities according to his or her skill and grade level. The objective of this study was to determine the correlation between the reading levels attributed to the books in the Escalire series, as well as the level of complexity of these books as indicated by SATO, a text-based computer analysis system. The results indicated a significant correlation of 0.668 and a percentage of agreement of 35.26%. In total, 95.51% of the books in the Escalireseries were either at the level corresponding to the SATO scale or within one level (either one level higher or lower than what has been proposed by SATO).

Cette série, nommée Escalire, contient 156 livres au total, qui sont répartis en 15 niveaux, soit de A à O. Cette échelle correspond aux niveaux établis par Fountas et Pinnell (2012). Cette série de livres gradués consiste en un programme d’intervention en lecture ciblant les élèves de la première à la quatrième année du cycle primaire. De tels programmes d’intervention en lecture peuvent mener l’élève vers l’acquisition et l’amélioration de la lecture en fonction de ses habiletés et son niveau scolaire. L’objectif de cette étude était de déterminer s’il y a corrélation entre les niveaux de lecture attribués aux livres de la série Escalire et le niveau de complexité indiqué par le système d’analyse de texte par ordinateur (SATO), pour ces mêmes livres. Les résultats ont démontré une corrélation modérée (r = 0.668, p < 0,001) et un pourcentage d’accord de 35,26 %. Somme toute, 95,51% des livres de la série Escalire se sont trouvés soit au niveau correspondant à l\u27échelle SATO ou à un niveau près (soit un niveau supérieur ou inférieur à ce qui a été proposé par SATO).

La réduction des complications de macrosomie, de déchirure périnéale et de césarienne à l’accouchement par la pratique d’activités physiques : état de la recherche
In Canada, there are more than 350,000 childbirths per year (Statistics Canada, 2019). In the final phase of pregnancy, women can have a birth with or without complications (WHO, 2018; Public Health Agency of Canada, 2018). According to several researchers, physical activity in several cases helps prevent some of these complications (Public Health Ontario, 2015 ; Government du Québec, 2019). The present study aimed to determine whether physical activity plays a part in reducing the etiological factors of three delivery complications: macrosomia, perineal tears as well as caesarean section and, through this analysis, determine whether physical activity acts as a preventative measure. The results obtained underline the key preventative role of a physical activity intervention and of leisure with regard to macrosomia and caesarean section. As for the contribution of physical activity in reducing the risk of perineal tears, more research is needed to determine if its role is significant
